Hospice is here to help educate you and your family about advance directives and advance care planning. We can also help you take the right steps toward having difficult conversations with your loved ones and making your healthcare wishes known.

A Health Care Proxy will ensure your wishes are honored if and when you are unable to make decisions for yourself.

Your decision to accept or reject medical treatment, including life-sustaining procedures, ultimately depends upon your personal wishes, values, and beliefs. Using an Advance Directive form, such as a Health Care Proxy, will help your family and friends understand your wishes and will give both you and them peace of mind.

Everyone over the age of 18 needs to appoint a Health Care Proxy* for these two situations:

1. Temporary inability to make healthcare decisions can happen no matter what your age. That’s because unexpected events can happen at any time. And, during those events there are often times when a healthcare decision needs to be made, even if you’re not able to voice those decisions for yourself. (For example, when having an outpatient surgical procedure and are under general anesthesia.) If you have a Health Care Proxy, decisions will be made according to your wishes — even if you are temporarily unable to make your own requests. Once you become conscious, the Health Care Proxy no longer has any authority to act.

2. Permanent inability to make healthcare decisions. This would arise if you were comatose from a terminal illness, in a persistent vegetative state, suffered from an illness that left you unable to communicate or, if elderly, suffered from senile dementia or Alzheimer’s disease.

If you don’t have a Health Care Proxy, all appropriate medical treatments would be provided to you. If you’ve appointed a Health Care Proxy, that person would represent your voice and act on your behalf in making healthcare decisions for you according to your pre-determined wishes or best interests.

*The New York Health Care Proxy Law allows you to appoint someone you trust — such as a family member or close friend — to make healthcare decisions for you if you lose the ability to make decisions yourself. Appointing a health care agent and discussing your wishes with them helps ensure that healthcare providers follow your requests.

These diets contain more than the government’s recommendation that between 10 to 35 percent of daily calories come from protein.

The aim: Weight loss.

The claim: You’ll lose up to 10 pounds within the first week, and continue at a clip of 2 to 4 pounds a week until you reach your goal weight – all while eating as much as you want (of the approved foods, of course). If you follow the rules to the letter, you’ll never regain.

The theory: Counting calories isn’t the key to weight loss; protein is. It’s a weight-loss powerhouse – it’s filling, takes time and work to digest, and has very few calories for each gram of food compared to carb-heavy foods. When protein supplies the majority of a diet. and fats and carbs are all but squeezed out, fast weight loss ensues. (Limiting carbs, the body’s preferred energy source, forces the body to turn to an alternative fuel – stored fat.) That’s motivation enough for dieters to follow a strict plan that rewards the faithful by slowly adding back the bread, cheese and fruit they so dearly missed.

Dos Don’ts

Prepare for lots of rules. All four phases of the Dukan Diet – named for former French physician Pierre Dukan, its creator – are heavy on do’s and don’ts, and even the slightest slip-up is considered destructive. You’ll move from the all-you-can-eat, pure protein “Attack” phase to “Cruise,” which allows selected vegetables on selected days. In the third phase, “Consolidation,” you’ll add more foods that, by now, you’re longing for, such as cheese and bread. By the last phase, “Permanent Stabilization,” you’re relatively free. With just a few (very crucial and very specific) parting rules, you’re permitted to eat anything you want.

Attack: A gourmand’s dream diet, this phase is all-you-can-eat, portion-wise. Any of the following is fair game: lean beef, veal, pork and venison; organ meat including liver and tongue; fish; shellfish; poultry; low-fat ham, turkey and chicken; eggs; vegetable protein such as tofu and seitan; nonfat dairy; water and other no-calorie drinks (yes, even diet soda); and 1 1/2 tablespoons of oat bran. Spices and herbs are encouraged to avoid monotony. Craving something else? Too bad – cheating is expressly forbidden. “[S]uccumbing to any other foods, as small as the lapse may be, will be like puncturing a balloon with a needle,” warns Dukan.

The Attack phase lasts between one to 10 days, depending on how much weight you have to lose. For the majority of dieters – those with 20 to 40 pounds to shed – Attack is typically five days and weight loss is usually 4 to 7 pounds, according to Dukan.

Cruise: In this phase, you’ll add back certain all-you-can-eat vegetables (the nonstarchy ones, such as cucumbers, mushrooms, zucchini, peppers and salad greens). Still, Dukan recommends alternating one day of the “pure-protein” Attack phase with one day of protein plus vegetables. (A five-day protein then five-day protein-plus-vegetables plan is also an option). The oat bran recommendation rises slightly to 2 tablespoons.

You should lose at a rate of about 2 to 4 pounds per week, according to Dukan. You’ll continue alternating until you reach your goal weight.

