Hospice and How It Can Help the COPD Patient AND Caregiver

Hospice can be an invaluable resource for people with end-stage COPD and their overworked, overwhelmed caregivers. But before I get into that, let me apologize. I’m writing 3 posts today, and they’re all a bit on the depressing side, because they deal with death.

I don’t mean to remove all hope for people fairly new to COPD as they read these posts. There are many opportunities for hope and a certain quality of life, even after a diagnosis of COPD. But it’s also a fact that COPD is a chronic, progressive illness for which there is no cure. Eventually, death does become a reality. It can take years, but it will happen at some point. So, while it’s important to live life to the fullest as long as you can, it’s also important for both patient and caregiver to prepare for what is coming in the future.

So, this post is about how hospice can help, and the sooner it is initiated once the end is approaching, the better. Unfortunately, many people do not get referred to hospice (or are not ready to accept it) until far too late. And at that point, there may not be time for hospice staff to do what they are so good at. easing the way to a dignified, peaceful death.

My mom was referred to hospice during a recent hospitalization. and it turned out to be far too late. Although I have said that she was “dying for the last 2 years” more than once, in the end, she deteriorated so rapidly that we weren’t even thinking hospice before she went into the hospital. And then it was too late for her (or us) to reap the benefits, for the most part.

Hospice is a concept of care that provides comfort and support (on several levels) to patients and caregivers when traditional medical care can no longer prevent death from coming on, usually within a few weeks or a few months.

Hospice is a team-oriented approach that neither prolongs life nor hastens death. According to the Hospice Foundation of America, “the goal of hospice care is to improve the quality of a patient’s last days by offering comfort and dignity.”

Hospice care is not just about providing physical care, though that is certainly included. It’s also about the emotional, social, and spiritual impact of the disease and of the dying process.

Hospice does not usually refer to a place. 80 percent of hospice care is provided in the patient’s home. The emphasis is on enhancing comfort and reducing any pain or distress.

When the doctor says that nothing more can be done to improve or even maintain respiratory function, and that death is likely within the next one to six months, then hospice should certainly be considered. Hospice doesn’t mean giving up. it means making a conscious choice to focus on the quality of your life in your last days and then to go out on your own terms!

When you accept hospice, though, you are accepting that you will no longer be accepting life-sustaining or invasive treatments, except in the case of an emergency. It also entails acknowledging “Do Not Resuscitate” (DNR) status. In other words, if your heart or lungs stop working, you do NOT want emergency personnel to initiate treatment that would get things going again.

However, if you were to fall and hit your head or if you broke a bone, then emergency treatment for those would be acceptable, even if you are on hospice.

Once you go on hospice, you are not a prisoner. You CAN change your mind and go off of it. But you can only do that once or twice. So do not take the decision about hospice lightly. Make sure you are ready.

What Does Hospice Offer?

As I’ve already stated, hospice can be beneficial both to the patient and to the caregiver. I have been a COPD caregiver for more than 4 years, and it takes a toll. So, even if patients are willing to “go it alone”, keep in mind how helpful hospice can be to your caregivers.

Most people who have hospice have Medicare health insurance, but both Medicaid and private insurance also often will cover hospice care in qualified people. Here are the hospice services typically covered by Medicare:

  • Nursing care and case management
  • Physician oversight sometimes home visits
  • Home health aide bath services
  • Medical appliances, medication, and supplies
  • Spiritual, dietary, and other counseling
  • Trained volunteers for patient companionship caregiver respite
  • Social work services
  • 24/7 on-call availability

Most people think of hospice in relation to cancer patients, but it can also be helpful to a respiratory patient. As death approaches in someone with COPD, breathing gets harder and harder and air hunger, fear and stress can develop.

Hospice nurses can help tweak oxygen levels and delivery methods, along with morphine and other medicines to control this air hunger and emotional stress. How much will that contribute to a more peaceful, gentler COPD death?

How to Learn More About Hospice

The Hospice Foundation of America website has lots of information about hospice, such as their Myths and Facts About Hospice page and their Hospice Stories section.

You can also talk with your doctor about hospice, and/or talk with the staff at one of the local hospices in your area. You should be able to find them listed on the Web or in the yellow pages of a phone book. Local hospitals may have their own hospices or be able to refer you to one as well.

My Personal Hospice Experience

My mom was diagnosed with COPD back in December 2005. Although she went on oxygen, for a couple of years, she still seemed pretty healthy. Then, in February of 2007, she developed pneumonia and was hospitalized. At that time, her doctor suddenly referred her to hospice, telling us it was not just for dying patients (which is not really true).

At that point in time, neither mom or I was prepared to think of her as imminently terminal, but we accepted hospice and they initiated services that seemed more like traditional homecare than anything else. Unfortunately, about 6 weeks later, mom’s femur broke and hospice was cancelled so she could have the leg surgically repaired.

2007 was a rough year, with 5 hospitalizations in total. On the 4th one, we thought we were going to lose Mom, but by 2008, she was on the mend and over the next year got better and better, often not really even needing her oxygen. Hospice was a distant memory.

Towards the end of 2009, Mom’s breathing started to gradually worsen and I started wondering how much longer she could go on. But she soldiered on, adapting to every little change. Her current doctor talked about hospice when Mom started to ask questions about what the end would be like, but it still didn’t seem like time.

Then, in early November of 2011, Mom just suddenly decompensated, breathing-wise, and had to be hospitalized. Within a week, the doctors decided her lung deterioration was so far advanced, there was nothing more to be done, and started to prepare her to come home to die, which is what Mom wanted so badly.

A hospice referral was initiated, and they helped us with the whirlwind of preparation to bring Mom home on 12 LPM of oxygen (2 concentrators!), a CPAP machine, and kinds of other equipment and medication. It felt like we were setting up an at-home hospital room.

Mom came home on her birthday, and it was a joyful day for her, though also exhausting and with a few scary moments for both her and me. The hospice nurse spent 4 hours with us, getting us situated and setting up support services for the future. I was grabbing that lifeline, as the thought of being chained to my home 24/7 (Mom couldn’t be left alone) was pretty overwhelming and terrifying.

We thought we had at least 3 months to prepare and deal with the dying process. But Mom had other ideas. She passed away early the next morning. In the end, all that the hospice team was able to do for us was set things up and then to come out and pronounce her so that we didn’t have to call the police or coroner. Those things were great, but there is so much more hospice could have done for us, had there been more time.

