    CORE-AT Electronic Medical Record and
    Injury Surveillance System

The CORE-AT electronic medical record (EMR) system is a web-based injury surveillance and EMR system for use by athletic trainers. The CORE-AT EMR was designed and developed by athletic trainers in partnership with Essentialtalk. an international technology communication company.

The CORE-AT EMR is compliant with the data acquisition, storage, and transmission standards set forth by the Health Insurance Portability and Accountability Act (HIPAA).

Health information obtained using the CORE-AT system are not stored locally on computers. Instead, data is uploaded in real-time and transmitted to secure, redundant servers.

    System Capabilities

The CORE-AT EMR system consists of documentation capabilities including individual patient/athlete registration (e.g. sex, age, grade), injury demographics (e.g. sport, season, position), comprehensive patient evaluation (e.g. diagnosis, injured body part, side, mechanism of injury), and daily treatment, progress, and discharge (e.g. athletic training interventions, rehabilitation protocols, date of return-to-play) notes.

A necessary feature of the patient record is the ability to assign ICD-9 CM and CPT codes to patient diagnoses and treatments, respectively, to enable the characterization of secondary school athletic training practice and for creating cost-estimates for secondary school athletic training services.

Also included in the system are single and multi-item patient-oriented outcome measures, both generic and region specific. Outcome instruments are automatically generated and included as part of the patient evaluation process based on diagnosis and injury region. An automated system not only assures collection of appropriate patient information but also promotes the use of outcome instruments as part of standard athletic training practice.

    Injury Surveillance

In addition to the capabilities of the CORE-AT system to collect patient outcomes, CORE-AT is also constructed to capture and integrate injury surveillance information specific to athlete injury and exposure (e.g. incidence, athlete-exposure, time loss/non time loss, sports participation).

With the surveillance component integrated directly into the medical record, information such as time loss/non-time loss, diagnosis, mechanism of injury, sport, and position are all collected and recorded during the initial evaluation, without the need for double entry. Data regarding athlete exposures are also captured and made available to the clinician.

    Additional Information

If you are interested in seeing the CORE-AT EMR in action, request a guest account for trial access to the system.

Login to the CORE-AT Electronic Medical Record and Injury Surveillance System.

    MCAT Test Scores
    and Medical School Requirements

New MCAT scoring and GPAs required by US medical schools and Canadian medical schools are provided in this website. To guide you in your journey to medical school, this page will provide you with helpful information such as new MCAT test scores range (good, high and highest scores), scores percentiles, your chance of medical school admission, and advice on how to attain a high MCAT score.

How the new MCAT is scored

The MCAT (Medical College Admission Test) is a prerequisite for admission to nearly all the medical schools in North America. This is administered by the Association of American Medical Colleges (AAMC) in hundreds of testing centers in the United States, Canada and around the world.

In 2015, over 64,000 premed applicants to American and Canadian medical schools submitted their MCAT test results. While the actual weight given to MCAT scores in the admissions process varies from school to school, often they are regarded in a similar manner to your premed GPA (i.e. your academic standing).

The MCAT has four sections, namely; (1) Biological and Biochemical Foundations of Living Systems; (2) Chemical and Physical Foundations of Biological Systems; (3) Psychological, Social, and Biological Foundations of Behavior; and (4) Critical Analysis and Reasoning Skills.

Each section is scored from a low of 118 to a high of 132 while the midpoint is 125. The total score results from a combination of the four sections’ scores. Total score range is from 472 to 528, with a midpoint of 500.

The MCAT score report that you will receive would reflect the scores’ percentiles, confidence bands that show your true score range, and score profiles that reflect your strengths and weaknesses. For your guide on new MCAT scores, click here: MCAT Score Report .

MCAT scores are released 30 to 35 days after the exam. Click here: MCAT Score Reporting System

    Average MCAT Test Scores

To look at average MCAT scores and GPA, click here: US Medical Schools MCAT Scores and Canadian Medical Schools MCAT Scores

Note: ‘Average MCAT score’ refers to the average of students who were accepted to medical school. Apart from your MCAT score, your GPA and non-academic factors like your personal statement and/or autobiographical material, letters of reference, and the medical school interview influence your chance of getting accepted to medical school.

