A historic sign that welcomed visitors to The Ahwahnee Hotel in Yosemite National Park was stolen just days before it was due to be covered up as part of a fierce legal battle between the park and its outgoing concessions operator.

Yosemite spokesman Scott Gediman said the sign was taken sometime Saturday night or Sunday morning. The Ahwahnee’s name, in place since 1927, will change officially at 12:01 a.m. Tuesday, when Aramark takes over the park’s concessions contract from Delaware North.

Curry Village, Wawona Hotel, Badger Pass Ski Area and Yosemite Lodge at the Falls also will change names. They will become Half Dome Village, Big Trees Lodge, Yosemite Ski Snowboard Area and Yosemite Valley Lodge, respectively.

Gediman said park rangers are investigating the theft of the iconic rounded sign. Whether the crime was a political statement or simple act of vandalism is irrelevant, he added, as this is a federal offense and is taken seriously.

As for the name changes, both park and Aramark employees will be hard at work just after early Tuesday morning, Gediman said. The park is responsible for changing road signage, while Aramark will cover the existing signs with temporary canvas signage.

“We feel strongly that these iconic facilities, and their historic names, ultimately belong to the American people,” Gediman said. “But with open litigation, these name changes are necessary to continue service without disruption.”

Aramark spokesman David Freireich said the company has worked with the Park Service for the last six months to make sure “tomorrow looks no different than today.”

However, the merchandise stands will look quite different.

“Due to the outgoing concessionaire’s claim, we are unable to sell certain items,” Freireich said. These items are: apparel, pens, mugs and stickers.

To combat this, the company will unveil a new line of merchandise branded simply as Yosemite, not Yosemite National Park. Items that do not fall into those four categories will continue to be sold in shops with the original name on them.

The company also will roll out a new website, www.travelyosemite.com. as well as a new phone number for reservations and general inquiries: 888-413-8869. Both will begin accepting reservations Tuesday.

Freireich said that 95 percent of current Delaware North concessions staff members were retained by Aramark.

“The names and faces of employees will remain the same to ensure a smooth and seamless transition,” he said.

Canvas signage will be used temporarily until Aramark can permanently remove the existing signs, Freireich said. Among the signs that will be coming down: the large Camp Curry Village sign at Curry Village, which opened in 1899.

The National Park Service announced the name changes in January. Delaware North contends the trademarks associated with these buildings came with the concessions contract – a claim that outraged the public and led to an intense legal battle with the park that still is working its way through the courts.

On Feb. 26, Delaware North sent a letter to Aramark President Bruce Fears offering the trademarks, which its lawsuit claims are worth $44 million. to the company for immediate use, and “the amount of compensation, if any, to which Delaware North is entitled from the National Parks Service and/or Aramark for that property would be determined in due course.” The names in question were legally trademarked by Delaware North, but the government believes their value to be less than $2 million.

Aramark issued a strongly worded statement that condemned Delaware North, saying it “forced” the National Park Service to change the names with its ownership claims and demanded “an outrageously inflated price over (the National Park Service’s) fair market value.”

The National Park Service filed a petition with the U.S. Patent and Trademark Office to void the existing trademarks held by Delaware North at Yosemite National Park.

“(Delaware North’s) attempt to offer to assign these names to Aramark or any another company misses the inescapable fact the names are not theirs to ‘assign’ in the first place,” the statement said. “Their proposal reinforces that (Delaware North) continues to completely miss the point the Yosemite names belong to (the National Park Service) and the American people, not (Delaware North) or Aramark or any other company.”

In a new turn, the National Park Service filed a petition with the U.S. Patent and Trademark Office to void the existing trademarks held by Delaware North at Yosemite National Park.

“These names were trademarked without our approval, and this petition asks the trademark office to cancel them,” Gediman said. “However, this doesn’t change what we’re doing tomorrow.”

Gediman said Delaware North offered the names to the Park Service in December in a similar proposal to the one recently sent to Aramark. It was turned down.

If the patent office grants the request, the Yosemite names will be changed back immediately, Gediman said.

Gediman did not have a timetable for the resolution of either the recent petition or the ongoing lawsuit. He hopes the changes will be temporary, but said all park officials will use the new names until the litigation is resolved.

The Ahwahnee is missing its sign, but that may become a moot point, at least for now. When it opens Tuesday, it will be The Majestic Yosemite Hotel.

DAMARIS ARGUERA Foothills Country Hospice Society The Foothills Country Hospice Society (FCHS) is a grassroots community sponsored organization directed by a volunteer board of directors and is a registered charity and not for profit Society WHO WE ARE WHAT WE DO FCHS is dedicated to providing expert compassionate care to people who are terminally ill and to their families.

The Board s primary work to date has been to establish a vision, raise funds and awareness, and to ultimately design and build one of the first rural, purpose-built, free standing adult Hospice homes in Canada ESTABLISHMENT WHERE WE OPERATE On January 28, 2008, the Foothills Country Hospice accepted its first patient.