So, our experience was that first we got hospice way too early and then in the end, way too late.

If you are starting to sense that your life is coming to a close, at least start thinking about hospice and what it can do for you and yours. Don’t wait until the end is right there before you take action!

    5 quick fixes that help when Outlook is slow

    We all know that working with Outlook each day can become frustrating quite quickly. Sometimes it decides to slow down, hang or stop responding for a time, other times it crashes and takes your whole system with it… in most people’s minds Outlook and “slow” have become notoriously associated with each other. What most forget, though, is that situations like these can be easily remedied by performing some simple things like:

    1.Update to the latest version of Outlook.

    I remember that about a year ago, Microsoft launched an update that caused many users to pull out their hair in frustration. Outlook was ridiculously slow and everything took up to 5 times as much to do (especially browsing folders) The Outlook team very quickly launched a fix for this, but even 3 months afterwards we received emails regarding issues caused by that buggy patch there were lots of users who simply didn’t install the latest updates. So, if you’re on Outlook 2003 or 2007 just go to the Help menu, then click on Check for Updates and follow the steps displayed; if you’re on 2010 you can achieve the same thing with the Windows Update entry in your Control Panel.

    If you’ve installed the latest updates and Outlook is still slow then you can just perform a Detect Repair procedure. This basically scans for and repairs any issues that may have arisen since you first installed it. It doesn’t erase any files, rules or accounts, so it’s a quick and safe procedure one that you can perform even if not on an Admin account (note though, that it’s quite slow, so be sure you won’t have need of Outlook for about an hour). To do this, Outlook 2003 and 2007 have the option in the same Help menu, accessible from the main window. For 2010, go to your Control Panel – Programs and Features – in the newly-opened window, right click MS Office 2010 – choose the Change function (notthe Uninstall one ) – click Repair, then follow the steps displayed. If Outlook is still slow afterwards, you can also try reinstalling it (that is, if you have the original CD those on corporate networks should consult their Administrator).

    3.Run the trusty Inbox Repair Tool (ScanPST).

    One thing that I’ve never understood is why a useful little program such as this doesn’t get its own shortcut in the Office folder from the Start menu, but rather you have to know about it in order to use it. I’m sure that at least once you have encountered a corrupt file on your hard-drive – be it a photo or a document. The same thing can happen with one of your emails, especially since your PST file is basically an archive that holds thousands of different files (one for each email). If just one of those files gets corrupted, it will slow down Outlook and all its operations. What ScanPST does is go through each byte of your PST and checks if “they fit together”. Once it detects a problem it either fixes it or puts the file away (if it can’t be fixed) so that it doesn’t slow down Outlook anymore. I recommend using it every 3 months, since even if Outlook isn’t slow errors may still be present. So, either go to the link above to see where you can find it or click here to run it straight away, from Microsoft’s servers.

    Usually, we install a program and expect it to perform as new for however long we will use it. This isn’t the case with Outlook, its complexity being its eventual downfall. With this in mind, from time to time you will have to perform the instructions detailed above, but if you want to squeeze every bit of performance and speed from it you will also have to remember these steps:

    a) Download full IMAP items. If you are working under an IMAP account (Gmail for example), then make sure that it behaves just like POP meaning, it downloads whole emails and not only their headers. If not, every time you click on a new item Outlook will slow down. To find out how to do this, just go here for Outlook 2003 or here for 2007 ; by default, Outlook 2010 downloads IMAP items in full. Even easier, just press Ctrl+Alt+S and then Edit your IMAP account.

    b) Disable add-ins that you no longer use. This is self-explanatory, actually – Outlook will perform slower if there are a million other things running in the background, accessing its data. You can manage them either from the Outlook Options screen or with the free Add-in Utility that comes bundled with Bells Whistles for Outlook (our performance add-in). You can download the trial version from here and keep the Utility afterwards – it’s absolutely free!

    c) Disable the RSS feature found in Outlook. Just go to the Options screen, then search and uncheck the option Sync RSS Feeds to the Common Feeds List . So that Outlook doesn’t slow up searching old RSS items, be sure to delete them all afterwards.

    5. Keep your PST file size under control.

    This one’s a doozy! Since your PST file is a database of all your emails, the larger it is – the slower Outlook will run. So, I found that it’s best to start a new PST every year or so (this is what I’m currently doing). In addition, you should also delete your duplicates (which can appear because of many factors) and your obsolete emails, compact your PST and archive old items. Another thing to remember is that by moving your attachments to your hard-drive instead of keeping them within the emails, Outlook will run much faster .

    John Taylor Hospice is found in Erdington. It is a hospice where patients come first. Not only do they care for patients in the hospice but they send support out to many families.

    We have already begun to raise funds for this magnificent cause and we aim to do more for this Hospice. JTH is no longer supported by the NHS. It would be devastating for them to fall short of funds

    We have set out to raise awareness about JTH and what it does, we hope to fundraise for refurbishments to JTH and also show that youth can contribute constructively to their communities on their own inititative.

    Help the Hospice have teamed up with a local restaurant in Kingstanding to create a charity meal! The meal will take place on the 22nd of April, starting at 6:00 pm.

    I realize it’s not in OC, but Cal State LB is pretty close to you, and you could get your bachelor’s in two years in the trimester program. I’m in my sixth semester there and it has been challenging but great. If you really don’t like where you are, look into transferring. There isn’t a waiting list, and I know Saddleback just switched their admissions and now they don’t have a waiting list either. I think Goldenwest does, though, so that might not be your best bet. Good luck!

