Caring for Terminally ill Patients

BENGALURU: When a person is diagnosed with a terminal illness, family members, friends and caregivers should be willing to listen to their concerns, say doctors.

Dr L Srinivasamurthy, medical director, Nightingales Home Care Service, says that in order to take care of patients with cancer and other life-threatening conditions such as liver and kidney failure, AIDS, old age senility and other mental illnesses during their terminally ill condition, a broad, multidisciplinary approach should be followed. In order to give effective palliative care family members should be supportive and make use of available community resources. This care can be successfully implemented even if resources are limited. Palliative care can be provided in tertiary care facilities, in community health centres and even in people’s homes and for this, family members should listen or understand the needs of patients.

Palliative care improves the quality of life of terminally ill patients and their families by providing pain and symptomatic relief as well. It also provides spiritual and psychosocial support from the time of diagnosis to the end of patient’s life, he says.

What to expect from palliative care?

  • Provides relief from pain and other distressing symptoms;
  • Affirms life and regards death as a normal process.
  • Intends neither to hasten nor postpone death.
  • Integrates the psychological and spiritual aspects of patient care.
  • Offers a support system to help patients live as actively as possible until their death.
  • Offers a support system to help the family members cope during the patient’s illness and in their own bereavement.
  • Uses a team approach to address the needs of patients and their families.

In most parts of the world, a majority of cancer patients are in advanced stages of cancer when they first seek medical help. For such patients, the only realistic treatment option is pain relief and palliative care. Effective approaches to palliative care are available to improve the quality of life for cancer patients. The WHO 3-step ladder for cancer pain is a relatively inexpensive yet effective method for relieving cancer pain in about 90 percent of the patients, says Dr Srinivasmurthy.

In the final stages of life-limiting illness, even the best possible care cannot extend life beyond a point, and hence the focus shifts to making the patient as comfortable as possible.

Depending on the nature of the illness and the patient’s circumstances, this final stage period may last from a matter of weeks or months to several years. During this time, palliative care measures can provide the patient with medication and treatments to control pain and other symptoms, such as constipation, nausea, or shortness of breath, says Dr Shoba Naidu, critical care consultant, Sagar Hospital

When to initiate palliative care on your loved ones:

  • The patient wishes to remain at home, rather than spend time in the hospital.
  • He/she stops receiving treatment for the disease.
  • The patient has made multiple trips to the emergency room, the condition has been stabilized, but the illness continues to progress significantly, affecting their quality of life.
  • If a patient has been admitted to the hospital several times within the last year with the same or worsening symptoms.

When caregivers, family members and loved ones are clear about the patient’s preferences for treatment in the final stages of their life, they’re free to devote their energy to care and compassion. To ensure that everyone in the family understands the patient’s wishes, it’s important for anyone diagnosed with a life-limiting illness to discuss their feelings with their near and dear ones before a medical crisis strikes, says Dr Shoba Naidu.

Experts also suggest that it is important to seek financial and legal advice while the terminally ill patient can still participate. Legal documents such as a living will, power of attorney, or advance directives can set forth a patient’s wishes for future healthcare so that the family members are all clear about his or her preferences.

MADISON — Saint Jude Hospice now offers loving care for patients and families facing terminal illness from two new locations in Wisconsin: Platteville and La Crosse.

Saint Jude’s mission is to serve all patients, in the comfort of their own home, who are facing terminal illness by bringing healing to those when their hope has changed from cure to comfort.

Nurses are available 24 hours a day, seven days a week. They have no wait list and will respond to all requests within two hours.

Saint Jude Hospice has been in Wisconsin since July of 2010, when it opened its doors in Madison.

A new inpatient unit at St Luke s Hospice – paid for in part thanks to The Star – has been given the Royal seal of approval.

Prince Richard, The Duke of Gloucester, toured the new extended and refurbished £5.5 million In Patient Centre at the hospice in Whirlow.

The Duke Gloucester has visited St Luke’s Hospice in Little Common Lane, Sheffield, to open the new In Patient Centre. Our picture shows the Duke signing the visitors book, watched by chief executive Peter Hartland.

The Star has so far raised more than half of the £100,000 it pledged for one of the single rooms in the new unit, with £50,000 alone coming from our Great Pie and Peas Up campaign.

