The community residential care sector encompasses a range of living options for people, primarily seniors, with different support needs. Databases and data sources

Continuing Care Reporting System

The Continuing Care Reporting System (CCRS) contains demographic, clinical, functional and utilization information. This data refers to individuals receiving continuing care services in hospitals or long-term care homes in Canada. Participating organizations also provide information on facility characteristics to support comparative reporting and benchmarking.

The clinical data standard for CCRS was developed by interRAI. an international research network. It was modified with permission by CIHI for Canadian use. Clinicians assess their residents using interRAI’s Resident Assessment Instrument–Minimum Data Set (RAI-MDS 2.0)©.

The information is entered into a computer at the point of care. Real-time reports flag resident risks and inform care planning. The information then flows to CIHI. With no additional collection effort, evidence is available to guide care and system-wide planning as well as quality improvement.

Palliative care: Toronto left with few residential hospice beds

Every time a Perram House patient passed away, a staff member wrote that person s name and date of death on a small white card and placed it near a candle in the front entrance.

It was the 10-bed residential hospice s way of honouring the deceased, allowing their memory to remain in the heritage home s grand foyer, if only for a few more days.

One final card was written last month, but it didn t feature a patient s name. To the disappointment of those desperately needing palliative care in this city, it read: Perram House. April 10, 2013.

The hospice a yellow brick house at 4 Wellesley Place closed suddenly after nearly a decade of providing palliative care to Toronto residents from all walks of life.

Outraged staff accused board chair Frank McCrea of shutting down the union, which had recently organized to demand a wage increase. McCrea, meanwhile, blamed dwindling donations that had forced the charity to operate at a loss for months.

Whatever the reason, the closure of Perram House leaves Toronto with only two residential hospices.

To me, it s really shocking, said Bill O Neill, executive director of Kensington Hospice, where the phone has been ringing constantly since Perram House closed. There s a real discrepancy between Toronto and other cities.

Kensington Hospice in downtown Toronto and Dorothy Ley Hospice in Etobicoke are now the city s only residential hospices, providing a total of 20 hospice beds for a population of 2.6 million. (Casey House, which serves AIDS patients, is classified as a hospital.)

By contrast, Saint John, N.B. has 10 beds for a population of 70,063.

Residential hospices are relatively new across Canada, but palliative care experts say they are a crucial option for patients who do not need to be in hospital and cannot die at home. They combine a home-like environment with 24-hour medical care and psychological support.

There are about 193 palliative care beds in Toronto hospitals, but most have a vacancy rate of close to zero and limit stays to 15 days. Patients can stay in a residential hospice for three months or longer.

As for home care, Toronto Central Local Health Integration Network (LHIN) funds three visiting hospice programs. But for patients with complex needs or without a family caregiver, it is only a short-term option.

The vast majority of Canadians say they want to die at home or in a hospice, but about 70 per cent die in hospitals. In the 2011 fiscal year, nearly 5,000 people died in an acute-care hospital in Ontario while receiving palliative care.

When there are no beds available. the patient generally ends up in a higher-cost setting, said Dr. James Downar, a palliative care physician at Toronto General Hospital. Acute-care beds are the most expensive setting in the entire system.

The daily cost of an acute-care bed in a hospital is between $850 and $1,100, compared with about $439 for a residential hospice bed.

Hospice patients receive care free-of-charge, but advocates say the savings are significant for a province trying to balance its books and hospices aren t just more efficient, they re more humane.

Coline Bettson s mother suffered a sudden brain aneurysm in March and moved into Perram House. The first night, nurses brought Bettson hot tea and set up the bed so she could cradle her 96-year-old mom in her arms.

Everything I asked for and she wanted, they agreed to. They couldn t have been more kind and caring, she said. I was stunned and appalled when I heard (the hospice closed).

In 2005, the Ministry of Health announced $115 million in palliative-care funding, including plans to fund the care costs of 34 residential hospices across the province.

Eight years later, 24 of those hospices have been built and receive annual funding of $90,000 per bed. The remainder of hospice budgets between 20 and 50 per cent must be raised through donations.

McCrea, the former Perram House board chair, said fundraising had always been a struggle.

New Democrat health critic France G linas said the ministry should fully fund residential hospices, as it does long-term care.

It s been unacceptable from the beginning, she said. Hospices deal with vulnerable people in their last days of life. It s wrong to let them rely on donations.

