Purpose:
The purpose of this bulletin is to provide information on implementation of new Medicare Hospice Conditions of Participation (COPs), related joint training being sponsored by, Centers for Medicare and Medicaid (CMS) and the Minnesota Department of Health (MDH), and implementation date by MDH.

CMS Training
Joint training will take place via three (3) CMS sponsored satellite training sessions which are archived at the CMS website.

CMS will host 4 satellite broadcasts scheduled from 12:00 noon � 1:30 pm Central Standard Time (CST) on Thursday, January 22, 2009; Thursday, February 19, 2009; Friday, March 13, 2009 and Wednesday, April 22, 2009. Broadcasts can be viewed by members of the public by satellite coordinates provided on the CMS website approximately 2 weeks prior to the broadcast.

Palliative chemotherapy: harms and benefits weighed in new study

Palliative chemotherapy is treatment designed for terminal cancer patients to prolong survival and ease symptoms but not cure disease. Now, researchers have found that the therapy comes with certain harms, which they say need to be addressed.

The researchers, from the Weill Cornell Medical College, Dana-Farber Cancer Institute and Harvard Medical School, have published their results in the BMJ.

They say their findings uncover a discrepancy between what type of care cancer patients want and what they actually receive.

Overall, the team found that terminal cancer patients who receive chemotherapy during the last months of their lives are less likely to die where they wish and are more likely to undergo invasive medical procedures – including CPR and mechanical ventilation – than patients who did not receive the therapy.

Dr. Holly Prigerson, of Weill Cornell Medical College, says that the reasons for the link are complicated, but they may originate in misunderstanding of the purpose and consequences of palliative chemotherapy.

For the study, investigators assessed data from 386 patients in a federally funded study, called Coping with Cancer.

This 6-year study followed terminally ill people and their caregivers until the patients died, and the researchers looked at how psychosocial factors influenced patient care.

After asking the caregivers to rate their patient’s care, quality of life and where the patient would have wanted to die, the researchers reviewed patient medical charts to determine the type of care they actually received during their last week.

Chemotherapy patients less likely to die where they want

The researchers found that patients receiving palliative chemotherapy were less likely to talk to their oncologists about the type of care they wanted to receive, to complete Do-Not-Resuscitate orders or to even acknowledge they were terminally ill.

The findings reveal that 68% of patients receiving palliative chemotherapy died where they wanted, compared with 80% of those not receiving the therapy.

In detail, 47% of patients who received the therapy died at home, compared with 66% of patients who did not receive the palliative care. Additionally, 11% of patients receiving the chemotherapy died in an intensive care unit, versus only 2% of patients not receiving the therapy.

Dr. Prigerson says:

“It’s hard to see in these data much of a silver lining to palliative chemotherapy for patients in the terminal stage of their cancer. Until now, there hasn’t been evidence of harmful effects of palliative chemotherapy in the last few months of life.”

“This study is a first step in proving evidence that specifically demonstrates what negative outcomes may result,” she adds.

The team also found that patients receiving palliative chemotherapy were more likely to be referred to hospice care much later than those not receiving the therapy.

Hospice care is a place that provides comfort and emotional support for terminal patients, and 54% of the patients receiving chemotherapy were referred to hospice late, compared with 37% of the patients not receiving the drugs.

‘Potential need for oncology practice changes’

The team notes that 56% of the patients from the study were receiving palliative chemotherapy in their final months, and they say this majority of use emphasizes the potential need for changes in oncology practice at academic medical centers.

Dr. Alexi Wright, assistant professor and medical oncologist at Dana-Farber, says:

“We often wait until patients stop chemotherapy before asking them about where and how they want to die, but this study shows we need to ask patients about their preferences while they are receiving chemotherapy to ensure they receive the kind of care they want near death.”