Hundreds of pie and pea suppers were hosted by readers, residents, businesses, schools and community groups, with proceeds going towards St Luke’s fundraising target.

The Queen’s cousin looked around the hospice before meeting patients and staff, unveiling a commemorative plaque, and signing the guest book.

He praised fundraisers and staff for their hard work, and the architects for their design.

The new unit, which aims to allow patients to carry on as normal a life as possible, accommodates 20 patients in 14 single rooms and two three-bedded rooms.

The Duke said: “I congratulate those who have played a part in developing St Luke’s over the decade and, in particular, this new wing which you have clearly designed with great care and attention and which is already full of your patients who feel extremely comfortable and well looked after.”

Following the Duke’s visit, BBC television presenter Harry Gration took part in a dedication ceremony with patients.

Gaynor Boon, aged 52, from Woodhouse, has ovarian cancer and has been staying in the unit for a week while her pain relief is managed.

The former curator at Sheffield Museums said: “This is my first visit. It’s amazing really. Coming in you feel like you are walking into a hotel environment and are treated like a hotel guest.

“You have got state-of-the-art nursing and pain management, but on top of that there is the hospitality side.

“You’ve got a lovely room with a balcony, a menu which offers three-course lunches and dinners, and a restaurant service.

“My dog Hunney, a long haired golden retriever, is also allowed to visit! She comes most days which makes it feel like a home from home.”

Una Moran, director of fundraising at St Luke’s Hospice, described the unit as ‘a gift by the people of Sheffield, for the people of Sheffield’.

She said: “Everyone who has contributed should be proud of the part they have played in creating this gift for the people of Sheffield, which will be here for many years to come.”

She added: “It never ceases to amaze me the ways people think of to raise money, and the generosity people find to stretch themselves to support this hospice, which supports about 6,000 people across Sheffield every single year, including patients and loved ones.”

A NEW lottery has been launched in aid of Salisbury Hospice.

The Your Hospice Lottery will offer people the chance to take part in a weekly draw with a top prize of £1,000 to raise money for the work done by the charity.

It is hoped the lottery will raise in excess of £500,000 for Salisbury Hospice Charity over the next five years, and there will also be a £100 prize, four £25 prizes and 80 prizes of £10 to win as well as the chance of a weekly rollover prize of £250, which could go up to a maximum of £10,000.

John Munro, fundraising manager with Salisbury Hospice Charity, said: “We rely on the generosity of local people to help us raise the £1.5m needed each year for us to continue to help families in our community who are living with life-limiting illnesses.

“A home visit from a community nurse costs £100, while one bereavement support session to help loved ones through a difficult time costs £20. By playing the weekly lottery, supporters will be providing a valuable and regular income for Salisbury Hospice Charity which will help to run services and to plan patient care.”

IN a special appearance in Glasgow, Laura Fraser, the star of hit television series Breaking Bad, encouraged people to put their heart into the Prince Princess of Wales Hospice.

The Hollywood actress, who grew up in Glasgow, is the newest patron of the hospice and put the first spade in the ground at the site of the state-of-the-art facility to be built at Bellahouston Park.

She also helped to launch the Foundation Appeal, the push to raise the remaining £333,000 of the £1 million needed to lay the groundwork.

“Message hearts have already begun pouring in from all over the city. They will be placed in a large chest and sunk into the foundations under the front door of the new hospice,” she said.

“These messages are incredibly touching and you can clearly see from the sentiments expressed, just how much the hospice means to the people of Glasgow.”

Friends, volunteers, dignitaries and supporters of the existing Carlton Place hospice gathered at the south side park where the new building will go up, due to open its doors in 2018.

Lord Provost Sadie Docherty said more than 36,000 people had been treated at the hospice since it opened 30 years ago.

“This is an emotional day for many of us,” she added. “We are witnessing the dawn of a new era for the care of the terminally ill.”

Jamie Hepburn, Minister for Sport, Health Improvement and Mental Health, praised the effort of the fundraising carried out to build the facility. In total it will take £21m to build the hospice on a hilltop site in the park, with £6.5m still to raise.

“The dedicated hospice staff play such an important role, and I would also to recognise the work of the many volunteers, fundraisers and supporters throughout the hospice movement,” he said.

