We have been providing compassionate, professional care to terminally ill patients and their families since 1982. We are a not-for-profit corporation, state licensed and Medicare/Medicaid certified. Because of our excellent reputation for care and service, we have served over 25,000 patients since 1982. We serve patients in greater Will, Grundy, and Kendall counties along with parts of Cook, DuPage and Kankakee counties.

The mission of Joliet Area Community Hospice is to provide comprehensive, holistic, community based support services and care for terminally ill persons, their caregivers, and loved ones without regard to economic status; to enable the dying person to live peacefully, in comfort, and with dignity to the last moment of life.

Consumer Assessment of Healthcare Providers and Systems (CAHPS)
Joliet Area Community Hospice has always been proud to offer excellent patient centered care. Below are just a few of the highlights from the Family Satisfaction Surveys from the fourth quarter of 2014:

  • 98.5% of respondents reported that the Hospice Team clearly explained the plan of care to the patient s family
  • 96.9% responded that the patient was treated with respect
  • 93.5% of respondents indicated that the hospice care received by their loved ones was consistent with end-of-life wishes
  • 96% of patients responded that hospice care helped with patient s feelings of anxiety or sadness
  • 96.9% indicated that the patient/family received the right amount of religious or spiritual contact
  • 89.1% indicated that the patient s personal needs were taken care of, compared to the national average of 78.6%
  • 83.3% of respondents reported that the quality of care improved after hospice was involved, compared to the national average of 63.4%

Hospice Home opens, May 2004 becoming the first freestanding hospice home in Illinois.

What makes Joliet Area Community Hospice Different?
Joliet Area Community Hospice is the only non-profit community-based, Medicare-certified hospice serving Will and Grundy Counties. Because of the community support, Joliet Area Community Hospice was able to build the first Hospice Home in the State of Illinois, bringing cutting edge hospice care to our area. The staff at Joliet Area Community Hospice go above and beyond the call of duty to meet the needs of both patient and family as they face the end of life looking at not only the physical and psychological needs, but also the environmental, social, spiritual, and emotional needs.

Joliet Area Community Hospice has a dedicated pediatric team that has received referrals from area hospitals as well as many specialty hospitals such as Children s Memorial Hospital, Ronald McDonald Children s Hospital of Loyola University Medical Center, University of Chicago Comer Children s Hospital and St. Jude Children s Research Hospital. In addition to the pediatric hospice team, there is a Pediatric Palliative Care Program for children that may not qualify for hospice services, but could still benefit from comfort care and family support services.

As a non-profit agency, Joliet Area Community Hospice receives additional funding from the United Way of Will County, United Way of Grundy County, grants, memorials, general donations and bequests.

Hospice Overview

The Dorothy Ley Hospice

Fostering hope and dignity through exemplary care, advocacy, education and research for individuals living with the challenges of a life-limiting illness or loss.

The Dorothy Ley Hospice is a volunteer-based community service organization offering compassionate care to people living with the challenges of a life-limiting illness or loss. Our goal is to provide comfort and help them manage so they can live every day to the fullest with dignity, meaning and hope. All of our services are offered at no cost to the individual thanks to the generosity of our donors, government funding and the support of our community partners.

Our programs and services meet a variety of support needs for individuals, families and care providers. Any individual facing the challenges of a life-limiting illness, regardless of age or medical condition, can access hospice support. We also provide support services for their family members.

The Dorothy Ley Hospice has two key programs — a community program and a residential program. Services offered through out community program are available from the time of diagnosis of a life limiting illness. The residential program is available to people who have three months or less to live.

Most of us – including our friends and family members – will ultimately face a life-limiting illness and loss. In fact, as the population ages and the structure of our health care system evolves, there are few options for the terminally ill who need physical, emotional and spiritual support. The Dorothy Ley Hospice fills that critical void.

Thanks to the generosity of donors, the Hospice is able to provide services at no cost to the individuals we serve. This is why every dollar we receive from individuals, foundations, corporations, organizations, churches and special events is appreciated and valued.

Our total budget to serve more than 3,000 individuals a year is almost $3 million. The provincial government funding covers 60 per cent of our costs and we need to fundraise the remaining 40 per cent or $1.1 million a year from the community.

