Providing excellent patient care is the most important aspect of nursing. Moreover, taking credit for the care given is also an important responsibility. Most nurses hate the paperwork more than any other aspect of their job, but it’s critical that it be done, and be done well. As with every aspect of quality patient care, charting should be exceptional; it should never be taken lightly.

The chart is a legal medical record, communicating crucial information to other members of the health care team so that they can make informed decisions. An accurate chart is critical to assessing the patient’s health status, and determining future care and treatment methods.

State Facts, Not Opinions

One of the first lessons a nurse learns is that if she doesn’t chart something, it wasn’t done. As a nurse, you can argue to the death that you gave the right meds or checked the heart monitor, but if you neglected to chart it, there s no legal record of it.

That doesn t mean you should go chart-crazy, and jot down every minute detail. Stick to the facts! For example, don’t get caught in the habit of charting every two hours that a patient was “resting comfortably in bed” until you have actually checked on the patient and noted the exact time. And don t get ahead of yourself: making a comment or checking off a box even two minutes in advance can come back to haunt you. If that patient falls, has a request that someone else responds to, or should code just a minute or two before your recorded entry, your whole level of care could come into question.

Your patient may be a mean, nasty, curmudgeon, but that’s not for you to say. Neither is it right to say she’s a sweet, adorable old lady. Look for the facts (signs, symptoms, and statements ) as to the basis for the behavior. Is your patient in pain? Is he scared or fearful of a procedure or diagnosis? Is he depressed or anxious? Has he had some issues with family or caregivers? Is the sweet old lady being stoic? Does she understand her illness? Is she in denial? Or perhaps her symptoms have been well-controlled with the current medication regimen?

Stayed here recently for 4 nites and whilst the property is reasonable – whilst it is 4 star – I have certainly stayed in other 4 star properties that are much better. The price however was very reasonable – and the room, whilst it has everything you need, was just a little jaded. It was clean, the bed was a bit old with a reasonably old mattress but it was certainly comfortable still. The thing that let this place down so badly was the service – or rather the housekeeping. It was certainly clean, however when i arrived there was no bath mat so needed to use the hand towel. Next day I returned to be met by the housekeeping staff to let me know they run out of time to do my room that day. I wasnt too fussed – they did provide bath mat, replenished the mini bar and had i have insisted would have made up my room.
On Sunday I was out for the day and returned to find the room had not been touched at all – so for a 4 nite stay – only had housekeeping for 2!
Parking whilst not allocated to individual units is ample and I was able to park out the front of my unit every evening – even though it was booked out every nite.
On the first nite I arrived home about 10.00 to find a small party being held outside the units and this was a little annoying with breaking glass and drunks just outside the bathroom window – of course this is not the fault of the property but perhaps indicative of the type of people who stay there. It seems each room is configured a little differently so would cater for all types of travellers.

All four phases of the Dukan Diet – named for former French physician Pierre Dukan, its creator – are heavy on do’s and don’ts, and even the slightest slip-up is considered destructive. You’ll move from the all-you-can-eat, pure protein “Attack” phase to “Cruise,” which allows selected vegetables on selected days. In the third phase, “Consolidation,” you’ll add more foods that, by now, you’re longing for, such as cheese and bread. By the last phase, “Permanent Stabilization,” you’re relatively free. With just a few (very crucial and very specific) parting rules, you’re permitted to eat anything you want.

Attack: A gourmand’s dream diet, this phase is all-you-can-eat, portion-wise. Any of the following is fair game: lean beef, veal, pork and venison; organ meat including liver and tongue; fish; shellfish; poultry; low-fat ham, turkey and chicken; eggs; vegetable protein such as tofu and seitan; nonfat dairy; water and other no-calorie drinks (yes, even diet soda); and 1 1/2 tablespoons of oat bran. Spices and herbs are encouraged to avoid monotony. Craving something else? Too bad – cheating is expressly forbidden. “[S]uccumbing to any other foods, as small as the lapse may be, will be like puncturing a balloon with a needle,” warns Dukan.

The Attack phase lasts between one to 10 days, depending on how much weight you have to lose. For the majority of dieters – those with 20 to 40 pounds to shed – Attack is typically five days and weight loss is usually 4 to 7 pounds, according to Dukan.