“This new facility will bring world-class hospice care to the people of Glasgow, offering privacy and compassionate care to the people who need it.”

This will be the first hospice in the UK to implement the Sengetun model of care – a Scandinavian model that puts patients and family at the centre of its focus alongside optimal operational efficiency.

It will also provide hotel style comfort with 16 en-suite bedrooms with private terraces, soft seated areas and domestic style, open plan kitchens. The accommodation will allow patients and families the opportunity to eat together. The aim is to provide more space, independence and privacy to allow greater opportunity for normal family activity.

The new hospice’s commitment to the palliative care of young people aged 15 and over will also include specialised accommodation to meet their particular needs. This will include family bedrooms, family lounges, social areas, complementary therapy rooms, spa bathrooms, a hydrotherapy pool, sanctuary, sensory room, young people’s lounge and training rooms.

Job Description Summary

The Registered Nurse Case Manager plans, organizes, and directs home care services. He/She is an experienced nurse with an emphasis on community health education or home health. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and their family within the home setting. The Registered Nurse Case Manager respects and promotes the mission, vision, and values of VNHC.

Essential Job Functions/Responsibilities

  1. Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
  2. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
  3. Uses health assessment data to determine nursing diagnosis.
  4. Initiates the plan of care and makes necessary revisions as patient status and needs change.
  5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  7. Regularly re-evaluates patient nursing needs.
  8. Counsels the patient and family in meeting nursing and related needs.
  9. Provides health care instructions to the patient as appropriate per assessment and plan.
  10. Provides supervision of LVN’s and home health aides per CHAP standard and regulatory requirements.
  11. Responsible to reassess the patient within 24 hours following a change in condition or discharge from an inpatient setting. Case manager should be the first RN to visit the patient status post hospitalization; updates the plan of care.
  12. Accountable for financial resources and eligibility under various third party payor sources, Medicare and Medical.
  13. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  14. Knowledge of state, federal, local and accreditation (CHAP) regulations for the delivery of home health services.
  1. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
  2. Communicates with the physician regarding the patient’s needs and reports and changes in the
    patient’s condition; obtains/receives physician’s orders as required.
  3. Communicates on a regular basis with the team and the area team leader.
  4. Communicates with community health related persons to coordinate the care plan.
  5. Teaches the patient and family/ caregiver self-care techniques as appropriate provides medication, diet and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/ caregivers.
  6. Provides and maintains a safe environment for the patient.
  7. Assists the patient and family/ caregiver and other team members in providing continuity of care.
Additional Duties
  1. Participates in on-call duties as defined by the on-call policy.
  2. Actively participates in Quality Management program.
  3. Attends and participates in the agency team care conferences.
  4. Orients and mentors new staff as requested.
  5. Ensures that arrangements for equipment and other necessary items and services are available.
  6. Meets daily average productivity standard set by the organization.
  7. Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

Position Qualifications

  1. Graduate of an accredited school of nursing. Bachelor’s degree, with one year of home health care experience preferred.
  2. Current licensure in State, CPR certification and valid driver’s license.
  3. At least one year of recent experience working as a professional nurse in an acute care setting.
  4. Management experience not required. Responsible for supervising home health aides/ LVNs.
  5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

I had never been in a hospice center. I liked this center was really nice. All the staff were very nice and respectful of my situation and my loved one. The place was very clean with many services for families who are facing the death of a relative. Everything was explained to me as what it was ahead of me.

Things to know, if you do have a someone who is going to die, you must know all of their personal information and the funeral arrangements, because as soon as the time comes, you must call the funeral home. There is not waiting time for this.

In care at the Haven was excellent but we had the VNSNY for home hospice care before my husband was admitted. It was horrible and has taken me two years since his death to stop blaming myself for not changing from them to a home hospice agency that really cared about the patient.
He was assigned a nurse that never showed up. We had a different nurse every time and they didn t even come every week.
Jim had kidney cancer that had spread to the lungs and spine. One week when his aid was on vacation, a young woman was sent to take her place. What a disaster. His bed sore became unbearable and he was out of some of his pain medication. There was no consistency of care. No one should ever go through what happened to him under VNSNY in home hospice care.
I m sorry Jim. I should have done more to make them care.

