    Just a note to say thank you for your wonderful hospitality and effort to make my latest visit the best stay ever! As a frequent traveler I have discovered the characteristics of exceptional hotels and consider yours among the best. The front desk welcome was pleasant, room perfect, food great and all staff went out of their way to make me feel “at home.” The cookies are a nice touch. The Burlington Quality Hotel is my preference for accommodations when in Burlington and I look forward to my next stay.

    The experience which I have with this hotel is always a good one. When you enter the hotel they now have a water jug with limes in it which is just calling your name after a long drive. The rooms are great. This past stay we had an executive room on the top floor which has been refurbished and very quite. The room had a shea with a bathrobe and slippers. The room was well equipped with space. The bathroom had a walk in shower with a rain shower from the ceiling. You had a choice of a regular shower or a rain shower which was very welcoming because you do not see that at many places. The breakfast has also been upgraded also to serve hot food of somewhat better quality – the hash browns are the triangle ones- with bacon and scrambled eggs.

    My stay at Quality Hotel Burlington was enjoyable because the service was very good and the team was attentive to my needs. The location is convenient for access to the McMaster University DeGroote School of Business Ron Joyce Centre. Overall the stay was good value for money.

    HQRP Overview

    The Patient Protection and Affordable Care Act (ACA) mandated the initiation of a quality reporting program for hospices (HQRP). The Center for Medicare and Medicaid Services (CMS) determines the quality measures that hospices must utilize and the processes hospices must use to submit data for those measures. Hospices that fail to comply with HQRP requirements will incur a 2 percentage point reduction to the market basket percentage increase for the corresponding fiscal year.

    HQRP Requirements

    Hospices are required to comply with requirements for two HQRP program components that comprise the data sources for the measures:

    Hospice Item Set (HIS) . A component of the HQRP for the FY 2016 APU and subsequent years. HIS is a patient level data collection tool developed by CMS. Hospices are required to submit an HIS-­Admission record and an HIS­-Discharge record for each patient.

    CAHPS® Hospice Survey . A component of the HQRP for the FY 2017 APU and subsequent years. CAHPS Hospice is a post­death family caregiver survey developed by CMS for the assessment of patient and family experiences with hospice care.

    What’s New

    Hospice Quality Reporting: Annual Payment Update. On June 29, 2016 CMS mailed notifications to hospices that were not in compliance with Hospice Quality Reporting requirements. If you received a notice of non-compliance you have the opportunity to submit a request for reconsideration on quality data submissions affecting your FY 2017 APU.     The Role of Psychology in End-of-Life Decisions and Quality of Care

    Psychologists can contribute to end-of-life care before illness strikes, after illness is diagnosed and treatments begin, during advanced illness and the dying process, and after the death of the patient, with bereaved survivors.

    Findings

    Medical doctors, nurses, social workers, and the clergy have traditionally been the main players in helping care for people near death, but psychologists are increasingly using their expertise to help people have a so-called “good death” or perhaps the more accurate “least worst death.” End-of-life – defined as the period when health care providers would not be surprised if death occurred within about six months – is a time when psychologists can treat depression and anxiety associated with pending death, offer grief counseling, help people understand confusing medical terms, and help provide compassionate care for the dying and their loved ones.

    Several factors are shaping the expanding role of psychological practice in end-of-life care, according to psychologist William E. Haley, PhD of the University of South Florida. Psychologists are already trained and involved in the mental health treatment of major chronic illnesses such as heart disease, cancer, AIDS, dementia and chronic pain. Psychological intervention with these people includes psychotherapy for depression and anxiety, stress and pain management, relaxation training and family and group psychotherapy. Also, a broad-based movement to improve the final moments of life has led to research identifying major failures in hospital care of the seriously ill. This research finds that many patients are dying following prolonged hospitalization or intensive care in which their final days involve unrelieved pain and their preferences concerning life-sustaining treatments are not fully discussed, documented or followed. These findings have led to the rapid expansion of the end-of-life field, allowing for expanding contributions of psychologists.

