How to File a Grievance

Patients may request that the Health Safety Net conduct a review of an eligibility determination, or of provider compliance with the Health Safety Net regulation. To file a grievance with the HSN, send a letter to

At a minimum, the letter should include your name and address. If possible, it should also include information about the situation, the reason for the grievance, the provider’s name (if a provider is involved), and any other relevant information. The more information you give, the better. It is very important to include the provider’s name if a provider is involved.

1. What is the Health Safety Net?

The Health Safety Net is a fund set up to help pay for health services for certain low income uninsured and underinsured individuals. The Health Safety Net used to be called the Uncompensated Care Pool (UCP), or Free Care.

2. Where can I use the Health Safety Net?

You can use the Health Safety Net at hospitals and community health centers. However, at most hospitals, the doctors bill separately. The Health Safety Net will pay for hospital facility charges (for example beds, nurses, and equipment), but you may have to pay bills for the doctors and for services like lab tests and x-rays. Be sure to check with your doctor first to see if the Health Safety Net will cover all the services you receive, or just some.

3. How long will I be eligible for the Health Safety Net?

You may have Health Safety Net eligibility for up to a year, but you may receive review forms before the year is over. If you get review forms or any other forms asking for information, please complete them as soon as possible. If your address, phone number, health insurance, or income changes, you need to call the MassHealth office (1-888-665-9993) immediately to give them your new information.

4. Do I need to be a U.S. Citizen to be eligible for the Health Safety Net?

You do not need to be a U.S. Citizen to be eligible for the Health Safety Net. You do need to live in Massachusetts and provide proof identity. More information can be found in the Member Booklet for Health Coverage and Help Paying Costs (ACA-1) .

5. What are Health Safety Net Primary and Health Safety Net Secondary?

If you do not have any other insurance, and qualify based on income, you will have Health Safety Net Primary. If you have other insurance, and qualify based on income, you will get Health Safety Net Secondary. If you have Health Safety Net Secondary, your other insurance will need to be first billed for health services you get before the Health Safety Net is billed.

6. How do I apply for the Health Safety Net?

You may apply using the MassHealth application called the Application for Health and Dental Coverage and Help Paying Costs (ACA-3) form. You can get a paper application by calling MassHealth at 1-800-841-2900 or meet the financial counselors at your local hospital or community health center for help with the application.

7. I have a letter that says I am eligible for ConnectorCare. What happens if I do not enroll in ConnectorCare? Can I still get health services from the Health Safety Net?

If you have been determined eligible for ConnectorCare, you have 90 days of HSN eligibility starting on your date of application to enroll in a ConnectorCare plan. If you do not enroll within this time period, you will no longer be eligible for HSN.

8. I have a deductible listed for my Health Safety Net. Where should I send that payment?

You can pay your deductible to the hospital or community health center after you get services. The hospital or community health center will bill you for the amount that you owe. It is very important to keep track of your payments so that you have a record of when you reach your deductible.

9. I have Medicare. Am I still eligible for the Health Safety Net?

Yes, as long as you meet all the eligibility requirements for the Health Safety Net, including income below 400% FPL. The Health Safety Net can pay for services that Medicare does not cover, as long as you get the care at a hospital or community health center. The Health Safety Net pays for your services after Medicare or any other insurance you may have has already been billed.

10. What are the co-payments for the Health Safety Net?

Patients ages 21 and older who use the Health Safety Net have to pay co-payments for prescription drugs. The co-payment amounts are $1 for generic drugs in the following drug classes: antihyperglycemics (diabetes drugs), antihypertensives (high blood pressure drugs), and antihyperlipidemics (high cholesterol drugs). The co-payment is $3.65 for other generic drugs and $3.65 for brand-name drugs. There is an annual maximum of $250 per patient on pharmacy co-payments.

11. Where can I fill my prescriptions with the Health Safety Net?

HSN has a limited number of CHC or hospital outpatient pharmacies in its network and each requires that your prescription be written by a clinician who works at that affiliated facility. In most cases, you will need to see a doctor at the hospital or community health center where the pharmacy is located in order to have your prescription filled there. The Health Safety Net will not pay for prescriptions you get filled at a local retail pharmacy (for example, CVS, Walgreens, etc.) unless they have a special agreement with a neighboring Community Health Center.