    * The percentage of students whose scores were lower than yours. Percentile ranks are not used as criteria for medical school admissions. The scaled score is what matters for medical school admissions. Every year, the AAMC releases MCAT score percentiles which gives exam takers an idea on how they fared compared to others who took the exam. For MCAT scores percentiles for past MCAT test dates, click here: MCAT Research and Data

Old and New MCAT Scores Accepted by Medical Schools

MCAT scores that date back two or three years are accepted by most medical schools. Although the MCAT has changed in 2015, some medical schools will accept the old MCAT scores until the 2018 application cycle, while some require the new MCAT score for the 2017 application cycle. For a list of medical schools and their acceptance policy, click here: MCAT exam policy .

Doctor of Nursing Practice, Nurse Anesthesia (BSN-DNP)

    DNP Purpose statement

The Doctor of Nursing Practice (DNP) is a practice-oriented doctor that prepares graduates with the knowledge and skills to translate research into practice and improve patient outcomes across populations. In addition, the BSN-DNP Nurse Anesthesia Program is designed to prepare the next generation of nurse anesthetists with the essential skills to serve society as compassionate clinicians, leaders, and educators.

A CRNA is an advanced practice registered nurse that has received at least a Master’s degree related to the practice of nurse anesthesia. According to the American Association of Nurse Anesthetists, CRNAs safely deliver approximately 40 million anesthetics in the U.S. each year for patients undergoing surgical and obstetrical procedures, pain management, and trauma services. CRNAs are often the sole anesthesia provider in rural hospitals and the main provider of anesthesia for the U.S. Armed Forces. CRNA’s administer anesthesia in a variety of settings including hospitals (urban and rural), outpatient ambulatory centers, physician offices, military installations, and pain management clinics.

    Vocational Growth

In three years, you can earn a Doctor of Nurse Practice (DNP) with a specialization in nurse anesthesia (CRNA). The program’s full-time, on-campus enrollment provides the most efficient path to achieve your personal and vocational goals. You want to be a leader. The University of Saint Francis will get you there.

    Reputation

Employers recognize and trust the skills of University of Saint Francis nursing graduates. Our university is nationally recognized for academic excellence, distinguished faculty, and exceptional graduates who lead the profession with vision and compassion. The USF Nursing Department’s commitment to Catholic Teaching and Franciscan Values emphasizes care of the whole person.

    Why Nurse Anesthesia?

Improved Skills: A DNP allows you to provide nurse anesthesia care before, during, and after surgical and diagnostic procedures. You will learn to care for your patients so anesthesia can be safely administered.

Better Care: A Doctor of Nursing Practice (DNP) prepares you to effect greater change in healthcare. DNP graduates have the knowledge and skills to deliver improved healthcare and patient outcomes. The healthcare industry’s increased emphasis on population health has increased reliance on DNPs.

Expected Growth: The National Center for Health Workforce Analysis (HRSA) projects an 87% increase in job availability by 2025, making the demand for CRNAs substantial.

It’s Required: Starting in 2025, the Council on Accreditation of Nurse Anesthesia Educational Programs will require a DNP to practice as a nurse anesthetist.

    Health Marketing Basics

    Introduction

Health marketing and communication is an emerging field that draws from traditional marketing theories and principles. Health marketing is defined as creating, communication and delivering health information and interventions using customer-centered and science based strategies to protect and promote the health of diverse populations.

    Marketing

Marketing is the process by which products are used to meet human or social needs. By acting as the link between vendors and consumers, marketing is an essential part of the exchange of goods and services. The fundamental aspects of marketing are the same whether they meet the need for a commercial product or a public health service. Therefore, health marketing is a form of traditional marketing.

For both traditional marketing and health marketing, the end user of a product or service is called a consumer and a group of consumers is referred to as a market . Characteristics of the market are considered at every stage in the marketing process, including the initial development of a product.

