Options for Extra Care: Nursing Homes and Hospice Facilities

The services offered by nursing homes, assisted living facilities, or hospice are all very different. Find out more about each one and the costs associated with them.

Most people hope to live out their last years at home, but health circumstances may necessitate moves to nursing homes or assisted living facilities. When a loved one s illness is terminal, hospice facilities or hospice care at home may be the most caring solution.

According to admissions director Janice Harris of Pickersgill Retirement Community in Towson, Md. each facility serves a different purpose and entails different costs.

Assisted Living Facilities

If your loved one is not incapacitated to the point that you need to consider the skilled care offered by a nursing home, assisted living can be a viable solution. Many are in pleasant, planned communities and can be a terrific option for people who are unable to maintain their own homes any longer or take care of daily needs such as cooking or driving; they may be experiencing decreased sensory perceptions and delayed reaction times or confusion, but don t need round-the-clock nursing care.

The down side is the high cost. Assisted living facilities can run from $1,000 to $5,000 per month or more, says Bill Guerra, RN, BSN, with the Seven Hills Surgery Center in Henderson, Nev. Some assisted living facilities also charge extra for services like administering medications, providing trips to the physician, bathing, and so forth.

Medicaid will pay for assisted living for certain patients, depending on their income and personal resources.

Nursing homes provide care when patients can t take attend to their basic needs, are incapacitated by illness, or have dementia, says Guerra. Some, like memory centers, cater to patients with dementia who are prone to wandering leaving home and getting lost or hurting themselves unintentionally, though they can usually walk and talk.

Nursing homes can be private-pay facilities or state-run homes covered through the Medicaid system for people who can t afford the costs on their own. Qualifying for Medicaid is a complicated procedure, and it might be a good idea to consult with an elder law attorney. If you have time to plan ahead, you can take out a long-term care insurance policy to cover some or all costs, should a nursing home be necessary later in life. Medicare pays only for very limited stays under certain circumstances and not continual, long-term residence.

Hospice facilities offer end-of-life care care that is often administered in the patient s home, though there are physical hospice facilities, too. Some hospice programs are independent, others might be within hospital settings. Tina Daniel, RN, a nurse manager with Gilchrist Hospice Care in Towson, Md. says that anyone can request an evaluation for hospice care: the patient, the doctor, friends, or family members. At some facilities, a representative from the hospice or a nurse can determine if hospice is appropriate by visiting your loved one in their home, assisted living facility, or nursing home, says Daniel.

It does not matter whether you have personal insurance, Medicare, or Medicaid. If your loved one is terminally ill, you do not ever have to pay for hospice care services as there are non-profit hospice facilities that offer services for free, says Guerra.

Gilchrist Hospice Care, where Daniel works, is an example of a non-profit facility and admits patients for hospice services regardless of their ability to pay. Medicare covers 100 percent of services relating to terminal illness at home and at a long-term-care facility, says Daniel. However, Medicare does not pay for room and board. The services covered are clinical support, equipment, and medication relating to the terminal illness. Insurance may assist with a portion of the cost of room and board. Daniel says, We work with all insurances and discuss the benefits and co-pay with you.

Your loved one s medical team, the support staff at the hospital where treatment was given, and non-profit organizations are ways to get suggestions for local hospice facilities.

Your first step is deciding which facility is right for your loved one s needs at this stage of his or her life. Then you ll need to research costs, figure out what resources are available to pay for it, and whether your loved one qualifies for financial assistance or has any insurance. Investigate what your loved one may be entitled to from Medicare and Medicaid. Depending on the person’s age and health status, there may be time to get long-term insurance for the future.

Once you know your budget, you can begin interviewing facilities about the level of care they offered and find the best one to meet your loved one s needs.

Embracing Life’s Path Through Comfort, Dignity, and Compassion

In order for a recipient to qualify for and be admitted to hospice services, the patient must first have been diagnosed with a terminal illness. Terminal illness is a medical prognosis of limited expected survival of approximately six (6) months or less if the disease runs its natural course.

Common Terminal Illnesses

  • Cancer any type
  • End Stage Liver Disease
  • End Stage Renal Disease
  • End Stage Heart Disease
  • End Stage Neurological Diseases
  • End Stage Pulmonary Disease
  • End Stage AIDS/HIV
  • End Stage Alzheimer’s Disease
  • End Stage Dementia

There are various end stage diseases that are considered terminal in nature, though they may not be listed above. For any questions regarding a diagnosis that may qualify for hospice services, please contact our office at 318-222-5711 or contact your physician for additional information regarding the specific disease or illness.

