Planning Resources by Setting

Long-term, Acute, and Chronic Care:

Individuals living in long-term, acute, and chronic care settings are among the most vulnerable populations during a public health emergency. Therefore, it is important for caregivers and planners who care for persons residing in these settings to be prepared and know how to respond in the midst of an emergency. The following resources are intended to provide guidance and assistance for long-term, acute, and chronic care facilities on how to plan for public health emergencies.

Healthcare Preparedness Activity Tools

  • Long-Term, Home Health, and Hospice Care Planning Guide for Public Health Emergencies
The Long-Term, Home Health, and Hospice Care Planning Guide for Public Health Emergencies was developed to help improve the planning efforts of these healthcare entities and their coordination and integration within the community. This Planning Guide focuses on six topic areas: situational awareness, continuity of operations, facility or agency operations, crisis standards of care, staffing, and fatality management. Each topic area is presented in a manner that allows the user to develop needed sections in a facility or agency’s emergency preparedness and response plan.

Other Tools

  • AHRQ Emergency Preparedness Atlas: U.S. Nursing Home and Hospital Facilities
This Atlas was designed by AHRQ to support state and local planning and response efforts for a bioterrorism or other public health emergency. It is intended to stimulate discussion about how nursing homes may be used to handle a surge of patients in response to emergency situations.
  • Emergency Planning Checklist (80 KB/6 pages)
CMS developed this tool for persons in long-term care facilities and their family members, friends, personal caregivers, guardians, and long-term care ombudsmen.
  • Emergency Preparedness Checklist for Long Term Care Homes (74 KB/5 pages)
This checklist was developed by the Survey Certification Division of CMS to evaluate current preparedness plans for every emergency.
  • Long-Term Care and Other Residential Facilities Pandemic Influenza Planning Checklist
This checklist developed by HHS and CDC helps long-term care and other residential facilities assess and improve their preparedness for responding to pandemic influenza. This tool identifies key areas for pandemic influenza and can be used to self-assess the strengths and weaknesses of current planning efforts.
  • Nursing Homes in Public Health Emergencies: Special Needs and Potential Roles and Appendix B: Model Long-term Care Preparedness Needs Assessment
This AHRQ assessment tool provides background about the preparedness level of nursing home administrators and staff on disaster plans, including bioterrorism response. The findings in this document assess the special needs of the older adult population in nursing home settings during a public health emergency.
  • Pandemic Influenza Workbook for Long Term Care Providers (1.71 MB/92 pages)
The California Association of Health Facilities provided this workbook as guidance to long-term care providers and to explain what providers should consider in preparing and making preparations for an influenza pandemic.

Reports and Guidance

  • AARP Tips to Help Older Americans Prepare for Emergencies
AARP has compiled a list of resources to aid caregivers of older adults with preparedness planning.
  • Caring for Vulnerable Elders during a Disaster (215 KB/62 pages)
This report is provided by the Florida Health Care Association looking at ways state and local leaders can keep vulnerable elders and other persons with disabilities residing in long-term care facilities safe during a disaster.
  • Emergency Planning for Long Term Care / Seniors Nursing Home Residential-Care-Facilities
This guide was developed by OnWellness. info’s to provide senior and long-term care facilities with an overview of risk factors and practical contingency plans.
  • Just In Case: Emergency Readiness for Older Adults and Caregivers
This is a supplement to Aging in Stride, developed by the Administration of Aging within HHS. It describes a three step approach for older adults and caregivers to improve their personal emergency readiness and includes an Emergency Readiness Checklist for Older Adults and Caregivers.
  • Kidney Community Emergency Response Coalition
This website provides patients, providers, and public health emergency coordinators insight into emergency and disaster preparedness for the entire end-stage renal disease community.
  • Preventing and Controlling Influenza Transmission in Long-term Care Facilities
A list of Ggeneral guidance resources provided by the CDC for the prevention and control of influenza transmission in healthcare facilities, including specific guidance for long-term care facilities.

Representative APR Range

LoanSolo.com is not a lender and does not provide short term or online personal loans but refers consumers to the lenders who may provide such loans. LoanSolo.com is unable to supply you with an exact APR (Annual Percentage Rate) that you will be charged if you are approved for a loan. APRs depend on multiple factors, including the lender’s requirements and offers, your credit worthiness, your state of residence, and the type and term of the loan you request. You will be given the details on the APR, loan fees, and other terms by your lender when you are redirected to your loan agreement during the loan request process.

The APR on a short term loan can range from 200% to 2,290% depending on how the APR is calculated (nominal vs. effective), the duration of the loan, loan fees incurred, late payment fees, non-payment fees, loan renewal actions, and other factors. Keep in mind that the APR range is not your finance charge and your finance charge will be disclosed later on. See a Representative Example

Borrow $200 for 14 days with a $30 to $60 lender fee. Your estimated APR is 391% to 782%
Calculation: (lender fee / loan amount) x (amount of days in a year / duration of the loan) x 100
Low End of Range: ($30 / $200) * (365 days / 14 days) x 100 = 391.07%
High End of Range: ($60 / $200) * (365 days / 14 days) x 100 = 782.14%
Payday loans are relatively expensive when compared with other loan products. Payday loans are not recommended as a long term financial solution and they should only be taken for emergency financial needs.

The APR on online personal loans can reach up to 1,386% depending on the lender’s terms, borrower’s residence state, the duration of the loan, loan fees incurred, late payment fees, non-payment fees, loan extension plan, and other factors. Some states cap APR and the amounts of charges for online personal loan.

Implications of Late Payment

You are encouraged to contact your lender as soon as possible if you are unable to repay your loan on the scheduled repayment date or make regular installment payment on time. Your lender may set late payment fees in accordance with state regulations, and your lender has several courses of action to deal with late payment. For more information about your lender’s specific procedures as they apply to late payments, please review your loan agreement or contact your lender directly. Origination, documentation and other additional fees may apply to your online personal loan deal. Late payment, non-payment fees and other penalties may apply to both short term and online personal loan deals in case of missed payments. Please review your loan agreement carefully for information about the financial implications before you provide your electronic signature.

