    Hospice Care Options

    Committed to ensuring the quality of life and respecting the choices of the patient and family, our team of hospice experts offers a wide variety of specialized services designed to meet the patient’s needs. The team works together to meet the unique physical, emotional and spiritual needs of each patient and family.

    Care for the whole person

    • The patient drives their plan of care through personal choices.
    • Family and loved ones are taught how to safely care for the patient. They are given support throughout the end-of-life process.
    • The nurse case manager is an expert in pain and symptom management. The nurse will institute the plan of care, and continue to assess changing needs.
    • Our medical director is available for home visits and consultation with the patient’s physician and nurse.
    • A social worker addresses the psychological and emotional needs of the patient and family, assuring access to community resources.
    • Chaplains can provide for spiritual needs.
    • Home health aides assist with personal care and light housekeeping.
    • Trained volunteers provide companionship, friendly visits and support for caregivers.
    • A registered dietitian assists with dietary needs.
    • Physical, occupational, respiratory, and speech therapy are available when deemed necessary for the patient’s comfort by the team.
    • A bereavement counselor provides support for at least thirteen months after a patient’s death.
    • SandCastles Grief Support Program for Children and Families. offers open-ended support for children who have experienced the death of a loved one.

    Where can we provide care?

    The hospice plan of care, successful pain management and family support make it possible for most hospice patients to remain at home where their loved ones can care for them. Families learn basic caregiving skills and techniques from hospice team members who regularly visit the home. When home care is not an option, hospice care provided to patients in:

    • Nursing homes
    • Assisted living centers
    • Adult foster-care homes
    • Homes for the aged
    • Senior citizen apartments
    • Hospitals

    Hospice admission

    When a patient requests admission to the hospice program, the hospice staff will contact his or her primary physician to make sure admission is appropriate at the time of the request.

    The patient will be asked to sign consent and insurance forms similar to the forms patients sign for hospital admission. An important form specific to hospice says that the patient understands that hospice care is aimed at pain relief and symptom management, not curative treatment. Additional forms outline hospice services being offered, and describe the Medicare hospice benefit.

    The nurse case manager will assess the patient’s needs and assist with obtaining any medical equipment that may be necessary for safe and efficient home care. The hospice staff also will develop an individual care plan and set up a schedule for home visits by team members.

    Your right to quality hospice care

    Henry Ford Hospice is dedicated to providing compassionate care of the highest quality to all of their patients in the community, regardless of ability to pay and without discrimination. Hospice is less expensive than conventional end-of-life care because of the way it is administered. Family, friends and volunteers provide much of the day-to-day care in the patient’s home. Expenses are further held in check because less high-cost technology is used.

    In Michigan, hospice is paid for by Medicare, Medicaid, Health Alliance Plan, Blue Cross/Blue Shield of Michigan and other commercial and private insurance plans. Insurance coverage rarely provides for the full cost of care. To fill the gap between reimbursements and actual cost of care, Henry Ford Hospice relies on grants and community support for both donations and volunteer staff.

    Patrick joined Hospice of Michigan in May 2004 as Service Line Director – Facilities, and has since served our organization as Corporate Director Of Clinical Services, Corporate Director Of New Business Development, and Corporate Director Of Organizational Integrity. Serving as Senior Vice President, Service Operations, Chief Operating Officer since 2007, Patrick was most recently appointed Executive Vice President, Chief Operating Officer in March 2014.

    As Executive Vice President, Patrick is the lead member of Hospice of Michigan’s Executive Team, providing guidance in all areas of operations. Patrick continues to focus on improving Hospice of Michigan’s quality and satisfaction outcomes through of our five diamond model of care, which creates operational efficiencies that help us achieve our mission to innovate end-of-life care. He is responsible for oversight of At Home Support, Hospice of Michigan’s advanced illness management program, all clinical and service operations, pediatric programs, as well as our human resource discipline, Work Life Services.

    Patrick is a graduate of the Second Degree/Second Career program at Wayne State University College of Nursing. In addition, he earned a Bachelor of Science degree in biology, an MBA and a MHSA from the University of Michigan.

    Before joining HOM, he was executive director of Avalon Hospice in Flint. He also worked at Henry Ford Hospital where he was a staff RN and assistant manager for three years.