    Dr. Haley and other members of the American Psychological Association’s (APA) Ad-Hoc Committee on End-of-Life Issues, identify four time periods when psychologists can contribute to end-of-life care:

    Before illness strikes;

    After illness is diagnosed and treatments begin;

    During advanced illness and the dying process; and

    After the death of the patient, with bereaved survivors.

    As psychologist Phillip M. Kleespies, PhD, notes in his 2004 book, Life and Death Decisions: Psychological and Ethical Considerations in End-of-life Care, it is difficult to think of a more intensely emotional and psychological time than when a patient is facing his or her decline and eventual death. “Working with professionals from other health fields, psychologists have much to offer dying patients, their families, and those who bear the burden of caregiving,” according to Dr. Kleespies.

    The role of psychologists in helping people with HIV and AIDS offers a powerful example of how psychological interventions can make a difference in coping with and adapting to loss and advanced illness across time, from prevention (see Understanding How People Change Is First Step in Changing Unhealthy Behavior ) to helping people who have contracted the life-threatening disease (see APA Office on AIDS-HOPE Program).

    Significance

    The U.S. Supreme Court says Americans should expect palliative care, which combines active and compassionate therapies to comfort and support people and their families nearing the end of life. Psychologists can make significant contributions to improve the quality of end-of-life care and decision-making.

    Practical Application

    Psychologists are increasingly taking a more active role in end-of-life issues. The American Psychological Association identifies the following four main roles that psychologists play in this area:

    Clinical Roles – Psychologists treat clinical depression if and when it arises in end-of-life matters, as well as other mental health problems associated with pending death. Psychologists also help caregivers and family members with facilitating emotional expression and how to effectively be good listeners for people who are dying. Properly trained psychologists also work effectively with issues of mourning and loss, traumatic stress, and serve as advocates for good medical care. The participation of psychologists in hospital ethics committees, palliative care, and other multidisciplinary teams is equally important.

    Education and Training Roles – Psychologists are teaching people to understand loss, grief, and mourning and to understand the differences between normal sadness and clinical depression at the end of life. They are also providing information about advance care planning and decisions (including “living wills”) and teach coping mechanisms. In 2004, APA received a grant from the National Institutes of Health Small Business Innovation Research program to develop an Internet-based program for educating psychologists and other mental health providers about helping those near the end-of-life and their loved ones. APA is working with a small business in Seattle to develop the program. Psychologists, such as Dale Larson, PhD, of Santa Clara University, were also major players in the Finding Our Way: Living With Dying in America community education project which reached millions of Americans through newspaper articles and website.

    Research Roles – Psychologists have played key roles in conducting research on major issues relevant to the end-of-life such as death anxiety; decision making at the end-of-life; family caregiving; psychological aspects of pain and symptom management; and grief and bereavement. For example, psychological research has increasingly demonstrated that most individuals who experience bereavement are more resilient than was conventionally thought to be the case. This research has demonstrated that individuals who do not experience intense grief symptoms after a loss are not at high risk for subsequent pathological grief reactions.

    Policy Roles – There are numerous opportunities and the federal, state and local levels for psychologists to advance the quality of care at the end of life. Many times dying people and their families are not fully aware of various end-of-life care options or not fully apprised of the probable benefits and burdens of these various options. Psychologists can work with other health care professions in advocating for the development of policies to ensure that people know what types of interventions and services are available to them. Other policy issues that psychologists can address include advocating for systemic changes in legal and organizational obstacles to quality care, advocate for ongoing discussion of death and dying issues in the media, the community, and in professional meetings to overcome society’s reluctance to address these issues that many consider a taboo subject, and to advocate for more equitable end-of-life care for people with disabilities.

    Chapter 3: Resistance and Static Electricity

    As we have learned, some kinds of atoms contain loosely attached electrons. Electrons can be made to move easily from one atom to another. When those electrons move among the atoms of matter, a current of electricity is created.