Cruise: In this phase, you’ll add back certain all-you-can-eat vegetables (the nonstarchy ones, such as cucumbers, mushrooms, zucchini, peppers and salad greens). Still, Dukan recommends alternating one day of the “pure-protein” Attack phase with one day of protein plus vegetables. (A five-day protein then five-day protein-plus-vegetables plan is also an option). The oat bran recommendation rises slightly to 2 tablespoons.

You should lose at a rate of about 2 to 4 pounds per week, according to Dukan. You’ll continue alternating until you reach your goal weight.

Consolidation: Now, the name of the game is keeping the pounds off. You’re very vulnerable to weight regain, says Dukan, and the only option is a transition phase that lasts five days for every pound you’ve lost. That means dieters who’ve lost 20 to 40 pounds will stay in Consolidation between 100 to 200 days. The proteins and vegetables from the previous phases are still all-you-can-eat, but you can now mix them together as you please. Each day, you also get a serving of fruit, two slices of whole-grain bread and 1 1/2 ounces of cheese. Each week, you’re also allowed two servings of starchy foods like pasta or quinoa, three specific protein indulgences (leg of lamb, roast pork and cooked ham), and one or two “celebration meals” where you can eat anything you want, provided you don’t binge or have them back-to-back. The daily 2-tablespoon oat bran requirement stays.

You’re still required to revert to the “pure-protein” Attack phase once per week.

Permanent Stabilization: Freedom! (Well, sort of.) You can now eat whatever you want six days out of the week, provided you don’t abandon what you learned during Consolidation. And 3 daily tablespoons of oat bran is now a must. The seventh day is a purest-of-pure protein day that mirrors the Attack phase but restricts even more the acceptable foods list. As its name suggests, this phase is meant to last a lifetime.

I think it’s important to point out that I only sell annuities, and burned the boat many years ago by changing my name to Stan The Annuity Man. With that being said, I am the first one to scream from the rooftops that annuities aren’t for everyone.

Annuities aren’t one size fits all panacea solutions, and they are not too good to be true, even though too many are sold that way. They do work well when placed properly in a portfolio, and fully understood for the contractual realities of the policy. So let’s take a closer look at the reasons you might want to stay away from the annuity dream slingers.

You don’t need an annuity if.

1.If you can professionally manage the money yourself

Annuities should only be owned for their contractual guarantees, so if you are adept at managing your own money and feel comfortable performing that task, then you should not deviate from that plan. If you have an adviser who has proven themselves to be really good at managing your money, then consider yourself fortunate and stay the course with that professional.

2.If you want real market type growth

Annuities aren’t growth products, period. Even the best no-load variable annuities have limited investment choices and restrictions on moving the money between separate accounts (i.e. mutual funds). Indexed annuities were designed to compete with CDs, and their historical returns reflect those realistic return percentages. Load variable annuities have limitations and restrictions from an investment standpoint, and their typical high fees severely eat into any annual returns. Facts are facts, regardless of the hypothetical growth dreams that are overly promoted.

3.If you are thinking about replacing your LTC policy

The best long-term care coverage that you can buy is a traditional long-term care policy. Annuity type coverage should only be looked at as a supplement to a traditional policy, or a last resort choice if you cannot pass the underwriting requirements of real LTC solutions. Confinement care riders or doublers that can be attached to some deferred annuities should not be considered as legitimate term care type coverage.

4.If you want everything packed into one product

Riders are attached benefits that you can add (with an annual fee for the life of the contract) to some annuity policies to try to solve for solutions like lifetime income, legacy, or long term care. Carriers and agents love to promote these one size fits all product scenarios, but you are best served by solving for one solution at a time with any annuity strategy. In most cases, the contractual guarantees are higher when you implement a single focus solution. The more isn’t the merrier with annuities.

5.If you are only being shown one product from one insurance carrier

Because annuities should be owned for their contractual guarantees, you should always shop numerous carriers for the solution you are looking to solve for. My apologies to the captive agent, but it just makes sense to shop around just like you would for any product. It is also important to remember that annuity guarantees are only as good as the company backing them up, so do your homework on the safety of the carrier.