Implications of Non-Payment

The costs associated with loans of up to $500 can range from 15% to 40% of the entire loan amount, and the charges associated with loans of more than $500 can be even more. Your lender may also charge you late fees as well as fees for non-sufficient funds. As an example, your lender may charge you a $20 nonsufficient funds fee as well as 15% of the loan balance as a late fee. Please review your loan agreement carefully for information about the financial implications of non-payment before you provide your electronic signature.

Non-payment may involve debt collection practices as it is set by applicable law. The majority of the lenders in our network do not intend to sell your debt to outside collection agencies. Instead, they will attempt to collect the debt in-house via telephone, email, postal mail or even text message. Similarly, they will not threaten criminal charges or sue borrowers; they will generally offer debt settlements over time instead. Every lender in our network is required to adhere to the Fair Debt Collection Practices Act, which protects consumers from being abused or harassed by debt collectors.

Impact on Credit Score +

Lenders are within their rights to report your failure to repay a loan to one or all of the major credit reporting agencies — Experian, Equifax and Transunion. This negative record can be reflected on your credit history indefinitely until the loan is repaid in full. After the lenders receive payment in full, they can report it to the credit reporting agencies. We remind that late payment or non-payment of your loan can have negative impact on your credit history.

Some of the lenders in our network may automatically renew your short term loan if it becomes past due. This term is clearly identified in your loan agreement. You should check your loan agreement for your lender’s policy on automatic loan renewal prior to e-signing it. If your loan is renewed, there will be additional charges as determined by your lender, and the minimum term can be set up. Your lender may offer you other options in addition to renewal, including the ability to repay your loan in full at a later date or repay your loan over time in a series of installments.

Not all the lenders in our network can provide up to $1,000 in payday loans and $3,000 in online personal loans. The limits and regulations vary from state to state. Cash transfer times and terms may vary from lender to lender.

Consumer Notice: We remind that payday loans and online personal loans for small dollar amounts are not a long term financial solution. These credit options imply relatively high interest rates and become a very costly solution if used improperly. Term life insurance is a simple and affordable way to ensure your beneficiary financial security in the event that you die within the period of, or term of the policy. The term of the policy can vary from one to thirty years of coverage. The face amounts stay level throughout the term and generally begin at $25,000 and increase to meet your specific needs.

Types of Life Insurance We Sell:

Term life insurance is affordable because it does not accrue a cash value and only pays the death benefit. Therefore, the initial premium is much lower, allowing you to easily budget for your coverage. This option offers great value to an individual that needs a death benefit for a specific period of time.

For example, term life insurance is a good choice for people that have started a family and need to consider paying for college, have amortized loans, or have a specific financial burden that will decrease over time. After the kids graduate from college, the amortized loans are paid down, or the specific financial burden has decreased, the need for cost effective high levels of coverage may not be as great. At this time your options vary.

Term life insurance policies (10 year, 20 year, 30 year) can usually be renewed at the end of the term. Perhaps at this point in your life a shorter term and smaller face value will adequately meet your needs.

Keep in mind that there is a direct correlation between age, health and premium costs. Subsequently, with advanced age and the health issues associated with age, your premiums will probably be higher and eligibility is not as easy. You may want to consider converting to a permanent life insurance policy at this time.

Long Term Disability Claims – Aaron Waxman Law #long #term #disability #insurance

Long-Term Disability Claims

Many employers offer long-term disability benefits to employees as part of their group benefit packages. Long-term disability benefits provide income replacement to those who are unable to work for an indefinite period of time due to an illness or injury. While every long-term disability policy is different, all policies require that an insured person satisfy a qualifying period (also known as an elimination period or waiting period), submit a complete application, provide updated medical application, participate in treatment/rehabilitation and be subject to a change of definition where the insurer s definition of disability changes from own occupation to any occupation.

The amount of your long-term disability benefit will depend on your policy. Benefit amounts can be equivalent to 50%-85% of your monthly income. Depending on your policy, your benefit may or not be taxable.

Long-term disability policies are complex and contain many provisions. Letters from the insurance company may contain terms that are ambiguous or unclear. A lawyer can help you to understand your insurance policy and any letters from your insurer.

If your application for long- term disability benefits has been denied, or your benefits have been terminated, you should not hesitate to seek legal advice. It is always important to understand your rights and your options with respect to a denial of benefits and to know that you can pursue a court action against your insurer.

The lawyers at Aaron Waxman and Associates have handled many claims for long-term disability benefit denials and helped many clients achieve the settlements they deserve.

If you have a question about long-term disability benefits, call or e-mail Aaron Waxman and Associates – there is no charge to call us and there are no bad questions.

Long-Term Disability Benefits for Fibromyalgia #long #term #disability #insurance

Long-Term Disability Benefits for Fibromyalgia

Long-term disability (LTD) insurance companies routinely deny or limit claims for long-term disability benefits based on fibromyalgia, a complex illness marked by chronic muscle and joint pain. Because there is no objective test for fibromyalgia and diagnosis is based largely on self-reported symptoms, many insurers specifically exclude fibromyalgia from coverage. Other carriers consider fibromyalgia primarily a mental disorder and will limit payments to 12 or 24 months.

Read your policy carefully to understand whether and for how long you can receive benefits based on fibromyalgia. The following language limiting eligibility is fairly typical.

Benefits will be terminated after 24 months for those with disabilities which are based primarily on self-reported symptoms, and disabilities due to alcoholism, drug abuse, or mental illness. Self-reported symptoms include manifestations of your condition that are not able to be verified using tests, procedures, or examinations commonly accepted in the practice of medicine, including headaches, pain, fatigue, soreness, numbness, dizziness, ringing in the ears, and loss of energy.

