We are thankful for you.

Would you recommend Heartland Hospice Services?

    by Dana D. (18 июля 2016 г.)

I volunteered to pray with patients but was never actually placed for that position. When the last volunteer coordinator left, the new coordinator never bothered to reach out and introduce himself or get to know me as a volunteer. I was really disappointed, especially with the good rapport I felt I had with the previous one. I feel like I wasted more time than actually helping people.

    by Laura B. (17 сентября 2013 г.)

I have been volunteering my time since my early 20’s and I volunteer for other organizations now, but I have never felt more appreciated or welcomed as a volunteer than Marisol, Dondra, and the nurses working for Heartland Hospice have made me feel. They really let you know that time is precious and you giving yours is the most selfless thing a person can do. I’m a friendly visitor for a very sweet lady and when she told me that she was glad she met me, I knew I was there for a reason. Volunteering with this organization is very fulfilling and you won’t be disappointed so if you’re looking to lend a hand or a compassionate ear, this is the place to do it.

    by Natalie B. (12 сентября 2013 г.)

Volunteering at Heartland Hospice has allowed me to work around people who are so passionate about what they do in caring for their hospice patients that they see everyday. Even on slow days there is no lack of things to do around the office. I would recommend this organization to anyone who is interested in working for a non-profit or even for someone who is looking for an opportunity to volunteer their time at. Thank you Heartland Hospice!

    by jihane s. (17 августа 2012 г.)

It’s for a very goog purpose. me and my friends have decided to take some time out of our time and help someone thats needs help.

    by Lyn G. (6 октября 2011 г.)

Volunteering with Heartland Hospice is not JUST gaining community service hours, or keeping busy during your summer vacation- it’s a whole lot more than that. Even though I’m going to school to become a nurse, anyone can volunteer. Heartland Hospice gives recognition to their volunteers and they consider you a part of the team. Being a volunteer someone will always say thanks for volunteering with us or it’s so good to have you here. Nothing is more welcoming than making people happy, whether it’s the team or our hospice patients. Spending time with our patients lets them know that we care and we want them to feel enriched. We want our patients to know that we’re not there just to do it we’re there because we want to be. We show compassion and do what it takes to make a positive difference in their lives. At the end of the day, it’s a breath of fresh air for them, and for me!

    What Makes Us Different

    • You don’t have to give up hope to get hospice. Many patients and the loved ones who care for them say they wish hospice care had started sooner.
    • Treatments that provide our patients with comfort and relief may continue, uninterrupted, even after we’ve started providing hospice care.
    • We don’t require that advance directive choices be made such as a “do not resuscitate” order (DNR) or living will to begin providing hospice care.
    • When you need care, we’re ready to provide it. Any day of the year, any time of the day, we can initiate hospice care for you or your loved one.
    • Our heart stretches across the nation. At any given time, Heartland provides comforting care to thousands of patients in communities and neighborhoods across the country.
    • We’re fully accredited by Accreditation Commission for Health Care (ACHC). We’ve chosen to follow their additional requirements in order to help advance the highest standards of community-based care.
    • Wherever you are in your health care journey, we’re there to meet you.
    • As mutual care providers, we work closely with your health care professionals to make sure specific protocols are followed closely.

Heartland at ProMedica, a collaboration between ProMedica and HCR ManorCare, is located on the ProMedica Flower campus. Heartland is dedicated to providing individualized, quality post-hospital care you need in a warm and comfortable environment. Most likely you are coming directly from the hospital and in need of a high level of care. Our clinical and therapy team is experienced in providing specialized care focused on your needs, interests and ability. This commitment results in a smoother and safer transition from the hospital to a higher level of ability.

You should look for a setting that specializes in providing vital medical care and rehabilitation in a warm and caring environment. We offer a range of services to meet your health care needs.

Email or call us for more information or to schedule a tour.

