    Much animal research into potential treatments for humans is wasted because it is poorly conducted and not evaluated through systematic reviews

    Clinicians and the public often consider it axiomatic that animal research has contributed to the treatment of human disease, yet little evidence is available to support this view. Few methods exist for evaluating the clinical relevance or importance of basic animal research, and so its clinical (as distinct from scientific) contribution remains uncertain. 1 Anecdotal evidence or unsupported claims are often used as justification—for example, statements that the need for animal research is “self evident” 2 or that “Animal experimentation is a valuable research method which has proved itself over time.” 3 Such statements are an inadequate form of evidence for such a controversial area of research. We argue that systematic reviews of existing and future research are needed.

    Assessing animal research

    Despite the lack of systematic evidence for its effectiveness, basic animal research in the United Kingdom receives much more funding than clinical research. 1 4 5 Given this, and because the public accepts animal research only on the assumption that it benefits humans, 6 the clinical relevance of animal experiments needs urgent clarification.

    Several methods are available to evaluate animal research. These include historical analysis, 7 critiques of animal models, 8 investigations into the development of treatments, 5 surveys of clinicians’ views, 9 and citation analyses. 10 However, perhaps the best way of producing evidence about the value of animal research is to conduct systematic reviews of animal studies and, where possible, compare the results of these with the results of the corresponding clinical trials. So what do studies that have done this show?

    CORE-AT Electronic Medical Record and
    Injury Surveillance System

    The CORE-AT electronic medical record (EMR) system is a web-based injury surveillance and EMR system for use by athletic trainers. The CORE-AT EMR was designed and developed by athletic trainers in partnership with Essentialtalk. an international technology communication company.

    The CORE-AT EMR is compliant with the data acquisition, storage, and transmission standards set forth by the Health Insurance Portability and Accountability Act (HIPAA).

    Health information obtained using the CORE-AT system are not stored locally on computers. Instead, data is uploaded in real-time and transmitted to secure, redundant servers.

    System Capabilities

    The CORE-AT EMR system consists of documentation capabilities including individual patient/athlete registration (e.g. sex, age, grade), injury demographics (e.g. sport, season, position), comprehensive patient evaluation (e.g. diagnosis, injured body part, side, mechanism of injury), and daily treatment, progress, and discharge (e.g. athletic training interventions, rehabilitation protocols, date of return-to-play) notes.

    A necessary feature of the patient record is the ability to assign ICD-9 CM and CPT codes to patient diagnoses and treatments, respectively, to enable the characterization of secondary school athletic training practice and for creating cost-estimates for secondary school athletic training services.

    Also included in the system are single and multi-item patient-oriented outcome measures, both generic and region specific. Outcome instruments are automatically generated and included as part of the patient evaluation process based on diagnosis and injury region. An automated system not only assures collection of appropriate patient information but also promotes the use of outcome instruments as part of standard athletic training practice.

    Injury Surveillance

    In addition to the capabilities of the CORE-AT system to collect patient outcomes, CORE-AT is also constructed to capture and integrate injury surveillance information specific to athlete injury and exposure (e.g. incidence, athlete-exposure, time loss/non time loss, sports participation).

    With the surveillance component integrated directly into the medical record, information such as time loss/non-time loss, diagnosis, mechanism of injury, sport, and position are all collected and recorded during the initial evaluation, without the need for double entry. Data regarding athlete exposures are also captured and made available to the clinician.

    Potential Drawbacks of Web Based Credit Card Merchant Account Terminals

    Many businesses sign up for merchant accounts where they want to be able to process credit cards through their computers via the internet. This can be a great alternative to traditional merchant account systems which typically require expensive purchase or lease terminals and the requirement to add a separate phone line for transmission of the data.

    Once approved, the merchant is given a user name and password that they use to log into their web portal merchant account web page also known as a “Virtual Terminal”.

    The page(s) of the Virtual Terminal have fields for data entry of the customer name, credit card number, expiration date, security code, billing address and transaction amount and details.

    Some virtual terminal systems such as Chase PaymenTech also allow for the addition of a low cost credit card reader so that the cumbersome data can be easily “swiped” into the system. This will not only save on time but also errors in data entry. However, many systems that do allow for a “swiper” system require only that particular merchant processor’s own expensive reader which can sometimes be quite cost prohibitive.

    Some of the drawbacks of this Virtual Terminal system need to be taken into consideration before deciding on this type of credit card processing system.

    One of the biggest problems a merchant can experience with this type of system is what happens when the internet goes down for whatever reason. When a web based credit card processing system loses internet, the entire system is rendered Worthless. Because the system is web based, losing internet loses all data entry and all processing capabilities. This can be devastating and amount to profit loss for a business.

    Also, logging in to the virtual terminal can sometimes be a long and tedious process depending on the speed of your internet and how busy the processors servers are. The other problem which can be a real nuisance is that if there is any period of time where you are not doing transactions, many virtual terminals will log you off which will then require you to re-log back in.

    A second problem with this type of system is the lack of receipt printing formats. Again, because it is web based, the only “receipt” a merchant can print is the actual web page itself. This will require a laser or inkjet printer which will end up being expensive for not only 8.5 x 11 printer paper, but also for toner. Also, depending on the type of printer being used, this can be a slow process that customers do not want to wait for.

    It is extremely important for a merchant to print a receipt not only to give to a customer but also to get signed for their own chargeback protections. There are a very few web based virtual terminal type systems that have formats built into them that will allow printing receipts to smaller thermal receipts such as the type you are used to seeing at a typical store that also has a signature line for customer signatures as well as some of the other formatting that will make a business look more professional.

    Another drawback to these systems is that most have no bar code scanning capabilities. Many businesses with a lot of products benefit from being able to do bar code scanning for fast point of sale checkout. A virtual terminal system will typically have no provision for this or even a drop down list of products, descriptions, or pricing. All information typically must be entered in for each transaction manually. This can result in wasted time and productivity.

    Evidence-Based Practice in Social Work: Lessons from Judgment and Decision-Making Theory

    This paper introduces a critique of the experimentalist strand of the evidence-based practice movement in social work. It does so by drawing on the available body of knowledge that is found within the field of judgment and decision making. The nexus between experimentalist evidence-based practice and rational choice approaches to social work decision making is demonstrated. Subsequently, it argues that the properties of social work decision tasks are more likely to facilitate naturalistic than analytical decision making strategies. Recognition-primed decision making is introduced as a possible alternative to practice guidelines and decision aids. Finally, the consequences of embracing the principles of recognition-primed decision making are considered regarding social work practice, education and research.