Even if your policy doesn’t specifically mention fibromyalgia as being excluded or limited, you should expect to meet substantial resistance if you file for LTD benefits based on fibromyalgia. Here are a few things to remember that could increase your chances of a successful claim.

Seek Appropriate Medical Treatment

Insurance companies are well aware that primary care physicians frequently diagnose fibromyalgia as a sort of “catch-all” when they cannot pinpoint the source of an individual’s pain. If you want your insurer to take your fibromyalgia claim seriously, it’s critical that you receive regular treatment from a rheumatologist, not just your family physician. Rheumatologists typically employ several different diagnostic techniques when evaluating a patient for fibromyalgia, including blood tests to rule out other illnesses and trigger point tests to locate areas of pain and tenderness. Generally an individual who reports pain in at least 11 of the 18 trigger points will receive a diagnosis of fibromyalgia. (Also see our article on getting Social Security for fibromyalgia. which discusses criteria used by the American College of Rheumatology (ACR) to determine when a patient has fibromyalgia.)

In addition to making a more accurate and credible diagnosis, a rheumatologist may be better able to prescribe therapies for your particular symptoms. Although there is no cure for fibromyalgia, it is often possible to control the pain and accompanying fatigue through medication, exercise, and a proper diet.

Record All Your Symptoms

In addition to chronic pain, many fibromyalgia sufferers also experience headaches, numbness, swelling, fatigue, IBS, and difficulty sleeping. Fibromyalgia may also adversely affect a person’s mental health, causing cognitive problems (often called “fibro fog”), depression, stress, and anxiety. Keeping a diary of all your symptoms, both mental and physical, will help your physicians diagnose and treat your illness, and may help to persuade your insurer that your condition is legitimate.

Obtain Opinions From Your Doctors

The opinion of your treating physician can literally make or break your LTD case, and it’s especially important in cases involving fibromyalgia, chronic fatigue syndrome, and other conditions that don’t lend themselves to objective testing. You or your attorney should ask your physician (preferably your rheumatologist) to identify the extent of your work-related limitations, especially in the following areas:

  • length of time you can sit, stand, and walk
  • amount of weight you can lift and carry, and
  • ability to bend, stoop, balance, crouch, and crawl.

Also, would you miss days of work or have to take unscheduled breaks during the workday due to pain, fatigue, or other issues? Would you need to recline or lie down during the day? Your doctor should be asked about any limitations that might prevent you from performing the demands of full-time work. It is also be helpful for your rheumatologist to explain what diagnostic testing was performed and why she ultimately believed a fibromyalgia diagnosis was supported by the medical evidence.

If you’re receiving mental health treatment for depression, also ask your doctor or psychologist for an opinion as to your mental limitations, including your ability to maintain regular attendance, sustain attention and concentration, understand and carry out instructions, and interact with supervisors, co-workers, and the general public.

Your lawyer should be able to supply a fibromyalgia assessment form for your doctor that encompasses all your possible limitations. Do not rely on the forms used by your insurance company, as they are often designed to elicit responses that could form the basis for a denial.

Reports from third-parties such as friends, family members, and former colleagues can also help bolster an LTD disability claim, but only if they’re limited to first-hand observations of your condition rather than opinions about your medical issues.

Pursue Your Claim With the Help of an Experienced Lawyer

Consumers that seek funds for longer terms or debt relief should consult credit advisor prior to making credit decisions of taking a short term or an online personal loan for small dollar amount.

Find rate of return on whole life insurance

When it comes to sorting the good whole life insurance policies from the bad, consumers focus on one number alone how much money their policy will generate.

To project how much money your policy will yield 10, 20 or even 30 years from now, insurance companies calculate the policy s expected yearly growth. Better known as the internal rate of return, your IRR will dramatically change the longer you own the policy and will be a crucial factor for deciding whether it s smarter to hold on to your policy or shop around for a new one.

Unfortunately, figuring out how much money your whole life policy is yielding now and what it will yield in the future is far more difficult than simply reading your policy s fine print. Here are some tips for determining how much cash your cash value policy is really providing.

Don t do it alone

Figuring out the rate of return on your whole life policy is almost impossible for consumers to do without help from the pros, says Tony Steuer, a life insurance analyst in Alameda, Calif. and author of Questions and Answers on Life Insurance.

There are tons of moving parts to a lot of life insurance contracts … (and) insurance companies usually don t disclose certain information to their customers, like if they ve had a change in mortality rates, he says.

Steuer adds that the method for calculating the rate of return on whole life insurance policies varies from company to company. While all companies factor in certain variables including how long you ve owned the policy, dividends (if applicable) and mortality charges, how much each factor counts for varies significantly between insurance providers and policies.

Approach projections with caution

While whole life providers frequently won t disclose how your rate of return is calculated, they will offer illustrations of how your policy is projected to perform in the future. All illustrations should include information on the costs of the policy, how much you ve paid, your current death benefit and the cash surrender value you would get for canceling the policy today. It should also include future predictions on how your policy should perform five, 10 and 20 years down the road. The bad news is that insurance illustrations are lengthy, frequently riddled with tough-to-understand industry jargon and could include predictions so outrageously optimistic, there s no way your policy will actually reach them.

(Insurance illustrations) include a guaranteed column, says Kelly O Connor, managing agent with Mountain Financial, LLC, a wealth management group in Greenwood Village, Colo. This explains what the policy is guaranteed to look like over a certain time frame. (Consumers) can trust that guaranteed piece of the illustration.

In certain whole life contracts, O Connor says that consumers can get an idea of how close other projections are by examining the company s dividend payout history.

All mutual insurance companies publish their dividend history, so if you look back and see that they ve never been under 5.5 or 5.25 (percent dividends) for the last 100 years, there s a pretty good chance that that will continue, he says.

Look ahead

Cash value life insurance policies are designed to be long-term investments, not short-term ways to make a quick buck, says Glenn Daily, a Certified Financial Planner and fee-only insurance consultant based in New York City. To understand the rate of return for your policy, check out how the policy is projected to perform over the next few decades.