But Dr. Wright adds that their findings should not be interpreted that patients should be denied or not offered palliative chemotherapy:

“The vast majority of patients in this study wanted palliative chemotherapy if it might increase their survival by as little as a week. This study is a step towards understanding some of the human costs and benefits of palliative chemotherapy.”

“Additional studies are needed to confirm these troubling findings,” concludes Dr. Prigerson.

The School of Management and the School of Public Health and Health Professions at the University at Buffalo offer a collaborative program leading to both a Master of Business Administration (MBA) and a Master of Public Health (MPH) degree (Health Services Administration concentration only).

Its purpose is to facilitate an integrated course of study that prepares students for high-level management careers in health care organizations and agencies. The program involves three years of study, with a total of 97 credit hours. The time required will be one semester to one year less than would be required to pursue the degrees separately.

Students will apply to both programs at the same time, although on each application they will indicate that they are applying to the other and that they intend to pursue the collaborative program. Administrators of the two programs will coordinate their reviews of the applications and will make every effort to respond to the applicant at the same time regarding the decision of their application. Each school will apply the same admissions requirements and standards that prevail for its other applicants. The School of Management will accept either the GMAT or GRE and SPHHP requires the GRE.

I had been doubly charged for one full year, and this came to my attention in January. I went to the Fulton Street location, spoke to the manager with the initials LR, who told me she would take care of it. On January 25th I had confirmed my address with her so that the corporate office could send me a refund, in which the manager had told me to expect it in 7-10 business days. On February 21st, I emailed LR asking what the deal was with the refund since I had not received it. She had replied on February 27 telling me I am sorry for the delayed response. I have checked on the status of your refund. There was a slight delay due to our department not being able to view the original email that was sent out. I have sent them all of the proper information and confirmed that they have received it and you will be receiving your refund within the next week and a half. I apologize again for any inconvenience. Please feel free to conntact me with any questions or concerns via email or phone. On March 16th, I emailed LR once again with news that I had not received the refund. No reply. On March 26th, I emailed LR once again with news that I had not received the refund. No reply. On April 4th (two days ago) I called the Fulton Street location and spoke to LR. I have been a member at the Planet Fitness – Canarsie, Brooklyn location for over four years, and as a loyal member I would like to draw your attention to several negative features that have been constantly occurring in the gym over time. First, I have noticed the chronic unsanitary and unhygienic conditions that have been prevailing and persisting for almost a year. Whether it s during peak hours or not, the gym has become a dumping ground filled with filthy paper littered all around the floors; on the machines; and along the walls. Also, the floors are constantly plagued with empty water bottles and the like. Second, the bottles of cleaning solution are almost always empty and/or defective, proving them unable to use. After approaching the front desk to notify them on several occasions, once instance I was told that, my co-worker left to pick-up some food and I m the only one on duty. Even, I m the only one here right now and I m not allowed to leave my post. I find it unacceptable that sanitary inspectors fail to assess the cleanliness of the gym on a regular basis throughout the day, especially on the weekends. Finally, the machines are frequently in a state of disrepair and are inoperable more often than not. Kevin was very professional and obviously very knowledgeable in .finding water leaks. He was easy to get a hold of and we were able to get an appointment quickly with him. He arrived on time and got straight to work finding our water leak which turned out to be a very quick process. He clearly knew Plumbing and gave lots of good recommendations to us which were helpful. I would say if you have a water leak and you re not sure where it s that don t call a plumber call Kevin he will find it for you and make sure you get the repairs you re supposed to get and not extra things done. We hired a plumber to find our leak and it cost us almost $1000 without any results before we called Leakmasters. Highly recommended!!