Munsinger Gardens

From Sawmill to Flower Garden

The history of St. Cloud’s Munsinger Gardens may be compared to the garden’s well-worn paths; for both have interesting and unexpected turns. During the 1880s the lower east bank of the Mississippi River was the site of a sawmill. But by 1915 the city purchased the area for use as a neighborhood park. Eventually, the site became known as Riverside Park and Munsinger Gardens, the latter section named in honor of a former superintendent of parks, Joseph Munsinger. Munsinger Gardens has experienced many different stages of development. Improvements to the gardens occurred during the Great Depression through the Works Progress Administration. Projects from this period included planting trees and flowerbeds. Other additions included the creation of rock-lined paths, a lily pond, and a fountain. However, most of the work in developing the garden was directed by Joseph Munsinger. The first greenhouse was built in 1938. While the original greenhouse is gone, new ones stand in its place, and garden staff continues the tradition of growing flowers that help beautify this wonderful jewel known as Munsinger Gardens.

I am a middle school teacher and brought some of my students here to volunteer and earn community service hours. Kayla, the Director of Volunteers, was very helpful, friendly, and the kids loved her! Because of their young age, my students were only able to complete clerical work in the Vitas office, but they had a fun time. It was a chance for them to see what the real world was all about. At the end of their two hour shifts, the students always asked Kayla if they could stay longer. I would highly recommend Vitas for anyone looking for a satisfying volunteer experience. If my students and I had a great time in the office just folding and labeling fliers, you will have a great time working one-on-one with hospice patients!

VITAS is a super great organization. They have excellent training and various opportunities to work with people in need. If you are busy with a full time job and still want to volunteer, you can still participate and volunteer as little as a few hours a week if you want. The staff is warm and welcoming. I highly recommend you give them a try.

Comments: From the time Amy arrived to provide an estimate through the time that Curtis stretched the carpet and Al did the cleaning, every step in the process was professional, pleasant and punctual. Amy explained that some of the furniture indentations might not completely come back up – but she was wrong. After 6 years of indentations EVERY spot bounced back up. It was AMAZING!! Curtis also did such a fabulous job of re-stretching the carpet, it looks better now than when it was first installed.

WAN – Wide Area Network (WAN) – Difference between WAN and LAN

WAN is the acronym for, Wide Area Network and refers to a network used to connect different equipments from remote areas. This technology connects sites that are in diverse locations. Wide Area Networks (WANs) connect larger geographic area, such as New York, Canada, or the world. The geographical limit of WAN is unlimited. Dedicated transoceanic cabling or satellite uplinks may be used to connect this type of network. Hence, a WAN may be defined as a data communications network that covers a relatively broad geographic area to connect LANs together between different cities with the help of transmission facilities provided by common carriers, such as telephone companies. WAN technologies function at the lower three layers.

Normally, network services are provided by a Common Carrier of, for example, telephone company. Users can use services on rent basis. Available services include telephone network, leased line, packet switched network, X.25, ISDN, frame relay and cell relay.

WAN is composed of a number of autonomous computers that are distributed over a large geographical area. LAN can be extended across large distances using Satellite Bridge but still this cannot accommodate many computers arbitrarily. WAN must be scalable to long distances and many computers. Therefore, network must replace shared medium with packet switches to span long distances or many computers. Each switch moves an entire packet from one connection to another. This mechanism is called packet switching. These switches are nothing but a small computer with network interfaces, memory and program dedicated to packet switching function. These packet switches may connect to computers and to other packet switches, typically high-speed connections to other packet switches, lower speed to computers. These packet switches can be linked together to form WANs. WANs need not be symmetric or have regular connections, i.e. each switch may connect to one or more other switches and one or more computers.

Data delivery from one computer to another is accomplished through store and forward technology. Packet switch stores incoming packet and forwards the packet to another switch or computer that has internal memory. Therefore, this can hold a packet in queue if outgoing connection is busy.

• When a network spans a large distance or when the computers to be connected to each other are at widely separated locations a local area network cannot be used.

• A wide area network (WAN) must be installed. The communication between different users of “WAN” is established using leased telephone lines or satellite links and similar channels.

• It is cheaper and more efficient to use the phone network for the links.

• Most wide area networks are used for transferring large blocks of data between its users. As the data is from existing records or files, the exact time taken for this data transfer is not a critical parameter.

• Another example of WAN is an airline reservation system. Terminals are located all over the country through which the reservations can be made.

• It is important to note here that all the terminals use the same common data provided by the central reservation computer.

