This is a difficult topic for many people to think about, and especially to bring up in a public forum, but people still want and need to know what they might expect as they or someone they care for are declining. It is understandable to fear the unknown, and to fear suffering. I would say that there are a few leading points here:

1) Most patients don t suffer much as their cancer progresses and as they transition through the dying process.

2) This process is quite variable from one person to another, but we typically have a good idea of what a patient s leading problems will be weeks to months before a person is experiencing a more rapid decline.

3) Engaging hospice services/palliative care can help guide expectations and generally manage many of the problems effectively by anticipating them, rather than waiting until very late to accept palliative care.

While I would never want to romanticize or minimize the challenges of the dying process, I find that the majority of my patients experience a controlled decline in which they really don t suffer. The most common pattern I see is that someone with a progressing cancer will slow down, eat less, begin to lose weight more steadily, and just gradually become less and less active. They no longer go out on walks, then leave the house less often, then spend more time alternating between sleeping and getting up in a chair, then eventually get to a point where they are sleeping most of the time and are pretty much bed-bound. They have less interest in eating, which is sometimes frustrating to the patient but often more so to the family and supporters of the person, who may pressure the patient to force down food that they have little or no appetite for (the danger is that some patients describe distress from the unyielding pressure from well-intentioned but nagging loved ones). Soon, they are sleeping more and more, to the point that they eventually sleep all of the time and are no longer communicative; this is followed by irregular breathing, more prolonged pauses between breaths (called agonal breathing, though it isn t uncomfortable for the person, just a reflex the last phase of the dying process), and then they stop breathing. It s important to remember that this common pathway of progression isn t a lack of will on the part of the person with progressing cancer: they would eat and be more active if they could, but the cancer pours out toxic proteins that mediate this inexorable process.

It s true that some patients have pain, or a terrible cough, or shortness of breath, agitation, and other problems, but I would say that most of the time, we have signals that these are going to be issues for a long while before the later stages, and it s advisable to deal with them as aggressively as possible before these symptoms become a crisis. Secondly, I most commonly see problems emerge when people (patients and/or their doctors) are very resistant to enlisting hospice support, which typically does a very good job of addressing symptoms proactively, before they become a crisis, and also being able to provide the comfort of helping patients and families/caregivers to understand what to expect in the near future. Just as on a plane, turbulence is much easier to understand if you know to expect it, what it represents, and how you ll get through it.

About 20 years ago, I spent some time in medical school doing home visits with the very compassionate and thoughtful Medical Director of a Boston hospice. I asked him about his feelings on euthanasia, and he told me that he didn t actually feel that it was a pressing need in almost any cases. Specifically, he noted that it was most common for people to fear two things about dying: being alone in being in pain. Though being a primary caregiver is a very hard job, it is a great blessing to have someone there to help as a caregiver to a terminally ill patient something that even attentive medical care can t substitute for. That care and the ability to ensure that people aren t alone is incredibly helpful. But a medical team today can usually do a good job to minimize the physical suffering of the process to relieve pain and other symptoms. And most of the time, what we see is a patient gradually withdrawing and eventually passing comfortably.

Many states have programs to help residents of the state purchase a new home. In Florida this is the S.H.I.P. or State Housing Initiative Partnership, Program. The program was established by the William E. Sadowski Affordable Housing Act of 1992 which provided for a dedicated revenue source with which local governments could help make housing more affordable.

There are several criteria that must be met for one to be eligible for the Florida SHIP program. Applicants may not have owned a home in the previous three years, they must not be purchasing property that will be occupied by a tenant other than themselves, use the home that is to be purchased as a primary resident, and meet the income eligibility requirements based on family size. The program is designed to help those family with incomes ranging from very low to moderate.

As with all things associated with buying a new home there are some general guidelines as well. Ones credit history must be adequate for mortgage purposes, the mortgage must not exceed the appraisal price of the home, and the home must be in good condition.

County governments are given money each year as long as they have a local housing assistance plan. They also usually have to ensure that mortgage payments don’t exceed thirty percent of the median income of the area. The money can be used for a variety of uses including new construction, down payments, closing costs, and mortgage buy-downs. The local governments are required to use the funds for certain demographics based on what the money will be used for and where.

