Patient Advocates are available to address issue of patients if the Department Level Advocate is unable. Our Patient Advocate program gives you and your family the security of knowing someone is available to focus on your individual concerns and rights.

This fellowship program is designed for candidates who have academic and clinical career goals in Hospice and Palliative Medicine. Fellows gain hospice and palliative medicine expertise as they provide consultations for medical, psychological, and spiritual concerns and coordinate team-based, holistic integrative approaches. Palliative care services are provided to adults and children through multiple settings, including inpatient wards, outpatient clinics, and day-hospital clinics. Trainees encounter patients with common and rare diseases from all walks of life, and in both research and community settings. Candidates must have:

  • a MD or DO degree from an ACGME-accredited medical school
  • satisfactorily completed three postgraduate years of training at the time of appointment
  • ABMS board eligibility or certification in one of the following specialties: internal medicine, family practice, pediatrics, psychiatry, neurology, oncology, anesthesiology, surgery, or radiation oncology
  • unrestricted state medical licenses and the ability to be licensed in Washington DC and Maryland by the beginning of training.

Program Structure
This one year fellowship program offers training that leads to board eligibility in Hospice and Palliative Medicine. Fellows receive broad, practical experience in palliative medicine and hospice care through inpatient and outpatient palliative medicine rotations at the NIH Clinical Center (approximately half the academic year), inpatient palliative medicine rotations at community Suburban Hospital (12 weeks), and home hospice (4 weeks), inpatient hospice (6 weeks), and long-term-care hospice (4 weeks) rotations at Washington Home and Community hospices. The fellowship includes a four-week elective rotation.
Additional program highlights:

  • Fellows are expected to demonstrate academic proficiency through literature review, a peer-review quality journal article, a book chapter, or other professional writing project, or a limited and closely-mentored research project
  • Fellows are exposed to and learn about complementary therapies such as acupuncture, relaxation therapy, hypnosis, biofeedback, guided imagery, Reiki, and art therapy
  • Longitudinal self-care activities through confidential, individual meetings with social worker and chaplain
  • Longitudinal educational and clinical collaboration with NIMH Psychiatry Consult Liaison Service

Application Information
This program participates in the ERAS system and all applications to this fellowship should be completed through ERAS. The completed application must include three letters of recommendation. The program also participates in the National Resident Match Program (NRMP). Additional information about the Hospice and Palliative Medicine Fellowship application cycle and deadlines can be obtained directly from the ERAS website.

Currently we do not sponsor J-1 Visa. Fellows receive a stipend based on the salary guidelines of the National Institutes of Health PGY4 level salary.

A Center for Addiction Recovery, CFAR, is an accredited rehab center, providing the most up-to-date Medical Detox, Intensive Outpatient and Residential Addiction Treatment Programs since 2002. Our addiction treatment programs provide treatment for male and female clients who are struggling with alcohol, drugs, prescription drugs and process addictions such as compulsive gambling and eating disorders.

CFAR embraces traditional and holistic approaches in the treatment of addiction which includes the non-12 Step SMART RECOVERY, and the 12 Step Recovery Program. All addiction treatment programs at A Center for Addiction Recovery focus on treating the physical, emotional and spiritual aspects of our clients’ recovery, helping them to identify and address the root causes that may have triggered their addictive behaviors. There are many instances that a past trauma has played a major role in someone’s addiction which in turns affects his or her treatment and recovery. In such cases we utilize the most promising trauma treatment approach, Rapid Resolution Therapy (RRT), which is exclusively delivered at A Center for Addiction Recovery by the founder and the developer of this therapy, Dr. Jon Connelly.

During RRT clients do not become overwhelmed or feel helpless by re-living the traumatic event. Neither do they go through hypnosis. They are able to clear their trauma while awake and present. Once the mind is clear from the trauma, clients have a far greater chance of achieving a successful recovery. With one-half to two-thirds of people with addictions suffering from Post-Traumatic Stress Disorder (PTSD), and about the same proportion of people with PTSD having developed an addiction, CFAR finds it absolutely essential to treat both conditions effectively at the same time to provide clients with the best chance for a successful recovery.

What does Hospice cost?