If you re thinking of buying a new policy, You need to see the average annual and year-by-year rate of return for at least the first 20 years, says Daily. Those who already own policies should focus on the average annual and year-by-year rates of return for the next five to 10 years. By examining how their whole life policy is performing on a yearly basis, consumers get a firm idea of what kind of performance they can expect in the near future.

Consumers should know that the year-by-year rate of return will substantially differ from the rate of return generated over several years, says James H. Hunt, a life insurance actuary for the Consumer Federation of America and former insurance commissioner of Vermont.

It may take as long as 10 years for the average annual rate of return on a cash value life insurance policy to turn positive, mainly due to heavy first year commissions and related sales expenses, he says.

When comparing policies or deciding if it s worthwhile to stick with the one you have, Hunt recommends hiring a third-party insurance analyst to evaluate whether a whole life policy makes sense in the long haul.

It s impossible to know what you re getting into without hiring an expert, he says.

  • I thank CADABAM’S who have really given me a new life. From being beaten by my husband and taking care of whole family and children he is back as a very responsible person understanding his responsibilities and quitting his habits completely.
  • It was a nightmare worrying about my mother who had depression and was all alone in India while I was in UK with my work and family. Its almost 2 years now that she is at CADABAM’S being very happy and taken care. I thank whole CADABAM’S Team for giving my mother a new life.
  • It was a rude shock to find our son with GANJA abuse and alcohol intake. Though there was gradual change in his behaviour we never could understand that it was because of his habbits. Just few months of treatment and support of counsellors and good medication he is back to studies and doing well. I thank doctors and counsellors for all the help and support.

How to Cancel Life Insurance: 13 Steps (with Pictures) #cancel #term #life

#

How to Cancel Life Insurance

There are many reasons why you many need to cancel life insurance for yourself or a loved one. You may no longer have the funds to pay for the policy. You may want to cash out the money that you have put into the policy, or you may want to cancel a policy with one company so that you can purchase a new policy with a different company. Whatever the reason, cancelling your policy is an easy business transaction that can be handled simply and professionally.

Steps Edit

Part One of Three:
Researching How to Cancel Edit

Visit the company’s website. If you have a policy with a large national or regional insurer, then it should have a website. Visit the company’s website and see if they have information about cancellation.

  • You can find a website address on any piece of correspondence that the insurer has sent you. Or you can search on the web: type the name of your insurer into your favorite web browser.
  • Look in the Frequently Asked Questions sections to see if there is information about who to contact.

Contact a broker. Before cancelling your life insurance policy, you should contact an experienced broker to ask any questions you may have about your decision to cancel. Even if you know for sure that you are going to cancel your policy, it is good to have someone to talk to about your choice who is an expert in life insurance issues.

  • If you do not have a broker already, contact your life insurance company to set you up with someone. Finding a broker through your insurance company will insure that you work with a broker who is properly licensed and who is recommended by experts.

Meet with an accountant. There can be tax consequences of cancelling a policy. Whole and universal life insurance policies build up cash value through excess premiums and earnings. When you cancel, the insurance company cuts you a check for this amount. [1] This money may or may not be taxed.

  • Only an experienced tax professional can answer questions about the tax consequences of cancelling life insurance.

Research partial withdrawal policies. If you have a universal life insurance policy, you may be able to withdraw some of the cash value from your policy without cancelling out of the policy altogether. [2] Doing so would free up some of the cash for you to use, but you would still have money in the policy in the event of your death.

  • However, taking a “loan” from your life insurance policy can have consequences, so make sure to speak with your broker before taking this option.
  • For example, some policies will reduce the amount of the death benefit and will not allow you to repay the loan to raise it again.
  • Additionally, in some circumstances, you could pay a tax on the amount that you take out of the policy.

Ask about the consequences of cancelling. You may have to pay a cancellation fee or penalty. [3] Also, the policy may state that some of the money that has accumulated in the insurance policy may be returned to you.

  • Look at your life insurance policy. It should include a clause detailing the cancellation fee or penalty. If the policy will return money to you upon cancellation, then the fee or penalty will be deducted from it. [4]
  • Some newer life insurance policies include a cancellation clause in which the penalty decreases over the life of the policy. In other words, the longer you have had the policy the lower your penalty will be. [5]

Try to keep the policy. If you want to cancel because of a temporary job loss or because your employer has stopped paying premiums, then consider if there is any way to continue making premium payments. You could pull money from other parts of your budget or get a part-time job.

  • Also talk with your insurance company. Some companies might let you reduce the face amount of the policy, thus lowering your premiums. Also, you may be able to use some of the plan’s cash value to cover premiums. [6]

Start early. If you intend to have a more affordable policy, then you should have a second policy in place before you cancel your first one.

  • The amount of time to process a life insurance application varies from 15 minutes to 2 months. [10] You should give yourself plenty of time.
  • If you have to undergo a medical exam or submit medical records, then you should account for that as well.
  • Getting a complete set of medical records can be cumbersome as offices are overwhelmed with requests for copies of records. [11]

Get quotes. If you cancelled the insurance policy because it was too expensive but still want coverage, then you will need to find affordable insurance. You should contact a broker or visit the websites for major insurers.

  • You can look into “term” life insurance. Term life insurance often has lower premiums because it is in effect for only a specific amount of time, during which you pay the premiums, whereas whole life insurance is in effect for the entirety of your life, even after the premiums have been paid. [12]
  • Term life insurance is recommended for those under 40 or those who do not have sufficient income to pay for whole life insurance. [13]

Apply for a policy. Gather the paperwork and fill it out. Keep a copy for your records and mail the original back.