6.If you don’t need additional lifetime income

Annuities were created in the Roman times for a pension type lifetime income guarantee. Even though variable and indexed annuities can be unbelievable complex and unfortunately represent the vast majority of annuities sold each year, the primary reason that most people should own an annuity is to solve for longevity risk (i.e. outliving your money). Whether you need income to start immediately or sometime in the future, annuities are the only product on the planet that will pay you regardless of how long you live. If you don’t see yourself needing additional income guarantees for the rest of your life and you are pretty good at managing your own money, then you probably have no need for an annuity.

7.If you see no need to transfer risk

Annuities in their purest form are transfer of risk strategies that can solve for specific things like principal protection, income for life, legacy, or long-term care. That easy to remember acronym that I’ve developed is P.I.L.L. If you don’t need to transfer risk to solve for these four items, then you probably shouldn’t own an annuity. If you are comfortable shouldering the risk yourself, then there is nothing wrong with that.

Another good tip when considering any annuity strategy is to always go with your gut instincts when making a decision. You need to trust that internal compass, because it is typically very accurate. If you are getting pressured into buying or a red flag is raised for any reason, then it’s probably best to move on.

If the sales pitch sounds too good to be true, then write down in detail exactly how you think the annuity being proposed is going to work according to what the agent said. Then sign and date that list, and have the agent sign and date it as well so you can have them fully endorse their recommendation. This is just an effective and simple idea to fully flush out the contractual realities of the policy.

Annuities can be fantastic strategies if placed and used properly. Use your common sense and experience in combination with these seven reasons to make sure you do the right thing when considering the purchase of an annuity.

    Texas SB4 Immigration Enforcement Law: 5 Things to Know

    The state of Texas has passed a law that has been described as a ban on “sanctuary” cities, but there is much more to the law that doesn’t get captured by that term.

    Opponents say it not only puts immigrants in the cross hairs of law enforcement, but also U.S. citizens, legal residents, public officials and law enforcement officers. The law’s supporters say it is designed to boost public safety by enforcing laws already in place.

    The Texas law, which goes into effect in September, follows one passed in Arizona that also tried to use local and state law enforcement to question the immigration status of residents. The Supreme Court gutted the Arizona law and a settlement removed a “show me your papers” provision requiring officers to investigate a person’s immigration status if they had “reasonable suspicion” the person was illegally in the U.S.

    The legal fight over the law has already begun, so here are five things to know about the law:

    It’s not just police who will enforce immigration

    The law enlists city council members, members of county commissions or other local governing bodies, sheriffs, district and city attorneys and even police on the campuses of colleges and universities in the effort. It prohibits them from adopting any policy to prohibit or “materially limit” a corrections officer, commissioned peace officer, booking clerk, magistrate, district attorney, criminal district attorney or other prosecuting attorney from checking into a person’s immigration status and reporting that information to the feds or another local entity.

    Government officials or campus police in those positions also can’t stand in the way of federal immigration officers enforcing immigration laws at a local jail.

    Officials can be removed for not complying with the law

    Some officials have said the law doesn’t mandate that local entities investigate a person’s immigration status, but it permits it.

    But the law allows state officials to remove from office any elected or appointed official who prohibits or “materially limits” enforcement or cooperation with immigration federal officials. Uncooperative sheriff, police chief, constable or jail administrators could face Class A misdemeanor charges. Defiance could mean fines of between $1,000 to $1,500 for the first violation and $25,000 to $25,500 for each one after that.

    Thomas Saenz, president and general counsel for the Mexican American Legal Defense and Educational Fund (MALDEF) said the state is treading precarious ground with those provisions. He argues a local official who voices disagreement with the provision could be seen as obstructing police officers from immigration enforcement. The provision may not stand up against First Amendment protections, he said.

    But Texas Governor Greg Abbott said the law has “already been tested at the United States Supreme Court and approved there.”

    It encourages people to turn in their local government or university officials

    The state’s cops, sheriffs and government officials may have to keep looking over their shoulder. Employees of the local governments or colleges and universities can turn in any employer who is not following the law. Then, the attorney general can order compliance or take the entity to court.

    Victims and witnesses could be asked about their immigration status

    There are protections for crime victims or witnesses from being questioned about immigration status. But the law also provides some exemptions.