• Because of the large distances involved in the wide area networks, the propagation delays and variable signal travel times are major problems.

• Therefore most wide area networks are not used for time critical applications. They are more suitable for transfer of data from one user to the other which is not a time critical application. Wide area networks are basically packet switching networks.

• A WAN provides long distance transmission of data, voice image and video information over large geographical areas that may comprise a country, a continent or even the whole world.

• WAN contains a collection of machines used for running user (i.e. application) programs. All the machines called hosts are connected by a communication subnet.

• The function of the subnet is to carry messages from host to host. The subnet consists of two important components; transmission lines and switching elements.

• Transmission lines move bits from one machine to another. The switching elements are specialized computers used to connect two or more transmission lines. When data arrive on an incoming line, the switching element must choose an outgoing line to forward them.

• The switching elements are either called as packet switching nodes, intermediate systems, data switching exchanges or routers.

• When a packet is sent from one router to another via one or more intermediate routers, the packet is received at intermediate router. It is stored in the routers until the required output line is free and then forwarded. A subnet using this principle is called a point to point, store-forward or packet switched subnet.

• WAN’s may use public, leased or private communication devices, and can spread over a wide geographical area. A WAN that is wholly owned and used by a single company is often called as an enterprise network.

• In most WANs the network contains a large number of cables or telephone lines each one connecting a pair of routers.

• If two routers which are not connected to each other via a cable want to communicate, then they have to do it indirectly via other routers.

Packet switching technologies such as Asynchronous Transfer Mode (ATM), Frame Relay, Switched Multimegabit Data Service (SMDS), and X.25 are used to implement WAN along with statistical multiplexing to enable devices to share these circuits.

Packet switched subnet

• When a packet is sent from one router to the other, via some intermediate routers, the packet received at each intermediate router is stored until the required output line is free.

• Once the line becomes free, the packet is forwarded.

• A subnet working on this principle is called as point to point or store and forward or packet switched subnet.

• If the packets are small and of same size they are called as cells.

Router interconnection topologies

• Fig. shows some of the possible router interconnection topologies in a point to point subnet.

• The LANs have a symmetric topology while WANs have irregular topologies.

• The WAN s can also be formed using satellite or ground radio system. Satellite networks are inherently broadcast type so they are useful when the broadcast property is important.

Difference between WAN and LAN

With LAN additional expanses are rarely required once it is installed. With WAN, users must continue to pay a communication cost to their contracted common carrier.

  • WAN is generally slower in transmission speed. Requesting the same level of speed as with LAN leads to a substantial increase in communication costs.
  • The Satellite Bridge can extend LAN across large distances while in case of the WAN, it spans over a wide geographical area.
  • LAN still can not accommodate arbitrarily many computers; WAN must be scalable to long distances and many computers.

The gym itself if really cool, but I m here rating an issue I had dealing with both the Fulton Street location in Brooklyn and the corporate office. I had been doubly charged for one full year, and this came to my attention in January. I went to the Fulton Street location, spoke to the manager with the initials LR, who told me she would take care of it. On January 25th I had confirmed my address with her so that the corporate office could send me a refund, in which the manager had told me to expect it in 7-10 business days. On February 21st, I emailed LR asking what the deal was with the refund since I had not received it. She had replied on February 27 telling me I am sorry for the delayed response. I have checked on the status of your refund. There was a slight delay due to our department not being able to view the original email that was sent out. I have sent them all of the proper information and confirmed that they have received it and you will be receiving your refund within the next week and a half. I apologize again for any inconvenience. Please feel free to conntact me with any questions or concerns via email or phone. On March 16th, I emailed LR once again with news that I had not received the refund. No reply. On March 26th, I emailed LR once again with news that I had not received the refund. No reply. On April 4th (two days ago) I called the Fulton Street location and spoke to LR. She told me that she had checked one week ago and was sure that they sent it out then. I asked if she could email me the contact information for the person she is corresponding with so I can contact them directly. She said yes. One second after I hung up the phone with her, I emailed her to remind her to send me the contact information. No reply. It s been three months since I was told I would get a refund.