    Marketing Mix

Marketing mix is a term used to describe the integration of four fundamental marketing elements, which are sometimes called the four Ps :

    • Product: the item, good or service that is being provided that delivers benefits to those who consume it; includes quality, packaging, design and brand name
    • Price: monetary and non-monetary costs to the market
    • Place: channels and locations where the product can be obtained
    • Promotion: direct communication, publicity and advertising

Each of these four components should be present in a marketing plan. However, it is the science of correctly using these elements in combination with one another that provides the effective marketing mix . Essentially, to be effective a product must be tailored to customer needs, priced realistically, distributed through convenient channels, and actively promoted to customers. *

    A Traditional Marketing Example

A soft drink company creates a new product under its brand name. To efficiently market the new drink, the product is advertised in the local papers with coupons that offer a price reduction. The company distributes the new drink with their existing products so that it is available at every major grocery store in the country. Here is how this company used the marketing mix:

    • Product: new soft drink; released as part of existing brand
    • Price: reduced price for introductory period
    • Place: widely distributed to convenient locations
    • Promotion: advertisements in local papers

    A Health Marketing Example

The CDC develops a new rapid HIV testing kit that provides results in half the time of current tests. To efficiently market the new product, the testing kits are announced by the national media and medical journals. The CDC sends free samples of the new testing kits to each of the state health departments, who deliver them to local health departments, clinics and hospitals. Here is how the CDC used the marketing mix:

    • Product: new HIV testing kit; released by a credible research agency
    • Price: free for trial use
    • Place: widely and evenly distributed throughout states using state and local health departments
    • Promotion: national media publicizes to public; journals inform medical community

As demonstrated in these examples, each of the marketing mix elements must be present in the marketing process. Tailoring the elements to match the target market and using each component in coordination with one another leads to a successful marketing mix.

    Marketing and Exchange

One of the fundamental aspects of marketing is exchange. Individuals pay a price (money) for goods or services in the traditional definition of an exchange. In health marketing, the exchange often involves a non-monetary price, such as effort or time.

As this example shows, different markets can value the same exchanges differently. Because senior citizens and teenagers have varying priorities and often use the same channels for different activities, they have opposite views of what constitutes an equal exchange. Understanding the specific market for each product or service is essential to effective marketing.

    Choosing a Target Market

A critical decision in marketing is choosing the target market for the product. A target market is a specific group of people who have similar needs, preferences and behaviors. Once a target market is selected by the marketer, a marketing plan is developed to match the characteristics of the target market. There can be one or more target with corresponding marketing plans for each. Choosing a target market is a first step in making appropriate distribution and promotion decisions. It is only through an understanding of the target market that one can develop an effective marketing plan and create value for customers.

There are several important steps in choosing a target market:

    • Clearly define who the individuals are that make up the market you are analyzing. These are people who share a common need for your product, including people who may not yet be aware of that need.
    • For example, if you want to design a campaign to encourage people to get tested for diabetes, the market for the testing procedure includes mainly people who are not yet aware that they may have the disease.
    • Determine the characteristics that you will use to categorize the people within the defined market. Some common ways to categorize, or segment, people are. age, gender, ethnicity, geographical location, education level, income level, risk factors, etc.
    • Following with the diabetes example above, imagine that you want to market the diabetes testing to young adults. You may know from your research about diabetes that the disease occurs more commonly among low-income populations and individuals with lower levels of formal education. In addition, if your intervention program is only offered in one city, such as Chicago, your market is reduced further. In this example, you could segment the larger market and select a target market of 17 – 24 year old individuals who live in low-income areas of Chicago and did not finish high school.

For this example, only one target market was selected. However, many marketing efforts choose more than one target market. The decision of how many target markets to select often depends on the amount of resources you have for the project. More target markets require more resources.

    • Now that you have chosen a clear target market, it is time to better understand the individuals in this group. Through research you can determine the characteristics and behaviors of the group. There are many marketing resources that provide research information on different markets. If you select a market that does not have a large amount of existing research, you can conduct new studies through surveys, focus groups and other methods of collecting data directly from the market.
    • Some things to consider when analyzing a market include: what media they use; what they do with the leisure time (television shoes, music, websites); how they communicate with one another; what languages they speak, including slang words; what their opinions and knowledge are about the target health issue (such as diabetes in the above example); what their values are; etc.

In order to avoid stereotypes, you must use research and evidenced-based information to build your analysis. Many times the research of these markets reveals that their beliefs and behavior are much different than could be predicted.

A complete analysis of the target markets will result in choosing an effective target market and then, an informed development of marketing plan(s).

    What Dentists Are Saying

    Dr. Jan Ormsby
    Ithaca, NY

When I first came to work with Jim, I was producing approximately $48,000 per month. At the end of the first year, I was producing 114% more than when I started. The value of new patients during that first year also doubled.