In a perfect world, a guy who likes to unwind lonely driving roads on a brisk weekend morning owns an implement dedicated to just such an activity. A Mazda MX-5, for example. Or a Pontiac Solstice. Or a Honda S2000. Or. fill in your favorite sports car. But that ideal presupposes a big budget for grown-up toys, and/or the absence of parental obligations, which immediately add extra doors and seats to the equation. As much as we love ’em, sports cars are pure automotive self-indulgence, with a low practicality index. And the sad truth is, many of us are economically limited to one ride that has to serve a variety of everyday transportation functions, as well as satisfying your inner Michael Schumacher. Perhaps you’ve noticed this?

Take heart. Ownership of a car with four doors needn’t mean the end of fun-to-drive, and to support this proposition we present this field of five sub-$30,000 sports sedans that are just the ticket, able to deliver speed with a soupçon of luxury, too. Okay, more than a soupçon. A modicum. And in a couple cases, a profusion.

As a conceptual group, our roundup of 30-grand sports sedans isn’t new. It’s one of our regular sports-sedan reality checks. Still, there is much that is new here-in fact, only one of these five packages is familiar. That would be the Acura TSX, a sophisticated sweetheart that’s been on our 10Best Cars list since its 2004 debut. Modestly freshened for ’06, the TSX prevailed in our last under-30-grand decathlon (“Sustainable Sports Sedans ,” October 2004) and, consequently, anchors this one. (As with our 10Best Cars protocol, the also-rans in that derby weren’t invited to this one.)

The other everyday heroes are new for ’06 in varying degrees, and only one of them-the Jetta GLI-has gone through our battery of instrumented tests (October 2005 ). The GLI is the liveliest member of the Jetta family, and it shares its chassis and components with the GTI, VW’s famous hatchback hot rod. The Jetta’s inventory of go-faster hardware includes a new 2.0-liter direct-injection turbo four that churns up 197 horsepower and 207 pound-feet of torque, a useful upgrade from the previous 1.8-liter turbo motor’s 180 horsepower and 173 pound-feet, although the gain in output is mitigated by an increase at the scales.

The other force-fed car in the field is the long-awaited Mazdaspeed 6, with all-wheel drive and 274 horsepower available from a turbo version of Mazda’s 2.3-liter four. That’s almost enough to put the Mazda on an equal performance footing with bad boys like the Subaru Impreza WRX STI or Mitsubishi Evo, and it was more than enough to hustle the Mazda to the front of this pack in terms of sheer zoom-zoom.

Bob Parks has recently joined Foster Coach Sales, Inc. Bob has been working in the EMS field since 1971, starting work for the family Superior Coach dealership in New England. Bob received his EMT certificate in 1973, worked as a volunteer for a small New Hampshire service, and grew to understand the needs of his customers.

As the industry developed the family business decided to stop selling ambulances and to concentrate on the funeral side of the professional vehicle industry. In 1991 Bob moved to Goshen, Indiana to work for Medtec Ambulance and has since worked supporting dealers in sales and design of ambulances for Medtec, McCoy Miller and lately for Horton Emergency Vehicles.

Bob will concentrate on sales to commercial accounts and specialize in the Medix product line. Bob and his wife Diana live in Goshen, Indiana and but he will travel most of the territory for Foster Coach filling in the voids as needed. He has been a friend of the Foster family for years and has a lot of the same history that Steve Foster has in the EMS field. Then he heard about how baclofen had been shown to reduce the cravings for cocaine in rats. Thinking that it might also work for alcoholism cravings, he began to self-medicate with the drug. Almost overnight he felt much more at ease with himself and the world; at the same time he also felt less of a compulsion to drink.

After further experimenting with the drug, he no longer needed to drink.

His experiences led him to the conclusion that it was his anxiety that led him to drink and this in turn resulted in alcohol dependency developing. His resulting book chronicles his years of alcohol abuse and demonstrates to others how to quit drinking by using Baclofen

How You Can Use Baclofen to Treat Your Alcoholism

Baclofen is not licensed for the treatment of alcoholism. You use it at your own risk, all the information you find on this page is just that, information. Anybody who believes they have a problem with alcohol should visit their health care provider for advice.