Another important factor to consider is that the Florida SHIP program is essentially a loan. This loan must be repaid if the property is not held for at least twenty years.

Signing up for the Florida SHIP program is relatively simple. The process begins in the county office for housing assistance. This can be located through the U.S Department of Housing and Urban Development. Most Florida banks, mortgage companies, and real estate agents are also familiar with the program and can assist home buyers in finding and filling out the appropriate applications.

Welcome to the “LARGEST” College Football Fight Song web site in the universe, where all the songs are now in MP3 format. I have always been a fan of college football and especially the Nittany Lions of Penn State. All the MP3’s on this site have been compressed 11 to 1 in size and reformatted in mono, all to help save webspace. It has been a rough ride these past 2-3 years as interest seems to hae waned a bit and it is more costly to keep the site up for FREE! And now certain schools like Miami (FL), Washington and Utah forced me to remove the school’s songs from the website, even though they are free to download and isn’t this kind of like FREE advertisement for those schools? And all those millions of dollars the football programs bring and to top that with the price of tuition, you would think they would want to share the songs with their alumni, current students and fans of those schools. If anyone can help, please do to get those songs back up and available on this site. I am also in the process of adding as many or all of the football schools from the FCS (Football Championship Subdivision). (5) major BCS conferences and who play in the other FBS mid-major football conferences. My collection now contains 21 different CD’s, with Volumes 1 & 13 focus on all five of the power five conferences (ACC, Big Ten, Big 12, PAC-12, SEC and Notre Dame). These same conferences are also focused on: Volumes 2, 3, 4, 5, 6, 8, 19, 20 and 14 (alma maters). Jeudi 29 juin a eu lieu la cinquième Soirée des partenaires de l’école, organisée par le service des Relations industrielles. L’événement a rassemblé environ quatre-vingt personnes, industriels, représentants du secteur économique régional, universitaires et étudiants.

Une table-ronde, animée par Fabien SOULIÉ, enseignant-chercheur, a abordé le thème de « La fabrication additive dans les applications industrielles, citoyennes, de la recherche et de la formation ».

Stéphane ABED, président de la société Poly shape, Yann LEFEBVRE, président du FabLab « LabSud », André CHRYSOCHOOS, directeur du centre PRO3D et Christian JORGENSEN, professeur au CHU de Montpellier et directeur de l’unité INSERM “Cellules souches, plasticité cellulaire, médecine régénératrice et immunothérapies”, sont intervenus pour partager leurs expériences et échanger sur les applications de ces technologies.

Les diverses interventions des invités ont permis de montrer le spectre très large des champs d’applications de ces nouvelles techniques de mise en œuvre de la matière, allant de l’industrie de pointe à la recherche médicale avancée, sans oublier les applications standardsaccessibles à tous ” précise Fabien Soulié.

Les présentations ont aussi su faire ressortir toutes les étapes de la démarche de conception, de dimensionnement et d’optimisation, préalables à la phase d’impressionproprement dite ” ajoute André Chrysochoos, concluant que “la fabrication additive est apparue aux participants comme une solution complémentaire aux techniques classiques de fabrication, mais possédant ses propres limites “.

How Brake Light Wiring Works

When you press the brake pedal, remember that you’re triggering the brake light switch, too.

­If your brake light switch isn’t working properly, there’s a chance that you could be driving under risky conditions. Your brake lights may not light up when you press on the brake pedal or they may stay lit regardless of whether you’re pressing the pedal or not. Or perhaps, some switch malfunction is causing the lights to operate intermittently. So you can see the problem: If you don’t regularly check your brake lights for proper operation, you may be driving around town with no brake lights at all; brake lights that are continuously on; or brake lights that work only part of the time. Either way, it’s dangerous to you and other drivers around you.

Before you get too anxious about the brake work that you may or may not need, you may want to investigate the problem yourself. Actually, it may be an easier fix than you think, just by investigating your brake light switch. You can see your brake light switch by looking under the dash, near the top of the brake pedal. The brake light switch is usually attached to a small bracket that holds the switch, activated when the pedal is depressed, in position.