The fruition of the Board s vision has been realized through the dedication and effort of over 100 volunteers from the Foothills area south of Calgary, Alberta

Prior to the Hospice being open, many people had to leave their families and community supports in order to receive care as they approached death, since there is no hospital in Okotoks nor a full time Home Care service. The desire to have people remain with family and friends in their home community was the motivating force behind FCHS drive to build the Hospice home in service of our communities most vulnerable members REFERENCES CONTACT INFORMATION OUR CLIENTS All Hospice admissions for Calgary and area are managed centrally through the Pathways system. A palliative care expert – usually a doctor or nurse in the Regional Hospice Palliative Care Service – either at hospitals or in the community, can request a hospice placement for a person after assessing their suitability for this kind of facility.

Criteria include having a terminal diagnosis with an expected length of life of less than 3 months. People being admitted to a hospice agree that they are not seeking life prolonging interventions nor further investigations.

Since there is often a waiting list for hospice placement, criteria have been established by Alberta Health Services in order to assign a bed according to care needs. People needing admission who are currently residing in their own homes usually are assigned the highest priority. HOW OUR PROGRAM BENEFITS SENIORS The FCHS benefits seniors in specific because hospice palliative care is an approach to care that recognizes the importance of many aspects of living and caring as people approach a time of death. It pays attention to physical needs, supporting conversations, and practical preparation, providing expert pain and symptom management, allowing opportunity for spiritual reflection, and preparation, dealing with family issues, and supporting friends and families before and after the death of a loved on.
These key components make up hospice palliative care. It allows individuals to live as well as they can, as they recognize approaching death, and can provide a healing and hope filled time.

TUESDAY, Nov. 2 (HealthDay News) — Many people with advanced dementia aren’t getting much-needed hospice care because the admission criteria is flawed, researchers say.

“Dementia is a leading cause of death in the U.S. and hospice care can benefit patients with dementia. The main hindrance to getting palliative [comfort] care is guidelines that try to guide practitioners to wait until an estimated life expectancy of six months,” said Dr. Susan L. Mitchell, a senior scientist at the Institute for Aging Research at Hebrew SeniorLife in Boston, and lead author of a new study.

Such end-of-life predictions are difficult to make with certainty in dementia cases. Instead of using life expectancy as the requirement for admission, hospice care for dementia patients should be offered based on the patient’s and family’s desire for comfort care, suggest Mitchell and colleagues in the study published in the Nov. 3 issue of the Journal of the American Medical Association .

Hospice, or palliative, care is most often associated with cancer patients. The goal is to provide comfort and support to patients and their families, instead of life-prolonging treatments.

For people with cancer, the decision to switch to palliative care is more clear-cut. It generally occurs when someone decides to forgo traditional cancer treatments, such as chemotherapy or radiation, that don’t seem to be working anymore, and instead receive comfort care, which includes better pain management and discussions about important end-of-life care decisions.

For people with dementia, the decision process is murkier. Most people with advanced dementia are already in nursing homes, receiving around-the-clock care, but palliative care can provide families with additional support and help families make difficult decisions, such as whether or not to treat infections with antibiotics or to use a feeding tube to deliver nutrition. Palliative care may also provide better pain management and symptom relief, said Mitchell.

To improve the likelihood of dementia patients getting palliative care, Mitchell and her co-authors tried to come up with a better tool to assess their potential life expectancy.

This new method, dubbed the Advanced Dementia Prognostic Tool (ADEPT), includes 12 items, such as body mass index, ability to perform tasks of daily living like self-feeding, bowel incontinence, shortness of breath and oral food intake.

The researchers compared their assessment tool with the standard Medicare hospice eligibility guidelines on 606 residents in 21 nursing homes.

Their tool accurately predicted a life expectancy of fewer than six months 67% of the time, versus 55% for the Medicare guidelines, said Mitchell.

“While ADEPT was better than the Medicare criteria, its predictive ability isn’t perfect,” said Mitchell. “The delivery of palliative care should be guided by a preference of comfort care rather than by life expectancy,” she added.

A 2009 study by Mitchell and her colleagues was the first to label dementia a terminal illness like cancer and other incurable diseases.

Dr. Joseph Shega, an associate professor in the section of geriatrics and palliative medicine at the University of Chicago Medical Center, said he agrees that the issue of comfort care for dementia patients deserves attention.

“It’s important to recognize that we’re not really good at figuring out how long someone with dementia might live, and I agree with these authors that we should focus more on the goals of care and stop spending resources on trying to figure out how long someone might live,” said Shega.

“Hospice provides more support for nursing home staff, better support for the family, and can help better educate the family on the natural process of dementia so they know what’s going on,” he explained.

Hospice also helps manage symptoms, like discomfort or agitation, Shega added, while making sure that care plans and treatment goals agree with the values and wishes of the patients and their families.

Ayrshire College students were delighted to once again deliver Ayrshire Hospice’s fantastic ‘Simply For You’ programme at the college’s Ayr campus.

‘Simply For You’ is an informal workshop designed to boost confidence and morale for ladies struggling with changes to their appearance.

