Meaningful use stage 3 is the third phase of the meaningful use EHR incentive program. The Centers for Medicare Medicaid Services (CMS ) and the Office of the National Coordinator for Health IT (ONC ) published the final rule on meaningful use stage 3 on October 6, 2015.

Despite the requirements set by stage 3, there are important upcoming changes through a new law called MACRA — or the Medicare Access and CHIP (Children’s Health Insurance Program) Reauthorization Act. MACRA will eventually modify the meaningful use program as a means to push forward with value-based reimbursement.

The meaningful use program, as it has existed, will now be effectively over and replaced with something better, said Andy Slavitt, the acting administrator of CMS, in January 2016.

As things stand, in summer 2016, meaningful use stage 3 will be optional for providers in 2017 and mandatory for all participants in 2018. Objectives and measures for all providers, regardless of prior participation, are finalized for the 2018 reporting period with this rule.

Along with meaningful use stage 1 and meaningful use stage 2. the third list of criteria and requirements mandates that all hospitals and eligible healthcare professionals use certified electronic health record (EHR ) systems.

Meaningful use program requirements

Meaningful use stage 3 includes all of the requirements that physicians must meet to receive their incentives and avoid any penalties. In this program, physicians must meet eight overall objectives, in contrast to the earlier requirement for them to choose from a core menu of options. The objectives cover the following areas:

  1. Protected health information (PHI): Eligible physicians must attest to conducting a security risk analysis to assess vulnerabilities to PHI that could lead to data breaches. In addition to the fact that the Health Insurance Portability and Accountability Act (HIPAA ) requires practices to perform risk analysis and other security audits. the requirements attached to meaningful use objectives make it a must-have in order to receive incentives.
  2. Electronic prescribing: Eligible physicians are required to have more than 80% of their permissible prescriptions queried for drug formulary and transmitted to pharmacies electronically.
  3. Clinical decision support (CDS): For this objective, there are two different measures available for eligible physicians. The first measure covers implementing five CDS interventions. The second measure relates to the active use of drug-drug and drug-allergy interaction checks during the reporting period, which are available within a certified EHR platform.
  4. Computerized provider order entry (CPOE): Eligible physicians are required, under this objective, to meet three different measures for medication, lab and diagnostic imaging orders.
  5. Patient electronic access: To help encourage patient engagement, meaningful use stage 3 includes an objective in which eligible physicians must provide access to EHRs to more than 80% of patients, with the option to view and download the records. In addition, eligible physicians must offer the option to receive educational data to more than 35% of their patients.
  6. Coordination of care through patient engagement. The measures included in this objective encourage patients to actively engage in their care by necessitating physicians to educate them on and offer capabilities to view patient health data. The measures in this objective cover three different aspects. The first measure requires physicians to have more than 25% of patients interact with their EHR. The second measure requires that more than 35% of patients receive a secure digital communication from a care provider. The third measure focuses on encouraging the collection of patient generated health data from fitness trackers or wearable devices from more than 15% of patients. Eligible providers must attest to all three measures, but meet the thresholds for two of the three.
  7. Health information exchange (HIE): The measures included in this meaningful use objective encourage interoperability. The first measure requires that more than 50% of care transition and referrals include the exchange of care records. such as continuity of care documents (CCD ), electronically. The second measure requires physicians who are seeing a patient for the first time to receive care documents electronically from a secondary source more than 40% of the time. The final measure requires physicians to use e-prescribing services to reconcile medication lists from online sources with their own for more than 80% of new patients they see. Eligible providers must attest to all three measures, but meet the thresholds for two of the three.
  8. Public health and clinical data registry reporting: In this objective, providers must choose three out of five available EHR reporting destinations to which they will submit data periodically. Reporting options include an immunization registry, syndromic surveillance, cases, a public health registry and a clinical data registry.

Stage 3 also promotes the use of APIs to bridge the gaps between health IT systems and to provide increased data access.

Meaningful use and MACRA

MACRA is federal legislation signed into law on April 16, 2015 that establishes ways to pay physicians for caring for Medicare beneficiaries and includes funding for technical assistance for providers and related measure development. The law also sets requirements for data sharing .