    MedBridge Rehab

A distinct, post-hospital unit within our center was specially designed for your short stay which requires more intense medical and rehabilitation care. Before being recognized with the MedBridge certification, we had to meet stringent clinical, rehabilitative and outcomes criteria. Our multidisciplinary approach to recovery may benefit you as you recover from:

    • Orthopedic injuries
    • Total joint replacements
    • Cardiac recovery and complications
    • Stroke and neurological recovery

At Heartland we also offer long-term nursing home care combined with quality caregiving. A customized individualized service plan is developed with the involvement of your physician, the care team, you and your family and adjusted as necessary. This comprehensive approach is designed to address not only your health needs, but functional capabilities, chronic illness and desired outcomes in a warm environment with amenities and activities to meet your needs such as:

    • Attractive and comfortable rooms
    • Homemade meals and snacks supervised by a registered dietitian
    • Elegantly appointed dining room
    • Special arrangements for private functions
    • Inviting lounges and activity areas
    • Planned activities, entertainment and excursions
    • Personal laundry and linen service

    The Expertise You Want

Heartland’s patient and resident care team is experienced in providing a breadth and depth of services for medically complex, functionally limited and/or cognitively impaired patients. We offer a proactive approach in:

    • Falls: focusing on minimizing fall risk factors and fall related injuries while continuing to promote quality of life
    • Mental wellness: helping boost patients’ and residents’ sense of well-being by assisting those who may have issues that arise when dealing with age, chronic illness, impairment or disability which may cause problematic depression or sadness.
    • Pain management intervention: offering pain and symptom management and interventions for many older adults dealing with the daily occurrence of uncontrolled pain to promote comfort and enhance residents’ ability to perform their daily activities.
    • Wound management: providing the special care our thinner and frailer skin may require as we age while focusing on the entire patient including nutrition, hydration and mobility.

    Impressive Results

At Heartland, our primary focus is caring for patients with complex recovery and rehabilitation needs in a homelike setting. Our specialized therapy team has years of combined experience with some very impressive results *

    * Statistics represent patients admitted for post-acute care for 2015 for centers nationwide

    Making the Decision

Whether you are looking for short-term rehabilitation stay before returning home or for a residential setting for a longer stay, we understand the challenges faced in making this decision. When possible, don’t wait for a crisis to force your hand because it is better to make that choice when not under the added stress and strain during an emergency. When evaluating communities, it is reassuring to know you have our expansive services across the continuum for support.

We understand the physical and emotional challenges of recovery, and how important it is to select the right provider with the experience to meet your needs safely and confidently. Key indicators to review when choosing where to recover includes:

    • Does the facility have experience treating your specific condition?
    • How successfully have their previous patients with your condition recovered?
    • Are the nurses knowledgeable and experienced in caring for patients similar to you?
    • Does the facility have hospital-level therapists and equipment?
    • Are there any systems in place to reduce unnecessary readmissions to the hospital?
    • What activities and events are available to keep you engaged or help you transition back to the community?

At Heartland, our volunteers have the opportunity to enrich the lives of others. This can be through interaction with patients and their family members and sometimes it can be through community events. Heartland Hospice Serving Tide Water held their first hospice awareness 5k run. One of the volunteers, Candice, came up with a super hero theme for the run: Hospice Heroes.

Heartland provided in home nursing PT and OT services for my elderly father. PT AND OT were fine, but the nurses were lacking – a nurse came out when my father had a bad UTI, and did not recommend that he be taken to emergency and the infection turned septic, and dad nearly died. The nurse should have told the family that that was a possibility and even called 911! I do not live in that.

Would you recommend Heartland Hospice – Blue Bell, PA?

    by MARIA W. (15 августа 2014 г.)

Heartland Hospice is great to volunteer with because they provide training and only gave me volunteer opportunities that worked with my schedule, and close to home!

    by mary T. (15 августа 2014 г.)

Heartland Hospice is a wonderful organization helping people in difficult situations. Looking someone in the eye and treating them with dignity and love is an incredibly rewarding experience. It seems like elderly dementia patients are not visited as often as others but I believe that a kind touch and word still touches the soul and makes a difference.

    by Joan E. (26 апреля 2014 г.)

Heartland has provided me with so many opportunities to serve a most diverse group of wonderful folks. The people I visit give to me tenfold anything I do for them. Heartland offers me the gift of connecting with very special people who will always live in my heart. I appreciate the support and encouragement of Lisa, the volunteer coordinator at Heartland. She is always available for my questions or concerns. If you want a powerful and amazing life experience, volunteer your time through Heartland.