  • Also schedule any required medical exam and request medical records.
  • You should receive confirmation within a month. [14]

Even Hotels, a new idea in hotels

Totally different from ANY hotel that I have ever stayed in. I called it a new idea because I had never seen a hotel that focuses so much on wellness. The food in they serve and sell is focused on [More] your health. They have FREE filtered water and ice in the lobby and all on all the EVEN floors. The rooms have an open space cork floors, an exercise ball, a workout station with elastic bands and ropes. The TV defaults to channels showing exercise techniques to use these devices. AND if you want to use the gym, they have a very large two story area with weights and all the latest tech in exercise stuff. OK, I said all that but I never look for these things in a hotel. I am a rotund person who rarely uses exercise equipment. The hotel suits the needs for a good, fun, friendly stay near major attractions (like a short walk to Barclay Center). The TV had many more channels than i see at most hotels including all the normal favorites. The TV was big and was mounted on a a flex arm so it could be adjusted for almost any angle. I loved the hotel, the rooms, the beds were comfortable, the filtered water was great and the bathroom and showers were fantastic. AND THE STAFF. GREAT. Friendly, fun and helpful. There is a large mall across from Barclays Center, and IHOP and CVS the next block over, a Shake Shack near and tons of local food nearby. The lobby is setup up to read or meet. They have comfortable chairs, couches, books, games and work area with new Apple Macs and printers. There is a cute enclosed garden area right off the lobby with comfy benches and flowers. The Brooklyn EVEN hotel had just opened in July but my grandson and I arrived for our stay in the last week of August and things were running well. Why is it called EVEN Hotels? Who knows. BUT they made the theme so they have only Even number rooms and the free water/ice stations are on the Even floors too. Love the place. I just hope that it doesn’t get so popular that the prices go way up. [Less]

Our first time at an Even hotel and we were impressed. Rooms are large by the metro area standards and were fresh. It was a good idea to have a partial rug around the bed with a cork-like flooring [More] for the rest of the room, which added to the clean lines. Work station is nice for office/business work you may have. The in room fitness equipment allows for a private in room morning stretch before taking a shower. Lobby is comfortablle and restaurant is nice for a light meal or late evening snack at the bar. [Less]

Convenient and Energetic Hotel in Downtown Bklyn

By Traveling NoVA

Hotel is well situated for restaurant row on Smith Street and near Barclay Center for sports events. It is also a short cab ride or long walk to DUMBO! Exercise equipment in room is great for [More] core workouts and there is a.TRX in the gym for those who use it. The ginger/lime water in the lobby was delicious and encouraged hydration. Breakfast in Kork and Kale was a little chaotic. Food was delicious with some clever healthy and tasty twists in the ingredients. [Less]

Really Beautiful Boutique

The receptionists Breun and Shakira were very friendly. The hotel staff greeted us with a glass of champagne and a nice welcome gift. The supervisor Sue was superb. She gifted me with a cheesecake [More] from Juniors on behalf of her and the hotel staff for my birthday. I loved the staff. They were friendly and down to earth. I would highly recommend. The hotel is located in Downtown Brooklyn, making it accessible to all of the amazing attractions that Brooklyn has to offer. [Less]

This is a great hotel. Kortney (sp) is so awesome! He was always friendly and informative. He helped us repeatedly. The hotel is nice, clean, very quick to respond when we needed something for our [More] room. I would love to see K-cup coffee machines in the IHG family. I will not recommend using their 3rd party parking garage. We were told at chick in that the garage was indoor, it is not. My car came back filthy, dried plywood on the hood, it had to be peeled off, a scratch on the passenger mirror where a car door hit it, a ding-dent on cabin, dried rusty water on paint, minor scratches on paint, scratches on door as if it hit something when opened. [Less]

Clean and good service

Clean and comfortable room. Informative front desk staff. [Less]

Conveniently located and good value

Candlewood Suites is a good value hotel centrally located near Times Square. Excellent staff, clean accommodations and good internet connectivity. The only drawback is that my room did not have a [More] closet, which was a bit of a challenge, given that I was in NY on business and had clothing that needed to be hung up. I ended up using the bathroom shower curtain bar and that worked reasonably well. [Less]

Good family experience

A great place for a family gathering stay. The suite was spacious and the whole family enjoyed the pool. Breakfast was very good. The kitchenette in the suite was well stocked and helpful. [Less]

This is our second stay at the Staybridge Suites for baseball tournaments. Our experiences here have been more than wonderful. Friendly staff, ease and convenience of group reservation and rates, [More] comfortable, clean, great amenities. The location is also superb, right across WestGate Center with lots of shopping, places to eat, things to do. like movie theater and Dave and Busters to name a few. Close to the neigboring baseball locations too! [Less]

very clean.well kept and friendly.a great place! will return if in area again will give two thumbs up [Less]

My suite of rooms was extremely comfortable and quiet even though we were next to a busy street. The lounge area in the lobby was nice and we all enjoyed ending the day over a glass of wine before [More] dinner. I would definitely like to stay here again! [Less]

Great location and service

The staff was very considerate and helpful. This was a combination leisure trip and medical visit. Staying at the Holiday Inn Express made it very enjoyable. [Less]

Pool is great. Breakfast is OK. Staff is wonderful. A/C wasn’t great in our 3rd floor (top floor) room, since it was well over 100 F every day. It only cooled the bedroom, not the living room / [More] bathroom areas. [Less]

Great Choice for the Price

Newly remodeled hotel (still some going on I guess). Rooms are nice, easy to book and check-in and out is easy. Only downfall is the lack of great food options within walking distance. [Less]

Newly renovated rooms

How I would love a mirror and light at the desk area. Other than that, the new rooms are wonderful. [Less]

Wonderful birthday gathering!

We had multiple guestrooms for family from out of town. Most, but not all, were able to be on the 4th floor, which was great (new renovation). We understood that the renovations were behind [More] schedule, so that was the only downside of our stay. The birthday party hors d’oeuvres, drinks and meal was excellent. The bartender (Mitch) really went with the flow. Our servers (Beatrice and Somi) were very attentive and efficient. The event/sales staff (Rosalen and Warren) made us all feel very special. Should You Get Life Insurance for Your Children

Whether you are just starting a family, or already have children, there are several important decisions that you may be thinking about.