Baclofen is not approved for the treatment of alcoholism, why this is so is open to debate. However all drugs need to go through a period of testing before they can be licensed to ensure their safety. Others might say it is due to vested interests in the alcoholism treatment industry. There is no money to be made from baclofen; the same cannot be said of 12 step programs and other newer (and thus more expensive) alcoholism medications.

Dr. Ameisen started with 5 mg and found it most effective (that is stopping the cravings for alcohol) at doses of 270mg. He still takes a daily dose of 50mg and is good as cured. Some have to take more to get rid of the urge to drink, others less. You should speak to a qualified doctor for dosage guidelines.

On January 25, Choice Hotels International announced that the first two Cambria Suites would open in Chelsea and Times Square, NYC, in early 2013. With 194 suites, the Times Square hotel will be the largest Cambria Suites hotel to date.

On January 18, the DiamondRock Hospitality Company announced that it would acquire a NYC hotel property upon its completion in 2013. The development is underway at West 42nd Street in Times Square. Depending on the final number of guest rooms, the purchase price will range from $112.5 million to $135 million or almost $450,000 per room.

In early January, RLJ Development bought the Doubletree Metropolitan Hotel in Midtown for $335 million. The hotel sellers, Highgate Holdings, Goldman Sachs Group Inc. s Whitehall Real Estate Funds and Rockwood Capital LLC, made a hefty profit. The Whitehall-led group bought the hotel in 2003 for $110.5 million and spent $35 million on renovations.

The Substance Abuse and Mental health treatment Administration reports that there are more than 400 addiction treatment centers in New York City alone. N.Y. Education Law Article 129-B Notice of Audit Issued to New York Colleges and Universities

Many institutions are reporting receipt of a letter dated June 26, 2017 from the New York Office of Campus Safety with an attached Notice of Audit (“Notice”) pursuant to New York Education Law Article 129-B (N.Y. Educ. Law §§ 6439–6449). The Notice seeks data submissions relating to the provision of Article 129-B and it includes 23 separate requests for information and documentation. The response to the Notice must be postmarked no later than July 7, 2017.

This audit comes at a time when key institutional personnel, including student affairs professionals, are away from the office on vacation and some institutions are closed. In addition, the short turnaround requested (fewer than 10 calendar days over a major holiday weekend) gives very little time to gather the responsive materials, let alone review and redact them if necessary. The time period is far less than what is required to respond to a discovery demand under the New York Civil Practice Law and Rules.

We believe that it is unclear whether FERPA permits the release of personally identifiable student information to the New York Office of Campus Safety, which is an office of the New York Division of Criminal Justice Services and not an office of an education agency.

The statute at § 6449 provides only for the collection of aggregate data, consistent with the statute’s emphasis on confidentiality and respect for the privacy of those involved in the process. Section 6449(3) emphasizes that, even when collecting aggregate information “the department shall not release the information, as provided for in this section, if it would compromise the confidentiality of reporting individuals or any other party….”

Eleven of the 13 requests in the Notice contain statutory references to the applicable section of Article 129-B as authority for the requested data. Two of the requests, numbers 9 and 10, contain no reference to the statute and there does not appear to be any specific section of the statute that supports the sensitive nature of the data sought in requests 9 and 10. Additionally, request number 4 seeks copies of all “no contact orders” issued by the institution, although there does not appear to be a statutory basis for such a request. Number 6(a) seeks data on all students subject to interim suspension, although that request also appears to be beyond the scope of the referenced statutory section.

Notwithstanding the unrealistic time frame to respond to the audit requests and credible questions about the statutory basis for specific requests, institutions must begin to prepare a response.

Request an Extension

We encourage institutions that do not anticipate that they will be able to comply with the aforementioned deadline to contact Deputy Director Stacey Hamilton by telephone to request an extension and follow up with a written request and/or confirmation.

Prepare Materials for Submission

Institutions should plan to submit easily accessible data such as policies, blank forms, website material by July 7, 2017, or the extended deadline, and include a cover letter indicating that, where applicable, additional materials will follow as soon as possible. In that cover letter, the institution may articulate the factors, if applicable, that make it difficult to respond within the narrow time frame allotted. One of those factors may be that the materials have to be carefully reviewed in order to redact confidential information in accordance with the privacy considerations emphasized in Article 129-B and other privacy laws.