Amid lots of fun, laughter and pampering, the Ayrshire Hospice ladies picked up a few style and colour tips from the students while enjoying the friendly support of other women facing similar challenges.

Students at Ayrshire College have been supporting the programme since 2014.

This year the Make-Up Artistry and Beauty students provided facials, hand care and make-up sessions, and colour analysis to the hospice women.

The day was a resounding success, summed up best by one of the ladies who attended. She said “I’ve had a great time! It’s been nice learning some new ideas from the young ones who are training. The Ayrshire Hospice team have been fantastic again.”

Lisa Flint, Curriculum Manager of Hair, Beauty, Complementary Therapy and Make-up at Ayrshire College, said “We were delighted to hold our first ‘Simply for You’ programme at the Ayr campus. The relationship we have built over the past couple of years with the Ayrshire Hospice provides our learners with invaluable real-life experience whilst supporting our local community.

“I am extremely proud of the professionalism, knowledge and high standard of treatments the learners provided. They were true ambassadors for Ayrshire College and have gained more insight into the Ayrshire Hospice. The feedback from guests and students proved that this was a huge success and a very enjoyable day for all involved. We are looking forward to holding our next event.”

Wife’s Zafira is not starting again.
The last 2 times this happened it was the starter, replaced and all was OK.

The difference between the last 2 times and this time is that the solenoid on the starter is not clicking (still clicking before)
And when I took the starter off it was still free. (was jammed before)

Also checked the old starter at the 2 live connections while off the car.
1) starter starts to spin OK
2) the solenoid clicks OK

Tried a new starter anyway but same symptoms.

The other symptoms is that the central locking is sluggish, and feels like the battery is quite low – however same symptoms as when the starter did go previously.
When trying to start all the dashboard lights go out. (or is that normal)

I have checked the earth wire from the starter to the battery earth and no resistance – so OK here
The battery was tested with Gunson’s Start check battery tester and was 75-100% so seems OK here too.
Battery has been smart charged anyway just in case.

It would appear that the solenoid is not getting power to get the starter spinning.

Any mechanics here that know what to check now, or what the likely cause is ?

If your dash lights are going out and theres barely enough juice to activate the central locking, while I’d check all major connections, I’d bet the battery is just about to give up the ghost.
Modern batteries appear to be all or nothing and can go off very suddenly.
Check the voltage across the battery terminals and if its less than 10.5 – 11 volts, it’s very tired. Get it started with a jump, check across the terminals again and make sure it’s at least 13v with the engine running, ideally around 14-14.5 volts to make sure the alternator is fine. Anything higher is just as much a problem too!

It ended up being the battery – a shorted cell.

What threw me was that it initially was showing OK.
After charging it was not showing OK and 10.6 volts
I checked it an hour later to confirm readings and checked out OK with around 12.5 volts – so intermittent readings.

So wasted my time and effort changing the starter motor twice, and that’s not easy to do on a Zafira.

    Why and when the iPad is the best e-reader

    When the Kindle appeared, readers realized that years of false starts and empty promises were over, that the e-reading revolution had really and finally arrived. And then, a kind of Cold War set in between two kinds of readers: bibliophiles who insist that wood pulp—the musty smell of real paper!—is the best way to read, and the rest of us, who increasingly love the convenience of the digital ebook era.

    They’re both right.

    For the first time in history, when we sit down to read a book, we’re faced with more than simply a choice of what to read—we must also decide how to read it. Perhaps you’ve found your favorite reading method, and tend to stick with it. But the truth is that different mediums offer different strengths, which are in turn tuned to different types of reading. Wedging yourself into a corner with just one device for all types of reading can deny you the pleasures and advantages of the other.

    In the last two years, I’ve read books in numerous ways: a desktop computer, a netbook, my iPhone, my iPad, an e-ink Kindle, and—believe it or not—a good old-fashioned paper book. (I even listened to Jonathan Franzen’s Freedom with the Audible.com iPhone app.) The strength of those first three options, in most cases, was that they’re available: If you need to read something now, they’ll do in a pinch.

    Mostly, though, I stick to the iPad, a basic model e-ink Kindle, or paper. The iPad is the closest thing to my default reading device, for reasons I’ll get to in a second, but it’s never an automatic choice. Why? Because each has its advantages.

    For studious reading. I prefer the iPad. Yes, I’ve been out of college for years now, but in one of my other journalistic pursuits I co-host a podcast featuring interviews with authors of books on politics and policy. That means lots of reading, lots of highlighting, and lots of note-taking. The iPad is clearly best at the latter two tasks, especially the note-taking. And it keeps getting better in that regard: Amazon recently updated its Kindle iOS app to offer several different colors of highlighter ink, making it easier to color-code your notes in a book as you go. That’s a feature shared by Apple’s iBooks app as well, as both companies work to appeal to the educational and textbook markets.