The hospice benefit will continue to cover palliative and comfort care for an individual with a terminal illness for the following periods

  • Initial 90-day period, and
  • Subsequent 90-day period, and
  • Indefinite subsequent 60 day periods

We view each case individually and will re-evaluate each patient at 60-90 day intervals to determine continued eligibility for hospice.

Medicare, Medicaid, managed care programs and commercial insurance programs provide coverage for hospice care.

Medicare

Most of our patients are eligible for Medicare coverage and our hospice agency accepts the Medicare hospice benefit as payment for 100% of a patient’s hospice care. We require no co-payments for our services for Medicare recipients and all services rendered are at no expense to the patient or their family.

Medicare Hospice Benefit includes

  • Physician services as it relates to the terminal illness
  • Regular home visits by a hospice nurse
  • Chaplain services for the patient and family
  • Hospice aide and homemaker services, such as dressing and bathing
  • Social work services and counseling services
  • Bereavement services
  • Medical equipment DME such as hospital bed, wheelchair, or oxygen
  • Medical supplies such as bandages or catheters
  • Medications to control symptoms and relieve pain or discomfort
  • Physical, speech and occupational therapy
  • Dietary counseling, as necessary

Online/Part-time Format

Flexibility for Working Professionals

Online/part-time students earn the degree in 2-3 years by taking, on average, 1-3 courses per term. Pre-recorded lectures allow students to learn on their own schedule each week. There are hard deadlines for turning in graded material, but students choose when to work on them.

They have the option of matriculating into the program during 3 different periods of the year, beginning with a 1- or 2-week on-site orientation in June, November or January.

On-Campus Requirement

Up to 80% of the curriculum may be completed online. For the remaining 20%, students have the opportunity to attend on-campus courses. The required “face-to-face” time adds up to a minimum of 4 weeks over the course of 2-3 years.

There is no limit on the number of on-campus courses that an online/part-time student can take. On average, these students complete about 25% of courses on campus.

The on-campus requirement can also be completed by enrolling in the Summer, Fall or Winter Institutes. The intensive 1-3 week courses allow students to earn academic credit in a short period of time. Each institute explores a specific area of public health, allowing students to gain specialized training based on their interests. Designing Your Degree

The core coursework comprises roughly half of the curriculum and is the same for both the full-time and online/part-time format.

For electives, all students have the freedom to customize their elective coursework based on personal interests and professional goals. With over 140+ online courses to choose from, there are endless possibilities for how to plan your education. A faculty advisor helps you determine elective courses that match your desired knowledge base and skill sets.

The ability to personalize half the curriculum is a defining feature of the Bloomberg School’s MPH degree program.

Health Economics is an applied field of study that allows for the systematic and rigorous examination of the problems faced in promoting health for all. By applying economic theories of consumer, producer and social choice, health economics aims to understand the behavior of individuals, health care providers, public and private organizations, and governments in decision-making.

From economic modeling to policy analysis, I am confident that the skills I have gained throughout the year will stay with me throughout my life. Health economics is used to promote health through the study of health care providers, hospitals and clinics, managed care and public health promotion activities. Health economists apply the theories of production, efficiency, disparities, competition, and regulation to better inform the public and private sector on the most efficient, or cost-effective, and equitable course of action. Such research can include the economic evaluation of new technologies, as well as the study of appropriate prices, anti-trust policy, optimal public and private investment, and strategic behavior.

Health economics can also be used to evaluate how certain social problems, such as market failure and inequitable allocation of resources, can impact on the health of a community or population. Health economics can then be used to directly inform government on the best course of action with regards to regulation, national health packages, defining health insurance packages and other national health programs.

Faculty of the Johns Hopkins Bloomberg School are currently conducting research on a wide variety of topics including the impact of health care, health insurance and preventative services on health lifestyles as well as providing research and advice to governments around the globe to enable a more effective and equitable allocation of resources. Surgical Technology

Coming to Scranton for Fall 2017!