Consolidation: Now, the name of the game is keeping the pounds off. You’re very vulnerable to weight regain, says Dukan, and the only option is a transition phase that lasts five days for every pound you’ve lost. That means dieters who’ve lost 20 to 40 pounds will stay in Consolidation between 100 to 200 days. The proteins and vegetables from the previous phases are still all-you-can-eat, but you can now mix them together as you please. Each day, you also get a serving of fruit, two slices of whole-grain bread and 1 1/2 ounces of cheese. Each week, you’re also allowed two servings of starchy foods like pasta or quinoa, three specific protein indulgences (leg of lamb, roast pork and cooked ham), and one or two “celebration meals” where you can eat anything you want, provided you don’t binge or have them back-to-back. The daily 2-tablespoon oat bran requirement stays.

You’re still required to revert to the “pure-protein” Attack phase once per week.

Permanent Stabilization: Freedom! (Well, sort of.) You can now eat whatever you want six days out of the week, provided you don’t abandon what you learned during Consolidation. And 3 daily tablespoons of oat bran is now a must. The seventh day is a purest-of-pure protein day that mirrors the Attack phase but restricts even more the acceptable foods list. As its name suggests, this phase is meant to last a lifetime.

In 2014, diet creator Pierre Dukan was removed from the French medical register for promoting the plan commercially, as reported by the Guardian.

How much does it cost?

Animal protein, vegetables and dairy products are pricey, especially if you’re eating a lot of them – which you will be.

You can buy books like “The Dukan Diet” and “The Dukan Diet Made Easy.” Plus, coaching is available for $29.95 per month.

Will you lose weight?

It’s hard to say. No clinical trials have evaluated the Dukan Diet. An online survey of 1,525 people in 2010 reported that dieters lost, on average, 15 7/10 pounds after the first two phases.

How easy is it to follow?

How much do you like rules? If you want to be told exactly what to do – no matter the difficulty of the request – you’ll appreciate the Dukan Diet’s many guidelines. And while the restrictive phases are short, they are very restrictive, so don’t underestimate the willpower you’ll need.

Convenience: Recipes are abundant and simple. Eating out is allowed, but alcohol is banned during early phases of the diet. Some online support is offered. And ShopDukanDiet.com offers resources such as packaged food and supplements. An 8-ounce serving of Goji berries, for example, is $16.99.

Eating out: No problem, as long as you order from your approved-foods list. If you’re in Attack, try steak with a side of shrimp. During Cruise, throw in some steamed veggies. Just make sure the chef doesn’t add any oil – or worse – butter. If you’re tempted by dessert, Dukan suggests ordering coffee or keeping a yogurt on hand to end your meal on a sweet note.

Alcohol: Off-limits until you reach the Consolidation phase when you can have a glass of wine or bottle of beer with your celebration meal.

Timesavers: None, unless you hire somebody to plan your meals, shop for them and prepare them.

Extras: Online forums and live chats connect you with other dieters and Dukan nutritionists. And for a fee, which varies based on your plan, you can get a personalized program and online weight-loss coaching. Prepackaged Dukan food that conforms to the diet is available online.

Fullness: Nutrition experts emphasize the importance of satiety, the satisfied feeling that you’ve had enough. You won’t go hungry on Dukan. Protein is filling, and you don’t have a calorie limit.

Taste: You’re making everything, so if something doesn’t taste good, you know who to blame. In the absence of many traditional condiments and oils, you’ll want to make full use of the approved herbs and spices to enliven your meals.

Health Nutrition

If you want a healthy diet, look elsewhere, our panel concluded. The high amount of protein could lead to long-term health problems, and shutting out entire food groups poses a risk of nutritional deficiencies. Possible side effects include lethargy, bad breath and constipation.

What is the role of exercise?

It’s required. Brisk walking is Dukan’s exercise of choice – 20 minutes a day in Attack, 30 to 60 minutes in Cruise, 25 minutes in Consolidation and 20 minutes in Permanent Stabilization. Dukan also offers instruction on toning your stomach, thighs, arms and butt.

    The plan can choose not to cover the costs of services that aren’t medically necessary under Medicare. If you’re not sure whether a service is covered, check with your provider before you get the service.

    Medicare Advantage Plans may offer extra coverage, like vision, hearing, dental, and/or health and wellness programs. Most include Medicare prescription drug coverage (Part D). In addition to your Part B premium, you usually pay a monthly premium for the Medicare Advantage Plan.

    BENCHMARK PASSAGES & RUNNING RECORDS

    Find students’ instructional levels by assessing their reading skills with developmentally appropriate texts while recording reading behavior. Benchmark Passages are short text selections that are one part of a three-part process to help place students at their instructional levels for leveled reading sessions and to assess their readiness to progress to the next level.