    Take a piece of wire. The electrons are passed from atom to atom, creating an electrical current from one end to the other. Electrons are very, very small. A single copper penny contains more than 10,000,000,000,000,000,000,000 (1×1022) electrons.

    Electricity “flows” or moves through some things better than others do. The measurement of how well something conducts electricity is called its resistance.

    Resistance in wire depends on how thick and how long it is, and what it’s made of. The thickness of wire is called its gauge. The smaller the gauge, the bigger the wire. Some of the largest thicknesses of regular wire is gauge 1.

    Different types of metal are used in making wire. You can have copper wire, aluminum wire, even steel wire. Each of these metals has a different resistance; how well the metal conducts electricity. The lower the resistance of a wire, the better it conducts electricity.

    Copper is used in many wires because it has a lower resistance than many other metals. The wires in your walls, inside your lamps and elsewhere are usually copper.

    A piece of metal can be made to act like a heater. When an electrical current occurs, the resistance causes friction and the friction causes heat. The higher the resistance, the hotter it can get. So, a coiled wire high in resistance, like the wire in a hair dryer, can be very hot.

    Some things conduct electricity very poorly. These are called insulators. Rubber is a good insulator, and that’s why rubber is used to cover wires in an electric cord. Glass is another good insulator. If you look at the end of a power line, you’ll see that it is attached to some bumpy looking things. These are glass insulators. They keep the metal of the wires from touching the metal of the towers.

    Another type of electrical energy is static electricity. Unlike current electricity that moves, static electricity stays in one place.

    Try this experiment.

    Rub a balloon filled with air on a wool sweater or on your hair. Then hold it up to a wall. The balloon will stay there by itself.

    Tie strings to the ends of two balloons. Now rub the two balloons together, hold them by strings at the end and put them next to each other. They’ll move apart.

    Rubbing the balloons gives them static electricity. When you rub the balloon it picks up extra electrons from the sweater or your hair and becomes slightly negatively charged.

    The negative charges in the single balloon are attracted to the positive charges in the wall.

    The two balloons hanging by strings both have negative charges. Negative charges always repel negative charges and positive always repels positive charges. So, the two balloons’ negative charges “push” each other apart.

    Static electricity can also give you a shock. If you walk across a carpet, shuffling your feet and touching something made of metal, a spark can jump between you and the metal object. Shuffling your feet picks up additional electrons spread over your body. When you touch a metal doorknob or something with a positive charge the electricity jumps across the small gap from your fingers just before you touch the metal knob. If you walk across a carpet and touch a computer case, you can damage the computer.

    One other type of static electricity is very spectacular. It’s the lightning in a thunder and lightning storm. Clouds become negatively charged as ice crystals inside the clouds rub up against each other. Meanwhile, on the ground, the positive charge increases. The clouds get so highly charged that the electrons jump from the ground to the cloud, or from one cloud to another cloud. This causes a huge spark of static electricity in the sky that we call lightning.

    You’ll remember from Chapter 2 that the word “electricity” came from the Greek words “elektor,” for “beaming sun” and “elektron,” both words describing amber. Amber is fossilized tree sap millions of years old and has hardened as hard as a stone.

    Around 600 BCE (Before the Common Era) Greeks noticed a strange effect: When rubbing “elektron” against a piece of fur, the amber would start attracting particles of dust, feathers and straw. No one paid much attention to this “strange effect” until about 1600 when Dr. William Gilbert investigated the reactions of magnets and amber and discovered other objects can be made “electric.”

    Gilbert said that amber acquired what he called “resinous electricity” when rubbed with fur. Glass, however, when rubbed with silk, acquired what he termed “vitreous electricity.”

    He thought that electricity repelled the same kind and attracts the opposite kind of electricity. Gilbert and other scientists of that time thought that the friction actually created the electricity (their word for the electrical charge).