Whether you are just starting a family, or already have children, there are several important decisions that you may be thinking about. Can your home accommodate growing children? Do you make enough money to support your children? What schools would you like them to go to and how will you be able to afford their future education?

These are just a few of the things that parents consider about their child or children. One thing that you may not think about immediately however, is life insurance for your children.

Many new parents don t want to think about the serious aspects of life insurance and instead want to enjoy the new additions to their family without the thought of what may happen. However, it may be in your child s best interest to look into the many benefits of life insurance.

Life insurance is frequently purchased by older adults, or young parents themselves thinking about the well-being of their children should something happen to them. But what you may not realize is that having life insurance for your children can be a great benefit to them for many reasons.

First of all, purchasing a life insurance policy for your child can help protect them should an illness or injury prevent them from getting coverage in the future. Young, healthy children aren t typically seen as needing life insurance. However, having that coverage in place could help prevent you and them from future financial burdens should an accident or illness occur.

Secondly, many people see life insurance for children as a great gift because of it gives them a financial head start. Purchasing life insurance for your child will enable that policy to accumulate cash value, which can be used for whatever need may arise in their future.

For example, buying a life insurance policy for your child when they are young can allow that policy to mature and can be used to help with their continuing education when get older.

Many people that buy life insurance for children are actually the grandparents, because older adults especially understand the importance and long-term value of life insurance.

Whether you are a grandparent, parent or are just starting a family, life insurance for your children is something that you should research and get information about before you buy.

There are numerous resources available on the Internet that can help guide you to information about this. If you are looking for information on life insurance for yourself as well, it may be advised to find a company that can offer affordable life insurance for you as well as your children.

Do some research, talk to your family and then decide what s right for your family.

71 Michigan Long Term Alcohol And Drug Rehab Centers

Let’s discuss the latest statistics—numbers that represent a larger whole, numbers that can be startling when you think about them. More than half of Michigan residents drink alcohol, with more than a quarter of the adult population admitting to binge drinking. Added to that, 16% of kids age 12-17 admit to drinking and 10% of those kids admit to binge drinking. What about illegal drug use? Just over 10% of Michigan residents 12 and older report using drugs in a 12 month period.

Is that startling to you? What should be more alarming is the fact that these statistics are dependent on the honesty of those polled. How accurate do you really think those numbers are?

Here are some statistics that can’t lie. In Michigan since 2009, poisonings have surpassed motor vehicle crashes as the leading cause of injury or death. Ninety percent of all poisonings involve drugs.

Long Term Rehab in Michigan Offers Hope

While there are many recovery options for those who have an issue with alcohol and/or substance abuse, long term rehabilitation offers the best hope for successful recovery. Long term rehab has a more extensive program that will keep patients in a secluded, clean environment for a longer duration, and create a recovery plan for life that addresses relapse triggers. These facilities can aid admitted patients with life changes and communication issues, as well as provide family counseling and job skills training.

Locating a Long Term Rehab Facility in Michigan

With hundreds of programs to choose from, the task of finding the right one might seem daunting. When you start to narrow down the facilities that offer long term services, you want to look at the specifics of their program to find the right fit. For example, in Ann Arbor, Dawn Farm offers a long term residential treatment program set on an actual farm. Part of the course of treatment includes caring for animals and organic gardening.

Brighton Hospital in Brighton, Michigan. also has a long term facility with 24 hour inpatient care. The grounds at Brighton are peaceful and transitional housing for both men and women is offered. Because a long term recovery needs a long term solution, Brighton offers programs for patients in recovery even after they leave the facility.

Sacred Heart Recovery in Detroit believes that financial, cultural, racial, gender, age, or lifestyle barriers to recovery should never prevent an addict from getting help. Their individual recovery plans include a combination of group and individual therapies combining principles of Alcoholics Anonymous with evidence based professional therapies.

You can find the right long term rehab program for you. Contact RehabCenter.net for more information on Michigan’s long term options.

Term life insurance: Protection for your big plans. (And for your littlest ones.)

Term life insurance is the simplest way to protect loved ones now—and lock in your lowest available rates for a set period of time. As long as you keep your policy active for a set “term” (like 10, 15, or 20 years), it can replace your lost income, tax-free, in the event of your death. Plus, you can add more coverage or upgrade to a permanent policy later.*

Yearly Convertible Term

If you’re on a tight budget or unsure what protection you need, starting with a term policy can make sense. Later on, you can renew or upgrade.

Level Premium Convertible Term

If consistency appeals, take comfort with a guaranteed rate that won’t go up until your term runs out (or you convert to whole life coverage).

Our wide range of riders, some at additional cost, offer extra benefits to help you meet your short- and long-term goals.

You look out for loved ones every day. Term life insurance helps protect their futures, too.

It goes without saying that you could never be replaced, but with life insurance, your loved ones wouldn’t be burdened with the financial pressure that would come with your loss. Here’s why: the proceeds of life insurance can be used to help preserve your beneficiaries’ standard of living and fulfill future goals, and generally come free from federal income tax.

HOW DOES TERM LIFE INSURANCE WORK?

Term life policies often start you at a lower premium for a set period of time. You choose the length of time you’d like to carry the policy, and at the end of the term, you typically have the option to easily renew or upgrade to a permanent policy.

In this way, purchasing term life when you’re young can lock in your access to affordable life insurance for the term period—even if your health status changes. You can convert your coverage to more rewarding permanent policies, like whole life insurance. possibly at a lower price than if you’d put off purchasing until you’re older.

Could a term policy be right for you?

Term can be the perfect solution for those balancing high expenses with a high need for protection, for example, when you’re starting a family. Ultimately, term life is a cost-conscious solution to get the most protection you can now, while still providing you with an opportunity to convert to a permanent policy later on.