More EHR incentive program updates

It is important to note that in July 2016, CMS published an outline describing changes that it proposed for the Hospital Outpatient Prospective Payment System (OPPS) for 2017. In the document, CMS requested that Congress enact changes to the meaningful use program, including:

  • Reduction of the EHR reporting period from a full year to 90 days.
  • The elimination of CDS and CPOE objectives.
  • Reduction of the thresholds for a subset of the remaining objectives and measures in meaningful use stage 2 for 2017 and meaningful use stage 3 for 2017-2018 for eligible and critical access hospitals.
  • Flexibility for those who have not successfully demonstrated meaningful use in a prior year, enabling them to attest to a modified stage 2 by October 1, 2017.

Lawmakers must formally vote in these changes to meaningful use in order for them to take effect.

Despite the recent meaningful use stage 3 final ruling release and proposed changes, physicians in 2017 will have access to a newly introduced program called Advancing Care Information (ACI) with the Medicare Incentive Payment System (MIPS), which are both part of MACRA. Under the new law, ACI will only affect Medicare physicians’ offices and not Medicare hospitals or Medicaid programs. These new programs are designed to offer flexibility such as:

  • Reduced number of measures to meet.
  • Elimination of CDS and CPOE.
  • More emphasis on interoperability and health information exchange .
  • Multiple options to receive incentives for physicians.
  • Exception for certain physicians from reporting when EHR technology is less applicable.

The recent proposed changes to meaningful use rules have received praise from a number of different medical associations throughout the nation, but it remains to be seen how physicians will navigate the different programs and requirements they face under each one.

Steps to Be a Coding Specialist

What do I need to do to be a certified coding specialist?

Step 1: Pursue a Postsecondary Education in Medical Billing and Coding

Many coding positions seek individuals with an associate’s degree in medical billing and coding. Courses provide knowledge in medical terminology, reimbursement methods, pathophysiology and commonly used coding systems, including CPT and ICD-9. Students are also taught the details of the Health Insurance Portability and Accountability Act (HIPAA) as well.

Step 2: Gain Work Experience

While work experience is not a requirement to become certified, obtaining 2-3 years of work experience is also highly recommended. Hospitals, clinics, physicians’ offices and insurance companies are common work environments for medical coders. Individuals enrolled in a degree program also have the opportunity to enroll in an internship to gain hands-on training and some recommended work experience.

Step 3: Pursue Certification

The AHIMA oversees the certification exam for aspiring coding specialists. It offers several certification designations, including the Certified Coding Specialist (CCS) and the Certified Coding Specialist-Physician-based (CCS-P). The CCS is geared towards hospital-based coding professionals. Both tests consist of two parts, including multiple-choice questions and patient-case scenarios for medical coding. Both the CCS and the CCS-P exams test on ICD-9 and CPT coding systems. The CCS-P also includes testing on the Healthcare Common Procedure Coding System (HCPCS).

Step 4: Take Continuing Education Courses and Renew Certification

The BLS reported that health information technicians (which include certified coding technicians) need to regularly renew their certification and take continuing education classes. The AHIMA offers seminars, conferences and webinars that provide beneficial experiences for certified coding specialists and meet these requirements. CCSs and CCS-Ps must complete at least 30 continuing education units (CEUs) like these every certification cycle, as well as pay a renewal fee. CEUs are completed through college courses.

Step 5: Consider Pursuing a Bachelor’s Degree

Coding specialists looking to pursue promotional opportunities may need a bachelor’s degree in health administration or a related field, so coding specialists should keep that option open to them if they wish to advance.

    People often ask What s the difference between the standard, free version of Google Analytics and what you get with Google Analytics 360? Well, we decided to put together a handy comparison one-sheeter to help clarify the differences. Simply enter your email below to get the download link.

    The main differences between the standard and enterprise versions of Google Analytics deal with the volume of and sources of data. Here s some elaboration on the areas in the comparison chart above.

    More Data Collection

    Both versions of Google Analytics enable you to collect data from websites, mobile apps for iOS and Android, and from custom data sources via the Measurement Protocol. GA360 has much higher limits on the amount of data that can be collected, with a tiered set of limits starting much much higher!

    It also has additional slots for custom data (custom dimensions metrics) and more flexibility in organizing your data through higher limits on the number of properties and views you can have within an account. (Sorry I can t be more specific on the exact limits on properties views, but they are not public numbers.)

    GA360 customers can also create Roll-up Properties, which allow you to combine data from multiple properties together (great for large organizations with collections of websites to avoid multiple sets of GA code to enable rolling up sites together).