Health Minister Deb Matthews said in an interview that the province had come a long way since before 2005, when hospices were entirely self-funded.

We are transforming how we deliver health care, Matthews said. We are committed to investing more in the community sector and getting people out of hospital who don t need to be in hospital.

She said the health ministry dedicated $260 million to community services, including hospice and home care, in the 2013 budget. The Toronto Central LHIN has convened a working group on palliative care access.

Rick Firth of Hospice Palliative Care Ontario said most hospices are able to fundraise successfully, but Perram House may have struggled because it prioritized marginalized and homeless people.

The donations from families don t come in at the same volume, he said. That may be something we need to look at in the future.

Firth said the loss of Perram House will be felt in Toronto for the time being. Two of the hospices promised funding in 2005 were supposed to be in the city, but neither has broken ground yet.

The Toronto Commandery Hospice finally acquired land last year in North York, but is awaiting approval of its zoning application. The Yee Hong Centre for Geriatric Care in Scarborough still has no timeline for opening a hospice.

But Emily s House a long-planned pediatric hospice at Gerrard St. E. and Broadview Ave. is nearing the end of construction at last and hopes to open its doors at the end of May.

As for Perram House, the stunning home of sweeping staircases and vaulted ceilings built by widower Mary Perram in 1887, it now occupies prime downtown real estate, on Wellesley Pl. between Jarvis and Sherbourne Sts.

McCrea owns the home but has said he hasn t made a decision about the property. As a designated heritage home, it would require city council approval to be demolished.

Pete Perram, Mary s 52-year-old great-great-grandson, said the worst-case scenario would be for it to be turned into a condo building.

Mary intended it to be for the good of the people, the good of the community, he said. I hope that, as a legacy to her and to our family, that continues.

We have been caring for residents of southeastern Minnesota for nearly 20 years with our home hospice services and residential hospice facility.

Hospice provides comprehensive care when a cure is no longer the goal. The emphasis of hospice care is on quality of life, and on maximizing comfort as conditions take their natural course. Hospice care is holistic, delivered by a team of professionals and trained volunteers, designed to address the physical, emotional, spiritual, social, and psychological issues that accompany end of life. We focus on controlling distressing symptoms, and on other concerns the patient or family may have.

Types of Service

The AlarmNet network’s reliability is assured with a state of the art network control center that houses dedicated communication services and hardware platforms. Hardware servers and communication paths are completely redundant, with hot back-up databases housed in two separate locations. If one system were to have a catastrophic event, the other system can take over without any service interruption. Both locations are equipped with battery and generator backup along with technical support 24 hours-a-day, seven days-a-week for uninterrupted service. This level of sophistication allows us to handle alarm communication for UL listed and critical high-line security installations.

Network Architecture

In the AlarmNet architecture, although each of the networks are independent of one another, subscriber radios and Internet transmitters may send messages from one network and be received at nearly any central station location in North America over another technology. The AlarmNet Network Control Center (NCC) accomplishes this integration. This design offers the following important benefits:

1. Integration of Multiple Networks

One of the fundamental principles behind AlarmNet is the integration of different network types. In the AlarmNet architecture, member central stations are equipped with AlarmNet wireless or Internet receiving equipment or both. This equipment communicates directly with the AlarmNet network. Depending on its needs at installation at the protected premise, an installer may choose to use any of the available types of subscriber radios or Internet transmitters, whichever works best for that installation. Although each of the different products may report through a different network, the central station does not need to incur the costs of time, money, or labor to operate, maintain, and integrate all of these networks.

When a central station signs up with AlarmNet, it establishes a network connection scheme between the central station and AlarmNet and as a result gains access to any of its customers at the protected premise through that scheme and the interaction of AlarmNet.

2. Learn One, Know Them All

All of the different subscriber devices used for the different network services have a similar look and feel. The same programming tool is used for most of the devices. All signals are delivered to the central station in a uniform, familiar reporting format. This assures that the central station can receive signals from any type of AlarmNet subscriber; past, present, or future. Once equipped to receive alarm messages from the AlarmNet network, the central station can provide network services to any subscriber without additional expense.