    Why Benchmark Passages

    Benchmark Passages assess comprehension and reward students’ progress from level to level. They are one part of a three-part process that provides a more complete assessment of reading behavior and comprehension than any of the parts independently.

    • Each level has 2 fiction and 2 nonfiction passages.
    • Each level has at least 1 fiction-nonfiction passage pair on the same topic.
    • Each passage uses a level-appropriate percentage of words from leveled books at that reading level.
      • aa-E = 100%
      • F-J = 95% + 5% new words
      • K-Z = 90% + 10% new words
    • The text of the entire passage is used in the Running Record.
    • Most are one page long, but upper level passages can be two pages.
    • ALL are available on Raz-Plus .

    How to Use Benchmark Passages

    1. Give a student a Benchmark Passage he or she has never seen before to read aloud. If you prefer to use familiar text, use the fiction-nonfiction topic pair at each level. Use one passage from the pair to support a student’s understanding of the topic before assessing with the other passage.
    2. Record the student’s reading behavior using the passage’s Running Record form.
    3. If a student scores 90 percent, assess the student’s comprehension using a Quick Check from Level A-Z and Retelling Rubrics .
    4. If a student scores from 90%-94% percent on the running record and answers comprehension questions at 80%-100%, he or she is at an instructional level. (For more details, see About Running Records ).
    5. Use Benchmark WOWzers to reward students’ progress from level to level.

    If there is one mistake small town businesses make more often than any other it’s, What ever is left over, we’ll use for advertising.

    Marketing and advertising is an investment, not an expense.

    I know it sure seems like an expense to me when I’m writing the check, but trust me it’s not.

    Without enough money put aside for advertising your sales can go down and you suddenly have less and less for promotion.

    When do you advertise the most? For most businesses it’s the first day of business.

    Don’t you have a Grand Opening, balloons, flyers, ads, on-site radio stations, contests, and prizes?

    Did the income from sales pay for that? No, it didn’t. You advertise most when you need business. You advertise more when you don’t.

    An average cost of advertising is usually 1 to 5% of gross sales, which can vary according to location, local advertising rates, and industry. Car dealers need more advertising than funeral homes.

    Before we get to the 25 tips let’s look at the basic strategies of successful advertising.

    * In order to be successful, your advertising must provide a consumer benefit or solve a problem.
    * That benefit or solution must be wanted by the consumer.
    * The product or service you are offering must be tied directly to that benefit or solution.
    * The benefit or solution must be distinctly communicated through medial advertising. In other words, be clear, forget the advertising glitz and make sure the message isn’t lost in the ad.

    A small-budget advertiser doesn’t have the ”deep pockets to develop big advertising campaigns. Some time you need to break the rules to be noticed.

    Avis did it by admitting they were Number 2 in the car rental business and that campaign took them from 6th place to second place.

    When they stopped that campaign they dropped back to 6th again. In the past year they have gone back to it.

    Budget conscious advertisers must achieve top results for their advertising dollar. Expand your dollars by adopting some creative techniques.

    Here’s 25 tips I hope will help you.