Center for Hospice Care can provide services to those who meet the qualifications regardless of financial status. We do not turn anyone away because they cannot pay. If you are worried about the cost of caring for a terminally ill person, or worried about your family s finances when you are terminally ill, please do get in touch with us. We can discuss your options and hopefully ease your concerns.

Hospice care is widely covered by private insurance plans, although your provider is the best person to talk to when it comes to details about what is and is not covered by your plan.

Medicare and Medicaid

Patients who are eligible for Medicare/Medicaid Hospice Benefit will be able to get most hospice costs covered. That includes both clinical services such as pain relief, and support services. Some services may also be available to caregivers and loved ones at no extra cost. Usually, families of Medicare/Medicaid patients will have to pay very little for services related to the terminal illness.

Those who are not covered by Medicare or any other insurance will still be provided with hospice care. We will not refuse assistance because of an inability to pay. In some circumstances we can also help find out if terminally ill patients are eligible for special coverage that they may not currently be accessing.

Through the support offered by the community of Eastern Connecticut in the form of volunteer time and philanthropic donations, we can help make sure no terminally ill person goes without hospice care because of the cost.

A network topology is the arrangement of a network, including its nodes and connecting lines. There are two ways of defining network geometry: the physical topology and the logical (or signal) topology.

The physical topology of a network is the actual geometric layout of workstations. There are several common physical topologies, as described below and as shown in the illustration.

In the bus network topology, every workstation is connected to a main cable called the bus. Therefore, in effect, each workstation is directly connected to every other workstation in the network.

In the star network topology, there is a central computer or server to which all the workstations are directly connected. Every workstation is indirectly connected to every other through the central computer.

In the ring network topology, the workstations are connected in a closed loop configuration. Adjacent pairs of workstations are directly connected. Other pairs of workstations are indirectly connected, the data passing through one or more intermediate nodes.

If a Token Ring protocol is used in a star or ring topology, the signal travels in only one direction, carried by a so-called token from node to node.

The mesh network topology employs either of two schemes, called full mesh and partial mesh. In the full mesh topology, each workstation is connected directly to each of the others. In the partial mesh topology, some workstations are connected to all the others, and some are connected only to those other nodes with which they exchange the most data.

The tree network topology uses two or more star networks connected together. The central computers of the star networks are connected to a main bus. Thus, a tree network is a bus network of star networks.

Logical (or signal) topology refers to the nature of the paths the signals follow from node to node. In many instances, the logical topology is the same as the physical topology. But this is not always the case. For example, some networks are physically laid out in a star configuration, but they operate logically as bus or ring networks.

Trauma Interventions

There are two types of trauma — physical and mental. Physical trauma includes the body’s response to serious injury and threat. Mental trauma includes frightening thoughts and painful feelings. They are the mind’s response to serious injury. Mental trauma can produce strong feelings. It can also produce extreme behavior; such as intense fear or helplessness, withdrawal or detachment, lack of concentration, irritability, sleep disturbance, aggression, hyper vigilance (intensely watching for more distressing events), or flashbacks.

A response could be fear. It could be fear that a loved one will be hurt or killed. It is believed that more direct exposure to traumatic events causes greater harm. For instance, in a school shooting, an injured student will probably be more severely affected emotionally than a student who was in another part of the building. However, second-hand exposure to violence can also be traumatic. This includes witnessing violence such as seeing or hearing about death and destruction after a building is bombed or a plane crashes.

Causes of emotional or psychological trauma

An event will most likely lead to emotional or psychological trauma if:

  • It happened unexpectedly.
  • You were unprepared for it.
  • You felt powerless to prevent it.
  • It happened repeatedly.
  • Someone was intentionally cruel.
  • It happened in childhood.

Emotional and psychological trauma can be caused by single-blow, one-time events, such as a horrible accident, a natural disaster, or a violent attack. Trauma can also stem from ongoing, relentless stress, such as living in a crime-ridden neighborhood or struggling with cancer.

Symptoms of emotional and psychological trauma

Following a traumatic event, or repeated trauma, people react in different ways, experiencing a wide range of physical and emotional reactions. There is no “right” or “wrong” way to think, feel, or respond to trauma, so don’t judge your own reactions or those of other people. Your responses are NORMAL reactions to ABNORMAL events.