Once you’ve located the switch, check to make sure that the brake light switch’s electrical connection is firmly attached and that all of the wires connecting to the brake light switch are in good condition (at least as far as you can see from your position). Then check to make sure that the switch isn’t out of position or stuck in the on or off position. Unless there’s been a failure somewhere within the switch itself — a highly unlikely problem, by the way — it’s relatively simple to decide if the switch is at fault.

If you’re able to determine that it is indeed your brake light switch that’s at fault, replacement is simple. In fact, it’s likely that you won’t even have to get your hands dirty to complete a brake repair of this sort. Remember, there are several different types of brake switches available, including ­hydraulic brake switches, so be sure to get the one that’s right for your specific vehicle. You’ll need to know the make, model and year of your vehicle but really that’s about all. Your local auto parts store should be able to locate the correct part for you.

It may seem elementary, but remember that it’s important to properly diagnose the problem prior to buying any repair parts. It won’t do you any good to buy a new brake light switch if the problem is in the wiring leading up to the switch, in the connector itself or in the wiring that goes from the switch to the brake lights at the rear of the vehicle.

Sure, you can create great works with nothing more than a camera, but if you want to show your absolute best, you need to have access to the best equipment, facilities, and software out there. A film school can offer you sound stages, lighting kits, editing software, cinematography rigs, and pretty much anything else you could want, allowing you to practice like a professional and show the world what you can do.

2. Human Resources

Beyond equipment, software, and facilities, there is another resource film school can provide that you might not have considered. People. When you try to create on your own, you take on most of the responsibilities of the filming process. You are the director, editor, screenwriter, producer, special effects artist, audio engineer, etc. Unless you have some talented friends who are into film, you probably manage every part of the film besides the acting. And even when you only need actors, it can still be hard to find enough people to make your vision a reality.

When you go to film school, you aren’t just gaining access to great equipment, you are gaining access to like-minded peers. You’re suddenly surrounded by aspiring actors, writers, directors, cinematographers, and editors. You have the ability to focus on the aspects of film that truly matter to you while letting other students take on the things that are important to them. The headache of gathering up enough people, and taking care of EVERYTHING is gone.

3. Staff of Industry Professionals

Whether you choose to admit it or not, Youtube is not as great a teacher as. well an actual teacher. And at CCH we don’t just hire amazing professors, we hire industry professionals. With personal feedback from pros (many of whom still work professionally) who put time into getting to know and understand your talents, goals, and challenges, you will receive an education that is unique to you. And t hat is something the internet can’t give you.

4. Detailed Path and Curriculum

The courses that you take in college aren’t just thrown together in some arbitrary way. The curriculum is designed to put student success before anything else and at CCH we continuously update and improve our courses and degree paths to match the ever-changing industry.

When pursuing a film career without the help of film school, you have to figure out what is important on your own. There is no guiding hand to help you along the way, just you deciding what you want to learn. This can be freeing in some ways, but there are certain things that are crucial to success. Without exposure to the industry through college, you are at risk of missing out on some of those “must know” things.

5. Networking

Networking is the key to success in any field, but it is especially important in the film world. If you don’t know anyone who is currently working on a project, if no one in the industry is aware of your skill level, or if no one is willing to give you your “big break,” it will be hard to gain any traction on your journey to becoming a professional.

Film school gives you the chance to work with industry professionals who will be cheering for you, wishing you success, and who might be able to provide you with job opportunities. In addition to this, film school gives you the opportunity to work with many other aspiring professionals who might remember you when they are in need of someone with your specific talents. If you make yourself known around your college as a talented person who is great to work with, you are bound to have opportunities come to you in the future.

Not Convinced

Film school isn’t for everyone; college isn’t for everyone, but before saying no to it, consider the above. Will you have the drive and determination to create without help? Will you be able to learn everything you need to with only your computer as a resource? Will you be able to find work experience without connections? For you, the answer could be yes. There are many artists out their who have made a name for themselves without a college education, but make sure you know what path is right for you before making your choice.