    1. Radio, newspapers and magazine specialists will frequently give free help in developing an advertising strategy. Things like demographic information, money-saving ways to produce your ads etc.
  • Place your ads in off hours or in unusual locations for less. Many times you can still reach your target market with these spots.
  • Instead of a one-time big splash ad, be consistent with frequent small ads that work.
  • Monthly magazines sometimes have unsold ad space at the end of the month they will sell at a discount.
  • If you have an 800 number, put it in every ad for immediate response and feedback.
  • Try advertising consistently in the classifieds. These ads may draw more customers than more expensive display ads.
  • Can you barter for the cost of ad production? Maybe the newspaper needs painting in exchange for an ad about your paint store.
  • Piggyback advertising are the ads you receive with your Mastercard bill. Is there someone in your town that sends out a lot of bills? Can you put a small flyer in with their bills and split the postage? Or pay a small fee?
  • Split advertising costs with the people who sell to you. Vendors and manufactures are always looking for exposure. Let people know you carry their products and have the vendor pick up part of the ad cost.
  • Are there up front advertising discounts for cash?
  • Consider advertising in regional issues of national magazines. The costs are lower and you can still reach your target market. TV Guide is a good choice. It stays around for at least a week. Time, Newsweek, and US News and World Report may stay in local doctors offices for years.
  • Share ad costs with neighbor business. Video stores and Pizza parlors are natural partners. Have coupons to each others stores or share the cost of flyers.
  • Try reducing the size of your ad (not in the Yellow Pages) or length of your radio spots. A 60 second spot is not twice as much as a 30 second spot but you won’t get twice as many customers for a 60 over a 30. Going with small ads or shorter spots will allow you to do more ads which normally pulls more customers. It’s better to be there every day with small ads than every month with one big one.
  • Develop tight production controls to minimize the need to reject finished ads. The message is more important than the messenger. Don’t try to produce ads that win awards, produce ads that sell.
  • Who are your very best customers? Aim your ads to talk directly to people like them.
  • What will suppliers give you in the way of point-of-purchase materials. Posters, stand ups, handouts, etc. Some have excellent display racks you can use.
  • Some national chains like Coke and Pepsi provide outdoor signs for businesses. There are also indoor lighted signs you write on with special markers to advertise your special offers.
  • Can you sponsor a community event? A fun-run, golf tournament, or other event that will be well publicized in the community. Your name may not be prominently displayed but sometimes the positive exposure in the community will bring in new customers.
  • Small businesses can seldom afford saturation advertising. You must be selective in the media that reaches your customers. Pin your ad reps down and make them show you exactly how their media reaches your target audience.
  • Exploit the media you choose to the fullest. If your message is verbal, you don’t need TV. Use radio, billboards and newspapers to the fullest.
  • Consider direct mail. A letter and brochure before customer contact can increase business. An IBM study concluded that selling time can be reduced from 9.3 to 1.3 total hours with direct mail advertising. A Sales and Marketing Executives International Study showed salespeople went from eight orders per 100 cold calls to 38 orders per 100 when direct mail was used.
  • Try an editorial style ad. These are ads that look like actual stories in the newspaper. They will have advertisement at the top of the article. Develop a good headline, and 50% more people will read the article than would read an ad of the same size.
  • You can’t match larger competitors dollar-for-dollar but, you can use unusual approaches (like the Avis idea above), color, music, slogans, humor (be careful here), or media selection to win your market away from the big guys.
  • Due to the high costs of conventional advertising on, radio, TV, newspapers, many cost conscious business have been forced to look for lower cost methods. Can you advertise on parking meters, taxi boards, balloons, blimps, and grocery shopping carts. Community bulletin boards, movie ads, and weekly newspaper shoppers.
  • Key your ads. Put something in the ad that will let you know which media it came from. On coupons, put a code that will record the paper and date of the ad. In radio or TV, have them mention the ad to get the discount. Ask every customer how they found you.
  • Plan for a rainy day. During the year put a small amount aside each month for emergencies. You never know when you’ll need to react quickly to whatever the competition is doing. You must be able to capitalize on breaking national events or news regarding your industry. If negative things happen in your industry you may need to respond quickly to make sure the right message is presented.
  • Always give the customer more than you promised and more than they expected. This is tip number 27 of the 25 we advertised. Maybe this last one is the one you needed.
  • I hope these tips will help your business grow. Not all may be relevant to your particular situation. Hopefully, they will illustrate the importance to plan and control your advertising budget.

    Medicare Advantage Plans must cover all of the services that Original Medicare covers except hospice care. Original Medicare covers hospice care even if you’re in a Medicare Advantage Plan. In all types of Medicare Advantage Plans, you’re always covered for emergency and urgently needed care .

    The plan can choose not to cover the costs of services that aren’t medically necessary under Medicare. If you’re not sure whether a service is covered, check with your provider before you get the service.

    Medicare Advantage Plans may offer extra coverage, like vision, hearing, dental, and/or health and wellness programs. Most include Medicare prescription drug coverage (Part D). In addition to your Part B premium, you usually pay a monthly premium for the Medicare Advantage Plan.

    In 2016, most people pay the Part B premium of $104.90 each month.

    If you need a service that the plan says isn’t medically necessary, you may have to pay all the costs of the service, but you have the right to appeal the decision.

    You can also ask the plan for a written advance coverage decision to make sure a service is medically necessary and will be covered.     What’s a care plan in a nursing home?

    The nursing home staff will get your health information and review your health condition to prepare your care plan. You (if you’re able), your family (with your permission), or someone acting on your behalf has the right to take part in planning your care with the nursing home staff.

    The basic care plan includes:

    • A health assessment (a review of your health condition) that begins on the day you’re admitted, and must be completed within 14 days of admission
    • A health assessment at least every 90 days after your first review, and possibly more often if your medical status changes
    • Ongoing, regular assessments of your condition to see if your health status has changed, with adjustments to your care plan as needed

    Nursing homes are required to submit this information to the federal government. This information is used for quality measures, nursing home payment, and state inspections.

    Depending on your needs, your care plan may include:

    • What kind of personal or health care services you need
    • What type of staff should give you these services
    • How often you need the services
    • What kind of equipment or supplies you need (like a wheelchair or feeding tube)
    • What kind of diet you need (if you need a special one) and your food preferences
    • Your health and personal goals
    • How your care plan will help you reach your goals
    • Information on whether you plan on returning to the community and, if so, a plan to assist you in meeting that goal
