Adoption at the Movies: The Boss Baby (SPOILERS) Adoption Movie Review #pregnant

Tim Templeton has the perfect life for a seven-year-old boy. His parents dote on him, play games with him, engage his imagination, and send him to bed each night with stories, hugs, and a special song. One day, Tim s parents ask him if he would like a baby brother he says that he doesn t want one because after all, he is enough. But, the baby shows up anyway. Tim s new baby brother The Boss dresses sharp and, as Tim finds out later, can talk. The Boss is concerned that there will not be enough love for babies in the world because people are starting to love puppies. In the meantime, Tim starts to fear that there will not be enough love for him in his parents hearts because they re falling in love with the new baby.

SPOILERS AHEAD THE REST OF THE WAY

The Adoption Connection

There is no talk of adoption in the film. However, any child who has welcomed a new sibling into their family might relate to Tim s fear that his parents won t be able to love him enough. The movie harps on that fear. The Boss Baby tells Tim, (roughly paraphrasing) You re seven and a half. You re old. It s time to make way There s only so much love to go around. Tim suggests that they could share, but the Baby says, There s not enough love to go around, and if you don t adjust, there eventually won t be a place for you. This leaves Tim wondering whether he can be fired from his own family; in order to protect his place in his parents hearts, Tim sets out to rid himself of the new baby, first by trying to prove that the Baby is sinister, and then by trying to catapult him out of a window.

Some kids who have been neglected prior to coming into foster care might connect with a question that Tim asks The Boss Baby, You missed childhood? You never had someone to love you?

Eventually, Tim and The Boss Baby come to care about each other, and see the value of family. Tim is ultimately assured that he ll never be fired from his family.

Tim eventually comes to nurture his brother; he even sings his baby brother the song that his parents used to sing to him.


In a heaven-like celestial baby factory, The Boss Baby is designated for management instead of belonging to a family because he doesn t quite fit in, and doesn t appear to be ticklish.

The Baby s words to Tim are very unkind, and they could be painful for some kids to hear. Tim s attempts to defeat his new baby brother could be very dangerous if a young child mimicked them with a real-life baby; in particular, after the baby threatened to destroy Tim s treasured stuffed animal, Tim prepares to use his baby brother s swing chair to catapult the baby from a second story window onto the street.

Tim s parents don t seem particularly sensitive to Tim s feelings; they cover Tim s pictures on the fridge with pictures of the new baby. Tim feels rejected when his parents don t do his regular bedtime routine with him.

Tim wishfully remarks that his parents might be able to return the baby to the store and get their money back. It s a quick remark, but it could catch some sensitive ears by surprise.

At one point, Tim expresses a sentiment that other big siblings probably have said or thought at times, but it still could be hard to hear; he tells the baby, My life was perfect till you showed up. I wish you d never been born. The baby, who did not appear hurt by other things Tim said, appears cut by this remark, and walks off.

One character intends to make it so that no one will ever want a baby again.

A kid behind me cried during a scene when it appeared that an infant was imperiled.

BIG SPOILER ALERT: When the Boss Baby leaves Tim s home, small kids wave magic wands to make Tim s parents forget that he ever existed. Although they do this to Tim s parents without their consent, Tim is given the option whether to forget The Boss; he chooses to remember his brother. Later, The Boss decides to be born into Tim s family. It s nice to see that the brothers have chosen to accept each other, but it could be hard for some kids to think that their parents could easily forget them this could be particularly painful for kids who wonder whether their birth parents remember them, or who fear losing their adoptive parents. On a side note, Tim has a very active imagination, and it s possible that his entire experience with The Boss Baby was imagined in response to his mother s apparent pregnancy; the film doesn t make that clear, though, and most kids probably won t make that assumption. END SPOILER

The Boss Baby had some funny moments and ultimately ended happily, but it didn t live up to the hopes I had for it, and it s got some aspects that make it hard to recommend for young or sensitive viewers. Most kids 11 and up should be fine, but younger kids especially those for whom permanency hasn t always been a sure thing might find certain aspects of this one very troubling. It would be helpful for parents to directly remind their children that the children will never be fired from the family, and that there is enough love to go around, no matter how many kids come into the home.


Questions for Discussion

Can parents love more than one child with all of their heart? Can a child love more than one parent (or more than one pet) with all of their heart?

Where does love come from? Do you only have a certain amount to give, or can you make (or find) more?