    Patrick is a COO Forum Member of the National Hospice Work Group and has been appointed by Governor Rick Snyder to be a public board member of the State Board of Mortuary Sciences. He is also a board member of the National Arab American Nurses Association (NAANA) and sits on the Michigan Oncology Quality Consortium Advisory Board (MOQC)

    Please note that the Bureau of Construction Codes will no longer administer the Electrical Examination. Once you have been approved to take the examination, you will be contacted by PSI to schedule your examination date and time at your convenience. All future examinations will be conducted by PSI at one of the five centers selected by the applicant. Examinations will be continuously scheduled 6 days a week.

    NOTICE: ONCE YOUR APPLICATION IS APPROVED YOU WILL BE NOTIFIED BY A THIRD PARTY TESTING AGENCY (PSI) WITH INSTRUCTIONS ON HOW TO SCHEDULE YOUR EXAMINATION. ALL EXAMINATION FEES WILL BE PAID TO PSI AT THAT TIME. PLEASE DO NOT SEND PAYMENT TO THE STATE OF MICHIGAN/BUREAU OF CONSTRUCTION CODES.

    IMPORTANT: Applications for electrical licensing examinations

    The state s electrical examination forms can only be used by persons who are eligible to be licensed by the state of Michigan. The state of Michigan issues electrical licenses to residents in most communities in the state of Michigan. However, if you reside in the following communities where electrical licenses are issued locally, you should not use this form nor should you submit it to the state of Michigan: City of Grand Rapids and City of Detroit .

    Qualifications for a Electrical Journeyman License

    • Must be at least 20 years of age.
    • Have not less than 8,000 hours of practical experience obtained over a period of not less than 4 years related to electrical construction or maintenance of buildings or electrical wiring or equipment under the direct supervision of a person licensed under Act 217 of 1956. (Maximum credit of 2,000 hours per year.)
    • Credit may be provided for graduation from or attendance at a recognized training or educational program.
    • Must take and pass the examination.

    Qualifications for a Master Electrician License

    • Must be at least 22 years of age.
    • Has held an electrical journeyman s license for not less than 2 years.
    • Have not less than 12,000 hours of experience obtained over a period of not less than 6 years related to electrical construction, the maintenance of buildings, or electrical wiring or equipment under the supervision of a master electrician.
    • Must take and pass the examination.

    Qualifications for a Electrical Contractor License

    • Holds a master electrician s license or has not less than 1 master electrician residing in this state who is in his or her full-time employ. The master electrician shall be actively in charge of and responsible for code compliance of all installations of electrical wiring and equipment.
    • Out-of-state corporations will not qualify for a contractor s license unless they employ a master electrician residing in the state of Michigan.
    • Must take and pass the examination.

    Qualifications for a Fire Alarm Specialty Technician License

    • Must be at least 20 years of age.
    • Be certified by the National Institute for Certification in Engineering Technology (NICET) as an associate engineering technician, level II, or the equivalent as determined by the board, in the field of fire alarm systems technology.
    • Must take and pass the examination.

    Qualifications for a Fire Alarm Contractor License

    • Holds a fire alarm specialty technician s license or has not less than 1 fire alarm specialty technician residing in this state who is in his or her full-time employ. The fire alarm specialty technician shall be actively in charge of and responsible for code compliance of all installations of fire alarm system wiring and equipment.
    • Out-of-state corporations will not qualify for a contractor s license unless they employ a fire alarm specialty technician residing in the state of Michigan.
    • Must take and pass the examination.

    Qualifications for a Sign Specialist License

    • Must be at least 18 years of age.
    • Have not less than 4,000 hours of experience, obtained over a period of not less than 2 years, related to manufacture, installation, maintenance, connection, or repair of electric signs and related wiring, as verified by a contractor licensed under this act.
    • Successful completion of of an approved training course per 1956 PA 217.
    • Must take and pass the examination.

    Qualifications for a Sign Specialty Contractor License

    • Holds a sign specialist s license or has not less than 1 sign specialist residing in this state who is in his or her full-time employ. The sign specialist shall be actively in charge of and responsible for code compliance of all installations, connection, and repair of electric signs and related wiring.
    • Out-of-state corporations will not qualify for a contractor s license unless they employ a sign specialist residing in the state of Michigan.
    • Must provide evidence of public liability insurance.
    • Must take and pass the examination.

    The exam site and parking are accessible. Individuals attending the exam are requested to refrain from using heavily scented personal care products, in order to enhance accessibility for everyone.     Why Major in Criminal Justice?