    I live in Aliso Viejo and I’m on a program at San Bernardino University. It is a long drive, but I don’t mind. I’m a partnership Riverside Community Hospital- CSUSB student and have all of my clinicals at RCH, where I would I will be working after graduating. This is an accelerated program and I will have my BSN in 2 years. I think that having BSN is better. I’ve heard that Long Beach is very impacted and it is hard to get in, so I did’t even try to apply. I’ve applied to San Marcos University, CSUSB and Saddleback. I got into CSUSM, CSUSB and didn’t get into Saddleback, but I didn’t care, since I was already going to CSUSB. It is much easier to get on a program at CSUSB because they accept 80 students 2 x a year. There is one transfer student in my class, she transferred from Loma Linda.

    i really did not want to deal w/a bachelors right now. i already have a ba from years ago, so i just wanted the rn. i heard that saddleback was crazy hard like cypress. is the time commitment outside of class insane? this is my typical tues: class from 800-1130, drive 40 min to hosp to p/u assign, get pat. and fill out paper work for about 1 1/2-2 hrs, drive home 45-1hr, eat dinner, start prep work for oh 3-5 hrs. go to bed hopefullly sleep 6 hrs (most stu sleep w/only 3 or 4, but i refuse, i’m too old ), wed. get up 430, work 630-1600, go home hr drive home, eat, home work 3-5 hrs. hellish, i’m telling you. is this normal for you all? thanks for the info!

    you mentioned you have a BA. some colleges have a 15 month program to get a second bachelor’s in nursing. it’s just as tedious as getting your associate’s degree in nursing too, but at least you’re getting a higher degree for the same work.

    I go to school only 2 times a week T-Th. Last quarter I had clinicals from 6:45 to 12:00 and a lecture from 14:00 to 15:50. Only 7 units, where at Saddleback there are 15 units/semester. Next quarter I will have 11 units and the same days of the week. The clinicals will be the same, but the lecture will be from 13:00 to 17:50. I also have BS degree and I’m not very young. If you will apply to CSULB, they will only consider you for the accelerated 1 y 4 mo program, which is very hard, my friend is on this program.

    saddleback doesn’t have 15 units of nursing a semester. that’s only if you include general education, which many students who enter have already fulfilled. saddleback has an average of 7 units of core nursing a semester.

    An IRS Payment Plan is for taxpayers who can’t afford to pay off their entire back tax debt at once or for those who can’t make large payments to the IRS without experiencing an economic hardship. The key word here is hardship. One could say that anyone who is paying back the IRS for past due taxes is experiencing an economic hardship in that there are always more pleasurable things to do with one’s money. Unfortunately, the IRS doesn’t see it that way.

    In order to set up a Payment Plan for IRS back taxes and stop collection action. a taxpayer must demonstrate to the IRS that paying all that is due or making excessively large payments to the IRS would result in an undue economic hardship to the taxpayer. This is done by showing that payment of what the IRS is demanding would result in the taxpayer taking food off their family’s table or doing without some other necessity of life to give the money to the IRS instead. Fortunately for many people, this is not that difficult to demonstrate. Other times it is a bit harder, but there is almost always some type of resolution available. If you contact us. we will help you deal with the IRS and correctly complete the right forms to set up a Payment Plan that works for you.

    The main benefit of setting up installment payments with the IRS is that it will put you back in compliance and you will no longer have to worry about the IRS knocking on your door. The IRS will have disappeared from your life and there will be no threat of levy and seizure (garnishment) of your wages, bank accounts, or any other property. The harassment by the IRS will be completely over, as long as you continue to make the agreed upon payments.

    It is possible that by having us take a look at your financial situation, you may actually qualify for a hardship deferment of collection action and be put on uncollectable status. If this is the case, you would then be completely left alone by the IRS and make no payments. Do not count on this, but it does sometimes work out that way. Furthermore, you will not have to wait long for an answer. According to the IRS’s internal rules, they must answer your request for hardship within ten (10) working days of receipt of your request. THAT IS FAST.

    In order to get set up on a really low payment plan or better yet on uncollectable status, you must demonstrate economic hardship to the IRS. You may be asking yourself: “How do I prove economic hardship to the IRS?” Economic hardship according to IRS rules and procedures is when paying the IRS would result in undue or unreasonable hardship to you, the taxpayer. As a taxpayer, you may think that not paying off your VISA debt or a loan from Aunt Mary and giving that money to the IRS instead is unreasonable, but the IRS doesn’t see it that way – and they never will.

    To view this in the proper perspective, you must look at it from the way the IRS Collections folks do. You see, they have all the power and hold most of the cards. What you have going for you are some reasonable outs provided by Congress and the IRS to keep the system efficient and within the bounds of reason. It is not efficient for the IRS to spend its time and money trying to collect a large sum of money from somebody who has no way of paying so large of an amount. The IRS and Congress have provided for this by instituting Payment Plans. The key is to make sure it is a Livable Payment Plan that you can afford and continue to pay without defaulting. This is what National Tax Relief makes sure of. You do not want to get coerced into a Payment Plan by a revenue officer in which the payments are so high that you will not be able to keep them up in the long run. If you get into a Payment Plan and later default, then you will be in bigger trouble than before.

    How to Qualify for a Payment Plan or Uncollectable Status

    You must show the IRS on the proper forms that your allowable expenses are equal to, or only slightly less than, your income. This is how you show the IRS that you can pay them only an “affordable and livable amount” without “undue economic hardship. Your allowable expenses are what the IRS considers the necessities of life. In general, they are: food, clothing, housing, utilities, transportation, medical, insurance and work-related expenditures.     Released in 1983 by Commodore, Magic Desk I is a ROM cartridge that presents the functions of the computer as a virtual desktop. The product was described as an attempt to make the Commodore 64 “people literate” (as opposed to making people “computer literate”).

    Produced during the “user friendly” frenzy of the Apple Lisa, Magic Desk I is interesting because it was an attempt to bring a similar visual metaphor to a much less powerful but more common home computer.

    In many ways, it feels similar to the interface used by the later Magic Cap PDA software. However Magic Desk I is rather primitive and quite lacking.

    Both the name and missing functionality imply this was to be followed up with an improved version. However no “Magic Desk II” was ever released.

    Because it is a cartridge, it starts up automatically, and faster than loading from disk.


    This is the Magic Desk I desktop. It presents you with a desk, typewriter, file cabinet, clock, and trash can. You move the “hand” with a joystick to select the item you want.

    Notice the door, calculator, phone, card file, and dollar bill graphics. Trying to select these does absolutely, completely, and totally nothing. That’s right, they don’t do anything. At all. This leaves one to speculate if these were supposed to be features of “Magic Desk II”, parallel release versions to the “Type and File” cart, or future disk based utilities.

    Either way, the typewriter would have looked lonely on the desk all by itself.