3. Supervision and Speed of Delivery

AlarmNet radios and transmitters are supervised on a regular basis. The supervision is performed automatically, and is transparent to the central station. The AlarmNet network communicates with all AlarmNet devices on a regular basis, assuring that they are operational. The AlarmNet network reports to the central station only when there is a problem. The central station does not need to worry about supervision or maintenance of the network, since the network performs these functions automatically. The central station only receives the signals it needs: actual alarm or trouble messages. When the network receives an alarm, it automatically and immediately forwards the signal to the appropriate central station. The AlarmNet transmission generally will reach the central station before the traditional dialer-based signal, in cases where an AlarmNet device and the digital dialer both send signals.

4. Secure, Redundant, Wireless Protection

Not all competitive wireless subscriber services offered in the market are wireless at both ends. For example, due to the design of the cellular network, competing cellular services deliver signals to the central station over the regular switched telephone network. In these cases, the communications integrity of the central station (and all premises protected by that central station) is vulnerable to the same weaknesses as any landline-dependent customer (line cut, service outages, and natural disasters). Limited services such as these protect only a portion of the communications chain. Standard AlarmNet wireless delivery, however, assures an end-to-end wireless system that provides redundancy, supervision, and verification of message delivery. It only makes sense that the central station should use wireless communications to maintain the same level of security and performance in its communications infrastructure as it does at the protected premises.

In order to maintain a redundant, invulnerable link to subscribers using wireline-terminated networks, a service provider must maintain at least two physically distinct and independent connections to every network type. AlarmNet maintains such connections at every level, saving the central station the expense and complexity of maintaining such an elaborate communications infrastructure. AlarmNet’s connection to the central station also has two layers of redundancy: every central station is equipped with a primary and backup transceiver.

Residential care homes are state-licensed group living arrangements designed to meet the needs of people who cannot live independently and usually do not require the type of care provided in a nursing home. When needed, help is provided with daily activities such as eating, walking, toileting, bathing, and dressing. Residential care homes may provide nursing home level of care to residents under certain conditions. Daily rates at residential care homes are usually less than rates at nursing homes.

Residential care homes are divided into two groups – Level III or Level IV — depending upon the level of care they provide.

  • Level III homes provide nursing overview, but not full-time nursing care
  • Level IV homes provide neither nursing overview nor nursing care

There are Level III homes which are designated as Enhanced Residential Care (ERC) providers. The ERC designation allows the residential care home to designate a number of beds for residents requiring a higher level of care.

Staying active at our residential care homes

Being interested in what is going on and taking part in fun activities is a vital part of life. Our approach to residential care focuses on ensuring all of our care homes offer a stimulating range of activities which are designed to reflect the wishes and interests of our customers and help them maintain close bonds with their local community.

Many of our care homes have a dedicated Activities Co-ordinator who arranges a variety of opportunities for our customers to enjoy daily activities in the home as well as in the local area. These can range from day trips to leisure facilities and places of interest, to arts, crafts, games, using the iPad and social events in the home. We work in partnership with the National Association for Providers of Activities for Older People (NAPA ) and encourage activities that enhance our residents’ lifestyle, hobbies, health and wellbeing.

Visitors are welcome at any time and if they wish can stay for meals. Some of our homes also have overnight guest rooms. Residents are encouraged to maintain links with the wider community and to come and go as they please.

Health and personal care

We work together with a number of health and social care professionals who either visit our homes on a regular basis or who will make visits by arrangement. These include the local GP, district nurse, dentist, optician, physiotherapist, chiropodist and holistic therapists. Most of our homes also have their own hairdressing salon.

Freshly cooked meals

Mealtimes are a social and enjoyable time of the day, and central to our residential care. All our menus offer freshly cooked, wholesome, appetising and nutritionally balanced meals with a different seasonal choice every day.

Our experienced chefs attend our in-house, nationally recognised training programme and we use the Standard Nutritional Assessment check in all of our homes. This enables us to understand the nutritional requirements of each resident and to meet their needs accordingly. Palliative care: Toronto left with few residential hospice beds

Every time a Perram House patient passed away, a staff member wrote that person s name and date of death on a small white card and placed it near a candle in the front entrance.

It was the 10-bed residential hospice s way of honouring the deceased, allowing their memory to remain in the heritage home s grand foyer, if only for a few more days.

One final card was written last month, but it didn t feature a patient s name. To the disappointment of those desperately needing palliative care in this city, it read: Perram House. April 10, 2013.