We suspected a large leak was the cause but had not been successful at locating it. After one household lost water I contacted Kevin and he came out on an emergency call. He looked over our well house and utilized a listening device on the pipes around the well house. He walked the water line with us and used his device at every location where we had exposed water lines. He checked the water pressure at every house. He identified one vacant home as a possible cause due to what he heard utilizing his device. He suggested we contact the owner to get into the home to check for leaks. He also pointed out that the drain field for that home was quite green and since the home had been vacant for some time and it had been a dry summer, this seemed unusual. When I finally located the property management company and was able to get into and under the house we found the leak immediately. It was very large and was bubbling like a fountain. Our community association members stepped up and we were able to cut and cap the water line. Once that was done the water pressure returned to normal. And you can be convicted of a less serious offense called a DWAI (driving while ability impaired) if as the result of ingesting alcohol, drugs, or a combination your ability to operate a vehicle as a reasonable and prudent driver has been impaired to any extent.

(The penalties for a DWAI involving only alcohol are less than those for a DWAI that involves drugs or a combination of drugs and alcohol.)

While most DWIs involve driving, it s possible to get a DWI in New York without actually moving your vehicle. For instance, you might be culpable for DWI if (while under the influence) you get into your car and start the engine driving isn t required.

And New York law says that if you re driving a vehicle, you ve given consent to submit to a chemical test for the purpose of determining the amount of alcohol or drugs in your blood. Although the phrase “you get what you pay for” seems to be a universal constant, there are a few options listed here that will help you get the most for your money.     To keep outlay as low as possible, knowing how to drive a stick shift is an advantage, since automatic transmissions are usually options instead of standard equipment. A manual transmission will affect resale value, however, so a decision has to be made whether to take that financial hit up front or down the road.

    This is officially the cheapest new car on sale in the United States, a title it’s held for a couple of years. Unexpectedly roomy for a subcompact sedan — and although no one benchmarks a Versa for driving dynamics, it doesn’t feel like a penance to drive. Air conditioning is standard, along with a CD player. A 1.6-liter engine making a tepid 109 horsepower and 107 lb-ft of torque is linked to a 5-speed manual transmission driving the front wheels. and that’s pretty much it.

    If you require an automatic transmission (a continuously variable automatic transmission, or CVT. is available), or Bluetooth, iPod integration, alloy wheels, a rearview camera, navigation or even one of those little mirrors in the front passenger’s sun visor, then you’ll have to look to one of the higher (and therefore more expensive) trim levels.     Giving the name Spark to something with only 84 hp and 83 lb-ft of torque from a 1.4-liter, 4-cylinder engine seems a tad optimistic. But where this subcompact 4-door (or five, including the hatchback) wins friends is with pleasing designs inside and out.

    As well as air conditioning, the Spark LS comes with power windows, alloy wheels, hill-hold assist and an impressive complement of 10 airbags. It’s another 5-speed manual transmission; CVT is an option. Now’s as good a time as any to mention continuously variable transmissions — without getting too technical, they’re basically one gear. They can be slow to respond and noisy. But that does make them cheaper than auto transmissions containing several ratios.

    Back to the Spark: It’s a car with decent interior space that stretches out as far as its headroom. Folding down the rear seats means removing the headrests, which is inconvenient, but that’s a minor gripe for what is generally a good buy.

    2014 Mitsubishi Mirage DE ($13,790; 34 mpg city/42 mpg hwy/37 mpg combined)

    This is an all-new model for 2014, in the United States at least. It seems the main reason why this 5-door subcompact hatch is so cheap is because the company didn’t spend any money making it decent to drive. Or including much sound insulation.

    It’s also one of the lightest cars on sale in North America: 1,973 pounds. The engine is certainly lighter by a cylinder. Yep, this one has three, straining out a wheezy 74 hp and 74 lb-ft of torque from 1.2 liters of displacement.

    Scrimping on cylinders and weight does have a great effect on fuel economy, though, returning the most impressive figures of this group. With the optional CVT, those numbers improve to 37 miles per gallon in the city, 44 mpg on the highway and 40 mpg combined. Yet somehow Mitsubishi still manages to include power windows, mirrors and locks, a 4-speaker stereo system and even an airbag for the driver’s knees. That said, it’s hard to get excited about.