    Instead of embracing the advantages that a computing device can bring to text editing, Magic Desk simulates a typewriter. The text on the screen even scrolls back and forth left and right as you type while the green “cursor” remains in the center.

    There are no block editing commands such as copy/paste.

    Using the icons at the bottom of the screen, you can return to the desktop, set tabs, return to typing, print what you have typed, or put your document in the trash.


    Once you have typed some text, a graphic of a sheet of paper appears above the desk.

    You can select the clock to the time.


    If a document is present, you can select the trash can. This will put the piece of paper in the “trash”, and it will be gone when you leave this screen.

    The content of a document is stored in RAM until it is saved to disk. You do that by selecting the file cabinet.


    The error handling in Magic Desk is rather poor. For example, if you select the file cabinet while there is no disk in the drive, it keeps you here until you insert a disk, and gives you no option to go back.

    Thankfully here in the enlightened, far off future we would never make such a hideous mistake. oh wait, I ran in to that in Windows 8!


    The file cabinet has three “drawers” you can select. Each drawer is already populated with a set of yellow “folders”. You then select a folder, give it a name if needed, and then click on the “files” icon to continue.


    Similarly, each “folder” is already populated with a number of files.

    If there is a document in memory you will see a document icon in the upper left. To save the document, select a file, give it a name, and select the “disk” icon.

    To load a document, select a named document, and select the document icon at the bottom.


    If you selected a document to load, it shows a preview before returning to the desktop. You can scroll this preview similar to the editor, and it again gives you the option to delete the document using the trash icon.


    Magic Desk also has some built in help. Pressing Control shows this screen, and selecting one of the icons will show some more information.


    Wow, it really wants to format that disk! The disk is full, so let’s reformat it? Great idea.

    That is really about all there is to Magic Desk I. In conclusion, were it more refined and complete, it probably would have been a decent tool for novice C64 users. A real Mac-like desktop came to the Commodore 64 later in the form of GEOS.

    If there is one mistake small town businesses make more often than any other it’s, What ever is left over, we’ll use for advertising.

    Marketing and advertising is an investment, not an expense.

    I know it sure seems like an expense to me when I’m writing the check, but trust me it’s not.

    Without enough money put aside for advertising your sales can go down and you suddenly have less and less for promotion.

    When do you advertise the most? For most businesses it’s the first day of business.

    Don’t you have a Grand Opening, balloons, flyers, ads, on-site radio stations, contests, and prizes?

    Did the income from sales pay for that? No, it didn’t. You advertise most when you need business. You advertise more when you don’t.

    An average cost of advertising is usually 1 to 5% of gross sales, which can vary according to location, local advertising rates, and industry. Car dealers need more advertising than funeral homes.

    Before we get to the 25 tips let’s look at the basic strategies of successful advertising.

    * In order to be successful, your advertising must provide a consumer benefit or solve a problem.
    * That benefit or solution must be wanted by the consumer.
    * The product or service you are offering must be tied directly to that benefit or solution.
    * The benefit or solution must be distinctly communicated through medial advertising. In other words, be clear, forget the advertising glitz and make sure the message isn’t lost in the ad.

    A small-budget advertiser doesn’t have the ”deep pockets to develop big advertising campaigns. Some time you need to break the rules to be noticed.

    Avis did it by admitting they were Number 2 in the car rental business and that campaign took them from 6th place to second place.

    When they stopped that campaign they dropped back to 6th again. In the past year they have gone back to it.

    Budget conscious advertisers must achieve top results for their advertising dollar. Expand your dollars by adopting some creative techniques.

    Here’s 25 tips I hope will help you.

    1. Radio, newspapers and magazine specialists will frequently give free help in developing an advertising strategy. Things like demographic information, money-saving ways to produce your ads etc.
  • Place your ads in off hours or in unusual locations for less. Many times you can still reach your target market with these spots.
  • Instead of a one-time big splash ad, be consistent with frequent small ads that work.
  • Monthly magazines sometimes have unsold ad space at the end of the month they will sell at a discount.
  • If you have an 800 number, put it in every ad for immediate response and feedback.
  • Try advertising consistently in the classifieds. These ads may draw more customers than more expensive display ads.
  • Can you barter for the cost of ad production? Maybe the newspaper needs painting in exchange for an ad about your paint store.
  • Piggyback advertising are the ads you receive with your Mastercard bill. Is there someone in your town that sends out a lot of bills? Can you put a small flyer in with their bills and split the postage? Or pay a small fee?
  • Split advertising costs with the people who sell to you. Vendors and manufactures are always looking for exposure. Let people know you carry their products and have the vendor pick up part of the ad cost.
  • Are there up front advertising discounts for cash?
  • Consider advertising in regional issues of national magazines. The costs are lower and you can still reach your target market. TV Guide is a good choice. It stays around for at least a week. Time, Newsweek, and US News and World Report may stay in local doctors offices for years.
  • Share ad costs with neighbor business. Video stores and Pizza parlors are natural partners. Have coupons to each others stores or share the cost of flyers.
  • Try reducing the size of your ad (not in the Yellow Pages) or length of your radio spots. A 60 second spot is not twice as much as a 30 second spot but you won’t get twice as many customers for a 60 over a 30. Going with small ads or shorter spots will allow you to do more ads which normally pulls more customers. It’s better to be there every day with small ads than every month with one big one.
  • Develop tight production controls to minimize the need to reject finished ads. The message is more important than the messenger. Don’t try to produce ads that win awards, produce ads that sell.
  • Who are your very best customers? Aim your ads to talk directly to people like them.
  • What will suppliers give you in the way of point-of-purchase materials. Posters, stand ups, handouts, etc. Some have excellent display racks you can use.
  • Some national chains like Coke and Pepsi provide outdoor signs for businesses. There are also indoor lighted signs you write on with special markers to advertise your special offers.
  • Can you sponsor a community event? A fun-run, golf tournament, or other event that will be well publicized in the community. Your name may not be prominently displayed but sometimes the positive exposure in the community will bring in new customers.
  • Small businesses can seldom afford saturation advertising. You must be selective in the media that reaches your customers. Pin your ad reps down and make them show you exactly how their media reaches your target audience.
  • Exploit the media you choose to the fullest. If your message is verbal, you don’t need TV. Use radio, billboards and newspapers to the fullest.
  • Consider direct mail. A letter and brochure before customer contact can increase business. An IBM study concluded that selling time can be reduced from 9.3 to 1.3 total hours with direct mail advertising. A Sales and Marketing Executives International Study showed salespeople went from eight orders per 100 cold calls to 38 orders per 100 when direct mail was used.
  • Try an editorial style ad. These are ads that look like actual stories in the newspaper. They will have advertisement at the top of the article. Develop a good headline, and 50% more people will read the article than would read an ad of the same size.
  • You can’t match larger competitors dollar-for-dollar but, you can use unusual approaches (like the Avis idea above), color, music, slogans, humor (be careful here), or media selection to win your market away from the big guys.
  • Due to the high costs of conventional advertising on, radio, TV, newspapers, many cost conscious business have been forced to look for lower cost methods. Can you advertise on parking meters, taxi boards, balloons, blimps, and grocery shopping carts. Community bulletin boards, movie ads, and weekly newspaper shoppers.
  • Key your ads. Put something in the ad that will let you know which media it came from. On coupons, put a code that will record the paper and date of the ad. In radio or TV, have them mention the ad to get the discount. Ask every customer how they found you.
  • Plan for a rainy day. During the year put a small amount aside each month for emergencies. You never know when you’ll need to react quickly to whatever the competition is doing. You must be able to capitalize on breaking national events or news regarding your industry. If negative things happen in your industry you may need to respond quickly to make sure the right message is presented.
  • Always give the customer more than you promised and more than they expected. This is tip number 27 of the 25 we advertised. Maybe this last one is the one you needed.
  • I hope these tips will help your business grow. Not all may be relevant to your particular situation. Hopefully, they will illustrate the importance to plan and control your advertising budget.