The hospice a yellow brick house at 4 Wellesley Place closed suddenly after nearly a decade of providing palliative care to Toronto residents from all walks of life.

Outraged staff accused board chair Frank McCrea of shutting down the union, which had recently organized to demand a wage increase. McCrea, meanwhile, blamed dwindling donations that had forced the charity to operate at a loss for months.

Whatever the reason, the closure of Perram House leaves Toronto with only two residential hospices.

To me, it s really shocking, said Bill O Neill, executive director of Kensington Hospice, where the phone has been ringing constantly since Perram House closed. There s a real discrepancy between Toronto and other cities.

Kensington Hospice in downtown Toronto and Dorothy Ley Hospice in Etobicoke are now the city s only residential hospices, providing a total of 20 hospice beds for a population of 2.6 million. (Casey House, which serves AIDS patients, is classified as a hospital.)

By contrast, Saint John, N.B. has 10 beds for a population of 70,063.

Residential hospices are relatively new across Canada, but palliative care experts say they are a crucial option for patients who do not need to be in hospital and cannot die at home. They combine a home-like environment with 24-hour medical care and psychological support.

There are about 193 palliative care beds in Toronto hospitals, but most have a vacancy rate of close to zero and limit stays to 15 days. Patients can stay in a residential hospice for three months or longer.

As for home care, Toronto Central Local Health Integration Network (LHIN) funds three visiting hospice programs. But for patients with complex needs or without a family caregiver, it is only a short-term option.

The vast majority of Canadians say they want to die at home or in a hospice, but about 70 per cent die in hospitals. In the 2011 fiscal year, nearly 5,000 people died in an acute-care hospital in Ontario while receiving palliative care.

When there are no beds available. the patient generally ends up in a higher-cost setting, said Dr. James Downar, a palliative care physician at Toronto General Hospital. Acute-care beds are the most expensive setting in the entire system.

The daily cost of an acute-care bed in a hospital is between $850 and $1,100, compared with about $439 for a residential hospice bed.

Hospice patients receive care free-of-charge, but advocates say the savings are significant for a province trying to balance its books and hospices aren t just more efficient, they re more humane.

Coline Bettson s mother suffered a sudden brain aneurysm in March and moved into Perram House. The first night, nurses brought Bettson hot tea and set up the bed so she could cradle her 96-year-old mom in her arms.

Everything I asked for and she wanted, they agreed to. They couldn t have been more kind and caring, she said. I was stunned and appalled when I heard (the hospice closed).

In 2005, the Ministry of Health announced $115 million in palliative-care funding, including plans to fund the care costs of 34 residential hospices across the province.

Eight years later, 24 of those hospices have been built and receive annual funding of $90,000 per bed. The remainder of hospice budgets between 20 and 50 per cent must be raised through donations.

McCrea, the former Perram House board chair, said fundraising had always been a struggle.

New Democrat health critic France G linas said the ministry should fully fund residential hospices, as it does long-term care.

It s been unacceptable from the beginning, she said. Hospices deal with vulnerable people in their last days of life. It s wrong to let them rely on donations.

Health Minister Deb Matthews said in an interview that the province had come a long way since before 2005, when hospices were entirely self-funded.

We are transforming how we deliver health care, Matthews said. We are committed to investing more in the community sector and getting people out of hospital who don t need to be in hospital.

She said the health ministry dedicated $260 million to community services, including hospice and home care, in the 2013 budget. The Toronto Central LHIN has convened a working group on palliative care access.

Rick Firth of Hospice Palliative Care Ontario said most hospices are able to fundraise successfully, but Perram House may have struggled because it prioritized marginalized and homeless people.

The donations from families don t come in at the same volume, he said. That may be something we need to look at in the future.

Firth said the loss of Perram House will be felt in Toronto for the time being. Two of the hospices promised funding in 2005 were supposed to be in the city, but neither has broken ground yet.

The Toronto Commandery Hospice finally acquired land last year in North York, but is awaiting approval of its zoning application. The Yee Hong Centre for Geriatric Care in Scarborough still has no timeline for opening a hospice.

But Emily s House a long-planned pediatric hospice at Gerrard St. E. and Broadview Ave. is nearing the end of construction at last and hopes to open its doors at the end of May.