What are the good parts of having a new brother or sister? What parts scare you?

This movie is not about a baby being adopted.
Notice the night before he came the parents were putting Tim to bed when the mother is pregnant and the baby is kicking.
Also, when the Boss Baby comes back into the world, he enters through a spiral or figuratively the birth canal.
This is a story of loss of a baby sibling.
Also note that when he goes back, he falls into an angel decorated cake and makes angel wings. Thus implying the child had died and went back to heaven or baby Corp where babies come from.

What I think was actually going on in the story, and I wonder your take on it, is that Tim has a pretty active imagination. I think the Boss Baby never happened, and it was just how Tim was processing getting ready for a baby brother. That s why no one but Tim remembers the Boss Baby — what do you think?

1. Select a location neighborhood for your hookah bar. A critical decision is where to open up the establishment. The same article notes the East Village in Manhattan also features many similar bars. Place these locations on your short list of possibilities. Keep in mind that prices in Manhattan tend to be higher than in Queens, which will be reflected in your monthly rent expense. On the other hand, you can probably charge more for the service in Manhattan than you could in Queens.

2. Rent a store. After you pick a location, you will have to find a store to rent. Start by deciding how large you wish your bar to be. 4. Finalize your menu. Before you begin purchasing the equipment to use in your hookah bar, decide on the food and drinks you will serve. Remember: the lighter the food, the less time customers will likely spend at the bar and the more people you can serve. Easy-to-prepare items, such as sandwiches and finger foods that are also consumed quickly, should enable you to server more people more often.

5. Furnish your bar to suit the taste of the target market. According to the New York Times travel section, the hookah bars in Astoria cater primarily to Egyptians, while those in the East Village serve a more diverse clientele. Therefore, if you have chosen Astoria for your establishment, a more traditional style with rugs and classic furniture will work better. In the East Village, a more modern and contemporary design is likely to suit the taste of your diverse customers.

When the Oracle DBA creates their tnsnames.ora file to define remote databases they often specify the host name of the foreign server that contains the remote Oracle database.

For example, a entry in the tnsnames.ora file for a remote database might look like this. In the example, we have added the service name berlin to our tnsnames.ora file:

Here we see a TNS service name of berlin. which defines a connection to a remote server named hun that contains an Oracle database named kraus. When a remote connection request is made from the UNIX server, the /etc/host file is accessed to get the IP address for the hum server.

From the listing below, we see that the hum server is located at 192.133.13.12. In sum, the /etc/host file is used to isolate the IP address from the tnsnames.ora file. If the IP address should ever change, the UNIX systems administrator only needs to change the IP address in one place.

SNF – Hospice Contracts Under the New Hospice Medicare Conditions of Participation

Our article in the December 2008 issue of Shorts on Long Term Care, about CMS’s revised hospice Medicare Conditions of Participation (CoPs) and their impact on the existing contracts that nursing homes may have with hospice agencies, generated some questions. Here are some of the questions we’ve heard and the answers:

Q: Do the new CoPs apply only to hospices or also to the SNF, where the SNF has a contract with a hospice agency to provide hospice care to a resident of the facility?
A: This question came from an SNF provider that expressed concern over some of the new language it was seeing in revised contracts sent by the hospice to be executed. The gist of the question was “if we choose not to accept the revised contracts, what happens?” Technically, these new CoPs apply to the hospice, not to the SNF. However, the new CoPs are very specific about issues that must be addressed in any SNFhospice contract (see our December 2008 article for details). So the hospice is required to include these new provisions in all their contracts with SNFs, and those requirements do impose some additional burdens on the nursing home as well as the hospice. For example, both parties must coordinate their care of a hospice SNF resident more closely, with specific documentation of how they plan to do this. There are other areas where the SNF will have to work closely with the hospice to meet the new CoPs. Because the hospice is required to meet the new CoPs, including the contract requirements, if an SNF refuses to enter into a new agreement with a hospice that complies with the new CoPs, that hospice would be within its rights to cancel any existing agreement and discontinue services. In addition, since nursing homes are required under state and federal law to provide care that is appropriate for and needed by a resident, a refusal to treat hospice patients consistent with the new CoPs could be a basis for deficiency citations against the nursing home. So, if they plan to continue offering hospice in the nursing facility, both parties need to find language they can live with that also complies with the new CoPs.