    FHA Refinancing

    Changes in the housing market have given you-and thousands of other Americans-the option to refinance your current home mortgage with a FHA Refinance.

    FHA Refinancing is a better option if you have a fair to good credit rating because, generally, FHA standards are less strict. Even if you likely will not qualify for conventional refinancing because of your income level, a FHA Refinance may still be an option for you.

    FHA Refinance Requirements

    There are some requirements that may apply when trying to refinance your current loan with the FHA. Keep in mind the following are simply guidelines-each borrower’s situation is different.

    • The mortgage that is to be refinanced must ultimately be insured through the FHA.
    • The current mortgage must be current and not delinquent.
    • The results of the refinance must lower the monthly principle and interest payments for the borrower.
    • No cash may be taken out on mortgages refinanced using the streamline refinance process.

    Other Information on FHA Refinances

    One Streamline Refinancing option you have is one that includes the closing costs into the new mortgage amount. This of course is only available if enough equity is in the home after it is appraised. The streamline refinance can occur without an appraisal but the new loan will not be able to exceed the original loan amount. If you are not living in the property (i.e. it is an investment property), the refinance can only occur without an appraisal.

    An IRS Payment Plan is for taxpayers who can’t afford to pay off their entire back tax debt at once or for those who can’t make large payments to the IRS without experiencing an economic hardship. The key word here is hardship. One could say that anyone who is paying back the IRS for past due taxes is experiencing an economic hardship in that there are always more pleasurable things to do with one’s money. Unfortunately, the IRS doesn’t see it that way.

    In order to set up a Payment Plan for IRS back taxes and stop collection action. a taxpayer must demonstrate to the IRS that paying all that is due or making excessively large payments to the IRS would result in an undue economic hardship to the taxpayer. This is done by showing that payment of what the IRS is demanding would result in the taxpayer taking food off their family’s table or doing without some other necessity of life to give the money to the IRS instead. Fortunately for many people, this is not that difficult to demonstrate. Other times it is a bit harder, but there is almost always some type of resolution available. If you contact us. we will help you deal with the IRS and correctly complete the right forms to set up a Payment Plan that works for you.

    The main benefit of setting up installment payments with the IRS is that it will put you back in compliance and you will no longer have to worry about the IRS knocking on your door. The IRS will have disappeared from your life and there will be no threat of levy and seizure (garnishment) of your wages, bank accounts, or any other property. The harassment by the IRS will be completely over, as long as you continue to make the agreed upon payments.

    It is possible that by having us take a look at your financial situation, you may actually qualify for a hardship deferment of collection action and be put on uncollectable status. If this is the case, you would then be completely left alone by the IRS and make no payments. Do not count on this, but it does sometimes work out that way. Furthermore, you will not have to wait long for an answer. According to the IRS’s internal rules, they must answer your request for hardship within ten (10) working days of receipt of your request. THAT IS FAST.

    In order to get set up on a really low payment plan or better yet on uncollectable status, you must demonstrate economic hardship to the IRS. You may be asking yourself: “How do I prove economic hardship to the IRS?” Economic hardship according to IRS rules and procedures is when paying the IRS would result in undue or unreasonable hardship to you, the taxpayer. As a taxpayer, you may think that not paying off your VISA debt or a loan from Aunt Mary and giving that money to the IRS instead is unreasonable, but the IRS doesn’t see it that way – and they never will.

    To view this in the proper perspective, you must look at it from the way the IRS Collections folks do. You see, they have all the power and hold most of the cards. What you have going for you are some reasonable outs provided by Congress and the IRS to keep the system efficient and within the bounds of reason. It is not efficient for the IRS to spend its time and money trying to collect a large sum of money from somebody who has no way of paying so large of an amount. The IRS and Congress have provided for this by instituting Payment Plans. The key is to make sure it is a Livable Payment Plan that you can afford and continue to pay without defaulting. This is what National Tax Relief makes sure of. You do not want to get coerced into a Payment Plan by a revenue officer in which the payments are so high that you will not be able to keep them up in the long run. If you get into a Payment Plan and later default, then you will be in bigger trouble than before.