As for Perram House, the stunning home of sweeping staircases and vaulted ceilings built by widower Mary Perram in 1887, it now occupies prime downtown real estate, on Wellesley Pl. between Jarvis and Sherbourne Sts.

McCrea owns the home but has said he hasn t made a decision about the property. As a designated heritage home, it would require city council approval to be demolished.

Pete Perram, Mary s 52-year-old great-great-grandson, said the worst-case scenario would be for it to be turned into a condo building.

Mary intended it to be for the good of the people, the good of the community, he said. I hope that, as a legacy to her and to our family, that continues.

The Seasons Hospice House

Nestled on 13 acres of beautiful woodlands on the southwestern edge of rural Rochester, MN, the Seasons Hospice House is calming and homey. Each of the eight private patient rooms has a lovely view of the expansive green lawn surrounded by pine, oak and maple trees. A large deck overlooking rose and flower gardens is available for patients and families to spend precious time together surrounded by nature’s beauty.

The common areas of the house are large and open. A family room and kitchenette are available for visiting families and patients.

How do we pay for care at the Seasons Hospice House?

Payment for residential hospice care varies for each person. Some private insurance companies provide for all or part of the care. The Seasons Hospice staff can help patients and family determine payment specifics as they relate to their particular situation. For patients that lack a payer source, there is a Patient Care Grant and Veterans Care Grant that may be available to offset expenses based on pre-determined criteria.

What services are available to patients residing at the Hospice House?

Residential hospice services include a private room, accessible bath facilities, 24-hour nursing care. meals and snacks as desired, medication administration. assistance with personal care, nurse call system, telephone service, cable television and wireless internet access. Hospice House patients also have access to services provided by Seasons Hospice social workers. music therapy. massage therapy. pet therapy. chaplain services. bereavement support. and volunteers who provide hospitality, cooking, and patient visits. Inpatient Care and Respite Care

Inpatient care is provided on a short-term basis for acute intervention to manage symptoms, allowing the patient to return to their place of residence when desired. This care is generally provided at the Seasons Hospice House.

Respite care is provided through Seasons Hospice to give the caregiver a needed rest or break from their caregiving tasks. Respite care is provided at the Hospice House.

Nursing Care

Registered nurses provide individualized care to meet the needs of each patient. The care is focused on pain and symptom management, promoting independence and offering choices regarding that care. The patient remains in control of their care and directs that care for as long as possible.

Nurses provide routine personal care, assistance with medications and treatment procedures and support of the patient and family.

Physician Services

The Seasons Hospice medical director visits patients to asses their condition and manage their level of comfort and medications. The patient’s personal physician remains involved in the management of the patient’s plan of care.

Social Services

Licensed social workers can provide psychological and emotional support for the hospice patient and their family and provide referral services for community programs and resources.

Medications

Seasons Hospice provides medications necessary to manage symptoms associated with the terminal illness. The goal of hospice care is to keep the patient as comfortable as possible throughout the last months of life. Medication needs are addressed by the hospice team and the physician.

Chaplain Services

Our staff chaplain offers spiritual support for the patient and their loved ones as requested, 24 hours a day, seven days a week.

Massage Therapy

Massage therapy specializes in the needs of the seriously ill by offering massage services to hospice patients at the Seasons Hospice House. Massage may be helpful in aiding relaxation, reducing pain, anxiety and other physical and emotional symptoms. It can create an overall feeling of well-being.

Music Therapy

Music therapy is the use of music within a therapeutic relationship to address a patient’s physical, emotional, cognitive, and social needs. It is highly personalized and the focus may be on the patient’s physical state, their emotional or spiritual concerns, or on supporting family or social gatherings.

Pet Therapy

Seasons Hospice’s therapy dog program serves to provide comfort, support, love, and animal companionship to patients, their families, and staff through the use of highly trained therapy dog teams acting in a volunteer capacity.

Registered therapy dog teams comprised of a trained therapy dog and trained volunteer handler, will visit hospice patients at the Seasons Hospice House upon a referral/recommendation from Seasons Hospice staff and permission from patient and/or family.

Bereavement Support

Bereavement support is available at the Hospice House to help patients and families understand and manage their experiences of loss, change, grief, and mourning while providing support and hope.

Continuing Care Reporting System

The Continuing Care Reporting System (CCRS) contains demographic, clinical, functional and utilization information. This data refers to individuals receiving continuing care services in hospitals or long-term care homes in Canada. Participating organizations also provide information on facility characteristics to support comparative reporting and benchmarking.