Garbage scam fitness company. Try to cancel because you move, good luck. Do not sign up here you can never leave. I tried calling corporate office and the execs phone messages system was full. Run away fast that will help you burn calories

He is not an interruption in our work. He is the purpose of it. He is not an outsider in our business. He is part of it. We are not doing him a favor by serving him. He is doing us a favor by giving us an opportunity to do so. I wonder if the staff at Planet Fitness – Canarsie, Brooklyn would benefit from being made aware of this little quote. I have been a member at the Planet Fitness – Canarsie, Brooklyn location for over four years, and as a loyal member I would like to draw your attention to several negative features that have been constantly occurring in the gym over time. First, I have noticed the chronic unsanitary and unhygienic conditions that have been prevailing and persisting for almost a year. Whether it s during peak hours or not, the gym has become a dumping ground filled with filthy paper littered all around the floors; on the machines; and along the walls. Also, the floors are constantly plagued with empty water bottles and the like. Second, the bottles of cleaning solution are almost always empty and/or defective, proving them unable to use. After approaching the front desk to notify them on several occasions, once instance I was told that, my co-worker left to pick-up some food and I m the only one on duty. Even, I m the only one here right now and I m not allowed to leave my post. I find it unacceptable that sanitary inspectors fail to assess the cleanliness of the gym on a regular basis throughout the day, especially on the weekends. Finally, the machines are frequently in a state of disrepair and are inoperable more often than not. This degradation of service is unfortunate because I greatly enjoy exercising at Planet Fitness – Canarsie. I would like to hear from you about your plans for updating and maintaining the gym so that I can decide whether or not to renew my membership for another year. If I have not heard from you soon, I am afraid I will have to cancel my membership indefinitely. I eagerly await your attention to this matter. Respectfully, Ashaki J.

Kevin was very professional and obviously very knowledgeable in .finding water leaks. He was easy to get a hold of and we were able to get an appointment quickly with him. He arrived on time and got straight to work finding our water leak which turned out to be a very quick process. He clearly knew Plumbing and gave lots of good recommendations to us which were helpful. I would say if you have a water leak and you re not sure where it s that don t call a plumber call Kevin he will find it for you and make sure you get the repairs you re supposed to get and not extra things done. We hired a plumber to find our leak and it cost us almost $1000 without any results before we called Leakmasters. Highly recommended!!

I contacted Kevin at Leak Masters after weeks of trying to determine why our community water association was experiencing significant water pressure loss. We suspected a large leak was the cause but had not been successful at locating it. After one household lost water I contacted Kevin and he came out on an emergency call. He looked over our well house and utilized a listening device on the pipes around the well house. He walked the water line with us and used his device at every location where we had exposed water lines. He checked the water pressure at every house. He identified one vacant home as a possible cause due to what he heard utilizing his device. He suggested we contact the owner to get into the home to check for leaks. He also pointed out that the drain field for that home was quite green and since the home had been vacant for some time and it had been a dry summer, this seemed unusual. When I finally located the property management company and was able to get into and under the house we found the leak immediately. It was very large and was bubbling like a fountain. Our community association members stepped up and we were able to cut and cap the water line. Once that was done the water pressure returned to normal. Dish Network Dispute Updates: Comcast SportsNet Stays; CBS May Vanish

Good news for sports fans: Dish Network and Comcast Corp. have gone into extra innings. In a move that will temporarily avoid a blackout of Comcast SportsNet channels in five major television markets, the two companies agreed to a short-term extension Tuesday to keep the programming on the air. No details of the temporary deal were disclosed.

Dish and Comcast have been engaged in a public battle over programming fees for more than a week, with the latter launching an outright media blitz in an effort to warn viewers that the channels were in danger of being yanked from the Dish lineup. The companies’ carriage contract expired on Monday.

Comcast’s NBCUniversal unit, which operates the SportsNet channels, had set up “microsites” aimed at the five affected markets: New England, the Mid-Atlantic, Chicago, California and the Bay Area. The sites encouraged viewers to contact Dish and voice their objections, or simply switch providers altogether. On Monday, an NBCUniversal spokesperson said hundreds of thousands of viewers had taken action (sent emails, tweets or placed phone calls) through the websites.

As of Tuesday, visitors to the sites are being greeted with a brief message saying a blackout has been temporarily averted.

Fans of “The Big Bang Theory” may not be so lucky.

CBS Corp. embroiled in its own carriage dispute with Dish Network, released a statement Tuesday saying it’s had enough with the extensions and that it will yank its programming from Dish if a deal can’t be reached by Thursday at 7 p.m. EST.