Lackawanna College’s Surgical Technology Program is designed to be completed over a two-year period with day, evening, and Saturday courses as well as on- and off-campus clinical hours. Each program also includes summer session courses and clinical lab hours.

Acceptance into Lackawanna College’s Surgical Technology Program is based on a selective admissions process with a maximum of 15 students selected annually.

Degree information

Through a balance of classroom instruction and clinical experience, the student will acquire applied skills and knowledge in the field of Surgical Technology.

Surgical technologists work in the operating room under the direct supervision of a surgeon and a registered nurse. Surgical technologists help facilitate the draping, and prepping of the surgical patient. They gown, and glove other team members. They get all the instruments, supplies, equipment necessary for specific surgical procedures. As a member of the sterile team, surgical technologists will hand all necessary instruments and supplies to the surgeon and other team members.

A surgical technologist has a broad knowledge of anatomy and physiology, microbiology, sterile techniques, and patient safety concepts as well as ethical and legal concepts. A surgical technologist must have an understanding of the hospital organization, policies, and procedures.

Job opportunities and salaries in the healthcare specialty of surgical technology have been rising steadily over the last few years, and the U.S. Department of Labor predicts employment of surgical technologists is projected to grow 30% from 2012 to 2022, much faster than the average for all occupations. Advances in medical technology have made surgery safer, and more operations are being done to treat a variety of illnesses and injuries. The median annual wage for surgical technologists was $41,790 in May 2012.

Most surgical technologists are employed by hospitals, day-surgery centers with the remainder working in the offices of physicians. After two years of experience, a certified surgical technologist may apply to become a traveling surgical technologist.

Lackawanna College’s Surgical Technology program combines classroom instruction with practical clinical externships at local hospitals. Both career-specific and academic core courses are included. Students successfully completing the program are eligible to sit for the National Certifying Exam (CST).

Surgical Technology Program classes are given days, nights and on weekends (based on the availability of the instructors). Clinical externships, however, must take place during the daytime.

  • To instruct students in the responsibilities which the performance of this role entails
  • To assist students in developing the knowledge of the organizational structure of the hospitals, its departments and the operating room
  • To develop a basic understanding of biological science as it relates to safe operating room procedure
  • To facilitate the hands-on, supervised experience in the operating room performing the duties of a surgical technologist.

Admissions criteria and requirements

Prospective students must meet the college’s minimum requirements. Applicants must submit proof of high school or GED completion with official high school transcript or GED equivalent. Lackawanna College will determine the applicant’s current academic ability based on a combination of following:

  • SAT or ACT test scores, if applicable
    • Recommended SAT scores: 470 Verbal and 470 Math.
    • Recommended ACT scores: 20 or higher in each relevant section.
    • Most scores are valid up to 3 years from test date.
  • College Transfer Credits
    • If transferring credits from another college/university, official college transcripts are required.
  • Placement Test
    • Applicants without valid SAT/ACT scores and who have not previously earned college credits will be required to take the ACCUPLACER exam for determination of current academic ability. Applicants must achieve College Ready scores in Reading, Writing, and Math to be considered qualified for the Surgical Technology program.

Due to the very specific nature of these degree programs, applicants with transfer credits may only transfer credits related to program core courses including the following:

Interviews will be scheduled for qualified Surgical Technology program applicants who submit a fully completed application. A mandatory informational meeting will be required prior to start of the selected program.Qualified students will be notified of the date and time upon selection into program. The meeting will review the Surgical Technology department’s policies and expectations. The meeting will also allow for question and answer session and tour of the Lackawanna College facilities.

Curriculum

Changes in the curriculum for this program have been approved by ARC/STSA and will be in effect for students beginning their studies in the Fall 2017 cohort.

There are thousands of colleges offering online courses in addition to the ones listed in the categories below. The schools included, however, have been chosen because they are both reasonably priced and regionally-accredited unless otherwise noted. Another emerging trend in higher education is the offering of creative tuition plans. Fixed rate tuition is a unique option being offered by a few colleges. Western Governors University in Utah charges a flat $3,000 per six-month term and the student can complete as many courses as they want in that period. Courses begin the first of each month and a student can enroll in as many sessions as they like.

Many colleges are now offering students the ability to begin taking online courses throughout the year instead of following traditional fall and spring semester formats. Some schools begin classes every month. Courses generally follow eight or sixteen-week schedules, but students can begin any month that fits them best. The following colleges in this category offer tuition beginning at less than $175 per credit.

