my spouse had lung cancer it was small cell and had spread she took 20chemo treatments 5 radiation to brain she was in pain she received pain medication I as care taker at home gave her her medication after she could not receive any more treatments they suggested hospice we really did not know how bad she was times my spouse would say I don’t need my meds she suffered a painfull death because we where not properly educated on what was happening to her body if fell the nurse that came did her best at saying u need to take this or that but not the real truth of what was happing to her body it was so hard we did not make the decision for hospice only way to have that was dnr who knew that that was the right thing to do we did not this must be fixed maybe everyone is not the same make it clear help help

Report: Stein Hospice has conflicts at OVH

Three area doctors who have government contracts to work at the Ohio Veterans Home might have improperly steered business toward Stein Hospice, where they also serve as physicians.

This has created a potential ethical conflict with the doctors who have received special treatment for their private business, according to a Ohio Inspector General s report released on Thursday.

The office handles complaints and pursues investigations into possible wrongdoing within state government.

The compliance officer concluded that there were significant problems with the relationship between Stein Hospice and the Ohio Veterans Home, the report indicated.

In this particular instance, there is a potential liability to the state of Ohio, according to inspector general spokesman Carl Enslen.

Enslen said this case has been forwarded to the U.S. Department of Health and Human Services to determine potential violations of federally regulated nursing care facilities.

The inspector general s fact-finding mission started in March 2014, when the home s former chief legal counsel sent a memo outlining possible misconduct.

Stein Hospice employees, according to the report, received high-level access to patient information and other government services, creating the appearance of an unfair advantage for their organization.

Of the 58 end-of-life veteran service referrals analyzed, the report indicated each one went to Stein Hospice and no other organization assisting terminally ill patients.

Those 58 patients received hospice care for an average of 472 days or almost five times what is considered a normal stay. One patient stayed in hospice care for nearly five years.

The report highlighted three other main areas of concern:

Possible federal anti-kickback violations, including self-dealing and false claims violations

Failure to clearly outline responsibilities between Stein Hospice and the Ohio Veterans Home

Possible violations of the rights for patients and beneficiaries to select medical providers and treatment

The arrangements appear to violate state and federal laws.

When accounting for possible damages and potential liabilities from this supposed improper relationship, it totals as much as $5.3 million.

Representatives from Stein Hospice, located in Sandusky, contend there is no substance to these allegations.

We did not do anything wrong. We have not done anything wrong. There has been no improper relationship, said Jan Bucholz, Stein Hospice s chief executive officer.

Ohio Veterans Home officials, who oversee veteran residencies in Perkins Township and southern Ohio, also offered comment on the situation.

As we were working to strengthen our operations at Ohio s two veterans homes, we identified issues of concern and asked the state inspector general to investigate, home spokeswoman Eileen Corson said.

Among the steps home officials recently implemented to remedy some issues:

Stein Hospice no longer has office space at either facility.

Veterans and their families now receive a list of all available hospice providers and make a choice in writing.

In addition to the referring doctor, a state department of veterans services doctor must also sign all hospice referrals.

Medical privacy issues have been addressed.

Doctors contracts are reviewed to make sure they comply with all federal and state laws.

We appreciate the inspector general s diligence in looking into this matter, Corson said. We will be closely reviewing his findings to see if there are any further steps to take to strengthen our operations. Providing the best possible care for Ohio s veterans is our top priority.

The Ohio Inspector General s Office on Thursday issued a report detailing possible violations committed by the Ohio Veterans Home in its relationship with Stein Hospice.

The Ohio Department of Veterans Services, the parent organization which administers the Ohio Veterans Homes, identified an estimated $5.3 million in federal penalties and loss to the state of Ohio, according to a related press release.

The report continued on: The former legal counsel of the Ohio Department of Veterans Services expressed concern that several physicians working at the veterans homes were also medical directors of Stein Hospice, a private medical contractor. The hospice was providing private services using state resources and equipment at the Georgetown facility, and paying no rent for use of the site.

Representatives from Stein Hospice and the Ohio Veterans Home were not immediately for comment.

“But the other side of that is, since the financial crisis four years ago, a lot of the competition that banks had have gone away, and that’s allowing them to earn higher margins on a lot of their products,” he added.

Nygren’s fund listed Capital One and JPMorgan among its largest financial holdings.

“We’re not trying to argue that these are fantastic businesses, but what we’re saying is the best banks sell at maybe three-quarters of book value. A lot of the others sell at about half of book value, and you don’t need tremendously good results from these banks to make them go up from these levels.”