“We would very much like to avoid going dark, thereby joining the more than 120 stations Dish has dropped since 2013 alone,” CBS said in a statement. “Unless agreements are reached, however, our viewers should be prepared to lose CBS from their Dish systems.”

The Dish-CBS contract expired last month, but the two sides reached a temporary extension.

Dish is also involved in a carriage dispute with Time Warner Inc.’s Turner Broadcasting. Several Turner-owned networks, including CNN, the Cartoon Network and TCM, went dark for a month before being temporarily restored on Nov. 21.

Dish viewers are growing increasingly frustrated with the constant fighting and many have taken to social media threatening to cancel Dish. The Dish Network Facebook page is littered with angry comments.

Dish says it is simply fighting to prevent unfair price hikes, and it has good reason to do so. In a report earlier this year, the Federal Communications Commission cited programming fees as one of the main reasons why Americans are seeing their cable bills rise so dramatically.

Some analysts say Dish has been among the most vocal pay-TV providers to push back against the fee increases, even if it is taking a public relations hit in the process.

How to Prevent Ingrown Pubic Hairs

  • Shave correctly. Using a razor often creates a sharp edge, which makes the hair easier to pierce and point into the skin. Our guide to shaving pubic hair includes information for both men and women to help you get a close shave while avoiding redness, razor burn, and bumps.
  • Take extra precautions when waxing. Sharp edges are common with waxing because hair is often broke above or below the skin s surface when removed.   • Exfoliate. Regularly getting rid of dead skin cells helps keep hairs pointing up. Generally, it should be exfoliated once a week. How many times a week you should exfoliate depends on your skin type, the other shaving products you use, how often you shave and the season. My pick is an Ayate washcloth (compare prices) that can be used with a mild soap or body wash. It s not very pricey, is made of natural fibers, it s more mold-resistant than your standard loofah and can be thrown in with your other wash.
  • Moisturize. Deodorant soaps, hot water, and cold weather conditions can all cause or aggravate dry skin. So make sure to use a lotion (keep on the outside of body) after showering so skin stays soft and pliable. When skin is soft it s not an easy ground for the hair to break through. Keep hydrated from the inside by drinking water throughout the day and limiting alcoholic drinks.
  • Watch your fashion. You may have the body to pull off tight pants, shorts or underwear. Be aware that any type of fitted clothing around your bikini zone can encourage the hair to grow back into the skin.
  • Keep a bump-fighter on hand. Even with the best of intentions and working to prevent ingrown hair, there may be a couple that fall through the cracks so to speak. Just in case you see some bumps forming, have an ingrown hair product ready so that you can tackle them right away. Not only will this help avoid the bumps, but also the pimples or infection that also comes along with them.

We have moved to Baltimore, Md and during one of the flooding rains our basement flooded. It is an old restored house that still has a stone basement. We are planning on finishing the basement and to a point we are glad it flooded now and not after remodeling. We got several bids, Anchor Waterproofing being one. The owner s son came, much later than the 2 hour window they gave us. He walked to the basement and saw the stone walls and said we needed to have a mason repoint the walls and he couldn t do anything. When speaking to the owner several days before to set the appointment, she said that any water problem had to addressed from the source and covering it up wouldn t solve the issue. This is what the other companies said and they gave us plans and solutions. We selected another company. It was apparent, since Anchor only spent 10 minutes max at our house, that they were NOT INTERESTED in working on a stone basement. This is odd as they are located very near our house. This is an area of old houses and many, many have stone basements. The time spent waiting for Anchor to come was a complete waste of time. But at least we did finds other companies that gave us solutions. Don t waste your time with this company.

The Downtown Nashville Community (aka subarea 9) is located in the heart of Nashville and includes the richest of Nashville History. North Nashville

North East Nashville

Another somewhat rural community is that of Parkwood and Union Hill (subarea 2) but its next door neighborhood, the Madison community (subarea 4) is the mirror opposite and has been populated and repopulated for decades. North West Nashville

The Joelton area (subarea 1) has historically been a rural community and one of the least populated of the Metropolitan Nashville-Davidson County communities although the closer you are into Nashville the more populated it is. The rural communities normally begin after you leave the Bordeaux area (subarea 3) and cross over Briley Parkway into White’s Creek which is also part of subarea 3.