Wait, Wait. Don’t Tell Me! Host Peter Sagal and Panelist Paula Poundstone, along with Car Talk’ s Tom and Ray Magliozzi, and other NPR folks, have joined forces with Car Talk’s Vehicle Donation Program. to tow listeners’ cars in support of the programs they love.

Here’s how it works: Car Talk sells listeners’ cars (with their permission, of course!), and the listeners’ local NPR Member Stations get the proceeds from the sale. This is Car Talk ‘s contribution to help pay the bills at stations across the country. We figure this is better than asking Carl Kasell to clean windshields for change in the NPR parking lot!

Recently, Sagal and Poundstone shared stories with us about their first set of wheels, and we got a little insight into their relationship with their cars.

Locks of Love is a public non-profit organization that provides hairpieces to financially disadvantaged children in the United States and Canada under age 21 suffering from long-term medical hair loss from any diagnosis. We meet a unique need for children by using donated hair to create the highest quality hair prosthetics. Most of the children helped by Locks of Love have lost their hair due to a medical condition called alopecia areata, which has no known cause or cure. The prostheses we provide help to restore their self-esteem and their confidence, enabling them to face the world and their peers.

Solicitations often occur on behalf of Locks of Love as part of an event being held to benefit Locks of Love. These events are normally registered with Locks of Love and have obtained approval to use our trademark name and logo in association with promoting the event. The location was great as it was walking distance to Brisbane Royal Show which I had to attend and offered at an additional fee secure parking. Walking distance valley for great food at China Town but thats were it ended.
The room was very average, very dated, the bathroom was small and is in desperate need of a re vamp, the room itself was light but had a horrible musty smell, even tried to air it out but no luck it still smelt terrible.

The Workers’ Compensation Claim Process

Most employers are required to carry workers’ compensation. or workers’ comp, insurance to cover employees who are injured on the job. Employees do not have to prove fault in order to recover, but benefits are not automatic. Specifics of the workers’ compensation claims process vary by state, but injured employees generally must notify their employer and the workers’ comp carrier. Regardless of state laws, however, injured employees are always encouraged to obtain any necessary medical treatment first.

Assuming you’ve already received medical attention, the workers’ compensation claim process involves several important steps. You must notify your employer in writing; fill out an official claim form (which should be provided by your employer); and keep detailed records about your treatment.

The workers comp claim process is discussed in greater detail below.

Get Immediate Medical Attention

Before filing a workers’ comp claim, make sure you get the necessary medical treatment. Some workers’ comp policies require injured employees to see a specified doctor, so you may want to ask your supervisor. But many state laws allow you to get a second opinion if you are not satisfied with the first one. Even if you don’t feel the need to get medical attention, it may be a requirement for the workers’ compensation claim process.

Keep in mind that a medical report will serve as an official record of your injuries and the basis for any workers’ comp reimbursement.

Notify Your Employer

Make sure you notify your employer about the injury within the statutory deadline, preferably soon after the injury occurs. In New York, for example, an employee has 30 days in which to notify their employer about a job-related injury. It’s a good idea to report all workplace accidents even if you don’t suspect an injury, just in case an injury is discovered after the deadline expires.

As with any legal process, make sure you notify your supervisor in writing. Even if you give verbal notification first, a written follow-up notification will provide an official record. The sooner you do this, the more details you will be able to recall.

Your employer probably will give you an official claim form as part of the workers’ compensation claim process. But if not, you should request one from the workers’ compensation board of your state. Generally, you will need to provide the following information on your workers comp claim form:

  1. Type of injury and affected areas of the body;
  2. Date, time, and location of injury;
  3. Parties involved in the accident;
  4. How the accident occurred; and
  5. Any medical treatment you have received

Workers’ Comp Claim Process: Employers’ Responsibilities

Employers that are required by law to provide workers’ comp coverage face the prospect of fines, criminal charges, and lawsuits if they fail to do so. Also, employers may not retaliate against a worker who claims workers’ comp. Therefore, employers have strong incentives to comply with the law and complete all valid claims.

Usually, your employer will file your claim with its insurer and the state workers’ comp board office. After your claim is evaluated by the insurer, an administrator will notify you about whether your claim has been accepted and the amount of benefits to which you are entitled.

