SINGAPORE: The Health Ministry does not require Integrated Shield Plans (IPs) to cover hospice care as an expansion of benefits will result in higher premiums. Minister of State for Health, Chee Hong Tat said this in Parliament on Monday (May 9), adding that IPs, together with MediShield Life, are designed to cover large hospital bills.

In response to a question from MP for Jurong GRC Dr Tan Wu Meng, Mr Chee said that hospice care is covered under some riders such as Great Eastern’s Total Health Riders and AXA’s Home Care Riders. He added that hospice care is affordable for most Singaporeans, based on his ministry’s data, when combined with government subsidies, Medisave and charity dollars. He highlighted that eight in 10 inpatient hospice bills did not have any out-of-pocket cash payment, while nine in 10 home palliative care patients did not have to fork out cash either.

Dr Tan asked about patients who fall under the ‘sandwiched class’- who may not qualify for as many government subsidies as patients from low income households, but who still want to take advantage of benefits provided by their IPs, which they purchased while still healthy. He asked whether the MOS would agree that the present system “makes it somewhat unfortunate that these patients can access their Integrated Shield Plan benefits in a restructured hospital, but not should they choose to seek end-of-life care at an inpatient hospice?”

In response, Mr Chee said government subsidies for hospice care are extended up to the 67 th percentile. Apart from subsidies, he said charity dollars are also an important source of support for patients. He said the government works closely with voluntary welfare organisations and charities in this area. Mr Chee reiterated that there are riders available but importantly, MediShield Life is a new scheme that started in November last year.

He said, ”It is important for us to focus the scheme on the original intent which it was set out to do, which is to cover large hospital bills. This is something we can review on an ongoing basis, to ensure MediShield Life is able to meet the needs of Singaporeans”.

    More and more people, especially Medicare patients, are having trouble finding a doctor.

    The doctor shortage is worse than most people think, says Steven Berk, M.D. dean of the School of Medicine at Texas Tech University. The population is getting older, so there’s a greater need for primary care physicians. At the same time, physicians are getting older, too, and they’re retiring earlier, Berk says. And graying doctors — nearly half the nation’s 830,000 physicians are over age 50 — are seeing fewer patients than they did four years ago, a 2012 Physicians Foundation survey reported.

    Soon, this fraying primary care network will face another huge challenge: Under the Affordable Care Act. millions of formerly uninsured men and women will have access to health care.

    We need to absorb these 30 million people, and that’s going to be a strain, says Russell Phillips, M.D. director of Harvard Medical School’s new Center for Primary Care.

    A fundamental change

    The approach most favored by experts at Harvard and elsewhere is to reshape traditional primary care: from a stream of patients waiting to see one harried doctor to a more efficient team practice in which patients with routine problems are seen by nurse-practitioners and physician assistants — trained specialists with master’s degrees. The team frees the doctor to spend more time with patients with more serious complaints.This change could be as fundamental as the one that took place when most family doctors stopped making Marcus Welby-like house calls.

    The Affordable Care Act encourages such a sea change, with provisions that aim to shore up and expand the country’s ailing primary care system while still reducing costs.

    The ACA authorizes money to increase the primary care workforce by training more doctors, nurses, nurse-practitioners and physician assistants. It includes more graduate medical education training positions, with priorities for primary care and general surgery, and more money for scholarships and loans for all health professionals. The law expands the number of patients seen at community health centers in areas with too few doctors and increases the number of staffers who work in the centers. It also expands nurse-managed clinics at nursing schools where nurses in training see patients who live in the area.

    Another key provision: a 10 percent bonus, through 2015, to primary care doctors who offer services to Medicare patients.

    But in these times of shrinking federal budgets, it’s unclear how much ACA primary care money will be available as Congress juggles competing priorities. Congress, for example, already has chopped about $6.25 billion from the ACA’s new $15 billion Prevention and Public Health Fund, which pays for programs to reduce obesity, stop smoking and otherwise promote good health. In addition, federal support for training all types of physicians, including primary care doctors, is targeted for cuts by President Obama and Congress, Republicans and Democrats, says Christiane Mitchell, director of federal affairs for the Association of American Medical Colleges, who calls the proposed cuts catastrophic.