When I first started, I had about 8-10 new patients a month. I was practicing alone in my home, so I had a very small practice. I knew I really had to grow the practice quickly because I moved into a new facility. With the different approaches the The Wealthy Dentist has given me, I was able to move the number of new patients up to 41 a month.

    Dr. Victor Sobrepena
    Foster City, CA

The major benefit that I have received from having been a member of the The Wealthy Dentist has been my transition from managed care insurance plans to fee-for-service patients.

Probably the biggest key that I ve seen that has allowed me to progress to where I am now has been really implementing a lot of the systems that I ve learned.

You know: making sure I have a complete team that s working for a common goal. In addition to that, I m really implementing a lot of the marketing strategies that I found were the most effective for me.

Especially when I first started, I tried a lot of different marketing techniques, and I wasn t sure whether they were going to work or not. With Jim and his team of dental management consultants, I ve really been able to copy genius instead of creating mediocrity.

Average GMAT Scores: Median GMAT Scores

What is the average GMAT score?

In recent years, the average GMAT score worldwide is 570. There is no acceptable or minimum score. What score you should obtain depends on which school you are going to apply to. Generally, the higher the school ranks, the higher score you must obtain. For the top 50 programs, the averaged GMAT score is 660.

Below is a table of median GMAT scores for some top MBA programs.

    Median GMAT Score

    Harvard MBA Program

    Wharton MBA Program

    Stanford MBA Program

    Sloan MBA Program

    Yale MBA Program

How many times can I take GMAT?

You may take the GMAT as many times as you want. But you could not take it more than once a month. The GMAT score is valid for five years. If you take GMAT more than once, the GMAC will forward the most recent three scores to school you apply to.

Considering the following factors when you are deciding whether to retake the test:

    • whether your new GMAT scores will be reported to the B-schools early enough to meet application deadlines
    • how well you think you already performed on the real GMAT relative to your realistic potential
    • whether the schools to which you are applying average your GMAT scores or consider only your highest GMAT scores

Can I cancel my GMAT scores?

During the test, you can quit the test at any time. When you quit, no score for any test section will be tabulated or recorded by the testing service, or reported to any school.

After the test, you may cancel your scores immediately after completing the test, while still in the testing room. However, once you request an unofficial score report (immediately after the test), you no longer have the option of canceling your scores.

    Damages in Medical Malpractice Cases

    Last updated: 12/18/2015

A patient injured by medical malpractice can recover a wide variety of damages — from medical bills to the loss of enjoyment of life to future earnings losses. If the medical malpractice results in the patient’s death, the patient’s family and heirs can also recover damages.

This article covers the types of damages that patients and their families can recover in medical malpractice lawsuits, as well as some of the limits that states impose on the amounts that can be recovered. (To learn more about medical malpractice claims, read Nolo’s article Medical Malpractice Basics .)

    General, Special, and Punitive Damages

To get a damage award, the patient must show that:

    • the medical malpractice caused the damages in some way, and
    • some kind of approximate price tag can be put on the damages.

The three categories of damages available in medical malpractice cases are general, special, and punitive.

General damages. General damages refer to the patient’s cost of suffering that, although real, cannot by its nature have a definite price. The most common examples are:

    • loss of enjoyment of life
    • physical and mental pain and suffering. and
    • loss of future earning capacity.

Every case is different and there are no clear rules about how the exact amount of damages is determined. To arrive at a dollar value, the patient and others will give evidence about the patient’s pain and suffering, loss of enjoyment, and so on. An expert might testify (give evidence) about the usual consequences of the patient’s injury. If the patient is relatively young and will be impaired long term, expert testimony about how to estimate the value of lost earning capacity may be necessary.

Keep in mind that general damages aren’t available for an injury existing before the malpractice, or the pain and suffering that a pre-malpractice injury, by itself, will cause in the future.

Special damages. Special damages cover the more quantifiable expenses caused by the medical malpractice, including medical bills and past missed work. Although there is often some guesswork involved, particularly when it comes to future medical expenses, special damages are typically more exact than general damages. An expert may still be useful, but in some states simply submitting a certified copy of the medical bill is good enough, depending on the facts of the case.

Punitive damages. In some circumstances, the patient may be able to recover punitive damages. The rules on when a patient may get punitive damages vary from state to state, but the general requirement is this: The doctor must have known that he or she was behaving in a harmful manner.

Example: A doctor intentionally leaves a sponge in the patient during surgery in order to create a reason for a second surgery to remove the sponge. This behavior would warrant punitive damages.