We suggest that with regard to request numbers 1, 2, 3, 5, 6(b), 7, 8, 11, 12 and 13, institutions collect the documents and data developed over the past academic year (Fall 2016 to Spring 2017). Note that for request number 12 regarding campus climate assessments, institutions should exercise care when preparing a response to prevent the identification of any particular student.

Concerns with Respect to Disclosure

Request number 4 asks for information and documents regarding each request for a “no contact order” received by the institution. Institutions may decide to provide a copy of the institution’s template “no contact order” language, rather than specific orders, together with data on the number of orders issued and the number of orders that were changed. Although the New York State Office of Campus Safety appears to be seeking copies of specific “no contact orders” that include the names of the students, it is unclear that they have the right to this personally identifiable information under FERPA.

Similar consideration applies to request number 6(a). It may be acceptable in the initial response to provide aggregate data on interim suspensions and not data that could identify a specific student. In light of the statute’s emphasis on confidentiality and privacy, and the fact that the statute refers to aggregate data, the Office of Campus Safety may not have the authority to receive personally identifiable information.

A separate issue is the scope of request numbers 9 and 10, which seek an academic year’s worth of records relating to all reports of incidents covered by Article 129-B and all records involving misconduct hearings covered by Article 129-B. These requests are overly broad, are seriously inconsistent with the statute’s emphasis on confidentiality and privacy, and are not in accord with the statute’s authorization to collect aggregate data. Institutions should be consistent in the documentation provided for each case and should make sure information does not contain personally identifiable information about students while this issue remains unresolved.

In a letter to the Office of Campus Safety dated June 29, 2017, the Commission on Independent Colleges Universities in New York (CICU) has raised the question of redacting personal information pertaining to students.

Echo Lake Beach

Beautiful Echo Lake, at an elevation of 1,931 feet, offers views of Mt. Lafayette and Cannon Mountain. Visitors can also enjoy swimming, fishing, boating and canoe and paddle boat rentals. Last rental boats go out an hour before closure. The waters of Echo Lake flow westerly toward the Connecticut River.

Franconia Notch Bike Path

Constructed as part of the Franconia Notch Parkway this path, completely separate from the Parkway, traverses the length of Franconia Notch and provides access to all park facilities and natural features.

Cannon Mountain Now Offers bike rentals and shuttle service. Bike Rentals are being offered by Sport Thoma and you’ll find a full range of Scott all-terrain bikes for adults and children.

Access to the Franconia Notch Bike Path from the base of the Aerial Tramway, Exit 34B, I-93. Shuttle rides will be available from the Flume Gorge back to the Tram. Visit the Franconia Notch Recreation Path page for more details on the bike path.

Lafayette Place Campground

Lafayette Campground is located in the middle of Franconia Notch State Park, along the Franconia Recreational Trail. The campground is open for walk-ins beginning May 13, 2016 and reservable stays from Memorial Day through Columbus Day weekend. Each campsite is wooded and has a picnic table, car parking space and an open fireplace. Brochures, weather reports and basic provisions such as wood, charcoal, ice, milk, and snacks are available at the lodge between 8 am and 9 pm. Coin-operated showers are open 24 hours. Winter camping is welcome; a vault toilet but no running water is available.

Cannon Mountain RV Park

The seven-site RV park, located on the north shore of scenic Echo Lake in Franconia Notch State Park, is open year-round. Three-way hook-ups are provided during the regular camping season beginning on May 20th, 2016. However, there is no water or sewer, including dump station, available from mid-October through May 1. RVs must be self-contained. Each site has a picnic table and fire ring. Bathrooms are available seasonally at the beach but are locked at night. In addition, many campers enjoy the convenience of winter RV camping at the base of popular Cannon Mountain Ski Area.

Open Virtual Tour

In 2006 I shot our very first spherical aerial panorama. from which the AirPano project started, in the beautiful place of the Colorado and the San Juan rivers confluence (USA):

The result was overwhelming by looking at the large screen one had a total illusion of flying over the canyon. And so we decided to continue our experiments.