    Importing Data and Product Integrations

    GA has a variety of integrations with Google advertising platforms you re probably already familiar with the AdWords integration. This is a two-way integration which both imports data from AdWords to Analytics (bringing in cost and impression data), as well as allows the export of remarketing audiences based on Analytics data to AdWords.

    GA360 includes these features for AdWords as well as DoubleClick platform products DoubleClick Campaign Manager (formerly DoubleClick for Advertisers), DoubleClick Bid Manager, and DoubleClick for Publishers.

    In addition to those direct integrations, GA allows importing data from custom data sources, including advertising cost data, product or content data, and more. GA360 enhances this with a feature called query-time import, meaning we can look at our historic data in the interface and view the new imported data, even if we ve changed or updated the import after the data was originally collected.

    Reporting Improvements

    If you were hoping that GA360 has some super-secret set of reports you ve never seen in the standard version of Google Analytics, you won t be blown away here. The table of contents of reports in GA360 is pretty much the same (although with expanded opportunities, given the additional data collection and import capabilities described above).

    One additional enhancement is in the area of attribution modeling. The standard attribution modeling tool can give some formula-based models for attributing conversions to channels (like first-touch, last-touch, time decay, etc.). But which of those is the right model? GA360 includes a data driven attribution model, which actually uses an algorithm on your data to understand where different channels make the most impact.

    There are also enhanced custom funnel reporting options for GA360 clients, enabling better flow reporting for on-site actions across users and sessions. This is great for visualizing multiple conversion paths through your website!

    Combating Sampling

    Just like GA360 allows you to collect more data, it also is designed to deal with large volumes of data better in reporting. As a reminder, when you request an ad-hoc report (a report that Google Analytics hasn t already pre-calculated, such as a custom report or a segment applied to a standard report), if the data exceeds a certain number of sessions, Google Analytics employs sampling to achieve answers quickly.

    GA360 raises the limits for sampling to be much (much!) higher, so you re less likely to run into sampling as you explore your data. Another huge benefit, they ve moved sampling to the View level, which means you can help reduce sampling in the interface by adding View filters, no website changes required. If you are still hitting sampling, GA360 also has a number of additional tools to prevent or eliminate sampling, like Custom Tables (to pre-aggregate commonly-used custom reports or other sampled data) and the ability to generate completely unsampled reports.     APIs and Google BigQuery

    Google Analytics has a variety of APIs for reporting and configuration. Again, the differences here are mainly in retrieving large volumes of data such as unsampled reports. Implementing Custom Tables will help to reduce sampling, even when pulling the data out of Google Analytics to use in another platform.

    GA360 also allows hit-level data to be automatically passed from Google Analytics into Google BigQuery, Google s big data storage and querying tool. This allows more complex analysis or can act as a conduit for exporting more granular data into a data warehouse. For companies looking to answer challenging questions with their data, like defining the customer journey, looking at user behavior across sessions, or joining together external data sources, Google BigQuery is the tool you ll be most excited about.

    Google Attribution will include a variety of attribution models. Unfortunately Google hasn t publicly released much of the detail around this so I can t say much right now about Attribution and how it compares with and integrates with Analytics/Analytics 360 at this point. The IRS and Congress have provided for this by instituting Payment Plans. The key is to make sure it is a Livable Payment Plan that you can afford and continue to pay without defaulting. You do not want to get coerced into a Payment Plan by a revenue officer in which the payments are so high that you will not be able to keep them up in the long run.     Maine Registry of Certified Nursing Assistants Direct Care Workers (CNA Registry)

    Notice. To report complaints of abuse,neglect, or misappropriation, please call the complaint line at 1 (800) 383-2441.

    To be placed on the Maine CNA Registry, you must be able to provide official documentation of formal CNA training that meets Maine s Rules and Regulations, whether for immediate placement on the Registry or for completing the Bridge Testing Process. If you cannot meet either of these, you will need to successfully complete an approved 180-hour CNA training course before you can be placed on the CNA Registry.

    Maine and federal laws allow you to work as a CNA for up to 4 months in a long-term care facility while you are fulfilling the requirements for getting placed on the CNA Registry. Employers can only hire CNAs who are listed on the Maine CNA Registry and who are in good standing.

    The CNA Registry lists CNAs who, through training, competency testing and clinical work experience, meet the federal and state requirements to work as a Certified Nursing Assistant in the state of Maine.