Self-Paced, Independent Study

The number of courses being offered in this category by today’s colleges and universities is growing very quickly. Students are demanding flexibility and schools are responding. Students can work through the course material at their own pace, which can prove to be very helpful both for those wanting to move quickly and for those needing to go more slowly.

The course material directs students through each subject and often requires written assignments and tests. Instructors are available for questions and grading, but the course is directed by the material and not the teacher. Students should assume these courses are just as challenging as traditional courses, but can offer greater flexibility to students with demanding schedules.

Many colleges such as Louisiana State University, Portland State University, and the University of Oklahoma have no admission requirements, and students simply register and pay for courses. Colleges usually allow students between six and twelve months to finish, but may give extensions of time for an additional fee.

Most colleges offering self-paced online courses also offer other online courses in traditional semester-based formats. However, one benefit of self-paced courses is the ability for students to begin any day of the year. These courses might also be referred to as “self-paced,” “quick entry,” “correspondence” and “open entry-open exit”. The colleges below offer flexible, independent, self-paced courses beginning at less than $100 per credit.

Access to Syllabus Before Enrolling

Many students may benefit from having the opportunity to review a course syllabus before enrolling. This allows students to research the course content, work requirements, grading system, and time commitment needed to earn a satisfactory grade. The following colleges place their syllabi online for students to review prior to enrolling. Colleges in this category can be found with tuition starting under $100 per credit hour.

Flexibility with Course Prerequisites

Colleges normally require students to take basic courses before proceeding to more advanced courses. As a result, students must follow college policies to prove that course prerequisites have been met before they will be allowed to enroll in an advanced class. College prerequisite policies can vary a great deal, but Louisiana State University’s policy is especially easy to follow for their online students.

LSU’s website is very informative and it clearly explains course content, lesson assignments, grading formats, and more. They offer more than 175 online, self-paced courses for less than $100 per credit hour, including many upper level courses.

Many colleges require students who take online courses to take their tests under the supervision of an approved proctor. Local college testing centers and libraries are common locations for students to arrange for a proctor. Some colleges, however, allow their students to take some of their exams without a proctor. Each course has different testing guidelines. Some instructors feel comfortable assessing a student’s knowledge through writing assignments, projects or group discussions, and allow their exams to be taken online. The following colleges have offered some courses with non- proctored tests. The colleges from this category begin tuition at less than $100 per credit hour.

Science Courses with Labs

One of the challenges of earning the majority of a degree online is the common requirement for a science course with a lab. Many colleges offer online science courses, but only a handful will include the lab. The following colleges include an online lab with some of their science courses. Colleges in this category offer courses for as little as $100 per credit hour.

Some colleges are including the cost of textbooks and learning materials in their tuition price. They sell themselves on the idea that their costs are all-inclusive, with no hidden fees. Two affordable online colleges offer students this option. Colleges in this category currently offer all-inclusive tuition beginning around $250 per credit hour.

Non-Regionally Accredited Schools

Some students may want to consider taking courses from Penn Foster College, even though this school does not have regional accreditation. Students who plan on graduating from a college that accepts ACE credit may be able to take advantage of Penn Foster’s more than 180 online courses that have been recommended by ACE. Colleges that do not normally accept credits, from non-regionally-accredited institutions, may still allow these courses to be transferred because they have been approved by ACE.

Penn Foster offers manageable, self-paced courses that students are allowed to take twelve months to complete. Students can begin these courses anytime and textbooks and study materials are included in the tuition. They also offer multiple options for acceptable exam proctors. These courses will not work for every student, but may be considered by students choosing colleges with generous transfer policies. Penn Foster currently offers courses for less than $100 per credit hour.

Online Christian Ministry Degrees

Some online private Christian colleges offer affordable tuition for students wanting to earn a ministry degree. Some of these colleges offer undergraduate degrees while others focus on the post-graduate level. Colleges in this category currently begin tuition at less than $200 per credit hour.

Low-Priced Online MBA Courses and Degrees

College tuition for a masters degree is usually higher than tuition for undergraduates. This often places these degrees out of the financial reach of many students. Students looking for affordable graduate-level courses may want to research the following colleges, with tuition starting at less than $250 per credit hour.