    This isn’t necessarily deep reading: The iPad facilitates note-taking and skimming—the kind of reading done by college students. But at the end of a book, it’s so much easier to go back, find your notes, and give yourself a Cliff’s Notes overview of what you just read. If you’re in information-processing mode, the iPad is usually the way to go. Making and navigating these sorts of notes on an e-ink Kindle is painful.

    For light reading. the basic e-ink Kindle is the way to go: It’s the cheap dime-store paperback that’s always with you. If you’re not into deep note-taking, but merely want to enjoy a good story, the vanilla Kindle makes good sense. The push-button page-turning often rewards propulsive forward progress more than it does a deep dive, but you might not notice if you’re reading the latest Harlan Coben mystery, say.

    The e-ink Kindle is also great for “lack of light” reading: Studies show that backlit displays, like the kind you get on the iPad and other tablets, reduce melatonin production and can disrupt your sleep—making the iPad or any other screen a bad idea for bedtime reading. If you’re curling up with a story at night, this is often the best way to go.

    Of course, “light” also has another meaning—the Kindle often offers the most ergonomically friendly version of a book, as well. When Robert Caro’s most recent volume about President Lyndon Johnson came out last year—weighing in at a hefty 736 pages—there was little temptation to buy that sucker in hardcover: My poor wrists would’ve rebelled, and my back was in no mood to carry it around in a bag everywhere. The iPad would’ve been a significant weight reduction, but I was also reading the book for pleasure instead of study. The even-lighter Kindle proved the ideal platform for such a monster.

    Of course, the Kindle has lost some of its advantage on this front thanks to the advent of the iPad mini. On the other hand, the Kindle doesn’t offer easy access to Facebook or Twitter, either—and when I’m in a reading mood, the lack of electronic distraction the Kindle affords is welcome.

    Of course, paper books don’t provide electronic distraction either. That’s why they’re often the best device, still, for old-fashioned immersive reading. The benefits here are admittedly less substantial, more theoretical and romantic, but I believe in them nonetheless. Paper books don’t require batteries. They don’t claim your attention with social media, games, or movies contained within. They simply are. And in the case of older books, those worn with time, one can feel oneself joining a lineage of readers. That’s not quite the case with today’s ebooks—though changes to the way used ebooks are traded and sold might someday endow those circuits and data with the romance of old-fashioned books. It hasn’t happened yet. Sometimes you want to read, stare out the window, read, then stare out the window. Books are still the best tool for that approach.

    But is there a best best?

    At this point, yes: The iPad mini. It’s got all the power of the iPad and its best reading apps; it features the ergonomic joy of the Kindle, and, well—if you want immersive reading free of distraction—you can always go ahead and turn your iPad to Airplane Mode to free yourself from distractions.

    Otherwise, it’s OK to accept some messiness in your reading life. Don’t just download the next book on your reading list; consider how you want to experience it, and let that decision be your guide.

    Had Your Credit Card Hacked Lately?

    My credit card information was just stolen — again. This is the fifth time in the past three years, which, if polling my Facebook friends is any indication, is about par for the course. It happens to everyone, it seems, about once every nine months. And for the record, each time I was hacked, my credit card was still physically in my possession.

    There’s no need to tell you what a pain in the ass this is, but I will anyway. You have to switch all direct billings from the old card to a new card — and you invariably forget a few which means you get dinged with late fees. You also have to update all those online shopping sites where you have your credit card info stored. And of course you have to check all three credit scoring sites to see if the theft is causing you any lingering damage. In terms of inconvenience, credit card theft is just one Dante circle removed from losing your whole wallet and having to cancel and replace everything.

    The good news is that none of my five thefts have cost me a penny, for which I am very grateful. And in four of the five instances, it was Chase — my credit card provider — that discovered the fraud and told me about it. I hadn’t had a clue.

    In all five cases, Chase closed my account, issued me a new one on the spot and sent me new cards overnight. I get the idea that they have an Amazon-size warehouse full of customer service reps who do nothing all day but close accounts that have been infiltrated by thieves.

    In my most recent theft, most of the charges were made in Tokyo and they included a dating app, some junk food bought in a mini-mart, and a bunch of other small purchases. The way credit card thieves frequently work is to first test the waters with small purchases to see if they go undetected before they try to charge something larger — you know, felony-like larger. In my case, it was a stay in an expensive hotel in Beverly Hills. Nope, I told the helpful Chase rep, I’ve never been to Japan or an expensive hotel in Beverly Hills.

    I can’t complain about Chase’s fraud detection unit; they seem to be right on top of things. But what I don’t get is why this keeps happening and why there doesn’t appear to be much of an effort to catch the bad guys. And I certainly don’t mean to single out Chase. Credit card fraud is an equal opportunity crime. No one is immune and it certainly doesn’t matter which lender you have although different cards hold you to different levels of liability.

    Given the prevalence of the problem, where’s the prosecution? For the number of thefts to be this widespread, doesn’t it mean that people see this as a crime that’s easy to get away with?

    The Nilson Report puts credit and debit card fraud at $11.27 billion in losses in 2012. That’s certainly nothing to sneeze at.