Q: If my nursing home already has a contract with our hospice agency that is not scheduled to expire or be renewed at this time, are we still required to enter into a new agreement with the hospice now, or can we wait until our current contract term ends?
A: Generally, you are not required to execute a new contract with an entity with which you already have an existing, ongoing agreement. However, most contracts contain language stating that if the existing contract is inconsistent with applicable law, the parties agree to negotiate in good faith to revise the contract to remedy any such noncompliance. Given the fairly extensive changes made by the new hospice CoPs to various types of contracts, hospice agencies have a pretty good argument that the contracts they had in place under the old regulations probably are not totally compliant with the revised CoPs. As a result, those agencies will need either to amend those contracts to meet the new CoPs, replace them entirely (which many hospices are opting to do, given the scope of changes required by the new CoPs) or terminate the services they were offering under the old agreement. Continuing to use contracts that are not fully compliant with the CoPs puts hospices at risk for deficiency citations, potential loss of payment and, at least in theory, loss of Medicare certification.

Q: When should my new or amended contract with my hospice agency be in place?
A: Technically, the effective date of the new CoPs was December 2, 2008. So, ideally, those revised contracts should already be executed by both parties and in place. Realistically, we don’t expect state surveyors to be out checking the detailed language of providers’ contracts right away, since the providers are still getting used to the new CoPs themselves. However, we have been told that if a complaint by a patient, family member or staff member leads the survey team to a contract as part of the problem, it’s fair game for the team members to request and review all contracts in detail, if they choose. Also, hospice surveyors are directed to request a list of certain hospice contracts during the survey entrance conference, so they can easily request copies as well, if they choose.

Optimal Hospice volunteer training

BAKERSFIELD, CA – Optimal Hospice is looking for volunteers. Whether it’s a skill or just time, organizers say everyone has something to offer.

Assistant volunteer coordinator Karen Budnick says volunteers play an important role in hospice.

Budnick said, They’re able to give family members a break so that they can step away and some times it’s just so they can go to the grocery store or go to a doctor’s appointment.

Thursday morning, new volunteers learned more about their role.

It’s end of life, which I think that a lot of us are not comfortable with the end of life, but when you realize that it’s living life to the fullest and helping somebody do that, it really captures your heart, said Budnick.

Volunteer Tim Brewer said, There’s a whole lot of satisfaction in just giving, period.

Brewer is part of the veteran to veteran program, providing support to patients and family members who served in the military.

We would love to have more veteran volunteers, pet therapy, really any type of volunteers. We have volunteers that cut hair, massage, visit, listen to wonderful stories, said Budnick.

    NHPCO Guidelines for Selecting a CAHPS Hospice Survey Vendor

    The Family Evaluation of Hospice Care (FEHC) is a post-death survey designed to yield actionable information that reflects the quality of hospice care delivery from the perspective of family caregivers. Hospices that utilize FEHC must be exempt from CMS-required CAHPS Hospice Survey.

    ATTENTION: NHPCO is no longer accepting FEHC survey submissions through the DART system. If you qualify for the CMS-required CAHPS Hospice Survey, CMS will not permit the use of another survey. Hospices that utilize FEHC must be exempt from CMS-required CAHPS Hospice Survey. Exempt hospices may download our FEHC Performance Calculator to obtain results for FEHC surveys.

    The following information applies only to those hospices that do not qualify for the CMS-required CAHPS Hospice Survey and would like to continue using FEHC:

    To access your archived FEHC Quarterly Reports, log in to the NHPCO DART System using your hospice’s NHPCO DART ID and DART Password. If you do not know the DART ID and password for your hospice, please contact NHPCO Member Services at 1-800-646-6460.

    FEHC National Annual Reports and supplemental reports are available for NHPCO Provider Members. To access, log in with your email address and NHPCO password (this is different from your DART ID login). If you are having trouble with your individual member password, click on Forgot Password at the top of the page.

    As the first process, authentication provides a way of identifying a user, typically by having the user enter a valid user name and valid password before access is granted. The process of authentication is based on each user having a unique set of criteria for gaining access. The AAA server compares a user’s authentication credentials with other user credentials stored in a database. If the credentials match, the user is granted access to the network. If the credentials are at variance, authentication fails and network access is denied.

    Following authentication, a user must gain authorization for doing certain tasks. After logging into a system, for instance, the user may try to issue commands. The authorization process determines whether the user has the authority to issue such commands. Simply put, authorization is the process of enforcing policies: determining what types or qualities of activities, resources, or services a user is permitted. Usually, authorization occurs within the context of authentication. Once you have authenticated a user, they may be authorized for different types of access or activity.