After the Claim

Most of your involvement with the workers’ compensation claim process is complete after filling out the necessary paperwork. But you still want to follow up on your claim and make sure you keep detailed records. For instance, you may want to keep a journal of how the injury affects your work and day-to-day activities. Also, be sure to keep receipts for out-of-pocket expenses and proof of any other hardships caused by the injury.

If your claim is rejected, you will have the opportunity to appeal in most cases.

    O.J. Simpson’s Former Manager Norman Pardo Says O.J. Was Paid $600,000 To Claim He Wrote ‘If I Did It’

    O.J. Simpson’s close friend and former manager claimed Tuesday that Simpson did not in fact write the controversial book “If I Did It,” which explores how Simpson would have hypothetically carried out the murders of Nicole Brown Simpson and Ron Goldman if he had actually committed the crime.

    Simpson’s longtime associate Norman Pardo joined HuffPost Live’s Marc Lamont Hill on the 20th anniversary of the infamous police chase that grabbed the attention of millions as Simpson led the pack in his white Ford Bronco. During the interview, Pardo claimed Simpson only agreed to claim authorship of “If I Did It” for a $600,000 payout.

    “If I Did It” was originally announced in 2006 and was planned to coincide with a television interivew, but the project was scrapped in November of that year by NewsCorp and its subsidiary ReganBooks amid intense public criticism. In July 2007, the family of Ron Goldman acquired rights to the book and announced they would publish it in an effort to satisfy what Simpson owed them for the 1997 wrongful death suit against him.

    Pardo told HuffPost Live that Simpson had no part in writing the book:

    I remember when that book was coming out. O.J. called me. He said, ‘Here’s the deal,’ because it was in the news that he was going to do an interview. I said, ‘O.J. don’t do it, it’s stupid.’ He said, ‘Hey, they offered me $600,000 not to dispute that I [wrote] the book.’ He said, ‘That’s cash.’ I said, ‘They’re going to think you wrote it.’ He said, ‘So? Everybody thinks I’m a murderer anyway. They’re not going to change their mind just because of a book.’

    So who was actually behind the words in “If I Did It”?

    “A ghostwriter for [the book’s publisher] wrote the book. [O.J.] was going to do an interview to say, ‘I wrote the book, blah blah blah,’ and they give him money,” Pardo said.

    FAQ: What might I expect to happen when someone I – m

    #

    This is a difficult topic for many people to think about, and especially to bring up in a public forum, but people still want and need to know what they might expect as they or someone they care for are declining. It is understandable to fear the unknown, and to fear suffering. I would say that there are a few leading points here:

    1) Most patients don t suffer much as their cancer progresses and as they transition through the dying process.

    2) This process is quite variable from one person to another, but we typically have a good idea of what a patient s leading problems will be weeks to months before a person is experiencing a more rapid decline.

    3) Engaging hospice services/palliative care can help guide expectations and generally manage many of the problems effectively by anticipating them, rather than waiting until very late to accept palliative care.

    While I would never want to romanticize or minimize the challenges of the dying process, I find that the majority of my patients experience a controlled decline in which they really don t suffer. The most common pattern I see is that someone with a progressing cancer will slow down, eat less, begin to lose weight more steadily, and just gradually become less and less active. They no longer go out on walks, then leave the house less often, then spend more time alternating between sleeping and getting up in a chair, then eventually get to a point where they are sleeping most of the time and are pretty much bed-bound. They have less interest in eating, which is sometimes frustrating to the patient but often more so to the family and supporters of the person, who may pressure the patient to force down food that they have little or no appetite for (the danger is that some patients describe distress from the unyielding pressure from well-intentioned but nagging loved ones). Soon, they are sleeping more and more, to the point that they eventually sleep all of the time and are no longer communicative; this is followed by irregular breathing, more prolonged pauses between breaths (called agonal breathing, though it isn t uncomfortable for the person, just a reflex the last phase of the dying process), and then they stop breathing. It s important to remember that this common pathway of progression isn t a lack of will on the part of the person with progressing cancer: they would eat and be more active if they could, but the cancer pours out toxic proteins that mediate this inexorable process.