Finally, term insurance is a great option for people who need:

  • Coverage for a well-defined period of time
  • Business protection (in the event you’d lose a key employee or business partner)
  • Extra coverage on top of other, permanent life insurance

IRS Provides Guidance on Supplemental Life Insurance #what #is #supplemental #term #life

#

Client Alerts

Life insurance is a common benefit that employers provide to their employees. Naturally, the provision of this benefit has tax consequences, and a recent private letter ruling from the Internal Revenue Service ( IRS ) reminds employers that they must carefully structure their insurance arrangements to avoid some unexpected tax consequences when their employees purchase supplemental insurance.

Basic Life Insurance

Employers often provide group term life insurance to their employees at no cost to the employee, usually with a benefit equal to a percentage of base salary. Internal Revenue Code ( Code ) Section 79 governs the taxation of this employer-provided life insurance. An employee can receive up to $50,000 worth of coverage tax-free. The cost of any insurance above $50,000, less any amount paid for the insurance by the employee, is taxable income to the employee. For example, if the employer provides basic coverage of $80,000 at no cost to the employee, the employee is taxed on the cost of $30,000 of insurance. The cost of the insurance is determined under tables published by the IRS in regulations under Code Section 79. The employer is required to report the cost of insurance coverage amounts over $50,000 as taxable imputed income on the employee s Form W-2. Since this amount is treated as wages, it is also subject to Social Security and Medicare taxation (which involves a tax on both the employer and employee).

Supplemental Life Insurance

In addition to providing basic life insurance, employers may give employees the opportunity to separately purchase supplemental group term life insurance (and even insurance on their spouses and dependents), but entirely at the employee s expense. Giving employees the ability to purchase additional insurance, even with after-tax money, could impact the tax consequences for both the employee and the employer because all employer plan life insurance coverage (basic and supplemental) generally must be aggregated in determining how the employee is taxed. The rate that the employee pays for the supplemental insurance may be less than the cost determined in the tables under Code Section 79. Even though supplemental insurance, and any insurance on spouses and dependents, is typically paid entirely by the employee, without a clear separation of the basic and supplemental policies, the IRS views the difference between the amount paid by the employee and the Code Section 79 cost as taxable compensation because the lesser cost of the insurance (compared to the Code Section 79 rates) arises in the context of the employment relationship. However, an employer can avoid this problem by electing to treat the policies as separate if the policies are structured so that premiums can be properly allocated among the policies. By doing this, the supplemental insurance can be taken out of Code Section 79 and the employee will not be subject to tax, regardless of whether the cost is below the Code Section 79 rate.

New Private Letter Ruling

Private Letter Ruling 201350032 describes a typical situation and approved the steps that the employer took for the premiums to be considered as properly allocated among policies and avoid aggregation of the policies. It involves a common scenario in which:

  • Employees were provided with basic life insurance at no cost to them by the employer.
  • Employees could separately purchase supplemental life insurance for themselves (and in some cases, their spouses and dependents) through the voluntary employees beneficiary association (VEBA) that the employer uses to fund certain welfare benefits for active employees and retirees. The employees were responsible for paying the entire cost of the supplemental coverage, and payments for this coverage were taken through after-tax payroll deductions.
  • The employer added sections in its employee benefit handbooks describing the availability of the supplemental insurance and provided administrative services to the VEBA as an independent contractor, but did not perform other functions for the supplemental life insurance.
  • Even though basic and supplemental insurance coverage was obtained from the same insurance company, the insurance premiums for the basic coverage were calculated separately from the premiums for the supplemental coverage offered through the VEBA.
  • The employer is not a party to the supplemental insurance contracts, and the supplemental policies are self-supporting in that the premiums for the basic insurance do not subsidize the supplemental coverage.
  • Experience factors for each policy are developed independently, reserves are not shifted between the policies, and dividend and rate credits attributable to each policy are determined separately. Neither the basic coverage nor the supplemental coverage were considered to be permanent benefits.

The IRS ruled that under the arrangement, the employer could treat the basic coverage and the supplemental coverage separately for purposes of applying the Code Section 79 rules. Therefore, the employees who purchased supplemental coverage would not be taxed on that coverage. The employees still were subject to tax on the cost of the basic coverage to the extent it exceeds $50,000, since that coverage was paid by the employer. In addition, employees (and the employer) would only be subject to Social Security and Medicare taxes on the amount by which the basic coverage exceeds $50,000. While a private letter ruling only applies to the taxpayer that requested it, the ruling is helpful to other employers because it describes a situation in which the IRS views the separation between the basic and supplemental policies as adequate to avoid aggregation under Code Section 79.

Employers would not expect that employees purchasing supplemental life insurance with their own after-tax dollars could see an income tax impact from this purchase. Therefore, employers who provide both basic and supplemental insurance to their employees should review the structure in this ruling to determine if they should revise the administration of their policies in order to avoid having employee taxation on the purchase of supplemental insurance.

Long Term Care Insurance

Long Term Care Insurance is more than just ordinary retirement planning. It removes the worry of having to earmark your nest egg for the uncertainty of needing care, so you can enjoy doing the things that make you happy.

  • Harvard Magazine says that women are living an average of 81.4 years and men are living an average of 76.8 years. Advancements in medicine increase the chance that a person will need long term health care. More LTC statistics.
  • New England Journal of Medicine reports that about 51% of people 65 years and older will eventually need these services.
  • The Wall Street Journal states that a staggering 55% of seniors 85 and older have Alzheimer’s or some other form of dementia, the number one reason for needing assistance. WSJ also reports that even children are buying policies for their parents to protect their inheritance.

Longevity Increases Need.

Modern health technology is helping the average United States citizen live a longer life. This extended longevity is increasing the likelihood a person will need Long Term Care services at some point during their life. In addition to Alzheimer s and dementia, there are other degenerative diseases such as obesity, diabetes, stroke, heart problems, cancer, Parkinson s disease as well as accidents and injuries resulting in things like bone fractures which may also create the need for services. Long Term Care Insurance can help pay for the cost of caring for those conditions so you will have peace of mind knowing you are protected if it were to happen to you.