    How to Qualify for a Payment Plan or Uncollectable Status

    You must show the IRS on the proper forms that your allowable expenses are equal to, or only slightly less than, your income. This is how you show the IRS that you can pay them only an “affordable and livable amount” without “undue economic hardship. Your allowable expenses are what the IRS considers the necessities of life. In general, they are: food, clothing, housing, utilities, transportation, medical, insurance and work-related expenditures.     David – 20 May 2008, 07:42 PM The hardest part for me, as someone who hadn’t taken a math class in more than 5 years, was adjusting at the beginning of the semester. It was easy for forgotten algebraic principles and equivalencies to get in the way of new concept learning in the beginning. I was a little nervous at first because the first few assignments were taking much longer than anticipated, but it became much more comfortable once the algebraic rust fell away. Do review algebra first! I got a great amount of information from the lectures and online forum, but I found it also helpful to reinforce those concepts by reviewing the corresponding textbook sections. The textbook is good for the general concept and formula checking, but when you have a more specific question just shoot it off to the forum and the feedback comes almost immediately. Trying to teach concepts in the forum to someone else, I also found, was really a great way to lock down new concepts. If you’re doing your homework early and not many people are discussing your question on the forum yet, check the stream channel for short video clips (a huge collection) that can provide that one needed concept (or trick) to the problem. Another source for learning (you will see there are very many) are great links to math websites and even designated times to personally chat online, or call, teaching assistants about your questions. Procrastination will penalize you in this course. There were some homeworks that were considerably longer than others that required early starts for a good grade. Overall it was a great learning experience that was just very convenient. You could probably be a full time business traveler and still take the course. I feel much better about my math skills now. Before I took this course, a function was just an equation that was hard to evaluate without a graphing calculator.     How to succeed: Tip #1: Do not take this course over the summer unless you are.
    a. Either very proficient with math (algebra and trigonometry);
    b. Or you have a lot of time to devote to this class and all the homework. Tip #2: Regardless of your answer to #1, you must be willing to do a lot of work, including posting and sharing questions and answers. Tip #3: Really listen to the very first lecture where Dr. Goetz explains how Calculus came into being. That sets the stage for the entire course and its primary objective. Throughout (and especially In the middle) of the course, it is easy to lose sight of how the little concepts add up to something worthwhile or useful. Thinking back on the first lecture will help you see the bigger picture and might even give you the proper perspective to understand that troublesome derivative problem or that confusing integration problem. Seriously! Tip #4: Try on your own first. Active learning is the only way to pass the final exam! Tip #5: When tip #4 fails, use all the resources at hand: the homework forum, video excerpts of the lecture or similar problems, the grapher, external resources, even that expensive dry text book! Tip #6: When tip #4 works, take the time to give others tips or weigh in on questions posed by other students. This serves 2 purposes: it reinforces your learning and it builds your body of postings for that all-important class participation credit (which is worth an entire grade improvement)! Tip #7: Convince yourself that you love to solve all kinds of problems, including Calculus, even if that’s not entirely true. If you can get into the spirit of things, you will have some measure of fun to help temper the frustration and make the long hours of doing homework less tedious. Tip #8: Dr. Goetz and his staff are the best and they are looking after your best interest: to learn and have an appreciation for Calculus. So trust them and don’t be afraid to ask anything that would help you to make the most out of the class.

    • The Grieving Teen: A Guide for Teenagers and Their Friends by Helen Fitzgerald is a compassionate guide that pays attention to the special needs of adolescents struggling with death and gives them the tools to work through their pain and grief.
    • Tiger Eyes by Judy Blume explains how a young girl and her brother cope with the murder of their father. This story is particularly useful in helping children deal with sudden and traumatic death.

    Service Desk Outsourcing Pricing Models: What You Need to Know #outsourced #help

    “How do you price your service desk outsourcing offering?” This is by far one of the most common questions that I am asked by companies that are researching options to outsource their service desk operations. The answer is “It depends.” In this latest article we will explore some of the factors that determine which pricing model is used and why as well as some of the most common formulas in the market today.

    There are two typical methods of pricing that ABS utilizes for our service desk outsourcing services: per-incident pricing and fixed fee pricing and in some instances a combination of both.

    Per-incident pricing is our most common model and is incorporated in conjunction with the shared model of our ABS Global Service Desk offering. Here’s how it works. We first work with each new customer to understand its unique support needs, call volumes, incident (ticket) volumes, estimated talk times, support hours, applications, SLAs, etc. As you can imagine, all of these elements will be factored into determining the actual per-incident price. Next, we establish a monthly incident volume (i.e. baseline) that we will use to staff the service desk in order to guarantee service levels and have enough bandwidth to cover typical variances. The agreed upon monthly volume is then multiplied by the per-incident price to establish the base monthly fee. In the event of any overages above the monthly incident volume, we simply apply the same per-incident amount to the number of additional incidents.

    Fixed-fee (monthly) pricing is most often used in conjunction with a dedicated service desk model. The math is very straightforward for this approach that is primarily based on the number of service desk resources that are dedicated to a client. Fixed-fee pricing with a dedicated model can be very helpful from a budgeting perspective. However, it may be possible that a client is paying for resources that are underutilized at times, especially if volumes vary significantly in different seasons or when paying for the staffing of dedicated resources during afterhours and weekends. It can also be more difficult to quickly scale up (or down) and react to significant increases (decreases) in volume that is typically easier to handle with a shared/per-incident model.

    Hybrid model: In some cases, a hybrid pricing model may be an option which is exactly what we offer to some of our clients when it makes sense. This combination can offer the benefits of the fixed-fee pricing for a dedicated team with the cost effective per-incident pricing model of a shared service for afterhours and weekends.

    Of course, these aren’t the only pricing models utilized. Per-user (or per-seat), per call (or per contact), and per minute pricing are also common; each with its own pros and cons. Here are some factors to keep in mind when considering any of these options.

    Per-user (per-seat): Many people like the per-user model since it can be a very easy calculation based on the number of users that are entitled to call the service desk. While this may be true, consider the following scenarios: First, vendors will need to assume an average number of calls and incidents per month for every user. Therefore it is quite possible that you may be over paying for volumes that you never actually use. Second, it can sometimes be tedious to administer the number of entitled users on a regular basis which varies as employees are added or deleted.

    Per-call: This pricing model is based solely on the number of calls handled by the service desk. Although this is similar to the per-incident model, on the downside you may end up paying for large spikes in very short calls as a result of an outage or other critical event. If you are considering this type of model, be sure that your vendor is factoring in such short calls and has the ability to immediately post outage messages to the IVR system in order to prevent unnecessary calls from the reaching the service desk.