    The criminal justice degree program provides a combined training and criminal justice education package to prepare men and women for a professional career in the criminal justice system.

    Acceptance into the criminal justice bachelor degree program (junior/senior level) is on a competitive basis, as space permits.

    The criminal justice degree program provides two primary entry points: (1) to accommodate criminal justice students who wish to complete all four years at Ferris; and (2) college criminal justice students who transfer into the Ferris State criminal justice degree program with an associate degree criminal justice or its equivalent.

    Criminal Justice College Graduates Get Great Jobs

    The criminal justice degree program at Ferris State has three options: law enforcement specialist; criminal justice corrections; or criminal justice generalist. Depending upon the criminal justice major or cj program option chosen, Criminal Justice degree students study criminal justice subjects such as administration of police units, police laboratory techniques, collection of evidence, the role of police in forming community opinion, cause and prevention of juvenile delinquency, criminal justice theory and practice of handling convicted law violators, probation and parole, criminal justice processes and problems, crime control policy, and the role of law enforcement in American society.

    During the summer internship between the junior and senior years, the Criminal Justice degree students are assigned to a cooperating criminal justice agency for practical experience in a criminal justice internship program.

    Bachelor’s Degree Criminal Justice Admission Requirements

    College students who major in criminal justice by enrolling in the criminal justice degree program prior to the junior level enroll in the Pre-Criminal Justice curriculum. Completion of the Pre-Criminal Justice degree program does not guarantee acceptance into the criminal justice bachelor degree program. The bachelors degree in Criminal Justice is awarded for successful completion of 128-134 semester credit hours of coursework. These criminal justice degree credit hours are a combination of the 63-64 credit hours as required in the first two years as a criminal justice major (or an equivalent from another college) and the 64-67 semester credit hours required in the junior and senior years at Ferris State. Graduation from the college criminal justice degree program requires a minimum 2.5 GPA for the law enforcement specialist option. Graduation from the criminal justice degree in the criminal justice corrections and generalist options requires a minimum 2.0 GPA.

    Graduates from the Criminal Justice degree program must complete all of Ferris State general education requirements as outlined in the General Education section of the criminal justice course catalog, and have an ACT math sub score of 24, or complete MATH 115 or pass the Math proficiency exam.

    What is hospice?

    It is health care that helps patients whose disease doesn t respond to curative treatment. They may choose hospice for palliative care.

    What is palliative care?

    It provides comfort, dignity and quality of life. it helps control pain and other symptoms. it also deals with psychological, social and spiritual issues. The goal is the best possible quality of life for patients and their families.

    What does hospice include?

    Doctors, nurses, social workers, spiritual counselors, home health aides, bereavement counselors and trained volunteers. They all work together to provide care. They help patients and their families according to their wishes.

    Where does hospice care take place?

    Wherever the patient resides – ideally at home. It may be in a hospice residence, a nursing home or even a hospital. The patient may move between home and a facility as their condition required. Hospice provides the most effective care in the best environment.

    Is hospice care concerned only with my physical needs?

    No. Hospice address physical, psychological, social, and spiritual needs. It must also meet the needs of the family. This is done through a coordinated plan of care, in consultation with a patient and family. Hospice also includes bereavement support. This helps people who have lost loved ones. Michigan law requires hospice to provide this at no cost for at least a year after the loss of a loved one.

    What are my rights as a hospice patient?

    Hospices must meet these standards. These rights are protected under Michigan and federal law.

    • Medical care must be supervised by a physician. It must be provided or supervised by a team made up of the doctor, a registered nurse, a social worker and a spiritual care or other counselor.
    • The care plan must address the specific needs of the patient and family.
    • Medical and nursing care must be available 24 hours a day.
    • The patient must have access to speech, physical, and occupational therapy.
    • The patient and family must get a statement outlining the rules and regulations.
    • The patient and family are entitled to know how to resolve problems they may have with the hospice.

    A patient in a hospice residence has additional rights:

    • Meal plans.
    • Visitors of any age, at any hour, as desired.
    • A place for family members to spend the might.
    • The residence must comply with state and federal regulations. This included fire prevention, infection control, and sanitation.

    A hospice patient is entitled to the same level of expert qualified care as they would get from any other health care provider.

    Will my insurance pay for hospice?

    Individual plans vary. You must be offered hospice coverage whenever you sign up for or renew health insurance. You must also be informed about any such coverage in your plan. Hospice care is covered by Medicare, Medicaid and most private insurance.