The next attempt was made in 2007 above the island of Manhattan. I remember this photo shoot by one funny detail: there were no headphones for passengers in the helicopter’s cabin. I had to shout from the back seat (while trying to overcome the engine roar that was coming from the open door) to let the pilot know where to go. He didn’t hear me anyway, so in order to change the shooting position I had to tap on his shoulder and then after he looked at me I had to point the direction with my finger where the thumb down meant moving lower. And guess how I showed him to go higher 🙂 It was fun.

After that incident, when hiring a helicopter I always ask: Do you have passenger headphones in the cabin?
And pilots are always surprised: But of course! Is it possible not to have them?
Yes, it is possible, I answer In New York 🙂

Despite of all the difficulties, this photo shoot was accomplished, delivering world’s first virtual photo tour above Manhattan .

There were many more flights after this one, and many like-minded people joined AirPano project in the process. As of today we shot around 100 spherical panoramas of the most interesting and beautiful places in all the continents of our planet. Except for Antarctica. We also made virtual photo tours over the world’s most beautiful cities.

In 2011 we returned to New York to once again photograph Manhattan: this time making it much better technically and artistically. Enjoy the daytime aerial panoramas of Manhattan now. The evening flight over Manhattan and the large photographic virtual tour will be published in the nearest future.

New York is the most famous city of the USA, and Manhattan is the most famous part of New York. It all started 400 years ago when the most famous and profitable in human history real estate deal was made: the Native Americans sold this ‘not so useful’ piece of land to Europeans for 24 dollars.

Since that time the island has changed dramatically. For 24 dollars one can probably buy a ticket to the Empire State Building.

Present day Manhattan is the most populated island on Earth. The land itself costs about 40 billion US dollars, and the cost of the entire Manhattan, including its real estate, reaches over 3 trillion US dollars.

This remarkable part of New York City is best to be discovered on foot not only because of the infamous New York traffic or cost of parking in New York. There is a joke in New York that only wasteful people can afford driving in New York, because the time one spends on finding a parking spot sometimes equals the time it takes to drive from one side of Manhattan to another.

New York is known to be a financial capital of the world, and the island of Manhattan is where all the banks, offices, largest insurance companies and corporations are located literally one on top of another. Basically the rest of New York, and eventually, the rest of the United States, work for Manhattan, making sure that this gigantic city inside of the city continues its mission processing the wealth of the whole world.

Besides a large number of museums, almost every building in Manhattan has a historical value. Old buildings harmoniously coexist with the best examples of the modern architecture. Most famous New York attractions are located in Manhattan, including Empire State Building mentioned above, Central Park and Times Square. There is also colorful China Town and bright Broadway they are all located on this island. It is a shame not to bring a photo of Manhattan from your trip to New York!

A provider for adults and pediatric patients with life-limiting illness, Vitas Innovative Hospice Care is planning an open house from 4:30-7 p.m. Sept. 26 at its newest location, Silver Cross Hospital Campus. 1890 Silver Cross Boulevard.

Palliative chemotherapy: harms and benefits weighed in new study

Palliative chemotherapy is treatment designed for terminal cancer patients to prolong survival and ease symptoms but not cure disease. Now, researchers have found that the therapy comes with certain harms, which they say need to be addressed.

The researchers, from the Weill Cornell Medical College, Dana-Farber Cancer Institute and Harvard Medical School, have published their results in the BMJ.

They say their findings uncover a discrepancy between what type of care cancer patients want and what they actually receive.

Overall, the team found that terminal cancer patients who receive chemotherapy during the last months of their lives are less likely to die where they wish and are more likely to undergo invasive medical procedures – including CPR and mechanical ventilation – than patients who did not receive the therapy.

Dr. Holly Prigerson, of Weill Cornell Medical College, says that the reasons for the link are complicated, but they may originate in misunderstanding of the purpose and consequences of palliative chemotherapy.

For the study, investigators assessed data from 386 patients in a federally funded study, called Coping with Cancer.

This 6-year study followed terminally ill people and their caregivers until the patients died, and the researchers looked at how psychosocial factors influenced patient care.

After asking the caregivers to rate their patient’s care, quality of life and where the patient would have wanted to die, the researchers reviewed patient medical charts to determine the type of care they actually received during their last week.