    CNA Registry Functions

    • Respond to inquiries
      • Respond to inquiries from the public, health care providers, individual CNAs and other state CNA Registries;
    • Protect the public
      • Enhance public safety by annotating information related to known criminal convictions as required by Maine law and substantiated complaints as required by Federal law;

    Substance abuse counselors provide therapy to individuals struggling with issues related to alcohol and/or drug use.     There are no federal regulations stating that individuals are required to have a high school diploma prior to attending nurse aide classes but some states such as Florida require a high school diploma or its equivalent prior to testing if you are under 18 years old and have not attended a training program. A program coordinator at the school or nursing facility which is offering CNA training classes may tell you if a high school diploma or equivalent (GED) is required in order to enroll in the nurse aide program. Some facilities do not require a diploma as long as you have completed a 10th grade high school education. In the state of Texas a high school diploma or a GED is not necessary in order to attend and complete a nursing assistant program.

    There is a wide disparity between CNA training requirements among the states. The difference lies in the mandatory number of total training hours necessary to complete state-approved CNA classes .

    While according to federal law each nursing assistant course should contain a minimum of seventy-five (75) instruction and clinical hours, individual states have the liberty to mandate extra training requirements for certified nursing assistants. The majority of states exceed the threshold federal minimum requirements for nurse aide training programs because they are considered insufficient to result in an optimal and safe level of care for clients.

    A review of nationwide state requirements for nurse assistant courses has found that the following states have mandated highest number of CNA program hours: Maine with a minimum of 90 hours of theory, 20 hours of laboratory skills and 70 hours of clinical instruction for a total of 180 hours; California with one hundred and sixty (160) training hours; Delaware and Oregon each requiring one hundred and fifty (150) hours; followed by the state of Alaska with one hundred and forty (140) hours; Virginia, Arizona, and West Virginia have a minimum of one hundred and twenty (120) training hours for state-approved classes.

One could say that anyone who is paying back the IRS for past due taxes is experiencing an economic hardship in that there are always more pleasurable things to do with one’s money. Unfortunately, the IRS doesn’t see it that way.

    In order to set up a Payment Plan for IRS back taxes and stop collection action. a taxpayer must demonstrate to the IRS that paying all that is due or making excessively large payments to the IRS would result in an undue economic hardship to the taxpayer. This is done by showing that payment of what the IRS is demanding would result in the taxpayer taking food off their family’s table or doing without some other necessity of life to give the money to the IRS instead. Fortunately for many people, this is not that difficult to demonstrate. Other times it is a bit harder, but there is almost always some type of resolution available.     The main benefit of setting up installment payments with the IRS is that it will put you back in compliance and you will no longer have to worry about the IRS knocking on your door.     To view this in the proper perspective, you must look at it from the way the IRS Collections folks do. You see, they have all the power and hold most of the cards. What you have going for you are some reasonable outs provided by Congress and the IRS to keep the system efficient and within the bounds of reason. It is not efficient for the IRS to spend its time and money trying to collect a large sum of money from somebody who has no way of paying so large of an amount. The IRS and Congress have provided for this by instituting Payment Plans. The key is to make sure it is a Livable Payment Plan that you can afford and continue to pay without defaulting. This is what National Tax Relief makes sure of. You do not want to get coerced into a Payment Plan by a revenue officer in which the payments are so high that you will not be able to keep them up in the long run. If you get into a Payment Plan and later default, then you will be in bigger trouble than before.

    You must show the IRS on the proper forms that your allowable expenses are equal to, or only slightly less than, your income. This is how you show the IRS that you can pay them only an “affordable and livable amount” without “undue economic hardship. Your allowable expenses are what the IRS considers the necessities of life. In general, they are: food, clothing, housing, utilities, transportation, medical, insurance and work-related expenditures.     The following eligibility criteria must be met at the time of application and examination.

    • Current, active, unencumbered license as a registered nurse in the US, its territories or Canada
    • Graduate degree from accredited NP program with concentration in oncology
    • 500 hours supervised clinical practice as an adult oncology nurse practitioner obtained within and/or following the graduate program within the past five years
    • One graduate level oncology course of at least 2 credits or 30 hours oncology Continuing Education within the past five years
      • Current, active, unencumbered license as a registered nurse in the US, its territories or Canada
      • Graduate degree from accredited nursing practice program with concentration in adult (primary or acute), family (across lifespan), gerontology, or women’s health
      • 1000 hours practice as an adult oncology nurse practitioner obtained within and/or following the graduate program within the past five years
      • One graduate level oncology course of at least 2 credits or 30 hours oncology Continuing Education within the past five years

    Applying for certification

    The certification test is a three-hour, 165 multiple-choice test is based on the AOCNP ® Test Content Outline (Test Blueprint). The Test Content Outline is based on the results of a role delineation study of advanced oncology nursing practice completed in 2014.