    In fact, credit card fraud — along with national security as it relates to terrorism and identity theft — are the top two things we most worry about, says Unisys. Almost 40 percent of us were deeply concerned about the security of shopping and banking online.

    How are these thieves doing it? Sometimes they just outfox you, technologically. They install skimmers in ATMs or at gas station pumps when no one is around that record credit card information surreptitiously. The information is later resold to criminals who create counterfeit
    cards or charge items over the phone or the Internet.

    Other times, though, it is simply you being careless. So what can you do about it (besides paying cash for everything)?

    1. Make sure no one is looking over your shoulder when you are paying with a credit card in stores. The Department of Justice’s website says that this is a still a common way to steal credit card numbers. It’s easy to get distracted.

    2. Lower your voice, or better yet, keep your mouth shut.
    Limit what you say on your mobile phone in public. Thieves get ahold of people’s credit card numbers by listening to cardholders’ conversations with hotel or rental car companies, according to the Department of Justice. Instead of holding those conversations within earshot, move to a quiet place. Never recite your credit card information in a busy lobby or from the back of a cab.

    3. Get a home shredder.
    Yes, it’s come to that. Destroy every check and credit card or bank statement before throwing them in the trash. Some thieves are dumpster divers, says the DOJ. It only takes one stray receipt to give a criminal all the access he or she needs.

    4. Watch where you eat.
    Dishonest wait staff have been known to use hidden or handheld devices to copy your credit card for later counterfeiting. They are often tied to a local crime ring, or seasonal and transient workers, said KrebsonSecurity.com .

    The Scam . The con artist sends a letter or email — purportedly only to a few recipients but actually to several thousand — making an offer that would result in a large payoff for the victim. The details vary, but generally the story is that a person, often the wife or son of a deposed African dictator, knows about some unclaimed fortune that they are willing to share with the scam victim in return for an advanced fee (e.g. bail money for the imprisoned millionaire). Once money starts coming in, the con artist will continue asking for more, claiming that problems have arisen. How To Protect Yourself . The FBI advises that if you “receive a letter from Nigeria asking you to send personal or banking information, send the letter to the U.S. Secret Service, your local FBI office or U.S. Postal Inspection Service.” You can also register a complaint with the Federal Trade Commission’s Complaint Assistant here.

    The technology world loves its buzzwords, no doubt. If you dig through the hype and hit upon the substance that lies beneath, however, you might be onto career-building gold.

    Take DevOps, for instance: It’s been among the buzzier terms in and around IT lately. But if you go deeper than the surface, and take note of some of the skills and technologies that DevOps environments increasingly need and make use of, your list will begin to look a lot like the lists that hiring managers and recruiters put together when seeking new talent.

    Consider the configuring and provisioning automation tool Puppet, for example, or project management and issue tracking application JIRA. Neither one of these is a “DevOps tool,” per se, but they are both increasingly found in DevOps environments, or ones that simply run on Agile development or comparable “continuous delivery” methodologies.

    Tech job site Dice gave InformationWeek a rundown of the IT job skills that have seen the greatest increases in the number of open positions listed on its site from January 2014 to January 2015. These are drawn from any category with at least 1,000 positions listed. JIRA has seen a 69% jump in the number of listings in that time period, while Puppet saw a 67% increase, making them two of the site’s top three fastest-growing tech skills areas.

    In other cases, as with security — the No. 1 fastest-growing tech skills area, according to Dice — the hype itself may be a key indicator of job market and hiring trends. The security field continues to dominate headlines. The Anthem breach is just the latest in a long string of high-profile security incidents. So, it should surprise exactly no one that security skills will be in very high demand in 2015 and beyond.

    Dice makes a distinction between two closely related security terms. It defines “cyber-security” as focusing on defending assets, such as networks and endpoints, from threats like denial-of-service attacks or malware, and “information security” as focusing on protecting data and privacy. In either case, hiring demand is way, way up: Cyber-security is the No. 1 fastest-growing skill area on Dice, up 91% in listings from January 2014 to January 2015. Demand for Information security skills increased 48% during the same period.

    “Skill [and] technology popularity is very reflective of current events,” said Dice president Shravan Goli in an interview with InformationWeek. “The multitude of security breaches that we have seen in the past few years has caused companies to want to be extra-cautious. That is, in part, why professionals with security skills, such as cyber-security or information security, are in such high demand.”

    Security is one of the hottest fields for IT recruiters this year. “Security is a huge issue. Mobile and cloud computing is everywhere,” said Laura McGarrity, VP of digital marketing and recruiting firm Mondo. in an interview with InformationWeek. McGarrity lists “security architect” and “security engineer” among her most sought-after titles this year. “Users are now connected on multiple devices, with multiple endpoints, making it tough to manage security risks.

    If you want to ensure your skills stay sharp, it certainly pays to keep tabs on emerging and, dare we say, trendy technologies, as well as on the technology news headlines. That doesn’t mean every marketable IT skill is a buzzword. In fact, far from it. Java developers are still highly employable. (The language remains ubiquitous in enterprise IT.) Ditto for those with Web development and open source skills. Yet, it can be extra beneficial — and potentially lucrative — to round out these “old” skill areas with newer technologies.