    Per-minute: If you are looking for a true service desk solution that can provide first call technical resolution, then you should be wary of a per-minute pricing model. First, this type of model is typically indicative of a call answering service where the agents are less technically savvy. Second, there is little incentive to resolve the user’s issue in an efficient and timely manner.

    In our experience of providing over 15 years of service desk outsourcing to our customers, we believe the per-incident model to be the most well-balanced approach. Here’s why:

    1. Pay for what you use – unlike the per-user or even the dedicated model, you aren’t paying for bandwidth that you’re not using.
    2. Scalable to meet changing business conditions – we typically review volumes on a quarterly basis to ensure that the established monthly incident volume is accurate and will adjust up or down as needed.
    3. Most cost effective for afterhours and weekend support – most companies that require afterhours and weekend support have significantly less volume during these periods. Using a dedicated/fixed-fee model can be quite expensive in these cases.

    Now that we have addressed the various pricing options available, it is important that you work with your vendors to understand what they are proposing and why. There is no right or wrong pricing model, it’s a matter of which one is the best suited to your company and finding a vendor that is flexible enough to deliver it.

    John Taylor Hospice is found in Erdington. It is a hospice where patients come first. Not only do they care for patients in the hospice but they send support out to many families.

    We have already begun to raise funds for this magnificent cause and we aim to do more for this Hospice. JTH is no longer supported by the NHS. It would be devastating for them to fall short of funds

    We have set out to raise awareness about JTH and what it does, we hope to fundraise for refurbishments to JTH and also show that youth can contribute constructively to their communities on their own inititative.

    Help the Hospice have teamed up with a local restaurant in Kingstanding to create a charity meal! The meal will take place on the 22nd of April, starting at 6:00 pm.

    Thousands of men and women meet online every day and while online dating has expanded the dating universe like never before; a few bad guys fly under the radar and attempt to scam innocent people. While scams asking for money are more common in online dating, it is important to be aware of other equally important fraudulent practices such as identity theft.

    What is Identity Theft?

    To put it simply, when someone steals or attempts to steal your identity it is known as identity theft. Identity thieves use the victim’s personal information and details to fraudulently pose as the victim, usually for financial benefits.

    How Do Thieves Steal Identities?

    The identity thief uses the victim’s identity for personal gain and this leads to identity fraud. They make use of personal information such as bank account details, social security or tax file numbers, credit card details or any other financial account information for their personal benefit. They can get hold of this information through various online and offline methods.

    • Phishing They send emails or pop-up messages, pretending to be your financial institution to get you to reveal your personal details.
    • Pretending Once they obtain your personal email address or phone number, again they pretend to be your financial institution to get a hold of your information.
    • Changing Your Address If they know your home address, they can change the address and divert your bills and bank statements to a different address.

    In the offline world, some identity thieves resort to literally stealing your information.

    • Stealing They steal wallets, credit cards, mail from mail boxes, pre-approved credit offers etc. At the corporate level, they even target companies and steal personnel information from their databases.
    • Dumpster Diving They rummage through the rubbish to look for identifying information or bills they can use.

    Staying Safe

    Don’t become a victim of identity fraud; take these steps to prevent it from happening to you while you’re dating online.

    • Do not post or share any personal information and details with anyone, no matter how safe you think it might be. Bank account details, a credit card number or a social security or tax file number is often all a thief needs for fraud.
    • If you are talking to someone on the phone, get to know them before you give them your cell phone number. Identity thieves have been known to use home and work phone numbers in reverse phone directories to reveal addresses etc so it s probably best you get to know someone a little before you share your contact details with them.
    • Keep a log of your chat or IM sessions wherever possible, just in case you need it in the future.
    • Do not entertain any requests to cash a cashiers check as these checks are usually fake and as a victim you not only lose your money but could also reveal sensitive personal information to the scammer.
    • Monitor your credit reports regularly to ensure there has been no unusual activity on your account. You can also freeze your report so credit issuers can only access your report when you give them permission to do so.
    • If you receive emails, text messages or phone calls asking you for sensitive information, make sure you double check with the bank or financial institution as they will usually not ask for sensitive information over the phone.
    • Use a locked mail box so your mail cannot be stolen and make sure you shred paper before you throw it in the garbage.

    Getting Help

    Sensitive information in the hands of a thief can lead to several kinds of identity fraud – credit card, bank and finance, government documents (getting a driver’s license in the victim’s name; using social security number), getting a job pretending to be the victim, renting services etc. Obviously an imposter is bad news and if you think you are a victim of identity theft/ fraud, you must get help immediately.

    • File a police report.
    • Immediately notify creditors.
    • Dispute unauthorized transactions and doing so will help restore your name and credit report.
    • Set up a fraud alert on your credit report to prevent the thief from opening further accounts
    • Close the accounts that have been tampered with

    GastricSleeve (Vertical Sleeve Gastrectomy)

    The gastric sleeve is the lastest weight loss procedure that shows promising results. By restricting food absorbtion and reducing hunger the gastric sleeve might be your best option. Please consult with our surgeon or staff to find out if this is the best procedure for you. Advantages of gastric sleeve include lower cost, less dumping, fewer complications and does not require adjustments after the initial surgery.

    The vertical sleeve gastrectomy is a restrictive form of weight loss surgery in which approximately 85% of the stomach is removed leaving a cylindrical or sleeve shaped stomach with a capacity ranging from about 60 to 150 cc, depending upon the surgeon performing the procedure. Unlike many other forms of bariatric surgery, the outlet valve and the nerves to the stomach remain intact and, while the stomach is drastically reduced in size, its function is preserved. Again, unlike other forms of surgery such as the Roux en Y gastric bypass, the sleeve gastrectomy is not reversible.

    Because the new stomach continues to function normally there are far fewer restrictions on the foods which patients can consume after surgery, albeit that the quantity of food eaten will be considerably reduced. This is seen by many patients as being one of the great advantages of the sleeve gastrectomy, as is the fact that the removal of the majority of the stomach also results in the virtual elimination of hormones produced within the stomach which stimulate hunger.

    Perhaps the greatest advantage of the gastric sleeve lies in the fact that it does not involve any bypass of the intestinal tract and patients do not therefore suffer the complications of intestinal bypass such as intestinal obstruction, anemia, osteoporosis, vitamin deficiency and protein deficiency. It also makes it a suitable form of surgery for patients who are already suffering from anemia, Crohn’s disease and a variety of other conditions that would place them at high risk for surgery involving intestinal bypass.