South Southeast Nashville

Maury County

    Joliet Area Community Hospice has been honored to provide services for thousands of veterans since 1982 and now enhances that care with Joliet Area Community Hospice’s Pride, Honor Dignity (PHD) Veteran’s Program. This comprehensive program focuses on respectfully celebrating veterans and providing care that recognizes the unique challenges that may exist in military families.
    This program provides care for those who served with Pride, will Honor their lives and service to country, and offers the Dignity so richly deserved.

    We Honor Veterans Program

    In order to provide greater community outreach, specialized education, and enhancements that honor the Veterans in our six county service area, Joliet Area Community Hospice is embarking on a two-fold project for Veterans and their families: Achievement of the We Honor Veterans program highest distinction and the creation of an outdoor
    Veterans Memorial Garden at JACH .

    The National Hospice and Palliative Care Organization (NHPCO) reports that one of every four dying Americans is a Veteran. Joliet Area Community Hospice has been honored to provide services for thousands of veterans since 1982 and enhances that care with JACH s own Pride, Honor, Dignity (PHD) Veteran s Program. JACH is committed to providing care for those who served with Pride, Honoring their lives and service to country, and offering the Dignity so richly deserved. This is accomplished by offering caregiver support, Veterans to Veterans programs, pinning recognition ceremonies, bereavement support, and community education in addition to guidance on resources and benefits available through the Dept. of Veterans Affairs.

    Joliet Area Community Hospice is currently working to achieve the We Honor Veteran s, Hospice Professionals on a Mission to Serve NHPCO program s highest level of distinction. This pioneering program has four levels of achievement that provides opportunities to educate our staff and community about the unique needs of Veterans at end of life. We recently achieved Level 1 designation, with the goal of reaching Level 2 as soon as possible. We now proudly display the We Honor Veterans logo on our website, promotional materials and advertising. We provide care for Veterans with an array of life limiting illnesses, including but not limited to cancer, AIDS, ALS, heart disease, liver disease, MS, Parkinson s disease, pulmonary disease, stroke, and end stage dementia and Alzheimer s disease.

    The hospice team consists of:

    • Patient Primary Physician
    • Hospice Medical Director
    • Registered Nurse
    • Certified Nurse Assistant
    • Licensed Social Worker
    • Chaplain
    • Volunteers
    • Bereavement Staff

    Members of our hospice team will make regular visits to assess the needs of the patient to provide additional care or other services, no matter where the patient resides. We will care for a patient living at home, in an assisted living facility, nursing home, or our Hospice Home. JACH will work with primary caregivers, family members and/or facility staff to meet the changing needs of the patient. In addition, a nurse is on-call 24 hours a day in order to meet any urgent needs and to provide guidance and support for caregiver concerns.

    Who pays for Veterans Hospice Care?

    Hospice care services are paid by, Medicare, Medicaid, and by private insurance. No one is ever denied care based on their inability to pay or their ethnicity, age, gender, mental or physical status, or religion.

    In addition to their proficiency on caring for those with advanced illnesses, JACH s staff and volunteers have undergone specific training, from the Department of Veteran Affairs (VA) and other local experts, that helps to identify and deal with issues that are unique to those who have provided military service.

    Many veterans have experienced traumatic events that have remained dormant for years only to emerge upon facing their own mortality. Treatment in each of these cases requires special sensitivity and JACH s clinical team of physicians, nurses, chaplains, social workers, certified nurse assistants and volunteers are prepared to identify these needs and recommend specific courses of action.

    Veteran to Veteran Volunteers

    Who understands as well as a veteran who has been there ? JACH welcomes and trains veterans for a variety of volunteering roles including but not limited to:

    Special expressions of gratitude and recognition for veterans on service, upon request, in the form of a unique Pinning Ceremony . This ceremony, performed by Veteran Volunteers, takes place where the patient resides.

    JACH PHD not only provides care for the veteran but for the family as well. This program offers assistance to the families in:

    • Accessing Military records
    • Accessing VA Memorial benefits
    • Accessing VA Burial Benefits
    • Supplying contact information for all other VA benefits that may be applicable

    Joliet Area Community Hospice has always been proud to offer excellent patient centered care. Below are just a few of the highlights from the Family Satisfaction Surveys from the fourth quarter of 2014:

    • 98.5% of respondents reported that the Hospice Team clearly explained the plan of care to the patient s family
    • 96.9% responded that the patient was treated with respect
    • 93.5% of respondents indicated that the hospice care received by their loved ones was consistent with end-of-life wishes
    • 96% of patients responded that hospice care helped with patient s feelings of anxiety or sadness
    • 96.9% indicated that the patient/family received the right amount of religious or spiritual contact
    • 89.1% indicated that the patient s personal needs were taken care of, compared to the national average of 78.6%
    • 83.3% of respondents reported that the quality of care improved after hospice was involved, compared to the national average of 63.4%

    Hospice Home opens, May 2004 becoming the first freestanding hospice home in Illinois.