    2014 smart fortwo pure ($14,020; 34 mpg city/38 mpg hwy/36 mpg combined)

    A successor is expected soon, because this sub-subcompact 2-seater is getting on in years. Space for the couple of passengers is fairly generous, and the trunk area can take just enough luggage for a pair’s weekend away.

    This was a good idea for European cities where streets are narrow and parking spaces are few. Things don’t quite work that way in the U.S. Instead, the American smart driver will have to contend with crosswinds gusting off the prairie and a shunt-with-every-shift mechanized semi-manual transmission, neither of which makes the car an attractive proposition. The short wheelbase also makes speed humps irksome.

    Most cars have crumple zones, areas of the body that absorb energy in a collision. The diminutive smart doesn’t have room for such things, but it still performs well in crash tests thanks to a safety cell that surrounds its occupants. And eight airbags in a 2-seater is good.

    Tipping the scales at a feathery 1,808 pounds means the tiny 1.0-liter, 3-cylinder engine doesn’t feel inadequate when it buzzes out just 70 hp and 68 lb-ft of torque. Air conditioning comes as standard for 2014, joining power locks.

    2014 Kia Rio LX ($14,700; 28 mpg city/36 mpg hwy/31 mpg combined)

    As prices rise, the cars become more desirable. The Rio subcompact sedan doesn’t compromise with weak engines or limited seating. It actually packs something of a punch: 138 hp and 123 lb-ft of torque from a 1.6-liter, 4-cylinder unit. This goes to the front wheels through a 6-speed manual transmission.

    The Rio looks vaguely sophisticated and comes with air conditioning, heated power door mirrors, a CD player and an iPod socket. Power windows, however, are part of an option package. Landing a hatchback version only means parting with another $100.

    Downsides include a noisy cabin, a disappointing driving experience and Kia’s seats don’t always provide the desired amount of support. So if you’re on a test drive, pay attention to your posterior.

    2014 Ford Fiesta S ($14,925; 27 mpg city/38 mpg hwy/31 mpg combined)

    At last, something for the enthusiast. This subcompact sedan feels confident through the curves as well as comfortable on the stretches. A 1.6-liter, 4-cylinder engine delivers 120 hp and 112 lb-ft of torque to the front wheels through a 5-speed manual transmission.

    The cabin is an agreeable place to be, with decent materials and build quality. Standard equipment includes power locks, air conditioning, hill-start assistance, an airbag for the driver’s knees and clever side mirrors that include a convex section to eliminate blind spots. The Fiesta is also rare in its class by offering tilt-telescopic steering wheel adjustment and a 6-speaker audio system.

    2014 Chevrolet Sonic LS ($14,995; 26 mpg city/35 mpg hwy/30 mpg combined)

    Just squeaking onto a list of cheapest new cars by costing five bucks short of $15k, the Sonic comes with blind-spot-mitigating mirrors, a tilt-telescopic steering wheel, hill-start assistance, Bluetooth, air conditioning and keyless entry. Both front occupants get knee airbags as part of the 10 found within the Sonic’s relatively commodious cabin.

    Less-than-stellar fuel figures have a silver lining: an energetic 138 hp and 125 lb-ft of torque from a 1.8-liter, 4-cylinder engine. Like the Fiesta, the standard transmission is a 5-speed manual. Though the Fiesta is the driver’s choice, the Sonic still feels composed.

    For those wanting to buy domestic, the Sonic is the only one of the bunch to be made in the States.

    A convenient motel for one of those horrible early morning international flights, so all we needed was a simple room with a comfortable bed to sleep and that is what we got, no frills but we didn’t need any. The young lady on reception was very friendly and efficient and arranged our early morning taxi pick up, directed us to a local restaurant which was walking distance and then gave a free wine which had not been expected but was gratefully received. Will stay again for the next early flight.