    Tom Renda, VP for the New England region at recruiting firm Eliassen Group. told us that developers who’ve added Hadoop experience to their resumes aren’t hurting for job prospects these days. “Open source developers, Java developers, and Web developers are all in high demand. When they have Hadoop experience as well, they are highly sought after in the big data and analytics space.”

    Metal Hips Fail Faster, Raise Other Health Concerns

    For many people with severe hip arthritis, total joint replacement brings pain relief and improved mobility for the life of the implant typically 15 years or more. But a new study shows that some metal-on-metal implants are likely to fail much sooner, especially in women. The study adds to a growing list of problems linked to metal-on-metal implants, including bone and tissue destruction and high levels of metal ions in the blood.

    For the study, published online in 2012 in The Lancet, researchers used data from the National Joint Registry of England and Wales the world s largest joint replacement registry to track more than 400,000 patients who underwent primary hip replacement from 2003 to 2011. Of these, 31,171 received metal-on-metal implants hips with a ball and cup made of a cobalt and chromium alloy.

    Compared with implants made of other materials, all-metal joints had a substantially higher overall failure rate. After five years, 6.2 percent of metal-on-metal hips had failed, whereas only 3.2 percent of ceramic hips and 1.7 percent of metal-on-plastic implants had. Failure rates were highest for younger women and for implants with larger heads 36 millimeters or more in both men and women. The head is the ball of the device that fits into the hip socket.

    Joel Buchalter, MD, a clinical assistant professor of orthopaedic surgery at New York University Medical Center, explains that manufacturers developed metal-on-metal implants with larger heads to prevent the problems that lead to early failure.

    Hip replacements can fail for several reasons, but the most common is loosening, in which components detach from the bone. Friction from normal wear produces debris that causes inflammation in the tissues around the joint. Over time, bone erodes and the implant loosens, leading to pain and decreased function. Implants can also dislocate meaning the metal ball slips out of the socket often within the first few weeks after hip replacement. Failed hips must be removed and replaced in a complex revision surgery. This operation is often less successful than the original hip replacement because bone loss makes the new implant harder to anchor.

    Dr. Buchalter says the metal-on-metal devices were targeted at active younger adults who needed a hip that lasted decades. In mechanical models, metal-on-metal implants with large-diameter heads showed extremely low wear and better lubrication, so they seemed an excellent solution, he says.

    The Lancet study authors thought so, too, until they looked at the data.

    We were surprised by the results, as studies suggest that larger heads should have lower wear and a lower risk of dislocation. However, we have seen an increase in revisions for pain and loosening, particularly in women, says Alison Smith, a study author and statistician at the University of Bristol in the U.K.

    Depending on patient age and implant head size, the revision rate for all-metal hips is three to four times higher in women and two to three times higher in men compared with other implants.

    Ashley Blom, MD, a study author and professor of orthopaedic surgery at the University of Bristol, suggests that hip anatomy or the presence of osteoporosis (which is more common in women) might account for the greater number of problems in women. He adds that several small studies show women also are more sensitive to the metal ions shed by the implants.

    Why all-metal hips fail more often still isn t entirely clear, and the British researchers conclude that more research is needed. We do not fully understand the mechanism of failure of these hips, they say. We need more studies to shed some light on this.

    In 2010, device maker DePuy recalled its articular surface replacement hips, or ASR hips, after they failed in about 12 percent of patients who received them. The recalled ASR devices were excluded from the current study.

    Metal-on-metal implants have raised other concerns in addition to early failure rates, including potential harm from cobalt and chromium ions released into the bloodstream when metal parts rub together. In February, the journal BMJ published results of a comprehensive investigation of metal implants. Among other findings, investigators concluded that systemic metal ions can cause cardiovascular problems and may damage DNA.

    However, Art Sedrakyan, MD, an associate professor at Weill Cornell Medical College in New York, points out that no good studies of the short- and long-term effects of ions exist. We don t know what the consequences of even moderately elevated ion levels might be over time, and there is no clear evidence that they lead to cancer or cardiomyopathy or other complications. We just don t know, he says.

    Dr. Sedrakyan, an authority on comparative effectiveness research, adds that in May 2011, the FDA ordered 21 makers of metal-on-metal hip implants to provide safety studies, including information on metal ion levels. Once that data is available and independent experts can analyze it, we can make better decisions, he says.

    David Lewallen, MD, an orthopaedic surgeon at Mayo Clinic in Rochester, Minn. and board chairman of the newly launched American Joint Replacement Registry (a national center for data collection and research on total hip and knee replacements), says it s also important to put current findings in context.

    Hip replacement is one of the most successful surgeries in the history of medicine, and the improvements it makes in the lives of patients are dramatic, he says. The majority of people, including those with all-metal hips, do extremely well, he adds.