    It s true that some patients have pain, or a terrible cough, or shortness of breath, agitation, and other problems, but I would say that most of the time, we have signals that these are going to be issues for a long while before the later stages, and it s advisable to deal with them as aggressively as possible before these symptoms become a crisis. Secondly, I most commonly see problems emerge when people (patients and/or their doctors) are very resistant to enlisting hospice support, which typically does a very good job of addressing symptoms proactively, before they become a crisis, and also being able to provide the comfort of helping patients and families/caregivers to understand what to expect in the near future. Just as on a plane, turbulence is much easier to understand if you know to expect it, what it represents, and how you ll get through it.

    About 20 years ago, I spent some time in medical school doing home visits with the very compassionate and thoughtful Medical Director of a Boston hospice. I asked him about his feelings on euthanasia, and he told me that he didn t actually feel that it was a pressing need in almost any cases. Specifically, he noted that it was most common for people to fear two things about dying: being alone in being in pain. Though being a primary caregiver is a very hard job, it is a great blessing to have someone there to help as a caregiver to a terminally ill patient something that even attentive medical care can t substitute for. That care and the ability to ensure that people aren t alone is incredibly helpful. But a medical team today can usually do a good job to minimize the physical suffering of the process to relieve pain and other symptoms. And most of the time, what we see is a patient gradually withdrawing and eventually passing comfortably.

    The Most Surprising Thing I Learned Working In Hospice Care

    In retrospect, hospice work isn’t so outrageous a field for me to enter.

    I hadn’t thought about what kind of kid I was until beginning my second-year clinical internship this year at an inpatient hospice in Philadelphia. I asked about death early, and I asked a lot of questions. Here and there, memories emerge of conversations I had early on while sitting in the backseat, listening to NPR. I remember a story about palliative care, and remember asking why it was controversial (this was more than a few years ago).

    Even so, I was scared to begin my internship. I was scared to hold people’s grief and anguish, scared of all the myriad ways that countertransference seeps out of our interactions with death and dying, scared to be considered weird and different for my interesting in working in thanatology.

    But I forged ahead. I took the internship, still slightly frightened and unsure, and gave myself permission to find a new placement if I couldn’t handle working in hospice.

    I could not be more pleased that I did just that. Working in hospice feels like it suits my soul, something at the very nature and essence of my being. People always ask me about what I do, and I grit my teeth before telling them, knowing that the reaction is a bit of morbid shock and awe, some fear, some respect hiding in the background.

    And I feel all those things, every single day, every interaction I have with a patient or their family. I am honored to participate in this process, to provide comfort, reassurance, and stability.

    Something I have realized during my work in hospice is that to have an interest in death and dying does not imply a morbid fascination with the end of life. To me, it suggests a deep reverence for the life we are all lucky to live, even if only for a short time.

    In assisting individuals and families through the dying process, we are also showing them how to continue living. To provide comfort to patients and their families and to do so with dignity, to lead by example in living a life that is affected by death but not overshadowed by it lets families know that they can do the same.

    To a certain extent, I believe that hospice work is the kind of work you do if you can. There is something of a moral obligation to respond to the calling, or inclination, or interest–whatever you want to call it. The nurses, social workers, doctors and volunteers I have been fortunate enough to work with this past year are such immensely dedicated, compassionate, strong individuals, who perform their work with strength, pride, but above all, tenderness.

    I feel blessed to be a part of a team that gives themselves to their work, that respects their patients and families, and that above all, honors life.

    The location was great as it was walking distance to Brisbane Royal Show which I had to attend and offered at an additional fee secure parking. Walking distance valley for great food at China Town but thats were it ended.
    The room was very average, very dated, the bathroom was small and is in desperate need of a re vamp, the room itself was light but had a horrible musty smell, even tried to air it out but no luck it still smelt terrible.

    Hvornår er det bedst at tage til Paris?