    The room was clean and comfortable, and though small, adequately furnished. My reason for selecting the motel was its proximity to the airport for an overnight stay after very late arrival, so the lack of availability of the shuttle after 10pm was disappointing (especially for the taxi driver who was angry about the small fare). However, the arrangements for after hours arrival worked well – I had been told the room number, and the door was unlocked with the key inside. I appreciated the availability of the shuttle whenever I wanted it the next morning for the trip back to the airport. I would stay here again in the same circumstances. Reasonable value at $119

    Exceptional service from Kylie (or Karley) who picked me up from the airport. That free Shuttle, the proximity to airport and pretty fast free Wifi and that is the end of the good news. Apart from that it was a very basic roadside inn and noting the proximity to the airport is really no big deal in Wellington. The bed was uncomfortable, the interior shabby (see the photos). Functional and basic really, and the sound of the wind blowing gave it a bit of a creepy feeling. And a bit depressing really. Should be less than $139 per night in my opinion – high double digits at best.

    We booked this at very short notice and found the reception staff very helpful in getting our booking arranged. Check in was a very friendly and positive experience, as was the check-out.

    We were only there for one night so only had a short time to enjoy the pool in the dark but it was very pleasant and well set up.

    The only disappointment was the very dated feeling, starting with a surcharge for using a credit card and a large sign on the fly spray advising you would be up for $8 if it went missing. The room was tidy but dated and lacking any comfortable chairs for relaxing. While everything worked, there was a feeling of the original decor having been touched up a few too many times.

    Although free WIFI was offered, I was unable to get it to work – it just kept dropping out.

    With regard to Wi-Fi a simple call to reception would have solved your issues, as on checking your stay record i find that you had no completed the logon to the network, which is two stages – selecting the Chelmswood Guest Wi-Fi service, then after logging on to that (limited service) then entering your password allows full service.

    Taupo stop

    The rooms are very out dated and are not organised very well. The kitchen cupboards didn’t close and it felt very much like staying in an old person’s home. While the staff were very friendly and the thermal pool was lovely, the rooms just didn’t match the quality of what I was expecting. Would be a great place to stay once they did an update of the rooms.

    As Lance Somerfeld learned, babies are excellent teachers.

    His son provided round-the-clock on-the-job training, free of charge.

    Within days of becoming a father, the 36-year-old New York City resident learned how to soothe a fussy baby. How to burp him, feed him and swaddle him.

    Yet in some ways, Somerfeld’s son began shaping him into a father even before delivery.

    Although men may not be aware of it, they actually undergo hormonal changes as they prepare for fatherhood, says neuropsychiatrist Louann Brizendine. author of The Male Brain (Broadway Books, $24.99), released in March.

    At first, those hormones tell them to panic, or at least pay attention.

    Levels of a stress hormone called cortisol the same ancient chemical that instructs men to fight or take flight tend to spike about four to six weeks after men learn they’re going to be fathers, subsiding as the mother’s pregnancy progresses, Brizendine says.

    “It is a cortisol surge that wakes our brains up every morning,” Brizendine says. “So this surge may put the father-to-be’s brain on alert and in a sense wake him up to the impending reality of a new baby’s coming, and alert him that he’d better get things ready.”

    Philip Andrew, who organizes a fathers’ group in Lincoln, Neb. says he tried to channel his pre-baby anxiety into educating himself about infants. Concerned that he was unprepared to handle a baby, he and his wife signed up for childbirth and parenting classes at the hospital. Learning about what to expect helped ease his mind and allowed him to remain calm on the big day, Andrew says.

    About three weeks before the baby arrives, levels of testosterone sometimes called the “male hormone,” associated with competitiveness, aggression and sex drive fall by roughly a third, Brizendine says.

    That may have helped the species to survive, she says. A human baby needs two full-time caregivers maybe more. So a baby is more likely to survive if Dad is at home to help, rather than out looking for new romantic conquests, she says.