    Officers can ask the victim or witness about their nationality or immigration status if the officer has “probable cause” to believe the victim or witness committed another crime. (Entering the country without inspection is a misdemeanor, so that would constitute as a crime).

    It promises to defend those who are sued for complying with the law

    The law says the state attorney general will defend anyone who is sued for complying with the law. The governor says it’s about public safety.

    “This bill will help keep dangerous criminals off our streets and protect innocent lives. For every ounce of criticism, there is a pound of praise from Texans who simply want laws to keep them safe,” stated a spokesperson for Abbott.

    Civil rights organizations like MALDEF say there is no question there will be litigation. They say public officials and law officers that honor detainers are certain to face Fourth Amendment legal challenges because there often isn’t sufficient cause for local officials to hold someone in detention beyond a release date. There also are certain to be civil rights and racial profiling accusations.

    Texas taxpayers will be footing state costs for defending the law and its enforcers.

    “It’s an open checkbook and there are going to be lots of suits,” said Saenz. “Why a fiscally conservative governor and legislature would write a check like that is the untold story.”

    When Exchange Online Protection scans an inbound email message it inserts the X-Forefront-Antispam-Report header into each message. The fields in this header can help provide administrators with information about the message and about how it was processed. The fields in the X-Microsoft-Antispam header provide additional information about bulk mail and phishing. In addition to these two headers, Exchange Online Protection also inserts email authentication results for each message it processes in the Authentication-results header.

    For information about how to view an email message header in various email clients, see Message Header Analyzer. You can copy and paste the contents of the message header into the Message Header Analyzer tool. When you select a message in the quarantine in the Exchange admin center, the View message header link also easily lets you copy and paste the message header text into the tool. Once in the Message Header Analyzer tool, click Analyze headers in order to retrieve information about the header.

    After accessing the message header information, search for X-Forefront-Antispam-Report and then look for these fields. Other fields in this header are used exclusively by the Microsoft anti-spam team for diagnostic purposes.

    Describes the results of the SPF check for the message. Possible values include:

    pass (IP address) indicates the SPF check for the message passed and includes the sender’s IP address. The client is authorized to send or relay email on behalf of the sender’s domain.

    Ride a motorcycle? Better make sure all your insurance coverage is in place, and get multiple quotes to ensure you get the best motorcycle insurance.

    “Best” doesn’t just mean cheapest. The minimum required coverage may leave you with stacks of bills if you have to lay your bike down.

    Yet motorcycle insurance can be less expensive than car insurance, and you are entitled to most of the same discounts.

    What insurance is needed to cover motorcycle injuries?

    Liability insurance pays for the damage you do to others. Comprehensive and collision pay to repair or replace your ride. Unless you buy some kind of specific medical coverage (either personal injury protection or medical payments), if you are at fault in an accident your own treatment costs are either paid by your health insurance or out of your own pocket.

    Your own health insurance, if you have it, is likely to cover hospitalization costs, but not all policies do. Ask. It may have a deductible — in some cases, as much as $5,000 to $10,000.

    Even if you have health insurance, it won’t cover injuries to your passenger.

    If another driver is to blame, it’s crucial to understand the importance of uninsured and underinsured motorist coverage, says Alex Hladkevych, motorcycle product director at Liberty Mutual. “If the driver responsible for a crash does not have adequate coverage,” he says, “you could be stuck with a big hospital bill.”

    The average treatment cost for a rider with traumatic brain injury was about $48,000, according to the Minnesota Department of Health.

    How much is motorcycle insurance?

    Motorcycle insurance can be anywhere from $71 to $297 a year depending on multiple factors like where you live and what type of coverage you get. Motorcycle insurance can be cheaper to insure than car insurance, but it depends a lot on where you live and the cost and performance level of the motorcycle you want to insure. Here’s a couple of examples to give you a clearer picture of how state and coverage affect rates. Also, how motorcycle rates compare to car insurance rates.

    Pricing out coverage on a used Honda Shadow Spirit 750, a 24-year-old rider in Jacksonville, Florida, looking for state minimum liability levels would pay about $71 a year, according to quotes provided by The same driver would pay about $830 a year for basic liability coverage on a 1998 Honda Civic.