12. I have private insurance with a high hospital deductible. Am I eligible for the Health Safety Net?

Yes, as long as you qualify based on your income. Your provider will first bill your insurance for services. Then your provider will bill you for any deductible required for the Health Safety Net. Only afterwards, will the hospital be able to bill the Health Safety Net for the deductible required by your private insurance. The Health Safety Net will pay for deductibles and coinsurance, but not for co-payments required by private insurance plans.

13. I had ConnectorCare but did not pay my premium. Am I eligible for the Health Safety Net?

No. Patients who fail to pay their ConnectorCare premiums are not eligible for the Health Safety Net. You may be able to work out a payment plan with the Health Connector, even after termination. Please contact the Health Connector at 1-877-MA-ENROLL for more information.

Course Outline and Method:

The Advanced Integrated Communication Programme is a part-time post-graduate Course designed and meant specifically for working professionals. Gmail mass email tips: Avoid the spammy look with the personalized touch

Gmail contact groups are great for sending a single email message to a specific set of people, but what if you want to give each email a personal touch, such as with a newsletter or promotional offer? Unlike Microsoft Office. Google’s productivity suite doesn’t have a native mail-merge feature you can leverage to do this. But don’t sweat it: With a little help from Google Sheets and a free downloadable script, you can personalize bulk email messages in Gmail in minutes.

Just remember, Google allows you to send only 100 emails per day using scripts. Here’s how to get started.

1. Create a spreadsheet

Log into your Google Drive account. On left sidebar, click New Google Sheets to create a spreadsheet. You’ll be importing your email recipients’ contact information here, so give it a descriptive name.

Next, go to the menu bar and select Add-ons Get add-ons. This takes you to Google’s add-on store. In the search window, type a name for your mail merge script—for this example, we’ll use Yet Another Mail Merge —and hit Enter.

This is a public mail merge script. Click the Free button, and a window will open, asking for various permissions to access Google Sheets, Contacts, and Gmail. Click Accept. A window will pop up under Add-ons on the menu bar, indicating that the script has been installed. Click the X to close it.

2. Import your contacts

The next step is to import your email recipients into the spreadsheet. Click Add-ons Yet Another Mail Merge Import contacts from a group. A small window will open with a drop-down menu of all the contact groups in your Gmail account. You can also go into Gmail, and create a new group with all the contacts you want to include in your mail merge. Then repeat the steps above and select this group from the drop-down menu. Click Import contacts. and those contacts will populate your spreadsheet.

3. Draft your email message

Now open a new browser tab and log into your Gmail account. Click Compose to open a new message. Type a title in the subject field and compose the body of your message. Then type $%headername% anywhere you want to insert column data from your spreadsheet. For example, type Dear $%First Name% if you want each email to use the recipient’s first name in the greeting. Once you’re done, click X to close the message, and it will automatically save to your Drafts folder.

4. Perform the mail merge

Go back to your spreadsheet and click Add-ons Yet Another Mail Merge Start mail merge. In the window that opens, select the title of your email message from the Drafts drop-down menu. Check the Track Emails Opened box if you want to see which of your recipients opens the email.

The text EMAIL_SENT will appear next to each contact in the Merge Status column of your spreadsheet after a successful mail merge.

To make sure everything is formatting correctly, click Send me a test email to receive a sample message. Toggle to Gmail and check the message. If everything looks good, return to that dialog box and click Send Emails. You’ll now see EMAIL_SENT next to each contact in the Merge Status column of your spreadsheet. If you want to perform another mail merge from that same spreadsheet, remember to delete that text from the Merge Status column of each recipient you want to email again.

A Guide to Mass Shootings in America

There have been at least 90 in the last three-plus decades—and most of the killers got their guns legally.