    Bill Stratbucker, 40, was surprised at the things he did after his twins were born such as going out to buy nipple shields for his wife, which help babies nurse. “Some part of me felt like I wanted to hold on to the way I was before kids,” says Stratbucker, a pediatrician in Grand Rapids, Mich. whose twins are now 6 years old. “You just change and mature a lot.”

    Reacting to Mom

    Even the male brain changes, Brizendine says.

    Tests show that men actually get better at hearing a baby’s cry zeroing in on the sound and responding to it as the due date of their own child approaches.

    Other hormonal changes are a head-scratcher, she says.

    At the same time that testosterone is falling, a man’s supply of prolactin a hormone that helps mothers make milk rises more than 20%, Brizendine says.

    “We still don’t know what prolactin is doing in dads,” she says.

    Male hormones begin to readjust when the baby is 6 weeks old, returning to pre-fatherhood levels by about the time the baby is walking, Brizendine says.

    Scientists can’t completely explain why men’s hormones fluctuate, Brizendine says. It’s possible that men are reacting to women’s pheromones, airborne chemical messengers secreted by their skin and sweat glands, she says. Scientists think these chemicals may trigger a man’s brain to “begin making the hormonal changes necessary for paternal behavior,” she says.

    These pheromones appear to work both ways, at least in animals. In a study of mice, the males released their own pheromones. That triggered the female mice to make more prolactin, which also stimulates the growth of maternal brain circuits, Brizendine says.

    Through these unconscious chemical signals, she says, mothers and fathers may be spurring more loving, nurturing behavior in each other, increasing the odds their babies will survive.

    It’s possible, Brizendine says, that “the baby is actually in control from before birth in just this very way: The baby is controlling the hormones of the mother, and she is controlling the hormones of the father-to-be.”

    Babies exert long-term effects on their fathers. Studies show that dads who are more involved in child care have lower testosterone levels than uninvolved fathers. But do lower testosterone levels really make men more nurturing? Or does nurturing change a man’s hormones? Scientists don’t yet know.

    Scientists have more solid answers when it comes to the ways that babies affect the brain.

    Looking at a baby’s face with its pudgy cheeks and big eyes causes the brain’s “parental-instinct” area to light up within one-seventh of a second.

    A baby’s smile activates the same brain circuits involved with falling in love, Brizendine says.

    As if practice makes perfect, Brizendine says, “the more a man holds and cares for his child, the more connections his brain makes for paternal behavior.”

    Parents say they need that fierce devotion to power through the tough times, waking every two hours to feed a hungry newborn or pacing the halls with a colicky 3-month-old.

    Rahul Parikh says even his training as a pediatrician didn’t adequately prepare him for those first few months of fatherhood. Unlike medical interns, who take turns working 24-hour shifts, parents never get a night off.

    “I was a ghost of myself for a little while,” says Parikh, 38, of Walnut Creek, Calif. who has a 2 -year-old daughter and another baby due in August. “Even starting medical school and becoming a doctor wasn’t like that. At home, when the baby screams, it’s all on you.”

    Parikh says he’s not surprised by new research suggesting that 14% of new dads develop postpartum depression about the same rate seen in new moms. Yet even the most involved fathers often notice that their wives experience parenthood differently.

    Many mothers find themselves consumed by the demands of their infants not to mention their own health needs, as they recover from childbirth. In spite of their willingness to pitch in, Andrew says some dads tell him that they feel a bit peripheral or even ignored.

    “I wasn’t the be-all and end-all for my wife anymore,” says Josh Hornick, 51, a father of two from Amherst, Mass. who trains life coaches. “And that’s as it should be. If I could choose whether to have my wife be more attentive to me or to our children, I would want her to be more attentive to my children. But it’s still a shock.”