    The AOCNP ® Test Content Outline includes eleven major subject areas, each with a percentage assigned to it. Many nurses who have passed the test tell us the Test Content Outline was essential in helping them know what areas to study. Remember, only generic drug names are used on ONCC tests. Once you pass the test, your certification is valid for four years.

    Note: If you’re eligible to test, you’ll receive an Authorization to Test (ATT) within 4 weeks of applying. This ATT will open an almost-immediate 90-day testing window. You must take the test within that 90-day window.

    Maintaining your certification

    You should be very proud that you’ve passed the certification test! In order to keep your certification, you need to earn points for certification renewal by completing Continuing Education, Continuing Medical Education, Academic Education credits, or have work published in publications or give presentations. Follow this step-by-step guide to maintain your certification.

    1. Determine where to log your information.
    If you passed a test in 2012 or later, use your Learning Plan on LearningBuilder. If you renewed via ONC-PRO in 2012 to 2015, you must first take an assessment to determine how many points you’ll need. After you assess, you will be notified that your Learning Plan is available for tracking your points.

    2. Plan your professional development.
    Your assessment results or test score report will tell you the subject areas and number of professional development points – ranging from 25 to 100 – you’ll need to renew your certification.

    3. Select the necessary subjects.
    Use your score report or assessment results to see which subject areas you need further professional development. Then, refer back to the current Test Content Outline (Test Blueprint) to help you identify whether a program is in the subject area you need.

    4. Enter your points on your Learning Plan on LearningBuilder.
    Make sure you keep track of all of your information, as you earn the points, in your Learning Plan on the new online tracking tool called LearningBuilder.

    5. Keep all documentation.
    We strongly urge you to record your points as they are earned. Keep copies of all grade reports, copies of publications, program descriptions, educational objectives, and outlines, in case ONCC asks to review them.

    The following eligibility criteria must be met at the time of application.

    1. Current AOCNP ® certification
    2. Current, active, unencumbered license as a registered nurse in the US, its territories or Canada
    3. A minimum of 1,000 hours practice as a nurse practitioner in adult oncology within the four years (48 months) prior to application.

    What is Advanced Practice? ONCC uses the definition of Advanced Practice nursing provided by the American Nurses Association.

    Renewing your certification

    All ONCC certifications are valid for four years. Renewal candidates must have a current, active, unencumbered RN license. Three renewal options are available in 2016:

    • Option 1: Practice hours + Professional Development (ILNA Points)
    • Option 2: Practice hours + successful testing
    • Option 3: ILNA Points + successful testing

    Candidates Who Are Renewing by Option 3 (ILNA Points + Testing)

    Candidates who are using renewal Option 3 (ILNA Points + Testing) should submit both the application and Learning Plans by the test application deadline date. Option 3 candidates must submit a test application and Learning Plan at the same time.

    Unfortunately, working as a certified nursing assistant (CNA) won t make you rich. If you can look past the salary, a CNA job can be your first foot in the medical field door and often allows you to create close relationships with patients.

    According to The U.S Bureau of Labor Statistics (BLS) Occupational Outlook Handbook, 2016-17, the median expected annual salary for certified nursing assistant is $25,090. Actual salaries may vary greatly based on specialization within the field, location, years of experience and a variety of other factors.

    What s my earning potential?

    Like other nursing jobs, a certified nursing assistant salary if often related to where the job is. According to the BLS, the top 10 percent of nursing assistants earned more than $36,170.

    How much competition will I face for a job?

    Essential Information

    Home health aides are responsible for administering medication, performing personal care tasks and general housekeeping and maintenance tasks for patients. Home health agencies require home health aides to pass state-regulated training programs before working with patients. Home health aides can meet these requirements by enrolling in home health aide courses or programs offered by local vocational or nursing schools. Training programs vary by state and employer, but most require aides to reach a minimum number of training hours in the classroom and in hands-on care, in addition to passing a competency exam. Additionally, home health aides can seek voluntary certification to demonstrate proficiency in home health care.

    Home Health Aide Certificate Program

    Continuing Education