Nygren also identified TE Connectivity as a stock that had upside potential, due to its healthy dividend yield, its share base declining 2 to 5 percent per year and top-line growth.

“We think you combine those three, you get a pretty nice rate of return, even if the PE-multiple doesn’t change,” he said.

How to Rebuild Your Credit After Bankruptcy — Fast

It’s about as popular as a root canal or a blown tire on the freeway. Yet like both of those dreaded occurrences, filing for bankruptcy is commonplace in modern America. In 2013 1,107,699 individuals and businesses in the U.S. had to file for bankruptcy according to government data. While it’s pretty safe to say that not many of the folks who filed for Chapter 7 or Chapter 13 bankruptcy were eager to do it, it’s worth remembering that bankruptcy is by no means a financial death sentence.

After all, bankruptcy protection is designed to provide people and companies with a way to discharge at least some of their debts and start over. And one of the very first steps to reboot your financial life involves rebuilding your credit score.

How Bankruptcy Affects Your Credit Score

There’s no sugarcoating the fact that any individual who has to file for bankruptcy is going to see a negative impact on their credit score. This matters because lenders rely on credit scores – which typically range from 300 on the low end to a maximum of 850 – to determine whether or not to offer you a mortgage or car loan and, if they do, at what interest rate. Put another way, a credit score is a quick way for lenders to decide how risky it is to loan you money.

Not surprisingly, a bankruptcy — which, by definition, allows someone to not repay all of their debts — hammers your credit score. How much it hurts depends on where you started. According to the Fair Isaac Corporation, which is better known as FICO and the best-known company that calculates credit scores, someone with a stellar score of 780 would see their rating plummet to between 540 and 560 after declaring bankruptcy. A person with a starting score of around 680 can expect to drop between 150 and 180 points, to between 500 and 530, after a bankruptcy. You can take a look at the credit profiles of the two people used in the FICO example here.

What does this all mean? In short, bankruptcy is a major red flag to lenders, a fact that is reflected in your credit score. In April of 2014 FICO reported that its median score was 711. Scores of 550 or lower are considered “deep subprime” by lenders and carry with them high interest rates – if you can get a loan at all.

Rebuilding Your Credit to Improve your Financial Health

Fortunately, a bankruptcy is by no means game over for your financial life. There are steps you can take to begin rebuilding your credit and, eventually, your overall financial health. “Bankruptcy is a very difficult and emotional time since there are severe consequences, but it also represents a chance to “start over” from a financial sense,” says Bill Harddekopf. CEO at LowCards.com.

According to Hardekopf, the road back to financial well being starts with rebuilding your credit score. “One of the first things to do is to get a free copy of your credit report, which you are able to do once a year from each of the three major credit reporting agencies. Check this for errors. Make sure you also determine your FICO credit score and work diligently to build up this score each month.”

Raising your score, says Hardekopf, is all about getting back to basics. “Pay your bills on time. Don’t spend more than you can afford. Get a credit card that reports to the credit reporting agencies and pay off the entire balance each month so you don’t incur any interest charges–this will help slowly build your credit score.”

Gerri Detweiler, author of Debt Collection Answers. How to Use Debt Collection Laws to Protect Your Rights, counsels that it’s important to avoid the urge to steer clear from credit after a bankruptcy. “Unfortunately, many people avoid credit afterward, which is understandable, but doesn’t help the situation,” she says. Instead, Detweiler says to focus on responsibly repaying any loans — such as a car or student loan — that may have survived bankruptcy. “Continue to pay them on time. You may also want to get a secured credit card. Use it for one purchase a month (your cell phone bill for example) and pay it off right away, and it will provide a valuable credit reference,” she says. “I’ve seen consumers boost their credit scores by 50-75 points or more in one or two years using this approach.”

Ways to Improve Your Credit After a Bankruptcy: A Checklist

As both Detweiler and Hardekopf make clear, being proactive about rebuilding your credit score after a bankruptcy is a must. Here are some other ways to take an active role in reinvigorating your financial health post-bankruptcy:

Know your credit score — Many people put this one off, because it can be painful. You know your credit score is low so does it really matter how low? Yes, it does. Get a copy of your credit report and know your score. Review it for any inaccuracies and make note of your debts. Only then can you come up with a plan to pay off your debts and improve your credit. Open a new bank account — Opening a new checking and savings account will demonstrate financial stability. It can also give you a fresh slate to practice good financial habits. Apply for a secured credit card — If big credit card bills led to your bankruptcy, you may feel like this is a terrible idea. Why go down that path again? The best answer is that secured credit cards are one of the easiest ways to build credit and improve credit scores. Compare interest rates of different cards, so you can select a card with the best rate and a low annual fee. A rate around 15% is good and an annual fee less than $30 is desirable.