Emotional and psychological symptoms of trauma:

  • Shock, denial, or disbelief
  • Anger, irritability, mood swings
  • Guilt, shame, self-blame
  • Feeling sad or hopeless
  • Confusion, difficulty concentrating
  • Anxiety and fear
  • Withdrawing from others
  • Feeling disconnected or numb
  • Insomnia or nightmares
  • Being startled easily
  • Racing heartbeat
  • Aches and pains
  • Fatigue
  • Difficulty concentrating
  • Edginess and agitation
  • Muscle tension

Physical symptoms of trauma:

These symptoms and feelings typically last from a few days to a few months, gradually fading as you process the trauma. But even when you’re feeling better, you may be troubled from time to time by painful memories or emotions–especially in response to triggers such as an anniversary of the event or an image, sound, or situation that reminds you of the traumatic experience.

Helping someone deal with emotional and psychological trauma

It can be difficult to know how to help a loved one who’s suffered a traumatic or distressing experience, but your support can be a crucial factor in their recovery.

  • Be patient and understanding. Healing from emotional or psychological trauma takes time. Be patient with the pace of recovery and remember that everyone’s response to trauma is different. Don’t judge your loved one’s reaction against your own response or anyone else’s.
  • Offer practical support to help your loved one get back into a normal routine. That may mean help with collecting groceries or housework, for example, or simply being available to talk or listen.
  • Don’t pressure your loved one into talking but be available when they want to talk. Some trauma survivors find it difficult to talk about what happened. Don’t force your loved one to open up but let them know you are there to listen whenever they feel ready.
  • Help your loved one to socialize and relax. Encourage them to participate in physical exercise, seek out friends, and pursue hobbies and other activities that bring them pleasure. Take a fitness class together or set a regular lunch date with friends.
  • Don’t take the trauma symptoms personally. Your loved one may become angry, irritable, withdrawn, or emotionally distant. Remember that this is a result of the trauma and may not have anything to do with you or your relationship.

One way of processing and dealing with Trauma is with EMDR.

What is EMDR?

EMDR (Eye Movement Desensitization and Reprocessing) incorporates elements of cognitive-behavioral therapy with bilateral eye movements or other forms of rhythmic, left right stimulation. One of the key elements of EMDR is “dual stimulation.” During treatment, you are asked to think or talk about memories, triggers, and painful emotions while simultaneously focusing on your therapist’s moving finger or another form of bilateral stimuli.

In a typical EMDR therapy session, you focus on traumatic memories and associated negative emotions and beliefs while tracking your therapist’s moving finger with your eyes as it moves back and forth across your field of vision.

Other forms of external stimuli that may be used in EMDR therapy include bilateral tactile sensations and sounds (e.g. alternating hand taps or a chime that pans back and forth from ear to ear).

How does EMDR therapy work?

At the time of a traumatic event, strong emotions interfere with our ability to completely process the experience and one moment becomes “frozen in time.” Recalling the traumatic event may feel as though the person is reliving the event all over again because the images, smells, sounds and feelings are still there and can be triggered in the present. When activated, these memories cause a negative impact on our daily functioning and interfere with the way we see ourselves and our world, and how we relate to others. EMDR therapy appears to directly affect the brain, “unfreezing” the traumatic memories, allowing you to resolve them. Over time the disturbing memory and associated beliefs, feelings, sensations become “digested” or worked through until you are able to think about the event without reliving it. The memory is still there, but it is less upsetting.

The exact mechanism for the effectiveness of EMDR is yet unknown. It appears that using rapid eye movements relieves the anxiety associated with the trauma so that the original event can be examined from a more detached perspective, somewhat like watching a movie of what happened. This enables you to access positive ways of reframing the original trauma and to release the body’s stored negative emotional charges around it (desensitization).

Some experts have noted that the eye movements involved in EMDR might be similar to what occurs naturally during dreaming or REM (rapid eye movement) sleep. It may be thought of as a physiologically-based therapy that allows a person to see material in a new and less distressing way. Others believe it reactivates parts of the brain that were “shut down” as a coping mechanism. In this way cognitive reorganizing takes place, allowing the negative, painful emotions to give way to more resolved, empowered feelings.