    Facts about the gastric sleeve or sleeve gastrectomy:

    Alternative names: vertical sleeve gastrectomy, sleeve gastrectomy, greater curvature gastrectomy, parietal gastrectomy, gastric reduction and vertical gastroplasty.

    Surgery for high BMI patients. For patients with a particularly high body mass index (typically 50+) many forms of weight loss surgery are either difficult to perform or present increased risk. As a result, a vertical sleeve gastrectomy (or increasingly a laparoscopic sleeve gastrectomy) is sometimes performed as the first of a two-part weight loss solution to provide an initial drop in weight which then makes other bariatric follow up possible at a reduced level of risk.

    Surgery for low BMI patients. For obese patients with a relatively low body mass index the vertical sleeve gastrectomy can also prove a good choice, especially where existing conditions (such as anemia or Crohn’s disease) prevent them from having other forms of bariatric surgery. In addition, patients may choose this form of surgery if they are concerned about the long-term affects of bypass surgery or object to having a ‘foreign’ body implanted into their body, as is the case with lap band surgery.

    Laparoscopic Sleeve Gastrectomy Surgery

    During sleeve gastrectomy, the surgeon will remove the larger, rounded part of the stomach.

    • The remaining stomach looks like a sleeve (or hose or tube) and holds about 15 percent as much food as the original stomach.
    • The surgeon will remove the larger, rounded part of the stomach from the body. (This is the only gastric surgery in which part of the stomach is taken out of the body.)
    • Unlike gastric bypass, which changes stomach openings, sleeve gastrectomy leaves the openings intact.
    • It may be a safer and more effective option than gastric bypass for patients with very high BMI, those with medical problems like anemia, Crohn�s disease, osteoporosis, extensive prior surgeries and other complex medical conditions.
    • Sleeve gastrectomy may be safer than gastric bypass for patients who have a number of health risks.
    • It lowers the risk of ulcers compared to gastric bypass.
    • The surgery cuts away the part of the stomach that produces grehlin, a stomach hormone that stimulates hunger.
    • Though the stomach is smaller, the openings are left intact, so digestion can go on as normal.
    • The body is free of foreign objects like the LAP-BAND� or Realize Band.
    • May be converted to gastric bypass or duodenal switch if necessary for additional weight loss
    • Expected excess weight loss for stand-alone procedure is 60 to 70% at two years
    • Sleeve gastrectomy is more common in Europe, but most American health insurance carriers still consider it an investigational procedure and do not cover the cost. But that is rapidly changing so please consult our office to see if your insurance will cover the gastric sleeve.
    • Anytime you have anesthesia or surgery, there is a risk of blood clots, other complications or death.
    • Do not smoke. Smoking would put you at high risk for infection, blood clots, slow healing and other life-threatening complications.
    • Complications can occur with the stapling, such as leaks or bleeding.

    • The smaller portion of the stomach may stretch.

    Rehabilitation Therapy after a Stroke

    Stroke is one of the leading causes of long-term adult disability, affecting approximately 795,000 people each year in the U.S. The very word “stroke” indicates that no one is ever prepared for this sudden, often catastrophic event. Stroke survivors and their families can find workable solutions to most difficult situations by approaching every problem with patience, ingenuity, perseverance and creativity. Early recovery and rehabilitation can improve functions and sometimes remarkable recoveries for someone who suffered a stroke

    There’s still so much we don’t know about how the brain compensates for the damage caused by stroke. In some cases, the brain cells may be only temporarily damaged, not killed, and may resume functioning over time. In other cases, the brain can reorganize its own functioning. Every once in a while, a region of the brain “takes over” for a region damaged by the stroke. Stroke survivors sometimes experience remarkable and unanticipated recoveries that can’t be explained. General recovery guidelines show:

    • 10% of stroke survivors recover almost completely
    • 25% recover with minor impairments
    • 40% experience moderate to severe impairments requiring special care
    • 10% require care in a nursing home or other long-term care facility
    • 15% die shortly after the stroke

    Rehabilitation actually starts in the hospital as soon as possible following a stroke. In patients who are stable, rehabilitation may begin within two days after the stroke has occurred, and should be continued as necessary after release from the hospital.

    Depending on the severity of the stroke, rehabilitation options can include:

    • A rehabilitation unit in the hospital with inpatient therapy
    • A subacute care unit
    • A rehabilitation hospital with individualized inpatient therapy
    • Home therapy
    • Returning home with outpatient therapy
    • A long-term care facility that provides therapy and skilled nursing care

    The long-term goal of rehabilitation is to improve function so that the stroke survivor can become as independent as possible. This must be accomplished in a way that preserves dignity and motivates the survivor to relearn basic skills that the stroke may have impaired – skills like bathing, eating, dressing and walking.

    Your recovery team

    To help you meet your stroke recovery goals, your rehab program will be planned by a team of professionals. This team may include some of the following:

    • Physiatrist. Specializes in rehabilitation following injuries, accidents or illness
    • Neurologist. Specializes in the prevention, diagnosis and treatment of stroke and other diseases of the brain and spinal cord
    • Rehabilitation Nurse. Specializes in helping people with disabilities; helps survivors manage health problems that affect stroke (diabetes, high blood pressure) and adjust to life after stroke
    • Physical Therapist (PT). Helps stroke survivors with problems in moving and balance; suggests exercises to strengthen muscles for walking, standing and other activities
    • Occupational Therapist (OT). Helps stroke survivors learn strategies to manage daily activities such as eating, bathing, dressing, writing or cooking
    • Speech-Language Pathologists (SLP). Helps stroke survivors re-learn language skills (talking, reading and writing); shares strategies to help with swallowing problems
    • Dietician. Teaches survivors about healthy eating and special diets (low salt, low fat, low calorie)
    • Social Worker. Helps survivors make decisions about rehab programs, living arrangements, insurance, and support services in the home
    • Neuropsychologist. Diagnoses and treats survivors who may be facing changes in thinking, memory, and behavior after stroke
    • Case Manager. Helps survivors facilitate follow-up to acute care, coordinate care from multiple providers, and link to local services
    • Recreation Therapist. Helps stroke survivors learn strategies to improve the thinking and movement skills needed to join in recreational activities