Get a gas card or a retail card — Gas and retail credit cards will also improve your credit. If you drive a car, you will have to purchase gas. You should make those purchases work for you. Gas and retail cards typically don’t require applicants to have good credit and, in fact, cater to folks with blemished credit.

Pay off your balance in full every month — While you are reestablishing your credit, it’s critical to pay off your full balance every month. This demonstrates to creditors that you are not a risk. Timely payments also have a significant impact on your credit score.

Continue to monitor your credit score — Check your credit score regularly (monthly is ideal) while you are actively improving your credit. Watching that number go up can make you feel like your hard work is really paying off.

Finally, it’s also important to remember to be patient throughout this process. A bankruptcy can impact your credit for as long as 10 years. But the more active a role you take and the sooner you get started, the quicker you can bounce back from bankruptcy. How do you know if your credit scores are good?

Credit monitoring is a service that alerts you whenever a change is made to the information in your credit files at one or more of the three credit bureaus. Triple-bureau monitoring obviously provides a more comprehensive overview of the changes being made in your credit files.

Do You Have to Place a Fraud Alert with each Credit Reporting Company?

Dear DRP,

No, you do not need to request another fraud alert with Experian.

How Requesting a Fraud Alert Works

When you request a fraud alert or security alert be added with any of the three major credit reporting companies, the company you contacted will notify the other two and alerts will be added with those agencies as well.

You should receive confirmation from each company that the alert has been added. If not, you can contact each agency individually to confirm that the alert is on file their files.

An initial security alert will remain on your report for 90 days. You can request a free copy of your report when you add the alert so that you can review it carefully and determine whether there are any signs of identity theft.

Keep in mind that each credit reporting agency may have slightly different information, so you will want to review all three credit reports.

Additional Things to Know About Fraud Alerts

Along with adding the initial alert, Experian will automatically opt you out of receiving pre-approved offers for six months.

Should you ever need to request an initial alert in the future, you can always add it directly by visiting Experian’s Fraud Center .

Be sure to contact your local police department to file an identity theft report and then contact Experian if you suspect there are fraud-related items on your report.

Once you’ve filed a police report you can request a seven-year victim statement be added. It will include two telephone numbers that you provide so lenders can contact you before granting credit in your name.

Instructions and Explanations on How to Replace Inputs

The Date (A1) – This will serve as the reference point for the Past Due Days column. Modify the date in cell B1 by using the numeric mm/dd/yyyy format: 11/07/2010

Account Number Column (A) – Simply index this column with the company’s control number for past due accounts receivable. This is important for maintaining the traceability of data against other accounting records, i.e. accounting entry tickets or vouchers, sales invoices, et al.

Invoice Number Column (B) – Input your invoice number — it is not necessary for the numbers to appear in proper numerical sequence, since not all accounts receivable are being aged. Nevertheless, this information is important to allow for audit trace-back purposes.

The Due Date Column (C) – The information in this column will be used to format the cells in column D to generate the maturity dates for each account.

The Maturity or Due Date Column (D) — In our example, the due date is automatically generated using the 30-day credit term; hence this cell will reflect the account’s maturity date, accordingly.

To modify the template according to your own credit terms, refer to the formula bar and then change the term that is added to the cell reference number. To illustrate, let’s say you’re moving from cell D4 to D5 but the term for this account is 15 days instead of 30: Modify C5+30 appearing on the formula bar to C5+15. The cell will automatically return with the corresponding maturity date for a 15-day credit term and will likewise automatically input the number of Past Due Days in column E accordingly.

The Past Due Days Column (E) – The input in this cell is automatically generated by the functions of the cells in column D. As mentioned earlier, the D column automatically computes the number of days the account has been past due, based on the corresponding due dates generated under the Due Date column (D) and as of the report date you indicated in column B.

Amount Due – Place the corresponding principal amount or balance of the receivable under this column.

Past Due Days Column (1-30/ 31-60/ 61-90/ 91-120/ 151-180/ and Over 181) – Extend the amount due under the applicable column according to the number of Past Due Days generated under column E.

Totals – Totals can be generated through the auto sum function, by highlighting all cells from G4 to G12 extending through N4 to N12.

Once you have customized the formula values for the D columns, you can simply change the report date every time a new report is prepared and the number of past due days will be readily available.