    A recent study by the Institute of Medicine and the National Research Council reports that, when compared with citizens of 16 other high-income democracies, including those of Western Europe, Japan and Canada, Americans not only die younger but have poorer overall health. The researchers traced U.S. health disadvantages to a number of causes, including the fact that Americans have more limited access to primary care.

    Where have all the doctors gone?

    Today, the United States is short about 16,000 primary care doctors — the very doctors (family practitioners, internists and pediatricians) who offer the treatments and preventive screenings that save lives and head off expensive emergency room visits and hospitalizations.

    Why the shortage? It starts with huge medical school debts and ends with a doctor who is often overworked and underpaid. While students may enter medical school wanting to practice primary care medicine, they graduate saddled with heavy debt — $250,000 is not unusual — which prompts them to switch to a more lucrative specialty. The starting salary for a primary care physician is $150,000 to $170,000; a radiologist or gastroenterologist can make two to three times that.

    Only one in five graduating internal medicine residents plans to go into primary care medicine, the Journal of the American Medical Association reports.

    Looking for a Doctor?

    The best place to start is your state medical association. Many provide doctor directories that often include new doctors just setting up practice.

    If you’re a Medicare beneficiary, go to medicare.gov . Under Forms, Help and Resources on the home page, select Find doctors, hospitals and facilities. Type in your ZIP code and you’ll get a drop-down menu of medical specialties, including primary care. The tool provides contact information for doctors according to the criteria you enter — geographic location, specialty, etc. Call the office to check whether the doctor is still taking new patients.

    Clay Buchanan, a former lawyer, is one of those graduating in May. At 48, the Little Rock, Ark. resident is older than most medical students. He shadowed a family doctor as part of his training — and was hooked. By noon the first day, I loved it, Buchanan says.

    New York University and several other colleges are planning to experiment with a three-year program.

    Health clinics offer primary care

    Community health centers offer another form of primary care. Nationwide, the centers serve 20 million patients a year using a team approach, and are open to all on a sliding fee scale. Under the ACA, they are expected to double their capacity to 40 million patients by 2015. To entice doctors to work at these centers, the National Health Service Corps repays up to $120,000 in loans for each doctor in return for four years’ service.

    Technology, including telemedicine — which could reduce patient trips to the doctor’s office — also should help expand health care.

    Another way to increase health services is to give physician extenders — nurses and other medical professionals — more autonomy. Patricia Grady, director of the National Institute of Nursing Research at the National Institutes of Health, supports increasing the role of trained nurses, allowing them to set up independent practices where they could do physical exams and advise patients on exercise and diet.

    But that position has met with some resistance. The American Academy of Family Physicians and the American Medical Association (AMA) favor training more physicians and nurses but want to keep nurses in teams led by doctors.

    Nurses and foreign doctors

    Permitting more foreign doctors to practice here also would increase the pool of primary care providers. Last fall, President Obama signed a three-year extension of a visa waiver program that allows states to place 30 foreign medical school graduates a year in medically underserved areas for three years. Most remain in the communities after they satisfy their commitment. Still, the shortage is so acute, even the AMA is lobbying Congress to increase to 50 the number of foreign-educated doctors each state is allowed.

    In the meantime, more Americans are anxiously searching for a primary care doctor — or waiting months to see one. Marcia Andrews, whose Washington doctor refused to take Medicare, finally found a new doctor who does. The search took her 18 months.

    When Can You Claim a Tax Deduction for Health Insurance?

    Each year, June 28 marks National Insurance Awareness Day, serving as a reminder that it’s a good time to review your insurance coverage and ensure that it still fits your needs.

    This is particularly true for health insurance and making sure you have the right amount of coverage at an affordable cost.

    Health insurance can be expensive, so being able to claim a tax deduction for some of your insurance costs can help you save come tax time.

    Since there are specific rules and qualifications you must follow, here’s an overview of when you can and cannot claim a tax deduction on your health insurance.