    The final plank in the AAA framework is accounting, which measures the resources a user consumes during access. This can include the amount of system time or the amount of data a user has sent and/or received during a session. Accounting is carried out by logging of session statistics and usage information and is used for authorization control, billing, trend analysis, resource utilization, and capacity planning activities.

    Authentication, authorization, and accounting services are often provided by a dedicated AAA server. a program that performs these functions. A current standard by which network access servers interface with the AAA server is the Remote Authentication Dial-In User Service (RADIUS) .

    Keytruda for Mesothelioma and Lung Cancer

    Keytruda is a brand name for the drug pembrolizumab, developed, manufactured, and sold by Merck to treat certain types of cancer. It was first approved by the U.S. Food and Drug Administration (FDA) in 2014 to treat metastatic melanoma and other types of cancer, including non-small cell lung cancer in patients with certain genetic features, called biomarkers. This is the first time has approved a cancer drug based on genetics.

    What is exciting about Keytruda for people affected by asbestos is that early studies are showing it may prove a useful chemotherapy drug for mesothelioma. More research is needed but the results are promising and could lead to a new therapy for mesothelioma patients who have few other options for effective treatment.

    Pembrolizumab and Immunotherapy

    Keytruda is an immunotherapy drug designed to help the immune system of the body attack and kill cancer cells. Immunotherapy is a newer line of treatment for cancer that harnesses the natural power of the immune system. The immune system works when cells, like white blood cells, recognize and attack foreign invaders in the body, like viruses. They recognize these pathogens because of receptors called antigens on their surfaces. Healthy, normal cells in the body have receptors on their surfaces that signal to immune cells that they are healthy and not to be attacked.

    Some types of cancer cells express these receptors, called PD-1 proteins. These allow the cancer cells to appear like healthy cells to the immune system, so they do not get attacked and continue to grow and thrive. Pembrolizumab acts on those receptors on the cancer cells, blocking their message and allowing the immune system to recognize them as foreign and to attack and destroy them. The strategy is referred to as immune checkpoint blockade.

    Indications and Side Effects

    The FDA approved Keytruda for specific types of cancers, for cancers in certain stages or with certain characteristics, and for cancers in patients with certain biomarkers. For instance, it is approved for melanoma, but only melanoma that has spread throughout the body and cannot be treated surgically. For other cancers, like non-small cell lung cancer, it is approved only in cases in which the patient’s cancer worsened after attempting to treat it with standard chemotherapy drugs .

    Side effects with pembrolizumab can be serious, but the most common are fatigue, rash, diarrhea or constipation, nausea, decreased appetite, shortness of breath, coughing, and itching. More serious, but less commonly, patients may experience bruising, anemia, numbness, dizziness, hot flushes, flu-like symptoms, hair loss, edema, irregular heartbeat, pain, kidney failure, infections, insomnia, diabetes and jaundice. Chest pains, shortness of breath, and a cough may indicate a serious reaction that should be addressed immediately.

    Keytruda for Mesothelioma

    Right now the indications for Keytruda do not include treating mesothelioma, but studies are underway to determine if it could be an effective and safe treatment for this cancer. Most recently, a study was published in The Lancet Oncology. and showed promising results. The study has shown that while other chemotherapy drugs are failing to treat this difficult type of cancer, immune checkpoint drugs may be a better alternative.

    The clinical trial is called KEYNOTE-028 and it involves 13 research teams in six different countries. The trial includes patients with various types of advanced cancer, including 25 patients with pleural mesothelioma. All of these patients had either received unsuccessful chemotherapy treatment prior to the trial or were not eligible for chemotherapy.

    The 25 mesothelioma patients received an injection of pembrolizumab every two weeks and the results were that 14 of the patients saw reductions in tumor size. They experienced an average of six months of reduction or lack of progression of the cancer. The average survival time was 18 months. Four patients are still alive and undergoing treatment with pembrolizumab, two years later.

    While 18 months survival may not sound that encouraging, it is a major improvement. Most patients with pleural mesothelioma that receive some type of second-line treatment will only survive an additional six months. With four patients surviving past the two-year mark, the treatment with Keytruda shows great promise over other options for mesothelioma patients. While there were common side effects in the participants, like fatigue and nausea, none had to stop because of the severity of these side effects.