Report: Stein Hospice has conflicts at OVH

Three area doctors who have government contracts to work at the Ohio Veterans Home might have improperly steered business toward Stein Hospice, where they also serve as physicians.

This has created a potential ethical conflict with the doctors who have received special treatment for their private business, according to a Ohio Inspector General s report released on Thursday.

The office handles complaints and pursues investigations into possible wrongdoing within state government.

The compliance officer concluded that there were significant problems with the relationship between Stein Hospice and the Ohio Veterans Home, the report indicated.

In this particular instance, there is a potential liability to the state of Ohio, according to inspector general spokesman Carl Enslen.

Enslen said this case has been forwarded to the U.S. Department of Health and Human Services to determine potential violations of federally regulated nursing care facilities.

The inspector general s fact-finding mission started in March 2014, when the home s former chief legal counsel sent a memo outlining possible misconduct.

Stein Hospice employees, according to the report, received high-level access to patient information and other government services, creating the appearance of an unfair advantage for their organization.

Of the 58 end-of-life veteran service referrals analyzed, the report indicated each one went to Stein Hospice and no other organization assisting terminally ill patients.

Those 58 patients received hospice care for an average of 472 days or almost five times what is considered a normal stay. One patient stayed in hospice care for nearly five years.

The report highlighted three other main areas of concern:

Possible federal anti-kickback violations, including self-dealing and false claims violations

Failure to clearly outline responsibilities between Stein Hospice and the Ohio Veterans Home

Possible violations of the rights for patients and beneficiaries to select medical providers and treatment

The arrangements appear to violate state and federal laws.

When accounting for possible damages and potential liabilities from this supposed improper relationship, it totals as much as $5.3 million.

Representatives from Stein Hospice, located in Sandusky, contend there is no substance to these allegations.

We did not do anything wrong. We have not done anything wrong. There has been no improper relationship, said Jan Bucholz, Stein Hospice s chief executive officer.

Ohio Veterans Home officials, who oversee veteran residencies in Perkins Township and southern Ohio, also offered comment on the situation.

As we were working to strengthen our operations at Ohio s two veterans homes, we identified issues of concern and asked the state inspector general to investigate, home spokeswoman Eileen Corson said.

Among the steps home officials recently implemented to remedy some issues:

Stein Hospice no longer has office space at either facility.

Veterans and their families now receive a list of all available hospice providers and make a choice in writing.

In addition to the referring doctor, a state department of veterans services doctor must also sign all hospice referrals.

Medical privacy issues have been addressed.

Doctors contracts are reviewed to make sure they comply with all federal and state laws.

We appreciate the inspector general s diligence in looking into this matter, Corson said. We will be closely reviewing his findings to see if there are any further steps to take to strengthen our operations. Providing the best possible care for Ohio s veterans is our top priority.

The Ohio Inspector General s Office on Thursday issued a report detailing possible violations committed by the Ohio Veterans Home in its relationship with Stein Hospice.

The Ohio Department of Veterans Services, the parent organization which administers the Ohio Veterans Homes, identified an estimated $5.3 million in federal penalties and loss to the state of Ohio, according to a related press release.

The report continued on: The former legal counsel of the Ohio Department of Veterans Services expressed concern that several physicians working at the veterans homes were also medical directors of Stein Hospice, a private medical contractor. The hospice was providing private services using state resources and equipment at the Georgetown facility, and paying no rent for use of the site.

Representatives from Stein Hospice and the Ohio Veterans Home were not immediately for comment.

How to Report a Fraud or Scam

If you have observed a scam or been the victim of a scam, spam or fraud and want to report it for enforcement, here is a list of where to report different types of scams in the US, UK, Canada and many other countries. In some cases there is more than one agency to contact. Some scams fit into more than one category, also.

Of course, in all cases, you can start by reporting it to us, using this form. We will use the information to warn other consumers and help you to direct it to law enforcement agencies.

Next, the Federal Trade Commission’s online complaint tool is the main tool to reporting a problem or scam to the government. They won’t likely get your money back, but at least you will help them to catch the scammer.

Another catch-all for bringing internet scams to the attention of the FBI and FTC is the U.S. government’s Internet Crime Complaint Center (IC3) at Internet Fraud Complaints Center. If you are looking for your local FBI Office Listings to report a crime or scam, click here.

    Reporting to Email account providers

    You can report spams and scams that are coming from specific email accounts to the providers. For example, if a scammer is using a Hotmail account to send scams or spams or receive replies, you can report it to Hotmail. Be sure to forward the email and copy it’s headers (click here to see how ) into the email you send to the provider. That helps them to trace the source.     Scams: AFF – Nigerian, Lottery scams, Money Transfer Frauds, Lawyers with deceased clients, etc.