    What makes Joliet Area Community Hospice Different?
    Joliet Area Community Hospice is the only non-profit community-based, Medicare-certified hospice serving Will and Grundy Counties. Because of the community support, Joliet Area Community Hospice was able to build the first Hospice Home in the State of Illinois, bringing cutting edge hospice care to our area. The staff at Joliet Area Community Hospice go above and beyond the call of duty to meet the needs of both patient and family as they face the end of life looking at not only the physical and psychological needs, but also the environmental, social, spiritual, and emotional needs.

    Joliet Area Community Hospice has a dedicated pediatric team that has received referrals from area hospitals as well as many specialty hospitals such as Children s Memorial Hospital, Ronald McDonald Children s Hospital of Loyola University Medical Center, University of Chicago Comer Children s Hospital and St. Jude Children s Research Hospital. In addition to the pediatric hospice team, there is a Pediatric Palliative Care Program for children that may not qualify for hospice services, but could still benefit from comfort care and family support services.

    As a non-profit agency, Joliet Area Community Hospice receives additional funding from the United Way of Will County, United Way of Grundy County, grants, memorials, general donations and bequests.

    Hospice Home in the State of Illinois to our community,

    The JACH Hospice Home is a home-like facility designed to provide short term care to terminally ill patients who are experiencing pain or symptoms related to their terminal illness that is not manageable at home. Hospice patients can also be admitted to the Hospice Home for short term respite care.

    The Hospice Home has 16 private patient bedrooms,( all with private bathrooms, with sleeping accommodations for family members), a family room, living room, dining room, kitchen, patient spa room, children s playroom and a solace room for patient and family use. Although we understand that there truly is no place like home for our patients and their families, we strive to provide the next best thing.

    Levels of Care at Hospice Home

    • In-Patient Care provides Short term pain control or management of uncontrolled acute clinical problems which can t be managed at home.
    • Residential Care provides routine care for patient. This level of care, here at the Hospice Home, is dependent on bed availability with room and board the responsibility of the patient/family.
    • Respite Care provides Caregiver/family relief from home care for a maximum of 5 days. The hospice team determines the appropriateness of the level and setting of care for the patient.

    Every patient and family served by Joliet Area Community Hospice Home will:

    • Receive around the clock support from our hospice team of physicians, nurses, social workers, chaplains, certified nursing aides, and volunteers.
    • Stay in a private room with accommodations for overnight guests;
    • Take comfort in knowing they are in a secure environment;

    We Honor Veterans Program

    In order to provide greater community outreach, specialized education, and enhancements that honor the Veterans in our six county service area, Joliet Area Community Hospice is embarking on a two-fold project for Veterans and their families: Achievement of the We Honor Veterans program highest distinction and the creation of an outdoor
    Veterans Memorial Garden at JACH .

    The National Hospice and Palliative Care Organization (NHPCO) reports that one of every four dying Americans is a Veteran. Joliet Area Community Hospice has been honored to provide services for thousands of veterans since 1982 and enhances that care with JACH s own Pride, Honor, Dignity (PHD) Veteran s Program. JACH is committed to providing care for those who served with Pride, Honoring their lives and service to country, and offering the Dignity so richly deserved. This is accomplished by offering caregiver support, Veterans to Veterans programs, pinning recognition ceremonies, bereavement support, and community education in addition to guidance on resources and benefits available through the Dept. of Veterans Affairs.