ABB has supplied the highest quality, most reliable components to data centers for many years and we recently initiated a concerted approach toward expanding our data center capabilities. We have focused resources through a dedicated data center industry sector initiative and accompanying growth strategy that has led to:

Our new facilities are really UNIQUE among other urgent cares. They are extra clean, spacious and well maintained. There is no other urgent care facility with similar warm, comfortable, clean and spacious waiting room.

All of our clinics are designed to be a “mini-ER”. Our exam rooms are up to date and each clinic is equipped with EKG, Cardiac/vitals Monitor and all other necessary tools for minor emergencies.

We use non-disposable surgical tools (much better than the ER disposable Tools).We have an autoclave in every facility to sterilize these equipment.

We have used an Electronic Medical Records system (EMR) since 2008 and we use electronic prescription system in all of our clinics. Our EMR system is Aprima. This offers a great advantage to our patients in terms of storing and reproducing the medical records. In addition, you do not have to worry about paper registration or about telling us your medications and allergies more than once.

We have been using the Digital X-Rays reading technology at all of our clinics. The FUJI Digital X-Rays reader offers greater accuracy in reading X-Rays, since it allows the doctor to magnify the image, change the contrast and rotate. It makes the interpretation of X-rays much easier. Patients are usually shown their X-rays immediately after it is done. Our X-rays are reviewed usually by a board certified radiologist within 24-48 hours from the time it is done (Fridays and weekends are read on Monday).

The Woodhaven and Garden city sites have a full service pharmacy attached to the clinic.

    Just returned from three weeks at Extended stay. this is our third time at this hotel. The rooms are neat and tidy. House keeping was OK linen changed weekly,towels every few days.The thing for Brit holiday makers to know is that this is not a traditional two week holiday venue that we are used to in europe. lots of people stay just a few days and move on or some are there working in the area and may stay several weeks or are at conventions so people are always changing although you don’t notice it. It is a perfect location for I drive with lots to do in walking distance it is also close to the two different routes for the trolley ride which is good breakfast is ok and covered in the other reviews

    The Extended Stay in Orlando is a delightful hotel offering rooms consisting of two double beds an open plan kitchen/dining area. Off the main room is a separate washroom w/wardrobe bathroom area within the room with bath/shower. Also within the main room is a sofa (pull out bed). A dining table/chairs. Kitchen contains fridge, dishwasher, microwave and hob. Also standard is a Tv and safety deposit box. Parking is free and swimming/ jacuzzi are modern and adequate. Staff are friendly and helpful. Really enjoyed our time here.

    One in five surgical weight-loss patients take prescription opioids seven years after surgery

    While the proportion of adults with severe obesity using prescription opioids initially declines in the months after bariatric surgery, it increases within a matter of years, eventually surpassing pre-surgery rates of patients using the potentially addictive pain medications, according to new research from a National Institutes of Health (NIH)-funded multicenter study led by the University of Pittsburgh Graduate School of Public Health.

    The findings—which come from one of the largest, longest-running studies of adults who underwent weight-loss surgery —indicate that improvements in obesity-related pain gained through bariatric surgery are not sufficient to counter the need for pain relief in the years following the procedure. Results are reported today in Surgery for Obesity and Related Diseases. the journal of the American Society for Metabolic and Bariatric Surgery.

    “Almost half of patients reporting opioid use at the time of surgery reported no such use following surgery. However, among the much larger group of patients who did not report opioid use pre-surgery, opioid use gradually increased throughout seven years of follow-up,” said lead author Wendy C. King, Ph.D. associate professor of epidemiology at Pitt Public Health. “Thus, post-surgery initiation of opioid use explains this phenomenon.”

    The U.S. Centers for Disease Control and Prevention recently presented an evidence-based guideline stating that opioids should not routinely be used to manage chronic pain.

    “Our nation is in an epidemic of opioid abuse. addiction and overdose. Recent reports have suggested that bariatric surgery patients are at elevated risk of chronic opioid use,” said co-author Anita P. Courcoulas, M.D. M.P.H. chief of minimally invasive bariatric and general surgery at UPMC. “Our study does not prove that bariatric surgery causes an increase in opioid use. However, it does demonstrate the widespread use of opioids among post-surgical patients, thereby highlighting the need for alternative pain management approaches in this population.”