    Keytruda for Asbestos Lung Cancer

    Pembrolizumab is now showing promise for mesothelioma patients, but it is already approved for treating non-small cell lung cancer, another type of cancer that can be triggered by asbestos exposure. Approval for treating this kind of cancer came after results from clinical trials showed it to be safe and effective. More recent studies have shown that it may actually be better than standard chemotherapy drugs like cisplatin and carboplatin.

    The study examined the results of the KEYNOTE-024 clinical trial of pembrolizumab in non-small cell lung cancer patients. The data showed that this drug had several advantages over traditional chemotherapy. Patients given pembrolizumab saw better overall survival rates, fewer side effects, less severe side effects, and better quality of life. Some side effects were worse with pembrolizumab, but overall adverse effects were lower and less severe. The benefits of pembrolizumab were seen in using it as a first-line treatment. This could mean that patients with lung cancer may be able to use the drug instead of cisplatin or carboplatin and avoid the adverse effects, getting better results sooner.

    The promise of immunotherapy drugs for treating cancer is overwhelmingly positive, and Keytruda in particular is leading the way in finding a new strategy for better treating mesothelioma in victims of asbestos exposure. Researchers are getting on board and there are now more than 90 clinical trials ongoing using Keytruda to treat various types of cancers.

    Two new mesothelioma trials are expected to start later this year. If you are struggling with mesothelioma, pembrolizumab is not currently approved for treatment, but you may be able to participate in a clinical trial. Talk to your doctor about how you can get involved if you qualify because this could prove to be a revolutionary and life-saving, or at least life-extending treatment for this difficult type of cancer.

    Your first step toward a career in Aviation Maintenance Science is obtaining your FAA Airframe and Powerplant (A P) Certification.
    Global’s hands-on approach trains students in the specific skills employers seek in the airframe powerplant mechanics and aircraft inspection fields. Students take courses in simulated work environments from instructors with real-world experience.
    Our curriculum was developed by educators and industry representatives to ensure that course offerings accurately reflect current industry standards. Students must complete the required course to qualify for the program’s Certificate of Achievement. Once trained, technicians will also be successful in related fields of aircraft manufacturing, electronics, hydraulics, pneumatics, welding, sheet metal, quality control, civil and military defense.
    F.A.A. Eligibility Requirements
    The A P mechanics course is for applicants who meet FAR 65.71-65.77 only. Under these regulations, you must have a certain type of mechanical experience before the F.A.A. will allow you to take the F.A.A. written exams. The required experience for the powerplant mechanic rating is a minimum of 18 months working with tools on any type of aircraft engine. The required experience for the airframe mechanic rating is a minimum of 18 months working with tools on any type of airframe. If you want both ratings, and your experience was a combination of airframe and powerplant, then you need a total of 30 months experience (not 36).
    Obtaining Your Authorization Slips
    To be enrolled in GLOBAL SCHOOL OF AERONAUTICS as a mechanic student, you must first meet the F.A.A. experience requirements. The purpose of this is to be sure the F.A.A. will permit you to take the written exam after you finish the course.
    Oral and Practical Examinations
    The F.A.A. requires the written results to be in the student’s hands before an Oral and Practical examination may be given. Therefore, in order to take an oral and practical examination with one of our examiners, you must bring your written score slips and your airmen certificate and/or rating application 8610-2. Your F.A.A. inspector should give you two copies of the 8610-2 form, both bearing original signatures. Photo copied signatures are not accepted.
    Classroom Time
    The average time to obtain the A P rating is from 11 – 13 days. Our instructors work closely with each student, progressing him/her based on their own capabilities. Our course is guaranteed 100% – we keep working with you until you pass – good for 36 months.

    The dish was excellently prepared. Grilled, juicy, and fresh without a hint of fishiness. A glaze of tangerine sauce brought a hint of tart sweetness. The fish was placed on a mound of sweet plantain rice. The combination of the fish and rice alone was to die for!

    However, as only expert chefs can achieve, additional garnishes provided even bolder, beautiful tastes. Pickled onions topping the fish made for an even finer taste experience, while green beans hidden under the fish added freshness and completed each bite

    Galway Bay FM will run the inaugural Galway Radiothon on Thursday 26th, Friday 27th and Saturday 28th February 2015 and they will be broadcasting live from An Taibhdhearc for the three full days.