    Still, many physicians and leading orthopaedic institutions, such as Rush University Medical Center in Chicago and the Mayo Clinic, have halted or scaled back use of all-metal implants.

    But what about the 500,000 Americans who already have metal hips? Unlike Britain, no formal guidelines or protocols exist in the United States for dealing with metal-on-metal implants, so doctors are developing their own.

    Dr. Lewallen urges all hip replacement patients to see their doctors at regular intervals, even if they have no symptoms. And he recommends surveillance of metal ion levels for patients with metal-on-metal implants, especially if they have kidney disease or other health problems. Ultimately, he says, care should be individualized to the needs of each patient.

    Nicolas Noiseux, MD, an assistant professor of orthopaedics at the University of Iowa in Iowa City, also endorses a patient-specific approach.

    I see patients who have large-head metal implants once a year, and at the slightest complaint, I suggest testing for blood cobalt and chromium. If the numbers are seven parts per billion or above, I follow patients more closely than a yearly interval and may recommend further imaging, such as an MRI, to assess fluid accumulation around the joint. And certainly in that case, or in the face of metal ions above seven parts per billion, some patients have elected to have [the implant removed], he explains.

    Dr. Buchalter endorses a similar program for patients with metal hips, but believes annual monitoring for the life of the implant now required in Britain is essential. He says regular blood tests and MRIs can alert doctors and patients to impending implant failure.

    Dr. Sedrakyan agrees that all options should be discussed in order to decide the best course for each patient. But, he says, as more information accumulates, we will be able to frame these recommendations based on better evidence.

    As for the future, Dr. Lewallen says metal hips may even have a silver lining. Patients are now aware that being seen regularly is crucial, and that awareness may help prevent more serious problems, he says.

    He adds that concerns about metal implants have helped increase interest in an American Joint Replacement Registry. Until recently, the United States lacked a large, nationwide registry to track long-term outcomes of joint replacments. Dr. Lewallen says that by uncovering problems sooner, registry data should improve patient safety and overall quality of care.

    SSO (Single Sign-On) Authentication on RDS

    Single Sign-On (SSO) is the technology that allows an authenticated (signed on) user to access other services without re-authentication. Applied to the Remote Desktop Service, SSO allows a user logged on to the domain computer not to reenter account name and password when connecting to RDS servers or launch published RemoteApp applications.

    In this article, we ll describe the peculiarities of configuring the transparent SSO (Single Sign-On) authentication on RDS servers running Windows Server 2012 R2.

    • The Connection Broker server and all RDS servers must be running Windows Server 2012
    • SSO works only in the domain environment: Active Directory user accounts must be used, and servers together with the workstations must be included in the domain
    • RDP 8.0 or later has to be used
    • Windows 7/8/8.1 on the client side
    • SSO works with password authentication (smart cards are not supported)

    The procedure of Single Sign-On configuration consists of the following steps:

    • SSL certificate has to be issued and assigned on RD Gateway, RD Web and RD Connection Broker servers
    • Web SSO has to be enabled on RDWeb server
    • The group policy for credentials delegation has to be configured
    • The certificate thumbprint has to be added to .rdp trusted publishers using GPO

    Firstly, you have to issue and assign an SSL certificate (the EKU of the certificate must contain Server Authentication). We won t describe the procedure of getting the certificate since it goes beyond the scope of this article.

    The certificate is assigned in the Certificates section of RDS Deployment properties.

    Then you have to enable “Windows Authentication” on all servers with Web Access role for IIS RDWeb directory and disable Anonymous Authentication.

    After you save the changes, restart IIS:

    If you are using RD Gateway, make sure that it is not used for connection of the internal clients. (Bypass RD Gateway server for local address has to be checked.)

    The next step is the configuration of the credentials delegation policy. This policy is located in Computer Configuration – Policies – Administrative Templates – System – Credential Delegation – Allow delegation defaults credential. The policy allows certain servers to access the credentials of Windows users.

    • The policy has to be enabled (Enabled)
    • You have to add the names of RDS servers to the list of servers, on which the authentication takes place. The format of adding a server is as follows: TERMSRV/rd.contoso.com. If you have to give this permission to all terminal systems in the domain (it is less safe, though), you can use this construction: TERMSRV/*.contoso.com Authentication single sign on

    Then, to prevent a window warning of the remote application publisher being untrusted to appear, add the address of the server with the Connection Broker role to the trusted zone on the client computers using GPO:

    User/Computer Configuration – Administrative Tools – Windows Components – Internet Explorer – Internet Control Panel – Security Page- Site to Zone assignment list

    Specify FQDN server name RDCB and Zone 2 (Trusted sites)

    Then enable Logon options policy in User/Computer Configuration – Administrative Tools – Windows Components – Internet Explorer – Internet Control Panel – Security – Trusted Sites Zone and in the dropdown list select Automatic logon with current username and password.