    Starting in 2006, King and her colleagues followed more than 2,000 patients participating in the NIH-funded Longitudinal Assessment of Bariatric Surgery-2 (LABS-2), a prospective observational study of patients undergoing weight-loss surgery at one of 10 hospitals across the United States.

    About 70 percent of the patients underwent Roux-en-Y gastric bypass (RYGB), a surgical procedure that significantly reduces the size of the stomach and changes connections with the small intestine. The majority of the remaining participants had a less invasive procedure, laparoscopic adjustable gastric banding, where the surgeon inserts an adjustable band around the patient’s stomach, lessening the amount of food the stomach can hold.

    Before surgery, 14.7 percent of the participants reported regularly taking a prescription opioid. Six months after surgery the prevalence decreased to 12.9 percent, but then it rebounded, progressing to 20.3 percent of participants regularly taking opioids seven years after surgery. Among participants who were not taking opioids at the time of surgery, rates increased from 5.8 percent six months after surgery to 14.2 percent seven years later. Hydrocodone was by far the most commonly reported opioid medication. followed by Tramadol and Oxycodone.

    There also was an increase over time in the use of medications typically prescribed for opioid dependence. although use of such medications remained rare, with less than 2 percent of patients using them through the years of follow-up.

    Recently, King and her colleagues reported that undergoing RYGB, compared to gastric banding, was related to increased risk of alcohol use disorder, illicit drug use and treatment for substance use disorder following surgery. However, in this study, the risk of starting or continuing opioid use post-surgery did not differ by surgical procedure.

    Most factors related to substance use disorder that were examined—such as gender, age, income, social support, mental health, smoking, alcohol consumption and illicit drug use—were not related to continued or post-surgery opioid use. However, more pain before surgery, worsening or less of an improvement in pain following surgery, and starting or continuing non-opioid pain-killers were each associated with a higher risk of continuing or starting opioid use after surgery.

    Some researchers have hypothesized that opioid use increases following bariatric surgery due to discontinuation of non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, which are contraindicated post-surgery. However, King and her colleagues found that starting NSAIDs post-surgery was associated with a higher risk of also starting to take opioids, while stopping NSAIDs was associated with lower risk of taking opioids.

    “This likely reflects that opioid and non-opioid pain-relief medications often are used in tandem, versus as alternatives to each other,” said King. “There is an urgent need for research into adequate alternatives to opioids for the long-term management of chronic pain following weight-loss surgery .”

    Aspect Software Files Bankruptcy to Cut Debt Load

    Aspect Software, a provider of software systems and equipment for call centers, has filed bankruptcy so it can reduce a $795 million debt burden that has limited its ability to invest in next-generation products and services.

    In a Chapter 11 petition filed Wednesday, Aspect said a capital restructuring plan backed by its creditors would eliminate $320 million of second-lien debt and convert $60 million of first-lien debt into 100% of the reorganized company’s equity.

    “By resetting our capital structure and dramatically improving our balance sheet, we will be well-positioned to compete over the long-term while continuing to accelerate investments in our product and service capabilities,” Aspect CEO Stew Bloom said in a news release.

    Over the past three years, Aspect has invested $160 million in acquisitions, technology agreements and partnerships. It serves 2,200 call centers in more than 70 countries, generating annual sales of more than $400 million.

    But in a bankruptcy court declaration. Bloom said staying on the “cutting edge of software solutions has been especially challenging given Aspect’s highly-leveraged capital structure,” which includes annual cash interest payments of approximately $34 million on about $475 million of first lien secured debt and about $320 million of second lien secured debt.

    The chief executive noted that, as with other call-center software companies, Aspect has been shifting to cloud-based solutions, but has incurred downward revenue pressure in part because of the high costs of adapting to another technology platform.

    Cloud revenue is also recognized over the period of the subscription contract, resulting in a deferred revenue backlog of more than $100 million, Bloom said.

    The planned restructuring will bring Aspect out of Chapter 11 under the control of its lenders. Aspect said it expected the reorganization to be completed within 105 days.