    Donations came be made during the Radiothon by popping into An Taibhdhearc on Middle Street, or at anytime by clicking on to the Galway Radiothon website or by clicking on the link to the right.

    During the Radiothon, they will play stories which feature people’s personal experience with cancer and life limiting illnesses.

    Funds raised will purchase state of the art equipment and make improvements to facilities to ensure those living with cancer and life limiting illnesses get the care they deserve close to home.

    Funds raised through the Make a Difference Galway Radiothon will be granted to the benefitting charities in the manner mutually agreed with Galway Bay FM.

    Under New York law, there are several offenses for impaired driving:

    DWAI stands for driving while ability impaired. It is typically charged when a driver registers a BAC between 0.05-0.79% and a law enforcement officer suspects the driver has consumed alcohol to a point an impaired ability to safely and prudently operate a vehicle. This is a traffic offense rather than a criminal act.

    DWI is typically defined as a motorist having a blood alcohol content at or above the threshold of 0.08%. However, you can be charged with NY DWI even if you have not submitted to a breath, blood or urine test to measure BAC. Officer observations again are key to the type of charge filed.

    Aggravated DWI is charged when a driver has an elevated BAC in excess of 0.18%.

    DWAI – Drugs relates to a driver impaired by drugs to the point of operating a vehicle with physical or mental abilities below those reasonably required to safely operate a vehicle.     Hospice care, according to Hospicefoundation.org, is a concept of care focused on supporting patients with a terminal prognosis and their families. Hospice staff members work in conjunction with volunteers to improve patients’ quality of life in their final days. The focus is on offering patients pain management, dignity and comfort. The hospice volunteer coordinator, a paid role within a hospice setting, has an integral role in making the work of the hospice possible.

    Primary Duties

    The main responsibility of a hospice volunteer coordinator is recruiting, selecting, training and coordinating volunteers for the hospice. She provide volunteers with guidance, encouragement and coaching, and she develops and implements the hospice’s volunteer training program. The coordinator keeps the lines of communication open with volunteers, patients and family members to ensure that a high level of care and compassion is provided. As needed, the coordinator counsels volunteers on how to complete duties better and disciplines or releases volunteers who are not meeting expectations.

    Secondary Duties

    The administrative responsibilities of a hospice volunteer coordinator include updating information in medical charts, keeping records on volunteers, writing policies and procedures and sending out correspondence, newsletters and so forth to keep in contact with volunteers. The coordinator might additionally plan networking and community events in order to increase the profile of the hospice to facilitate recruitment.

    Work Environment

    A hospice volunteer coordinator usually works on site at a hospice facility but may be located off site as well. Much of the coordinator’s work is done during a regular 40-hour workweek; however, participation in community events means putting in hours on nights and weekends. Because the coordinator interacts regularly with patients, family members and volunteers who are dealing with the prospect of imminent death, the stress level and emotional toll can be high.

    Requirements

    A bachelor’s degree, preferably in social work, sociology, psychology or a related field is generally needed to gain employment in this field. In lieu of a degree, though, applicants might be considered if they have a minimum of 5 years experience in a related role and a high school diploma.

    A hospice volunteer coordinator needs to possess both strong administrative skills, like an eye for detail and a respect for protocol, and strong people skills, like empathy and diplomacy. The candidate should possess patience, helpfulness and pragmatism for providing training. Additionally, coordinators need to be open minded when it comes to different lifestyles, spiritual beliefs and cultures because the job entails interacting with patients and family members with a variety of backgrounds.

    Vagus Nerve Stimulation (VNS)

    Vagus nerve stimulation (VNS Therapy®) is designed to prevent seizures by sending regular, mild pulses of electrical energy to the brain via the vagus nerve. These pulses are supplied by a device something like a pacemaker.

    • The VNS device is sometimes referred to as a “pacemaker for the brain.” It is placed under the skin on the chest wall and a wire runs from it to the vagus nerve in the neck.
    • The vagus nerve is part of the autonomic nervous system, which controls functions of the body that are not under voluntary control, such as the heart rate. The vagus nerve passes through the neck as it travels between the chest and abdomen and the lower part of the brain.

    What is the surgery like?