    Businesses: Complaints with businesses with fixed addresses

    • Complaints with any US or Canadian business: Better Business Bureau – Contact the Better Business Bureau where the company is located.
    • Phishing and Spam: Internet Crime Prevention Control Institute , a cooperative effort between Zero Spam Network Corp. and the University of Miami.     Triggers for palliative care report from Marie Curie: NCPC comment

    Marie Curie has today published a new report on improving access to care for people with conditions other than cancer.

    The new report, Triggers for Palliative Care. highlights some of the key reasons people with conditions other than cancer are failing to access palliative care. It cites uncertain disease trajectories and a shortage of research as well as a lack of understanding of what palliative care can offer as some of the barriers. Recommendations include better resourcing from government, better coordinated care, imrpved understanding of palliative care within organisations, and a call for more research.

    Commenting on today’s report from Marie Curie on palliative care Claire Henry, Chief Executive of the National Council for Palliative Care said:

    “Palliative care can transform the lives of people who are terminally ill, which is why it’s unacceptable that so many people who are approaching the end of life and who could benefit from it are not being given the chance to do so.

    People need to have meaningful choices about their end of life care, but this will only happen with a greater awareness of how palliative care can help across a range of conditions. Caring for all dying people must become a priority for policymakers, commissioners and service providers alike, and the healthcare workforce should be supported, including through mandatory training, to become better at discussing dying and identifying palliative care options. It is wrong and unfair that we have a diagnosis lottery where the quality of care you receive depends on which illnesses you have.”

    I booked a room at (The Inn At Lundy’s Lane owned by Niagara Hospitality Hotels) through cheaprooms.com which is run by hotels.com (Confirmation #: 047888188). Upon arrival at my room it was in unacceptable condition; the room smelled, it was dirty. I sat on the bed to try it, it was hard as a rock.

    I went to the “Manager” Donna Mancuso to ask for a refund. She said she would not charge me for 8.18.07 but would still charge me for 8.17.07. She became very rude after I told her this was not acceptable since the room was dirty and I had not stayed. She told me I owed $5 for outgoing telephone calls, I agreed to pay this. She told me to contact www.hotels.com as I booked the room through them.

    I contacted hotels.com they told me the can not issue refund it was up to the hotel, opposite of what Donna had told me. They also charged $4.70 to my credit card, I am assuming for the telephone calls.

    Hotels.com would not do anything for me. I also contacted Niagara Hospitality Hotels by email with no response. I have also filed a complaint with the Ohio Attorney General (Complaint #:340740) continuing investigation.

    After filing my complaint with Ripoff Report I got a rebuttal from Hotels.com. Hotels .com agreed to refund me $180, I checked my credit card statement and as promised the credit was there. I greatly appreciate the help from all participating parties.

    Hi, my name is Matt and I am a Lead Agent with Hotels.com Customer Care. My office is in Springfield, MO.

    In regards to your reservation, Mr. Freetage, I have taken a look and I see why the agents initially told you what they did; the policy included as part of your change/cancel policy states that there are no refunds for early checkout. This is due in large part to the fact that most hotels will still continue to bill us for the remaining nights; if we always refunded while still being billed, we’d lose a lot of revenue very quickly. However, in some cases the hotel will not bill us, and if that happens to be the case, we are more than happy to refund those nights.

    The fact remains that the hotel has told us we’re being billed for both of the nights you reserved. However, your complaint regarding the condition of the hotel/room adds some special circumstances to the mix. You were previously advised that you wouldn’t receive any credit, but were offered $80 in Hotel Bucks, which are vouchers good toward that amount off a future prepaid booking through us. I don’t show that you ever responded accepting or declining them. We never heard from you again after that offer, which was back on August 31st.

    Rather than offer you vouchers, I have done the following: I have adjusted your booking for a refund of the 2nd night, plus I have adjusted the charge for the 1st night down to our cost. Together, this brings about a credit of $180.00. This amount should appear on your account within 30 days or by your next billing cycle.

    For the record, Hotels.com did not make any other charges to your card aside from the total charge of $248.86 that was applicable for your booking when you made it on August 13th. The amount of $4.70 that you referenced must have been billed to you by someone other than Hotels.com. We do not charge our customers in relation to any incidentals they may incur at a hotel, including phone calls. All payments for incidentals are made to the hotel property directly.
