Pain Management and Symptom Control

The spirit of The Connecticut Hospice, Inc. is to encourage quality of existence for patients and families. Because of this abiding philosophy, pain and symptom control lie at the very heart of the Hospice program of care. Medical and pharmacological therapies control a range of debilities that, if untreated, sap a patient’s strength, will, and even human dignity.

Professional expertise and an individualized care plan make possible a control of pain rarely achieved in other health care settings. Nowhere is this more evident than in the work of the hospice physician, nurse, and pharmacist. The physician, nurse, and pharmacist are important team members in evaluating pain and treating it pharmacologically. Artists, social workers, clergy, nurses, and professional and lay volunteers work with psychological and spiritual pain. As a team, they review the patient’s status daily. In addition, consultants in several fields of health care complement the efforts of the medical and nursing staffs.

Prior to admitting a patient to the Hospice program, the Hospice physician confers with the family’s physician. They retain open lines of communication. In the home care program, the family’s physician acts as the primary physician. The role of the Hospice physician varies from case to case. In the inpatient unit the Hospice physician assumes the primary physician role. In both programs, medical direction is available twenty-four hours a day.

The care plan must be creative, innovative, and flexible to respond to the constantly changing challenges of irreversible illness. Some symptoms can be relieved by simple measures such as repositioning, massage, relaxation techniques, and distraction through arts and other activities. Yet medications are often crucial in alleviating physical discomfort.

In treating the patient, the Hospice physician and pharmacist are guided by repeated assessments, with adjustments in medications, and times of administration so that the patient will be as comfortable as possible.

An important principle in The Connecticut Hospice approach is that drug doses are carefully adjusted to each patient’s physical make-up. This assures pain relief without loss of alertness. The Connecticut Hospice goal is to control symptoms while maintaining optimum functioning.

At Hospice, the patient’s needs dictate the medication level. Medications are administered on a regular schedule, to eliminate not only pain, but also the fear of pain. Ease of administering is a key consideration also. Patients ar

When a medication is not available commercially in the exact dosage needed, the Connecticut Hospice pharmacist is able to meet individual needs. An active participant in patient care, the pharmacist attends morning rounds and weekly team meetings, serving as a source for current drug information and a consultant for changing drug regiments. Both the Hospice pharmacist and physician strive to educate others on the goals and parameters of hospice caregiving. Advances in symptom control present diversified and constant challenges. As Hospice physicians and pharmacists discharge their demanding duties, they are in the forefront of palliative care. Hospice remains identified by its excellence of symptom management and support. e spared injections whenever possible to make their lives more comfortable. Over 90 percent of the medications at Hospice are taken orally. Because of this, it is often possible for patients to be cared for at home.

Claude Benny Johnson, age 72, of Gray, died Friday, March 3, 2017 at his residence. He was the son of the late Roy Ledford and Marie Fry Johnson, born to them on December 28, 1944 in Gray, Kentucky. Benny was a retired clerk for the Burlington Northern Santa Fe Railroad Company. He was a United States Army Veteran, having served in the Vietnam War. Benny enjoyed all sports especially U.K. Basketball, Photography, Hiking, hunting morel mushrooms, was an avid Chicago Cubs fan and loved watching his great niece and nephew playing sports. Along with his parents, he was preceded in death by two brothers: Lee Roy and Hershal Ledford Johnson. Benny is survived by one sister: Karen Moses and husband, Kevin of Gray, Kentucky. Two brothers: Louie Johnson and wife, Pattie of Caledonia, Minnesota and Keith Johnson and wife, Robyn of Gray, Kentucky. American International Group Inc. said it has named Peter Zaffino as its new chief operating officer, effective August 1. Zaffino comes to the role from Marsh LLC, a unit of insurance brokerage Marsh McLennan Cos. Inc. where he was chief executive. Zaffino has also done stints at Guy Carpenter Co. and at a GE Capital portfolio company. He will have a base salary of $1.25 million, a short-term annual bonus target of $3 million and an annual long-term incentive award of $4.25 million. The executive will also receive a signing-on bonus of $15 million, partly in the form of stock options. AIG shares were slightly lower premarket, but are down 3.8% in 2017, while the S P 500 has gained 8.7%.