1 Some organization health care providers are made up of components that furnish different types of health care or have separate physical locations where health care is furnished. These components and physical locations are not themselves legal entities, but are part of the organization health care provider (which is a legal entity). A covered organization provider may decide that its subparts (if it has any) should have their own NPI numbers. If a subpart conducts any HIPAA standard transactions on its own (e.g. separately from its parent), it must obtain its own NPI number.

Heartland Hospice provides care to patients wherever they call home: in the house they ve lived in for years, a skilled nursing center, an assisted living facility or independent living facility.

Every patient is different. With different challenges, different needs, different family and friends. So the care we give each patient is different.

Our caregivers work with patients to create individualized care plans and set personal goals. They encourage patients to make the right decisions on their own behalf and help fill their lives with meaningful experiences. Pain and symptoms are managed so patients can be as comfortable as possible. Our social workers and spiritual counselors help patients and families share thoughts, hopes, concerns and dreams.

We take our roles in our patients lives very seriously. Their dignity must be preserved, and respect always shown to them.

Everyone experiences a life-long health care journey. We support our patients and their families wherever they are on that journey, and promise to uphold their wishes, goals and values. We call that the Heartland Experience, and we encourage you to read more about our Heartland Promises.

Hear firsthand what our patients have to say about our hospice services. Меньше

Ty: So what is for breakfast? Jack: You want breakfast in bed, sleep in the kitchen.

Lou: Where is Mallory when we actually need her.

Jack: How’s he doing? Amy: Scott was right, this horse is stubborn Jack: Well, at least he’s met his match Amy: Thanks. I think

Ty: Yep, plenty of cows down here. Jack: Shut up. Ty: As a matter of fact I think I see one now! Oh wait, that’s just your horse’s ass in my face.

Mallory: Why can’t people just tell each other what they really think. Ty: That’s probably a good thing. Mallory: No! It isn’t! I mean these days the street are filled with lonely and depressed people who didn’t say the right thing at the right time. they just keep everything to themselves. Ty: Well that’s one problem you don’t have. Mallory: Thank you. But think about it–how many people go through their wholes lives without telling their special someone that they love them. And then when they finally take the leap, it’s too late. I don’t want to ever be that person.

(Ty is trying to fix Jack’s truck)Ty: I wish the old man would just take this heap to a mechanic, but he’s so damn cheap–(gets in truck and turns it on the truck won’t start)Mallory: You’re flooding it. Ty: No I’m not. Mallory: Yes, you are.

(Talking to Jack about how he likes Lisa Stillman)Mallory: Do you like her? I mean, like her like her? Because when she was talking you were acting all funny. And if that’s true, you do you should do something about it because someone your age doesn’t have all the time in the world to be playing hard to get.

Mallory: Do you think she’s pretty? Jack: She’s not half bad looking. She’s got nice eyes, like a husky dog. Mallory: Sweet–that’s just what every girl wants to hear. Jack: Husky’s have nice eyes. Mallory: So do horses but you don’t go around comparing women’s eyes to a horses!

Making the Skilled Nursing Center Decision

Many tell us they don’t have the words to describe their appreciation and simply say “Thanks.” Below are comments and letters from patients and family members sharing their gratitude.

“I am continually impressed by the quality care that Heartland Hospice provides. When I call upon them I know they will arrive in a timely manner, and spend the time with patients and families to assure all needs are addressed; but most importantly I know when I discharge my patients with Heartland Hospice they are in good hands. The passion and professionalism of the Heartland Hospice nurses and other providers is extraordinary. They are a true example of what a hospice should be!”

-Kristin Starr, Palliative APRN. St. Joseph Medical Center, Kansas City, MO

“On behalf of my family, I wanted to say thank you for all that you and your staff did for us while my aunt was a patient of Heartland. Your team definitely showed that they are a shining jewel in the crown of Heartland Hospice.”

-Family member of hospice patient. Baltimore, MD

“Six months ago, Andrea walked through my door and my life was forever changed. I was so frightened to know that hospice would now be taking care of my dad.”

-Family member of hospice patient. Northern Virginia