The exact amount of punitive damages is up to the judge or jury, but typically cannot be more than several times the amount of the special and general damages.

    State Limitations on Damages

Many states place a cap on the maximum amount of damages the patient can recover. Some states put a cap on all damages combined, saying a patient cannot recover more than, for example, $500,000. Others have a cap only on “general damages,” which means compensation for things like pain and suffering, loss of enjoyment of life, and the psychological impact of scarring or disfigurement. “General damages” are sometimes called non-economic damages, because they cannot usually be measured by a dollar amount.

For example, California sets a $250,000 limit on non-economic damages in medical malpractice cases. But there is no limit on economic damages, including compensation for past and future medical care, loss of past earnings, and diminished future earning capacity.

Many states also have rules that reduce the damages the doctor must pay by the amount the injured patient received from other sources like insurance. Finally, many states also have statutes that limit the amount the patient’s attorney can charge for a malpractice case.

Damages Based on the Death of the Patient

All states have laws determining what damages can be recovered if the medical malpractice results in the patient’s death. These are called survival statutes and wrongful death statutes.

Survival statutes. Survival statutes allow the deceased patient’s heirs or estate to recover damages that occurred during the time period from the initial medical malpractice to the death of the patient. These damages generally include everything allowed in a malpractice suit had the patient survived, except for damages relating to the future, like earning capacity. Some survival statutes also provide for recovery of funeral expenses, although this is usually part of the wrongful death statute.

Wrongful death statutes. Wrongful death statutes are designed to compensate the patient’s family for their future monetary loss. The calculation is more thorough than a simple projection of future salary — it also considers factors like the patient’s spending, saving, and working habits. Compensation for the family’s loss of companionship or emotional harm is typically not allowed under the wrongful death statues, although recently some states have allowed that kind of recovery. Depending on the state, not all family members can recover. For example, a state may allow the patient’s spouse and children to recover damages, but not the patient’s parents (at least in the case of an adult patient).

    Getting Help

Because medical malpractice law is highly regulated by a complex body of rules, which vary considerably from state to state, it is often essential to get advice or representation from a lawyer.

For help on choosing a good medical malpractice attorney, read Nolo’s article Finding a Personal Injury Lawyer. Or, you can go to Nolo’s Lawyer Directory for a list of personal injury attorneys in your geographical area (click on the “Types of Cases” and “Work History” tabs to learn about a particular lawyer’s experience, if any, with medical malpractice claims).

    Get the compensation you deserve.

    The Patient-Centered Medical Home (PCMH)

    What is the PCMH?

The patient-centered medical home (PCMH) is a model of care that aims to transform the delivery of comprehensive primary care to children, adolescents, and adults.

Through the medical home model, practices seek to improve the quality, effectiveness, and efficiency of the care they deliver while responding to each patient’s unique needs and preferences. No matter where you fall on the spectrum of PCMH transformation—managing current projects, enhancing basic concepts, or advancing to more complex initiatives—the adoption of PCMH concepts can benefit your practice, your patients, and your bottom line.

    Defining the PCMH

The 2007 Joint Principles of the Patient-Centered Medical Home (3 page PDF) —as developed by the American Academy of Family Physicians, the American College of Physicians, the American Academy of Pediatrics, and the American Osteopathic Association—are as follows:

    • Physician-led practice: Patients have access to a personal physician who leads the care team within a medical practice.
    • Whole-person orientation: The care team provides comprehensive care, including acute care, chronic care, preventive services, and end-of-life care, at all stages of life.
    • Integrated and coordinated care: Practices take steps to ensure that patients receive the care and services they need from the medical neighborhood, in a culturally and linguistically appropriate manner.
    • Focus on quality and safety: Practices use the quality improvement process and evidence-based medicine to continually improve patient outcomes.
    • Access: Practices commit to enhancing patients’ access to care.

    PCMH Recognition and Accreditation

A number of nationwide PCMH recognition and accreditation programs exist. Although some practices are required to complete a recognition program (e.g. for participation in an accountable care organization or a PCMH incentive program), PCMH recognition for many practices is voluntary. You do not have to participate in a recognition program in order to begin or complete PCMH transformation.

Although some practices are required to complete a recognition program (e.g. for participation in an accountable care organization or a PCMH incentive program), PCMH recognition for many practices is voluntary.