The clinical data standard for CCRS was developed by interRAI. an international research network. It was modified with permission by CIHI for Canadian use. Clinicians assess their residents using interRAI’s Resident Assessment Instrument–Minimum Data Set (RAI-MDS 2.0)©.

The information is entered into a computer at the point of care. Real-time reports flag resident risks and inform care planning. The information then flows to CIHI. With no additional collection effort, evidence is available to guide care and system-wide planning as well as quality improvement.

The Seasons Hospice House

Nestled on 13 acres of beautiful woodlands on the southwestern edge of rural Rochester, MN, the Seasons Hospice House is calming and homey. Each of the eight private patient rooms has a lovely view of the expansive green lawn surrounded by pine, oak and maple trees. A large deck overlooking rose and flower gardens is available for patients and families to spend precious time together surrounded by nature’s beauty.

The common areas of the house are large and open. A family room and kitchenette are available for visiting families and patients.

How do we pay for care at the Seasons Hospice House?

Payment for residential hospice care varies for each person. Some private insurance companies provide for all or part of the care. The Seasons Hospice staff can help patients and family determine payment specifics as they relate to their particular situation. For patients that lack a payer source, there is a Patient Care Grant and Veterans Care Grant that may be available to offset expenses based on pre-determined criteria.

What services are available to patients residing at the Hospice House?

Residential hospice services include a private room, accessible bath facilities, 24-hour nursing care. meals and snacks as desired, medication administration. assistance with personal care, nurse call system, telephone service, cable television and wireless internet access. Hospice House patients also have access to services provided by Seasons Hospice social workers. music therapy. massage therapy. pet therapy. chaplain services. bereavement support. and volunteers who provide hospitality, cooking, and patient visits. The Seasons Hospice medical director regularly visits patients at the Hospice House.

Inpatient Care and Respite Care

Inpatient care is provided on a short-term basis for acute intervention to manage symptoms, allowing the patient to return to their place of residence when desired. This care is generally provided at the Seasons Hospice House.

Respite care is provided through Seasons Hospice to give the caregiver a needed rest or break from their caregiving tasks. Respite care is provided at the Hospice House.

Nursing Care

Registered nurses provide individualized care to meet the needs of each patient. The care is focused on pain and symptom management, promoting independence and offering choices regarding that care. The patient remains in control of their care and directs that care for as long as possible.

Nurses provide routine personal care, assistance with medications and treatment procedures and support of the patient and family.

Physician Services

The Seasons Hospice medical director visits patients to asses their condition and manage their level of comfort and medications. The patient’s personal physician remains involved in the management of the patient’s plan of care.

Social Services

Licensed social workers can provide psychological and emotional support for the hospice patient and their family and provide referral services for community programs and resources.

Medications

Seasons Hospice provides medications necessary to manage symptoms associated with the terminal illness. The goal of hospice care is to keep the patient as comfortable as possible throughout the last months of life. Medication needs are addressed by the hospice team and the physician.

Chaplain Services

Our staff chaplain offers spiritual support for the patient and their loved ones as requested, 24 hours a day, seven days a week.

Massage Therapy

Massage therapy specializes in the needs of the seriously ill by offering massage services to hospice patients at the Seasons Hospice House. Massage may be helpful in aiding relaxation, reducing pain, anxiety and other physical and emotional symptoms. It can create an overall feeling of well-being.

Music Therapy

Music therapy is the use of music within a therapeutic relationship to address a patient’s physical, emotional, cognitive, and social needs. It is highly personalized and the focus may be on the patient’s physical state, their emotional or spiritual concerns, or on supporting family or social gatherings.

Pet Therapy

Seasons Hospice’s therapy dog program serves to provide comfort, support, love, and animal companionship to patients, their families, and staff through the use of highly trained therapy dog teams acting in a volunteer capacity.

Registered therapy dog teams comprised of a trained therapy dog and trained volunteer handler, will visit hospice patients at the Seasons Hospice House upon a referral/recommendation from Seasons Hospice staff and permission from patient and/or family.

Bereavement Support

Bereavement support is available at the Hospice House to help patients and families understand and manage their experiences of loss, change, grief, and mourning while providing support and hope.