    Upgrading to a newer bike that’s financed, a 2013 Honda CBR250R, requires collision and comprehensive coverage; that would bring the annual bill for the 24-year-old to $297. The same level of coverage on a financed 2007 Mazda3 would be $1,558.

    In California, those numbers look a lot different: $209 a year to insure the paid-off motorcycle, but $1,940 for full coverage on the new bike. Insuring the paid-off Civic would cost the driver $768 a year, and the financed Mazda is actually cheaper than the new motorcycle at $1,544 a year.

    You typically pay more to insure a sport bike or high-performance bike than a cruiser.

    Many motorcycle policies include some level of accessories coverage — $3,000 is typical — and roadside assistance.

    Recommended motorcycle insurance coverage

    Like auto insurance, motorcycle insurance requirements vary from state to state. Managing Editor Des Toups and experts at Liberty Mutual Insurance and Progressive Insurance recommend:

    Bodily injury and property damage liability, which covers you if you’re at fault for an accident that injures another person or damages someone’s property. This is mandatory in most states.

    Guest passenger liability, which covers anyone riding pillion. Not all liability policies automatically cover your passenger, Toups says. You may have to buy additional coverage, depending on your state laws and your insurance company. Ask.

    Comprehensive and collision coverage, which pays to repair or replace your motorcycle if it’s stolen or damaged in an accident or stolen. This is optional unless your lender requires it.

    Uninsured/underinsured motorist coverage, which covers you when the person at fault for an accident doesn’t have insurance or doesn’t have enough insurance. Some states require it; others don t.

    Medical payments, which covers the cost of medical care you require as the result of a motorcycle accident. It can be used regardless of who is at fault. (States with no fault laws requiring automobile owners to buy personal injury protection typically exempt motorcycles, making this coverage even more important.) It can also be used to pay the deductible on your health insurance.

    What kinds of motorcycle insurance discounts are offered?

    Most discounts that are available to automobile drivers are available to those insuring a motorcycle. A few are specific to bikes, though. Ask your insurance company about:

    Safety course discount: Almost all insurers will give you a discount of at least 5 percent for completing an approved safety course, usually within the previous three years.

    Riding association or affinity discount: Usually 10 percent, for members of approved ridership groups such as the American Motorcycle Association, BMW Motorcycle Owners of America, Gold Wing Touring or Road Riders Association, Harley Owners Group, Honda Riders Club of America, Motorcycle Safety Foundation or Motorcycle Touring Association.

    Good rider or safe rider discount: Depending on the carrier, three to five years without a claim or major violation wins you preferred status.

    Layup discount: Bikes stored for the winter typically can get reduced rates.

    Anti-theft discount: Motorcycles equipped with a GPS-driven locating device get as much as 20 percent off comprehensive coverage. Some carriers will waive your deductible.

    Antilock brake and airbag discounts: While these features are not common, they are becoming more so. Nationwide and Farmers offer discounts for antilock brakes; Farmers has one for airbags as well.

    Helmet discounts: Not a discount, exactly, but you can save money by wearing one. For example, Farmers Insurance offers additional medical payments coverage for riders who use a DOT-approved helmet. The state of Florida requires that riders who go without a helmet carry at least $10,000 worth of medical payments coverage.

    Can I get a multi-policy discount? How about a home and auto bundle?

    If you insure more than one vehicle or have multiple types of policies with the same company, there is almost always a discount. It may be a:

    Multi-bike discount: For those with more than one insured motorcycle with the same carrier.

    Multi-policy discount: If you have a car insurance policy and a motorcycle policy with the same company. (Multi-car policies typically require two automobiles rather than a bike and a car.)

    Bundling discount: A home and auto bundle isn’t the only way to get a discount. A motorcycle insured with the same company that insures your home typically will get you a discount on both. Bundling works with renters insurance, too.

    Most other forms of automotive discounts apply to your motorcycle coverage as well. You can get discounts for early renewal, payment in full and even a transfer discount for switching companies.

    Bear in mind that while discounts can add up, the total amount is almost always capped at no more than 40 percent of your premium. That means it always makes sense to shop around and start with the cheapest rates you can.