    Det er næsten altid muligt at finde billige rejser til Paris. men undgår du højsæsonen fra 1. april til 31. oktober, er det nemmere endnu.     Paris centrale placering gør den til et knudepunkt for luftfartstrafikken i Europa. Den største lufthavn hedder Charles de Gaulle (CDG) og ligger 23 km nord for byen. Herfra er der tog til Gare du Nord, en af byens seks store banegårde. 14 km syd for byen ligger lufthavnen Paris-Orly (ORY), hvorfra der også er tog ind til Paris’ centrum. Paris Beauvais (BVA) ligger ca. 90 km – en time i regionaltog eller bil – fra Paris.

    Paris’ og Europas mest berømte vartegn er Eifeltårnet. der blev bygget i 1889 af Gustave Eifel. Det var oprindeligt konstrueret til at holde i kun 20 år. Men i dag kan mere end 7 millioner turister hvert år stadig se ud over byen fra 276 meters højde. Over for tårnet, på den anden side af Seinen. ligger bakken Trocadéro og på den det nye Palais de Chaillot fra verdensudstillingen i 1937, der erstattede det gamle fra 1867. Andre højdepunkter i Paris er: Mona Lisa s hjem Louvre. kirken Sacré Coeur i Montmartre-kvarteret. Triumfbuen og Versaille-slottet.

    This is a difficult topic for many people to think about, and especially to bring up in a public forum, but people still want and need to know what they might expect as they or someone they care for are declining. It is understandable to fear the unknown, and to fear suffering. I would say that there are a few leading points here:

    1) Most patients don t suffer much as their cancer progresses and as they transition through the dying process.

    2) This process is quite variable from one person to another, but we typically have a good idea of what a patient s leading problems will be weeks to months before a person is experiencing a more rapid decline.

    3) Engaging hospice services/palliative care can help guide expectations and generally manage many of the problems effectively by anticipating them, rather than waiting until very late to accept palliative care.

    While I would never want to romanticize or minimize the challenges of the dying process, I find that the majority of my patients experience a controlled decline in which they really don t suffer. The most common pattern I see is that someone with a progressing cancer will slow down, eat less, begin to lose weight more steadily, and just gradually become less and less active. They no longer go out on walks, then leave the house less often, then spend more time alternating between sleeping and getting up in a chair, then eventually get to a point where they are sleeping most of the time and are pretty much bed-bound. They have less interest in eating, which is sometimes frustrating to the patient but often more so to the family and supporters of the person, who may pressure the patient to force down food that they have little or no appetite for (the danger is that some patients describe distress from the unyielding pressure from well-intentioned but nagging loved ones). Soon, they are sleeping more and more, to the point that they eventually sleep all of the time and are no longer communicative; this is followed by irregular breathing, more prolonged pauses between breaths (called agonal breathing, though it isn t uncomfortable for the person, just a reflex the last phase of the dying process), and then they stop breathing. It s important to remember that this common pathway of progression isn t a lack of will on the part of the person with progressing cancer: they would eat and be more active if they could, but the cancer pours out toxic proteins that mediate this inexorable process.

    It s true that some patients have pain, or a terrible cough, or shortness of breath, agitation, and other problems, but I would say that most of the time, we have signals that these are going to be issues for a long while before the later stages, and it s advisable to deal with them as aggressively as possible before these symptoms become a crisis. Secondly, I most commonly see problems emerge when people (patients and/or their doctors) are very resistant to enlisting hospice support, which typically does a very good job of addressing symptoms proactively, before they become a crisis, and also being able to provide the comfort of helping patients and families/caregivers to understand what to expect in the near future. Just as on a plane, turbulence is much easier to understand if you know to expect it, what it represents, and how you ll get through it.

    About 20 years ago, I spent some time in medical school doing home visits with the very compassionate and thoughtful Medical Director of a Boston hospice. I asked him about his feelings on euthanasia, and he told me that he didn t actually feel that it was a pressing need in almost any cases. Specifically, he noted that it was most common for people to fear two things about dying: being alone in being in pain. Though being a primary caregiver is a very hard job, it is a great blessing to have someone there to help as a caregiver to a terminally ill patient something that even attentive medical care can t substitute for. That care and the ability to ensure that people aren t alone is incredibly helpful. But a medical team today can usually do a good job to minimize the physical suffering of the process to relieve pain and other symptoms. And most of the time, what we see is a patient gradually withdrawing and eventually passing comfortably.