Editor s note: In July 2012, in the aftermath of the movie theater massacre in Aurora, Colorado, Mother Jones created the first open-source database documenting mass shootings in the United States. Our research has focused on indiscriminate rampages in public places resulting in four or more victims killed by the attacker. We exclude shootings stemming from more conventional crimes such as armed robbery or gang violence. Other news outlets and researchers have published larger tallies that include a wide range of gun crimes in which four or more people have been either wounded or killed. While those larger datasets of multiple-victim shootings may be useful for studying the broader problem of gun violence, our investigation provides an in-depth look at the distinct phenomenon of mass shootings—from the firearms used to mental health factors and the growing copycat problem. Tracking mass shootings is complex; we believe ours is the most useful approach .

The interactive map below and our downloadable database have been expanded with 28 additional cases from 2013-2017. Dating from 2005, the FBI and leading criminologists essentially defined a mass shooting as an attack in a public place in which four or more victims were killed. We adopted that baseline when we gathered data in 2012 on three decades worth of cases. (It bears noting that there have been many similar indiscriminate gun rampages with fewer fatalities that would otherwise be included in our dataset .) In January 2013, a mandate for federal investigation of mass shootings authorized by President Barack Obama lowered that baseline to three or more victims. Accordingly, we include attacks dating from January 2013 in which three or more victims died. Our original analysis, which covers cases with four or more victims killed from 1982-2012, follows below. The cases we have documented since then using the revised federal baseline reaffirm our major findings.

Can the next attack be prevented?

It is perhaps too easy to forget how many times this has happened. The horrific massacre at a movie theater in Aurora, Colorado. in July 2012, another at a Sikh temple in Wisconsin that August, another at a manufacturer in Minneapolis that September—and then the unthinkable nightmare at a Connecticut elementary school that December—were some of the latest in an epidemic of such gun violence over the last three-plus decades. Since 1982, there have been at least 90 public mass shootings across the country, with the killings unfolding in 34 states from Massachusetts to Hawaii. Fifty-three of these mass shootings have occurred since 2006. Seven of them took place in 2012 alone, including Sandy Hook. A recent analysis of this database by researchers at Harvard University, further corroborated by a recent FBI study. determined that mass shootings have been on the rise .

We ve gathered detailed data on more than three decades of cases and mapped them below, including information on the attackers profiles, the types of weapons they used, and the number of victims they injured and killed. The following analysis covers our original dataset comprised of 62 cases from 1982-2012.

Weapons: Of the 143 guns possessed by the killers, more than three quarters were obtained legally. The arsenal included dozens of assault weapons and semi-automatic handguns with high-capacity magazines. (See charts below.) Just as a perpetrator used a .40-caliber Glock to slaughter students in Red Lake, Minnesota, in 2005, so too did the one in Aurora, along with an AR-15 assault rifle. when blasting away at his victims in a darkened movie theater. In Newtown, Connecticut, the attacker wielded a .223 Bushmaster semi-automatic assault rifle as he massacred 20 school children and six adults.

The perpetrators: More than half of the cases involved school or workplace shootings (12 and 20, respectively); the other 30 cases took place in locations including shopping malls, restaurants, and religious and government buildings. Forty-four of the killers were white males. Only one was a woman. (See Goleta, Calif. in 2006.) The average age of the killers was 35, though the youngest among them was a mere 11 years old. (See Jonesboro, Ark. in 1998.) A majority were mentally troubled—and many displayed signs of mental health problems before setting out to kill. Our focus is on public mass shootings in which the motive appeared to be indiscriminate killing. We used the following criteria to identify cases:

  • The perpetrator took the lives of at least four people. An FBI crime classification report identifies an individual as a mass murderer—versus a spree killer or a serial killer —if he kills four or more people in a single incident (not including himself), typically in a single location.
  • The killings were carried out by a lone shooter. (Except in the case of the Columbine massacre and the Westside Middle School killings, which involved two shooters.)
  • The shootings occurred in a public place. (Except in the case of a party on private property in Crandon, Wisconsin, and another in Seattle, where crowds of strangers had gathered.) Crimes primarily related to gang activity, armed robbery, or domestic violence in homes are not included.
  • Perpetrators who died or were wounded during the attack are not included in the victim counts.
  • We included a handful of cases also known as spree killings —cases in which the killings occurred in more than one location over a short period of time, that otherwise fit the above criteria.