As part of the deal, AIG has agreed in principle to acquire Hamilton USA, the U.S. platform of Hamilton Insurance. The companies will expand the target market of their Attune platform, which they launched in September 2016 to serve the U.S. small to medium-sized commercial insurance market. Attune will now target companies with annual revenue of up to $35 million, a market the companies estimate is worth up to $150 billion in annual gross written premiums. AIG and Two Sigma will enter a partnership to advance the latter s data science and technology for AIG s commercial insurance business. Hamilton Re and AIG will enter a reinsurance partnership, in which Hamilton Re will participate in more of AIG s ceded reinsurance. Putting data science and technology to work in our industry has been on my agenda for some time, said AIG s new Chief Executive Officer Brian Duperreault, founder and ex-CEO of Hamilton Insurance. Ricoh unveils solar array to power New Jersey facility, celebrates environmental sustainability milestones

MALVERN, Pa. April 24, 2017 /PRNewswire/ — Ricoh USA, Inc. today announced the latest in a series of sustainability milestones, including a new solar array that will power its West Caldwell, N.J. office facility. The array, which is set to provide for more than half of the facility’s electrical needs, literally represents the power of Ricoh’s ongoing sustainability efforts. This is the latest example of Ricoh’s environmental consciousness and measurable progress in sustainability efforts.

The solar array project began in March 2014. through collaborative work with EnterSolar. a leading New York City -based provider of solar solutions to commercial enterprises. Now live, the array is expected to generate more than 790,000 Kwh per year, providing more than 50 percent of the facility’s electrical needs. That translates to approximately 1.3 million miles driven by an average passenger vehicle, 555 fewer metric tons of CO2 produced per year and $1.9 million in energy savings over the next 10 years. The array itself is roughly 1.3 times the size of a football field.

“Concern for the environment is in Ricoh’s DNA. It’s core to our corporate identity,” said Donna Venable. Executive Vice President, Human Resources and Deputy General Manager, Shared Services, Ricoh Americas. “Since Ricoh established its Environmental Promotion Group more than 40 years ago, we have worked hard to foster sustainability from the top down and the bottom up. In fact, the West Caldwell facility itself has a strong sustainability history. The building has achieved ENERGY STAR® certification for three years in a row, and its on-site Biodiversity-Pollinator Garden achieved Conservation Certification from the Wildlife Habitat Council .

Unable to Log On

I reloaded this app on my newer iPhone after it developed some sort of crashing issue (something I assumed was isolated to the app on my phone). However when I go to log on, it doesn’t accept my information because the app still believes my user name to be something that it no longer is. No matter how many times I reloaded the app and reset my password (and entered in the correct, reset password) I was not able to gain access to my own account!

The clear solution to this is to allow users an option to enter their user name, even when redownloading this application from the iCloud.

Please take heed of this review and update the app. I love it, but if I can’t use it with this incarnation of the software, this version of the app is not any good.

GOOD BUT RESTRICTIVE

The only thing I do not like about pof site is that it dictates what user age group you are allowed to contact or browse. I am 43 male and I can not browse profiles of girls younger than 29 years old! Why? On the other hand they would match me with ladies up to 60+. I do not know if this rule only apply to the free membership or not. Over all pof is much better than many dating sites I have used. I hope they would allow people the freedom of choosing what they want while continuing to give members the choice of using filters. Thank you.

The official app is the best app

For years I used Tweetbot but as my primary Twitter client but always had the official Twitter app in the background for certain and uses. I decided to try the Twitter app full-time this week and man am I impressed. Using the official Twitter app breathes new life into Twitter, makes me much more excited and invested in using the platform. This app has polish that Tweetbot doesn’t have. It also supports features such as poll and moments that other apps don’t. After a week of using the official Twitter client, I am now regretting I hadn’t switched much earlier.

    I wish more people knew about hospice, or that I did sooner. They offer free grief counseling, have a fantastic lending library, a beautiful space, and lots of grief therapy groups. Luckily I haven t had to use too many of their resources but everyone is welcome and they understand the grieving process. The grief therapy group I was in (and there are lots of speciality ones, some for parents, some for art, some for suicides, some for terminal illnesses) was incredibly supportive and helpful. Meeting times are weird, but it s a free and wonderful service. Highly recommend.

    A special Thank You to the Norma F. Pfriem foundation for their generous donation of 52 beds.

    The Norma F. Pfriem Foundation has granted an extraordinary opportunity for patients and families to have the luxury of a first-class bed, upon their arrival. The Connecticut Hospice, Inc. the first palliative teaching hospital, who received the first Advanced Palliative Care certification in the United States of America, became the recipients of an enormous gift.

    A beautiful bed for each patient and family with a state-of-the-art mattress, as envisaged in the finest homes in America, arrived for our bedrooms on Thursday, March 20, 2014.

    As Spring evolves, our Norma F. Pfriem beds, together with our precious patients, will be seen on the Norma F. Pfriem Promenade. The Norma F. Pfriem Promenade, goes right to the sea and in their new Norma F. Pfriem beds, our patients will be able to sit up and enjoy the splendor of sunrises and sunsets with their families.

    One of our housekeepers, Luis, said that this would be “beautifully best for families.”