    Joliet Area Community Hospice is currently working to achieve the We Honor Veteran s, Hospice Professionals on a Mission to Serve NHPCO program s highest level of distinction. This pioneering program has four levels of achievement that provides opportunities to educate our staff and community about the unique needs of Veterans at end of life. We recently achieved Level 1 designation, with the goal of reaching Level 2 as soon as possible. We now proudly display the We Honor Veterans logo on our website, promotional materials and advertising. We are building capacity and collaborations as we achieve the goals of: improved quality of care for Veterans and their families, increased access to hospice and palliative care, strengthened partnerships with Veteran organizations, and adoption of best practice standards of care for Veterans.     What is Hospice?
    There comes a time when an illness cannot be cured. At that time, hospice services are a great benefit, a way to thank America s veterans by providing them with comfort, Pride. Honor. and Dignity in their final days.
    Hospice is a type of specialized care designed to treat an individual, not a disease. At Joliet Area Community Hospice, we assist people and their families facing the end of life with a full-range of medical, emotional, spiritual, and social needs. We provide care for Veterans with an array of life limiting illnesses, including but not limited to cancer, AIDS, ALS, heart disease, liver disease, MS, Parkinson s disease, pulmonary disease, stroke, and end stage dementia and Alzheimer s disease.

    The hospice team consists of:

    • Patient Primary Physician
    • Hospice Medical Director
    • Registered Nurse
    • Certified Nurse Assistant
    • Licensed Social Worker
    • Chaplain
    • Volunteers
    • Bereavement Staff

    Members of our hospice team will make regular visits to assess the needs of the patient to provide additional care or other services, no matter where the patient resides. We will care for a patient living at home, in an assisted living facility, nursing home, or our Hospice Home. JACH will work with primary caregivers, family members and/or facility staff to meet the changing needs of the patient. In addition, a nurse is on-call 24 hours a day in order to meet any urgent needs and to provide guidance and support for caregiver concerns.

    Who pays for Veterans Hospice Care?

    Hospice care services are paid by, Medicare, Medicaid, and by private insurance. No one is ever denied care based on their inability to pay or their ethnicity, age, gender, mental or physical status, or religion.

    In addition to their proficiency on caring for those with advanced illnesses, JACH s staff and volunteers have undergone specific training, from the Department of Veteran Affairs (VA) and other local experts, that helps to identify and deal with issues that are unique to those who have provided military service.

    Many veterans have experienced traumatic events that have remained dormant for years only to emerge upon facing their own mortality. Treatment in each of these cases requires special sensitivity and JACH s clinical team of physicians, nurses, chaplains, social workers, certified nurse assistants and volunteers are prepared to identify these needs and recommend specific courses of action.

    Veteran to Veteran Volunteers

    Who understands as well as a veteran who has been there ? JACH welcomes and trains veterans for a variety of volunteering roles including but not limited to:

    Special expressions of gratitude and recognition for veterans on service, upon request, in the form of a unique Pinning Ceremony . This ceremony, performed by Veteran Volunteers, takes place where the patient resides.

    JACH PHD not only provides care for the veteran but for the family as well. This program offers assistance to the families in:

    • Accessing Military records
    • Accessing VA Memorial benefits
    • Accessing VA Burial Benefits
    • Supplying contact information for all other VA benefits that may be applicable

    The Dorothy Ley Hospice

    Fostering hope and dignity through exemplary care, advocacy, education and research for individuals living with the challenges of a life-limiting illness or loss.

    The Dorothy Ley Hospice is a volunteer-based community service organization offering compassionate care to people living with the challenges of a life-limiting illness or loss. Our goal is to provide comfort and help them manage so they can live every day to the fullest with dignity, meaning and hope. All of our services are offered at no cost to the individual thanks to the generosity of our donors, government funding and the support of our community partners.

    Our programs and services meet a variety of support needs for individuals, families and care providers. Any individual facing the challenges of a life-limiting illness, regardless of age or medical condition, can access hospice support. We also provide support services for their family members.

    The Dorothy Ley Hospice has two key programs — a community program and a residential program. Services offered through out community program are available from the time of diagnosis of a life limiting illness. The residential program is available to people who have three months or less to live.

    Most of us – including our friends and family members – will ultimately face a life-limiting illness and loss. In fact, as the population ages and the structure of our health care system evolves, there are few options for the terminally ill who need physical, emotional and spiritual support. The Dorothy Ley Hospice fills that critical void.

    Thanks to the generosity of donors, the Hospice is able to provide services at no cost to the individuals we serve. This is why every dollar we receive from individuals, foundations, corporations, organizations, churches and special events is appreciated and valued.

    Our total budget to serve more than 3,000 individuals a year is almost $3 million. The provincial government funding covers 60 per cent of our costs and we need to fundraise the remaining 40 per cent or $1.1 million a year from the community.