    When health insurance is not tax-deductible

    If you didn’t pay for health insurance, you can’t take a tax deduction for it. If your employer pays your health insurance premiums, you can’t deduct those costs. However, if an employer only pays for part of your premiums, you still may be able to claim a tax deduction for the portion you paid.

    If you received a subsidy or premium tax credit to purchase an insurance plan in the Health Insurance Marketplace through the Affordable Care Act, any advanced-payment subsidy that lowered the cost of your health insurance premiums cannot be claimed as a tax deduction. However, the money you paid out of your own pocket for your premiums might be tax deductible.

    You can’t take a deduction for health insurance you paid for with pre-tax money. If you have insurance through your employer, the premiums you pay are usually taken out of your paycheck before your income taxes are calculated. Since these premiums are paid with pre-tax dollars, they’re already income-tax-free, meaning you can’t claim them as a tax deduction.

    Also note, you can not deduct health insurance unless you itemize your tax deductions or you are self-employed. You don t need to know if you qualify for itemized tax deductions, TurboTax will figure it out for you.

    When health insurance is tax-deductible

    If you’re self-employed, your health insurance premiums may be tax deductible. If you’re self-employed and not eligible for an employer-sponsored health plan through a spouse’s job, you may be eligible to write-off your health insurance premiums on your taxes. However, you can’t write off more in health insurance premiums than you earned.

    Health insurance premiums paid with your own after-tax dollars are tax deductible. For example, if you purchased health insurance on your own through a health insurance exchange or directly from an insurance company, the money you paid toward your monthly premiums can be taken as a tax deduction.

    Some Medicare plans are tax deductible. This includes Medicare Part B and Part D prescription coverage.

    There are limits to the amount of your health insurance you can deduct. If you are able to write-off your health insurance, there are limits to how much of your premiums you can write off.

    If you’re able to claim your health insurance as a medical expense deduction. you can only deduct medical expenses that exceed 10% of your adjusted gross income (7.5% if you’re 65 or older). If you’re self-employed and claimed the self-employed health insurance deduction, you don’t have to exceed the 10% threshold because you’re writing the premiums off as an adjustment to your self-employment income rather than as a tax deduction deduction.

    According to a report by the Harvard Joint Center for Housing, by 2035 more than one in five people in the U.S. will be aged 65 and older, and one in three households will be headed by someone in that age group. The Projections and Implications for Housing a Growing Population: Older Adults 2015-2035 report notes the growth will increase the demand for affordable, accessible housing that is well connected to services beyond what the current supply can meet.

    Census data shows that income drops significantly after the age of 75, falling from an average of $54K to $36K. With data also showing that these citizens spend more than 1/3 of their income on housing, the average single family residence is not affordable.

    Right now, more than 19 million older adults live in unaffordable or inadequate housing, and that problem will only grow worse in the next two decades as our population ages, said Lisa Marsh Ryerson, president of AARP Foundation, which provided funding for the housing report.

    The CEO of Real Property Management, Lukas Krause, discussed with FOXBusiness.com what this means for seniors and the real estate industry.

    Boomer: What does the government need to do to rethink the types of housing needed for baby boomers?

    Krause: Most housing regulations are controlled at the local level and range from zoning restrictions to building codes, which can become impediments to building new housing for seniors. San Francisco and Portland, for example, restrict and regulate new construction, which has led to a shortage of housing and run-away inflation in housing costs. City plans, zoning regulations and building codes should be reviewed to address the growing elderly population, in addition to transportation, healthcare, and financial assistance/incentives.

    The number of older households with disabilities related to mobility, self-care, or household activity is also projected to increase by 2035. Many seniors want to ‘age in place’ and stay in their homes. To do so, partnerships between health care and housing are needed to support the trend of providing long-term care in the home.

    Governments may need to consider offering financial assistance to low-income older adult renters, and incentive programs for safe and senior-friendly renovations, energy efficient home upgrades, and property tax relief. Affordability will be key, since more than half of all boomers have less than $100,000 saved for retirement. Public awareness and education is needed to encourage older adults to consider their future housing needs earlier in life.