    Co-Chairman of the Board and of the Professional Advisory Committee, Dr. Wayne Southwick, M.D. together with Dr. Wen-Jen Hwu, M.D. Ph.D. Board of Directors and Chairman of Professional Advisory Committee, The University of Texas MD Anderson Cancer Center, Professor, Department of Melanoma Medical Oncology and United States Chairman of Woman in Cancer Research, and member Chief William Farrell, who is also Chairman of the President’s Circle, “never has a gift been beamed more intensely at our patients and families.” The only exception was 14 years ago when Norma F. Pfriem, herself, granted the state-of-the-art beds to each patient and family.

    The nursing staff all participate in the International Norma F. Pfriem Fellowship in Hospice and Palliative Care Nursing, has enabled over sixty-eight (68) of the nurses to sit and pass their national boards in hospice and palliative care and thirty-three (33) of our certified nurses aides.

    Some of those staff, pictured here in their ta-dah mode, were the very ones who selected the first* state-of-the-art, ICU superbly crafted mattress to support the remarkable patients that we are honored to care for.

    The luxury of this support is only matched by the magnificence of the Norma F. Pfriem nursing staff at the Hospice, as “The Connecticut Hospice, Inc. will celebrate its 40th Anniversary, or in a place where every minute counts, its 21st Million Minute, “Operation Blessings”.

    The Connecticut Hospice inaugurated hospice care in America in 1974. Since then, it has been the beacon and teacher of the growing hospice movement throughout the nation, and beyond.

    The Connecticut Hospice addresses physical, spiritual, social, and emotional needs of patients with advanced irreversible illness, and their families. Such care is provided regardless of diagnosis and as long as the Hospice level of care is needed by the patients.

    Hospice care is provided in the home, or inpatient setting by a medically directed, nurse-coordinated, interdisciplinary team, and continues throughout the period of bereavement.

    Hospice does not take a narrow view of complexities of the needs of patients with an irreversible illness and their families.     Pain Management

    Pain Management is one of the most important services provided by hospice. Although not all hospice patients will have pain, the control of pain is very important for those who do. The hospice philosophy recognizes that pain is whatever the person experiencing the pain states it is. The person alone is the expert regarding the pain that he or she is experiencing.

    What is pain? Pain is an individual experience which is expressed in many ways. Pain is classified as, 1) acute, which means it is sudden without warning, intense and generally doesn’t last very long; 2) chronic pain is a constant, persistent pain which lasts a long time.

    How do we manage pain? The hospice nurse will want to know where the pain is, when it started, how frequently it occurs, how long it lasts, whether it is burning, aching, sharp in quality and what you did to relieve the pain. The nurse will ask you to rate your pain using your pain scale journal.

    Chronic pain is a type of pain experienced by many people with certain conditions or diseases, such as cancer. Chronic pain causes physical discomfort and emotional discomfort. The hospice nurse teaches the principles of managing chronic pain. Our goal is to eliminate pain, or to reduce the pain to a tolerable level, and then prevent the pain from returning. In order to do this, pain medications must be used on a regular basis, in order to maintain an even level in the bloodstream at all times. If you wait too long, or wait until the pain emerges again, to take the pain medication, a larger dose will be needed to control the pain. We are trying to control and prevent pain, not wait until pain happens and then try to get rid of it.

    Drug addiction is a common fear of most people when trying to control pain with narcotics. When narcotics are used appropriately as prescribed by the physician for pain relief, they do not become addicting. Addiction occurs when people abuse the drug and they pursue the use of the drug to give them a state of well being. This is not the case when it comes to hospice patients, so please do not become concerned about addiction at this time. There may be a craving or an over concern about the next dose, in order to keep the pain from recurring. If this happens, it is not that you are addicted and need a drug, it may be that it is time for the medication to be increased to a stronger strength, in order to control the pain.

    Various Medication Forms. Medications come in many different types of forms: syrups, liquids, pills, capsules, rectal suppositories, injections, intravenous mixtures and also transdermal (“skin patches”). The physician will determine the medication which is most effective, and easiest to take, for the individual’s needs.

    About the Connecticut Hospice

    The Connecticut Hospice inaugurated hospice care in America in 1974. Since then, it has been the beacon and teacher of the growing hospice movement throughout the nation, and beyond.

    The Connecticut Hospice addresses physical, spiritual, social, and emotional needs of patients with advanced irreversible illness, and their families. Such care is provided regardless of diagnosis and as long as the Hospice level of care is needed by the patients.

    Hospice care is provided in the home, or inpatient setting by a medically directed, nurse-coordinated, interdisciplinary team, and continues throughout the period of bereavement.

    Hospice does not take a narrow view of complexities of the needs of patients with an irreversible illness and their families. While the control of physical pain and other physical symptoms is the central and primary concern for caregivers, it is not the only priority. The patient with an irreversible illness suffers from an array of emotional, spiritual, social and financial problems.