    In 1747, Benjamin Franklin in America and William Watson in England both reached the same conclusion. They said all materials possess a single kind of electrical “fluid.” They didn’t really know anything about atoms and electrons, so they called how it behaved a “fluid.”

    They thought that this fluid can penetrate matter freely and couldn’t be created or destroyed. The two men thought that the action of rubbing (like rubbing amber with fur) moves this unseen fluid from one thing to another, electrifying both.

    Franklin defined the fluid as positive and the lack of fluid as negative. Therefore, according to Franklin, the direction of flow was from positive to negative. Today, we know that the opposite is true. Electricity flows from negative to positive. Others took the idea even further saying this that two fluids are involved. They said items with the same fluid attract each other. And opposite types of fluid in objects will make them repel each other.

    All of this was only partially right. This is how scientific theories develop. Someone thinks of why something occurs and then proposes an explanation. It can take centuries sometime to find the real truth. Instead of electricity being a fluid, it is the movement of the charged particles between the objects. the two objects are really exchanging electrons.

    It’s been almost twenty years since the Health Care Finance Administration (now the Centers for Medicare and Medicaid Services ) sent a memo to hospices announcing the first widespread, in-depth review of hospice medical records and patient eligibility. Focused Medical Review, as it was called in 1994, was just the beginning of the government’s search for fraud and waste in hospice.

    Prior to this action, hospices that had largely avoided widespread regulatory scrutiny were suddenly being held accountable to a much higher documentation standard than ever before. During this audit, hospices struggled to demonstrate through their documentation that patients who had already died, often within days or weeks of admission, met eligibility requirements for hospice. In the days before Webinars or online education, hospices were challenged with the means to disseminate education to their staff to teach them to “paint the picture” of medical eligibility.

    Today, hospices are once again facing intense scrutiny of their documentation as Medicare Administrative Contractors (MACs) are sending notices of Probe Edits and Additional Document Requests (ADRs). The burden on staff and financial resources to respond to these audits is taking a toll on hospices under review. Once again, many hospices are finding that they lack the documentation required to paint a picture of eligibility.

    The criteria used to determine eligibility for the Hospice Medicare Benefit is specific to each MAC, and changes periodically based on new prognostication tools and research. Hospices should use the disease-specific criteria outlined in each MAC’s Local Coverage Determinations (LCDs) to determine if someone is or continues to be eligible for hospice under Medicare.

    Documentation of disease-specific criteria, along with supporting information such as description of the patient’s illness trajectory, pertinent co-morbid and secondary conditions, severity of symptoms, declining functional, cognitive and nutritional status, and overall burden of illness are critical to depicting why this patient will probably die within six-months or less.

    All hospice staff should be fully aware of and utilize their LCD guidelines and disease-specific criteria to document eligibility for each admission and every recertification.

    Ask yourself the following questions to determine if your hospice team has the education and tools that they need to paint the picture of eligibility:

    Are the LCDs easily accessible to all admission and IDT staff – either on their laptop, smartphone or in a handy printed resource?

    Does your team refer to LCD criteria when discussing recertification during IDT meetings?

    Does each patient’s medical record include documentation regarding his or her prognosis, both upon admission and in each recertification note?

    Do your physician narratives contain key elements based upon LCD guidelines that lead the reader to the conclusion that the patient is terminally ill?

    Do you conduct monthly chart audits to ensure that best practices for documenting medical eligibility are being followed?

    Are ALL staff trained and regularly retrained on how to document to the LCDs, including documentation of co-morbid conditions to support medical eligibility?

    If you answered “No” to any of the questions below, your hospice may be at risk for financial penalties in the event of an audit. Comprehensive documentation that paints the picture of initial and ongoing eligibility is paramount to the ongoing survival of your hospice. Training, along with ongoing chart reviews, is the best insurance you can have against future MAC audits, probes, and ADRs.