    • The surgeon first makes an incision along the outer side of the chest on the left side, and the device is implanted under the skin. A second incision is made horizontally in the lower neck, along a crease of skin, and the wire from the stimulator is wound around the vagus nerve in the left side of the neck. The brain itself is not involved in the surgery.
    • The device (also called an implant) is a flat, round battery, about the size of a silver dollar—that is, about an inch and a half (4 cm) across—and 10 to 13 millimeters thick, depending on the model used. Newer models may be somewhat smaller.
    • The procedure usually lasts about 50 to 90 minutes with the patient under general anesthesia. Usually the person can go home later the same day. Sometimes you may need to stay overnight in the hospital for observation.

    How is VNS used?

    • The neurologist (or licensed professional) programs the strength and timing of the impulses according to each patient’s needs. The settings can be programmed and changed by placing a wand over the generator on the left side of the chest. The wand is connected to a handheld computer.
    • For all patients, the device is programmed to go on (give stimulation) for a certain period (for example, 7 seconds or 30 seconds) and then to go off (stop stimulation) for another period (for example, 14 seconds or 5 minutes). The device is set to give stimulation at regular intervals during the day, usually with 30 seconds of stimulation alternating with 5 minutes of no stimulation. The patient is usually not aware that it’s operating.
    • Holding a special magnet near the implanted device (generator) triggers the device to deliver another burst of stimulation, outside of the programmed intervals. For people with warnings (auras) before their seizures, activating the stimulator with the magnet when the warning occurs may help to stop the seizure.
    • Settings (also called stimulation parameters) set by the neurologist typically include a stimulation amplitude of 1.0 to 3.0 mA (milliamperes), a stimulation frequency of 20 – 30 Hz (hertz), and a pulse width of 130 – 500 microseconds. By adjusting these settings, the doctor not only may be able to control more of the patient’s seizures, but often can also relieve side effects. One study, for instance, found that changing the pulse width eliminated pain that some patients were experiencing.
    • The battery for the stimulator lasts approximately 5-10 years, depending on the settings used.

    Eligibility Selection Criteria

    Eligibility

    • Individuals (adults and pediatrics ) must be under the care of a National Hospice Palliative Care provider member, have a life expectancy of one year or less and not have any other means to fund the specific request.
    • Requests must support “experiences for the living” (ex. flying people in to visit, special events like fishing trips or special dinners, opportunities to spend time with family and friends in a memorable way, etc.)

    At this time, we are not able to accept applications from hospices that are not current members of NHPCO, or directly from hospice and palliative care clients or family members.

    Selection Criteria

    Funds may not be used for.

    • Bill payments or cash
    • Legal assistance
    • Automobiles or automobile repairs
    • Property and home improvements
    • Travel outside the United States
    • Funeral arrangements
    • Requests that are political, legal, or dangerous in nature
    • Medical Treatment / Supplies / Equipment / Transportation

    This list is not all-inclusive. NHF reserves the right to deny requests not on this list.

    Grant Allocation

    • NHF staff reviews all grant applications. Hospice and palliative care providers are notified within 48 hours of receipt of application of approval or non-approval.
    • Funds are mailed to provider in the amount specified on the application.
    • If for any reason the experience is not able to be granted, the funds must be returned to the National Hospice Foundation.
    • Total grant allocation not to exceed $2,500.

    Impact Report

    A one page after-action report of the activity or event is required. If possible, please include a photograph of the event or activity and include a family impact statement. If the report is not received from the provider in a timely manner (30-days following the “experience”) future grant submissions will be returned without review.

    *For the purposes of this application, hospice and palliative care providers are defined as any member of the hospice interdisciplinary team that routinely has contact with the client.

    The power of poetry in hospice care

    There is a tension between the advance of medical science, that pursues its ability to fix and treat, and an acceptance of the natural order of death and how that may be experienced. Atul Gawande, a surgeon and health researcher, has recently reignited this debate with his book Being Mortal (and also in presenting the 2014 Reith Lectures on BBC Radio). He writes: As people become aware of the finitude of their life, they do not ask for much. They do not seek more riches. They do not seek more power. They only ask to be permitted, insofar as possible, to keep shaping the story of their life in the world to make choices and sustain connections to others according to their own priorities. He puts forward strong arguments for medicine, and society, to review its approach to end of life care.