    Boomer: What does this mean for the real estate industry as a whole?

    Krause: People currently over the age of 55 have saved only $150,000 for retirement, per Fidelity and Vanguard estimates. This savings amount will generate only $500 per month in income, if the recommended 4% withdrawal standard is followed. Social Security pays an average of $1,294 in benefits to retirees, so average monthly income will be $1,794 or $21,528 per year. If 34% – 38% is spent on housing, the average retiree will have a housing budget of $610 – $682 per month – half of today’s average apartment rental cost of $1,100 per month. This means cohabitation and new forms of housing will be needed in the future. It also means that retirees who have not already purchased a home, will be unlikely to afford one.

    Boomer: Should boomers be looking at renting instead of buying houses in retirement?

    Krause: Home prices are increasing more rapidly than the cost of rent, though both renters and homeowners pay relatively the same amount over a long span of time.

    If a boomer has the financial resources, buying with a fixed mortgage rate would lock their monthly mortgage payment until the house is paid off. However, the costs of maintenance, insurance, lawn care, utilities, taxes, etc. are likely to increase at the rate of inflation, so buying is not a hedge against inflation. Buying opens the door to a reverse mortgage in the future, or down-sizing to free up cash.

    Renting puts the cost and responsibility for maintenance, taxes, floor, hurricane or earthquake insurance onto the landlord. It gives the tenant greater flexibility to move for health or financial reasons. Renting in a community specifically designed for boomers can provide the amenities, services and technology needed by boomers, though the cost may be prohibitive.

    Boomer: With the impending growth of boomer retirees, how, in your opinion, should builders be incorporating into plans?

    Krause: Accessibility is crucial. Design elements should include: single-floor living without stairs, doorways and hallways at least 36” wide, non-slip flooring with smooth thresholds between rooms, handrails, adequate lighting, lever-style door/faucet handles, higher toilets with extra space around for easier assistance, showers equipped with a bench, and multi-height kitchen countertops. For the exterior of the home, zero-step entrances, level driveways, weather-protected walkway and entrance, and low-maintenance landscaping should be considered.

    Hospice care affordable for most Singaporeans: Chee Hong Tat

    SINGAPORE: The Health Ministry does not require Integrated Shield Plans (IPs) to cover hospice care as an expansion of benefits will result in higher premiums. Minister of State for Health, Chee Hong Tat said this in Parliament on Monday (May 9), adding that IPs, together with MediShield Life, are designed to cover large hospital bills.

    In response to a question from MP for Jurong GRC Dr Tan Wu Meng, Mr Chee said that hospice care is covered under some riders such as Great Eastern’s Total Health Riders and AXA’s Home Care Riders. He added that hospice care is affordable for most Singaporeans, based on his ministry’s data, when combined with government subsidies, Medisave and charity dollars. He highlighted that eight in 10 inpatient hospice bills did not have any out-of-pocket cash payment, while nine in 10 home palliative care patients did not have to fork out cash either.

    Dr Tan asked about patients who fall under the ‘sandwiched class’- who may not qualify for as many government subsidies as patients from low income households, but who still want to take advantage of benefits provided by their IPs, which they purchased while still healthy. He asked whether the MOS would agree that the present system “makes it somewhat unfortunate that these patients can access their Integrated Shield Plan benefits in a restructured hospital, but not should they choose to seek end-of-life care at an inpatient hospice?”

    In response, Mr Chee said government subsidies for hospice care are extended up to the 67 th percentile. Apart from subsidies, he said charity dollars are also an important source of support for patients. He said the government works closely with voluntary welfare organisations and charities in this area. Mr Chee reiterated that there are riders available but importantly, MediShield Life is a new scheme that started in November last year.

    He said, ”It is important for us to focus the scheme on the original intent which it was set out to do, which is to cover large hospital bills. This is something we can review on an ongoing basis, to ensure MediShield Life is able to meet the needs of Singaporeans”.