    At last, you have to get the certificate thumbprint and add it to the list of trusted rdp publishers. To do it, run the following PowerShell command on your RDS Connection Broker server:

    Copy the value of the certificate thumbprint and add it to the list of thumbprints in the policy Specify SHA1 thumbprints of certificates representing RDP publishers (Computer Configuration – Administrative Templates – Windows Desktop Services – Remote Desktop Connection Client).

    Now the SSO configuration is over, and after the policies have been applied, the user can connect to the RDS farm using RDP without re-entering password.

    If you have a credit card, you have to worry — at least a little — about credit card fraud. Have you ever wondered, though, if fraudsters ever actually get caught? If so, what kind of consequences do they face?

    States make the call

    For the most part, the statutes governing credit card fraud are determined by individual states. Those statutes differ from one state to the next.

    In California, the Penal Code is specific and dense to read. The most common crimes referred to are:

    • Using someone else s credit or debit card without that person s consent.
    • Using your own credit or debit card knowing that it has been revoked or expired or that your available balance is less than the purchase price.
    • Using a stolen or fraudulent credit or debit card to receive money, goods or services.

    Crime and punishment

    The test is really “fraudulent use,” so don’t worry if you accidentally use a card you canceled. There has to be intent, and other thresholds have to be met. One such threshold that goes beyond “fraudulent intent” is “to obtain a benefit,” often more than $1,000.

    Here we’re talking about forgery, alteration, stealing, counterfeiting and even publishing someone else’s information. What a thief actually does will determine if the charge is petty or grand theft, or forgery. The consequences (as in any criminal case) will also depend on the circumstances and the thief’s criminal history.

    In general, however, both grand theft and forgery are felonies, subjecting the perpetrator to 16 months or two or three years in California state prison and a maximum $10,000 fine, or as a misdemeanor to up to one year in a county jail and a maximum $1,000 fine. (Petty theft is a misdemeanor, punishable by up to six-months in a county jail and a maximum $1,000 fine).

    Texas is different. The statutes are pretty specific as to what is illegal and is punishable by what Texas calls a “state jail felony.” It sounds bad, but it s actually the least serious felony in Texas law. Here, a thief gets a minimum of six months in jail but no more than two years, and a fine not to exceed $10,000.

    Federal penalties dwarf states’

    Oh, but we aren’t done yet. There are federal violations that often are attached to credit card fraud. These appear under Title 18 USC 1029. If a thief pulls his stunt on government property across state lines, or against a government entity, then federal charges may apply. The consequences there are much worse – as in, 10 to 20 years in prison.

    Answer: You have to install R 05010001 Glow Tubes. These are the only ones that will operate and lit properly on 4.16kV line to line voltage.

    Question #2: Our switchgear is on 13.2kV. Shall we use R or RS style Glow Tubes on outgoing feeder cables?

    Answer: Either will work since R 05010001 Glow Tubes operate on 4kV+ line to line voltage and RS 05010002 Glow Tubes operate on 10kV+ line to line voltage. However, since RS 05010002 Glow Tubes have distinctive spiral light we will recommend it. Please refer to photo below.

    Question #3: Our switchgear is 50 years old. Our Glow Tubes are hold by lousy pieces of bent metal. What is your suggestion?

    We performed our cost-benefit analysis on a hypothetical single doctor medical office using industry averages from various sources. Our hypothetical offices sees 30 patient per day, from a patient base of 2500 patients. We will use an assistant to physician ratio of 3 to 1 and our office receives 100 phone calls per day of various nature.

    Costs

    Software License – EMR license prices range from 1k-25k. Where an average license for a FULL/TRUE EMR is $10,000 and a Light EMR is $1,000.
    Implementation Implementation costs are usually billed hourly at a rate of $75-$150 per hour. Average implementation time per provider is 35 hours. Where 10 hours are used for customization, 25 hours for training and 10 hours for computer/network setup. This becomes exponentially lower as more physicians are added. For our example we will use an hourly rate of $100 per hour.
    Hardware Where most physicians have a 3 to 1 assistant ratio, we ve suggested 1 Tablet PC for the provider, 3 workstations for the assistants and a Server. Tablet PC = $2,500, Workstation = $1,000, Server = $2,000.
    Support Maintenance Ongoing support costs will be incurred from both an annual support contract with the software vendor for updates and technical support and the increased need of hardware/network support through a local IT representative.

    Improved Coding Where down-coding and poor charge capture can both be improved through an EMR s E M Coder. A study by Medical Economics magazine estimated that a physician who is regularly down-coding may be losing as much as $40,000 to $50,000 annually. A study done by Partners Healthcare System found an increase of 1.5%-5% in overall billing simply through improved charge capture. We offer a disciplined and structured environment that emphasizes character development, spiritual growth and personal responsibility. Our non-profit therapeutic program for girls is designed to develop the whole person in four main areas: spiritual, social, academic and physical. For over 45 years, CGA has shown that a personal relationship with Jesus Christ changes people’s lives. To that end, students participate in chapel services, local youth group, and in biblical studies. We surround our students all day long with a Christian environment to help facilitate life change through the witness and guidance of our staff and counselors.