    Gawande s Reith Lectures in December were widely acclaimed. As 2015 begins, however, the press have been less encouraging about Dr Richard Smith s blog on the BMJ website: Dying of cancer is the best death . His romantic view was rather insensitive, but he was, however clumsily, arguing for a reformed dialogue and the good death in the face of over-enthusiastic treatment options. The best result from his clumsiness came on the morning of Sunday 4 January on BBC Radio 4 s Broadcasting House programme. Two Sue Ryder hospice workers (Carol Howard and Louisa Nicoll) were interviewed by Paddy O Connell, who referenced Dylan Thomas s poetry to question society s approach to death in the light of Richard Smith s blundering attempt. Our hospice workers reaffirmed that the best hospice care enables us to talk about death, not to rage against it but to seek to go gentle into that good night .

    I have been a volunteer poet at Bolton Hospice for over four years now. I don t take any lead from Dylan Thomas, or the many others, to write poems about death. I write about lives, about creativity, about the wonder and detail of simply being. When I read Atul Gawande s book it immediately affirmed my belief that my role was indeed helping patients to keep shaping the story of their life . It is a privilege for me to work with patients and to be at Bolton Hospice where there is a great commitment, not just to excellent medical care but also to the value of creative therapies: the hospice employs an art therapist, whose work includes supporting and encouraging poetry (a painting by a hospice patient can be seen below, right).

    I hope in this blog – my first ever! – to give some insight into my approach with a view to inspiring other poets to work in end of life care, not just in short, awareness-raising projects, but as long-term writing opportunities, integral to hospice care. Art and poetry in the hospice helps patients to be aware of their own creative significance. I am inspired to write poetry based upon each individual s story, or their own art or craft work. I think I can express my approach most clearly through a beachcomber analogy, to take the idea of a walk with a patient along a conversational or creative beach area. A work of art, a found poem, can be assembled from whatever we come across together. The value of the walk and the quality of the art are both equally important and the two are, in fact, inseparable. As I discover and create the poem I am simultaneously helping the patient to (re)discover something amazing about themselves and, potentially, about the legacy of art – which is able to signify a life as other , a presence in time and space not restricted to a medical history or a curriculum vitae.

    The writer Virginia Woolf talks about life being in the luminous halo of a journey. Keats advocates negative capability – to be at peace with uncertainties, mysteries and doubts. Coleridge asks for a willing suspension of disbelief to be able enjoy art. Ted Hughes speaks of the momentary as the vital signature of a human being. James Joyce finds wonder in the everyday and ordinary. My particular inspiration from talking to hospice patients has been to capture the sudden lightning flashes of significance that celebrate life. The numinous connection is to dwell, to practice being with patients among the mysterious flickerings when meaning and understanding prove inadequate.

    End of life care services have formed an approach to good death. I simply argue for poetry, and other creative activity, to be fully included. I have produced a 1,500 word Guidance Framework for Poets in End of Life Care and the following is an extract.

    A poem may take a single phrase or idea or a topic discussed. It may be a longer linear narrative of a life, or a particular experience in work or war or a childhood tale.

    A poem may form a series of snapshots or an abstract reflection of a story. It may contain no direct personal reference and yet capture an individual s lifelong passion for place or hobby. You may write as to accompany a patient s own art, a picture or piece of craft work. A simple guide is to allow yourself, through conversation, to be inspired by the patient. Explain this to the patient the first time you meet them and why you have a pad and pen and will scribble notes as they speak. Every day in the hospice I learn many things new a word, a story, a piece of history, a way of seeing something and I thank the patient every time. Such things are inspiration, occurrences which themselves declare something needs to be written about this . The poem is not an evaluation or a narrative or biography. It is not a eulogy. It is art.

    Poetry can be in the smallest thoughts, the remembered flickerings represented by the three-line Haiku-type poems. I ll finish this blog with a number of longer poems for you to dwell upon not explained or elaborated upon. Like patients, they just are; they may have been read at funerals, framed or placed in a memory box, but poems are also being with .

    Some places you never really leave.

    This silence remains. I am never sure if my
    ears or my eyes hear the easy soaring of

    The first hospice and home-care programs will be based in Oregon’s Clackamas County. The programs will voluntarily seek Community Health Accreditation Program (CHAP) status—a process which includes access to and adherence with a set of quality and performance standards geared to drive performance above the minimum standard.

    Bristol Hospice programs provide hospice services to meet the physical, psychosocial, and spiritual needs of patients and their families and caregivers. An interdisciplinary group of professionals and volunteers develops an individualized plan of care which includes, as appropriate, the following services: nursing, physician, home health aide, counseling, spiritual support, therapy, dietary, counseling, volunteers, durable medical equipment, supplies and bereavement services.

    Bristol Homecare programs are focused on the care continuum model.