    By maintaining a little emotional distance or a least a broader perspective Andrew says fathers can help keep a household running. While moms focus on essentials such as breast-feeding, dads can keep the rest of the household from falling apart by doing housework, driving older kids to school or making sure that bills get paid.

    “The dad has a bigger picture on things, instead of that laserlike focus on the baby,” says Andrew, 39, whose children are 6, 8 and 13. “It works as a balance.”

    Matt Schneider, whose sons are 5 and almost 2, decided to stay home full time after his first son was born. He’s enjoying fatherhood more than he ever imagined.

    “It’s fun to see to see them developing into real people and see their personalities grow and develop, and to see yourself in them,” says Schneider, 35, a former schoolteacher who organizes the NYC Dads Group for active fathers with Somerfeld. “I was planning to stay out of the workforce a year. Now it’s been almost five years.”

    Transfer Post-9/11 GI Bill to Spouse and Dependents

    The transferability option under the Post-9/11 GI Bill allows Servicemembers to transfer all or some unused benefits to their spouse or dependent children. The request to transfer unused GI Bill benefits to eligible dependents must be completed while servicing as an active member of the Armed Forces. The Department of Defense (DoD) determines whether or not you can transfer benefits to your family. Once the DoD approves benefits for transfer, the new beneficiaries apply for them at VA.     Type of Assistance

    Eligible Servicemembers may transfer all 36 months or the portion of unused Post-9/11 GI Bill benefits (unless DoD or the Department of Homeland Security has limited the number of transferable months). If you’re eligible, you may transfer benefits to the following individuals:

    • Your spouse
    • One or more of your children
    • Any combination of spouse and child

    Available Benefits and Eligibility

    Family members must be enrolled in the Defense Eligibility Enrollment Reporting System (DEERS) and be eligible for benefits at the time of transfer to receive transferred benefits.

    The option to transfer is open to any member of the armed forces active duty or Selected Reserve, officer or enlisted who is eligible for the Post-9/11 GI Bill, and meets the following criteria:

    • Has at least 10 years of service in the armed forces (active duty and/or Selected Reserve) on the date of approval, is precluded by either standard policy (by Service Branch or DoD) or statute from committing to four additional years, and agrees to serve for the maximum amount of time allowed by such policy or statute.
    • Transfer requests are submitted and approved while the member is in the armed forces.

    Transfer Process. While in the armed forces, transferors use the Transfer of Education Benefits (TEB) website to designate, modify, and revoke a Transfer of Entitlement (TOE) request. After leaving the armed forces, transferors may provide a future effective date for use of TOE, modify the number of months transferred, or revoke entitlement transferred by submitting a written request to VA.     Other Factors to Consider

    • A childs subsequent marriage will not affect his or her eligibility to receive the educational benefit; however, after an individual has designated a child as a transferee under this section, the individual retains the right to revoke or modify the transfer at any time.
    • A subsequent divorce will not affect the transferees eligibility to receive educational benefits; however, after an individual has designated a spouse as a transferee under this section, the eligible individual retains the right to revoke or modify the transfer at any time.

    If a Servicemember wants to reallocate transferred benefits they can do so using the TEB Portlet in MilConnect at https://www.dmdc.osd.mil/milconnect. If a Veteran wants to reallocate benefits they should contact the VA using our “Ask A Question” website http://gibill.custhelp.com .

    If transferred benefits are totally revoked for a dependent a Servicemember must resubmimt a transfer request for the dependent via MilConnect, a Veteran cannot re-transfer benefits to a dependent if the dependent’s transfer eligibility was previously totally revoked.

    Family member use of transferred educational benefits is subject to the following rules:

    • May start to use the benefit immediately
    • May use the benefit while the member remains in the Armed Forces or after separation from active duty
    • Are not eligible for the monthly housing allowance while the member is serving on active duty
    • Can use the benefit for up to 15 years after the service members last separation from active duty
    • May start to use the benefit only after the individual making the transfer has completed at least 10 years of service in the armed forces