    She believes lung cancer patients should start palliative care with their first visit to a cancer doctor. “When I first talk to a patient who is newly diagnosed with lung cancer, you can see that they feel like they are in the twilight zone. Their head is spinning,” she says.

    Your oncologist will go over your symptoms and examine you to see what kind of palliative care is right for you, Thompson says. The care should be tailored to your needs. Your doctor can adjust it as they change.

    About the Program

    This part-time program would be of interest to people employed in quality engineering functions – quality control and quality assurance, receiving and in-process inspection, and to co-ordinate measuring machine operators, manufacturing technicians and technologists. Topics in quality control and assurance are covered in depth and in a practical and realistic manner. On completion of the program, the graduate will be able to apply the proper techniques and procedures to solve practical manufacturing problems with respect

    It’s been almost twenty years since the Health Care Finance Administration (now the Centers for Medicare and Medicaid Services ) sent a memo to hospices announcing the first widespread, in-depth review of hospice medical records and patient eligibility. Focused Medical Review, as it was called in 1994, was just the beginning of the government’s search for fraud and waste in hospice.

    Prior to this action, hospices that had largely avoided widespread regulatory scrutiny were suddenly being held accountable to a much higher documentation standard than ever before. During this audit, hospices struggled to demonstrate through their documentation that patients who had already died, often within days or weeks of admission, met eligibility requirements for hospice.     Ask yourself the following questions to determine if your hospice team has the education and tools that they need to paint the picture of eligibility:

    Are the LCDs easily accessible to all admission and IDT staff – either on their laptop, smartphone or in a handy printed resource?

    Does your team refer to LCD criteria when discussing recertification during IDT meetings?

    Does each patient’s medical record include documentation regarding his or her prognosis, both upon admission and in each recertification note?

    Do your physician narratives contain key elements based upon LCD guidelines that lead the reader to the conclusion that the patient is terminally ill?

    Do you conduct monthly chart audits to ensure that best practices for documenting medical eligibility are being followed?

    Are ALL staff trained and regularly retrained on how to document to the LCDs, including documentation of co-morbid conditions to support medical eligibility?

    If you answered “No” to any of the questions below, your hospice may be at risk for financial penalties in the event of an audit.     Palliative Care Team Goal: Improve Quality of Life, Coordinate Care

    Every person and every illness is unique. After you’ve asked your primary care doctor for a referral, you’ll meet with your palliative care team to discuss your goals and desires during your illness.

    Once the members of your palliative care team understand your needs, they will work with your primary care doctor and other experts to create an individualized palliative care plan. The goals are:

    • Relieve pain and other symptoms
    • Address your emotional and spiritual concerns, and those of your caregivers
    • Coordinate your care
    • Improve your quality of life during your illness

    For example, a palliative care doctor may prescribe medications and other therapies to treat pain, constipation. shortness of breath, and other symptoms. A social worker may coordinate your care and serve as an advocate on behalf of you and your family. A chaplain may offer spiritual support and help you to explore your beliefs and values.

    The palliative care team can also help your family by offering medical information, emotional support, and home care assistance.

    Who is on the palliative care team?

    In general, the interdisciplinary palliative care team includes a doctor, a nurse, and a social worker. But other experts often fill out the team, according to a patient’s needs. These include chaplains, counselors, pharmacists, dietitians, rehabilitation specialists, physical therapists, music and art therapists, and home health aides.

    There’s no single model for a palliative care team. Hospitals have their own types of palliative care programs. Often, large hospitals have more extensive palliative care services, but smaller hospitals, nursing homes, and hospices also offer palliative care.

    Your palliative care team can provide the following services:

    • Expert treatment of pain and other symptoms
    • Open discussion about treatment choices for your illness (including difficult and complex choices) and management of your symptoms
    • Coordination of your care with all of your health care providers
    • Help with navigating the health care system
    • Help with making a smooth transition from the hospital to home care or a nursing home
    • Emotional, spiritual, and practical support for you and your family