Chemotherapy patients less likely to die where they want

The researchers found that patients receiving palliative chemotherapy were less likely to talk to their oncologists about the type of care they wanted to receive, to complete Do-Not-Resuscitate orders or to even acknowledge they were terminally ill.

The findings reveal that 68% of patients receiving palliative chemotherapy died where they wanted, compared with 80% of those not receiving the therapy.

In detail, 47% of patients who received the therapy died at home, compared with 66% of patients who did not receive the palliative care. Additionally, 11% of patients receiving the chemotherapy died in an intensive care unit, versus only 2% of patients not receiving the therapy.

Dr. Prigerson says:

“It’s hard to see in these data much of a silver lining to palliative chemotherapy for patients in the terminal stage of their cancer. Until now, there hasn’t been evidence of harmful effects of palliative chemotherapy in the last few months of life.”

“This study is a first step in proving evidence that specifically demonstrates what negative outcomes may result,” she adds.

The team also found that patients receiving palliative chemotherapy were more likely to be referred to hospice care much later than those not receiving the therapy.

Hospice care is a place that provides comfort and emotional support for terminal patients, and 54% of the patients receiving chemotherapy were referred to hospice late, compared with 37% of the patients not receiving the drugs.

‘Potential need for oncology practice changes’

The team notes that 56% of the patients from the study were receiving palliative chemotherapy in their final months, and they say this majority of use emphasizes the potential need for changes in oncology practice at academic medical centers.

Dr. Alexi Wright, assistant professor and medical oncologist at Dana-Farber, says:

“We often wait until patients stop chemotherapy before asking them about where and how they want to die, but this study shows we need to ask patients about their preferences while they are receiving chemotherapy to ensure they receive the kind of care they want near death.”

But Dr. Wright adds that their findings should not be interpreted that patients should be denied or not offered palliative chemotherapy:

“The vast majority of patients in this study wanted palliative chemotherapy if it might increase their survival by as little as a week. This study is a step towards understanding some of the human costs and benefits of palliative chemotherapy.”

“Additional studies are needed to confirm these troubling findings,” concludes Dr. Prigerson.

The School of Management and the School of Public Health and Health Professions at the University at Buffalo offer a collaborative program leading to both a Master of Business Administration (MBA) and a Master of Public Health (MPH) degree (Health Services Administration concentration only).

Its purpose is to facilitate an integrated course of study that prepares students for high-level management careers in health care organizations and agencies. The program involves three years of study, with a total of 97 credit hours. The time required will be one semester to one year less than would be required to pursue the degrees separately.

I have sent them all of the proper information and confirmed that they have received it and you will be receiving your refund within the next week and a half. I apologize again for any inconvenience. Whether it s during peak hours or not, the gym has become a dumping ground filled with filthy paper littered all around the floors; on the machines; and along the walls. Also, the floors are constantly plagued with empty water bottles and the like. Second, the bottles of cleaning solution are almost always empty and/or defective, proving them unable to use. After approaching the front desk to notify them on several occasions, once instance I was told that, my co-worker left to pick-up some food and I m the only one on duty. Even, I m the only one here right now and I m not allowed to leave my post. I find it unacceptable that sanitary inspectors fail to assess the cleanliness of the gym on a regular basis throughout the day, especially on the weekends. Finally, the machines are frequently in a state of disrepair and are inoperable more often than not. Whether it s during peak hours or not, the gym has become a dumping ground filled with filthy paper littered all around the floors; on the machines; and along the walls. Also, the floors are constantly plagued with empty water bottles and the like.

Second, the bottles of cleaning solution are almost always empty and/or defective, proving them unable to use. After approaching the front desk to notify them on several occasions, once instance I was told that, my co-worker left to pick-up some food and I m the only one on duty. Even, I m the only one here right now and I m not allowed to leave my post. I find it unacceptable that sanitary inspectors fail to assess the cleanliness of the gym on a regular basis throughout the day, especially on the weekends.

Finally, the machines are frequently in a state of disrepair and are inoperable more often than not.

This degradation of service is unfortunate because I greatly enjoy exercising at Planet Fitness – Canarsie. I would like to hear from you about your plans for updating and maintaining the gym so that I can decide whether or not to renew my membership for another year. If I have not heard from you soon, I am afraid I will have to cancel my membership indefinitely. I eagerly await your attention to this matter.