What Does Long Term Care Insurance Pay For?

Your Long Term Care Insurance benefits can be used to pay for Home Health Care, Assisted Living Facilities, Adult Day Care and Nursing Homes. In fact, 72% of all LTC claims are paid for Home Health Care. This service is where an aide comes into your home to help with with your Activities of Daily Living and also provide help with cooking and cleaning. Long Term Care Insurance will help ensure you will be able to stay in your own home to receive care.

The problem: The high cost of needing Long Term Care could drain your life savings.

When planning, consider the high average annual cost of receiving Long Term Care:

  • Nursing Home $86,525
  • Assisted Living Facility $46,880
  • Home Care $42,215

The solution: Plan ahead with Long Term Care Insurance.

The high cost of receiving Long Term Care can erode your nest egg that you have worked so hard to save. The average stay in a nursing home is now 3.3 years. That length of time adds up to an average cost of $285,532 per person or $571,065 for a married couple. That high expense is figured at today s costs and inflation will likely triple that cost in the next 30 years. The plan will have inflation protection so your benefits do not lose purchasing power over time.

What if you could protect yourself from the greatest financial risk you will face in your retirement? Having Long Term Care Insurance coverage can save your nest egg and your retirement. Imagine how relaxed you ll feel knowing that you planned ahead and now you and your family are protected.

LTC Tree s assistance was invaluable in helping us decide which Long Term Care Insurance policy was best for us, while saving us money and providing peace of mind. -Kelly W. More Testimonials

Short-Term Drug Rehab #short #term #drug #rehab

Short-Term Drug Rehab

The following is a definition according to the White House Office on Drug Control Policy of Short-Term Drug Rehab:

Short-Term Residential Programs provide intensive but relatively brief residential treatment based on a modified 12-step approach. These programs were originally designed to treat alcohol problems, but during the cocaine epidemic of the mid-1980 s, many began to treat illicit drug abuse and addiction. The original residential treatment model consisted of a 3 to 6 week hospital-based inpatient treatment phase followed by extended outpatient therapy and participation in a self-help group, such as Alcoholics Anonymous. Reduced health care coverage for substance abuse treatment has resulted in a diminished number of these programs, and the average length of stay under managed care review is much shorter than in early programs.

All of the above is true, but it certainly doesn t tell you the entire story of short-term drug rehab. First of all, insurance companies and their managed care have reduced the length of stay in residential treatment facilities to one week at a time and sometimes only one day at a time, and it is also true that the total numbers of short-term treatment centers has fallen. But, if you were to go on the web and look for alcohol treatment or drug rehab, you would find that most of the referral people you would speak to are recommending short-term drug rehab.

We had a call from a new center in Virginia that wanted our endorsement and, mostly our referrals, to their 30-day rehab program that cost $1,500/day or $45,000 for the month. When asked what types of treatment they were providing, we were told that it was a combination of Heart, Mind, and Spirit .

Great! That tells us everything we needed to know this program is designed to help on an individual basis only and that individual is the owner of the program and not the clients. We have learned through the years that there are many rehab programs that are cookie-cutter duplicates of the original Care Units or Charter Hospitals which were an attempt to get as much insurance money as possible and to make the programs just meaningful enough to ensure that the person would ride out his allowed number of insurance covered days allowed. (This was before managed care)

The outcomes from these types of programs for a period of one year post graduation was under 5% success. That is about the same number of people that get well or off of drugs on their own without treatment.

One thing you must know before you decide on a rehab center or treatment program, and that is the fact that it is very difficult to detox anyone who is on any types of drugs in less than six weeks and one of the reasons that these programs fail so miserably is because they are trying to educate someone that is still toxic from their years of drug use.

On a serious note, the damage that these short-term treatment programs did to hurt the image of alcohol and drug treatment is immeasurable. These programs set up the attitudes that alcoholics and drug addicts are incurrable and that they are not cut from the same fabric as the rest of us good people.

Don t consider any program that fits under the classic Short-Term Drug Rehab model.

Term deposits Vs savings accounts: What’s best for you?

Where should one stash their cash: savings accounts or term deposits?Andrea Sophocleous investigates.

September 7, 2009

Saving hasn t always been fashionable. Before the global financial crisis began taking its toll on years of economic prosperity and consumer hedonism, excessive spending was a national in fact, global pastime.

Australia may have escaped the full grunt of the GFC, but the new mood of thrifty restraint appears to be hanging around a little longer. This is the perfect environment in which to begin saving for that next rainy day or home loan deposit. And with interest rates set to rise further, so could your savings.

Higher interest rates won t just increase your mortgage repayments ; on the plus side, they could also increase the interest you earn on your high interest savings account .

Savings accounts differ from day-to-day transactional accounts by being designed to help you save for a long-term goal. While you can access your everyday account for all your spending needs using debit cards, ATMs or the internet, any money you deposit in a savings account remains out of everyday reach and accrues interest on the growing balance.

You can make withdrawals whenever you like, but this would make a dent in the amount of interest you are paid. A more prudent option, therefore, is to make regular fortnightly or monthly deposits but no withdrawals.

A high interest savings account, however, is not your only option if you want to get serious about saving. If you are easily tempted into spending rather than saving, a term deposit may be a more suitable path for you to take.

Like savings accounts, term deposits are a low-risk, convenient way to earn higher interest on your money, but because you cannot withdraw any money during the life of your term deposit, your cash will remain safely tucked away, growing at a healthy rate.

Term deposits require a minimum opening balance usually at least $5,000 and can last anywhere from 30 days to five years. The rate is fixed for the duration of the term deposit, so you know exactly how much money you will end up with.

If you already have a few thousand dollars saved, a term deposit is a smart choice because you will start earning a decent amount of money in interest straight away. And if you won t need access to the money too soon, opting for a longer term makes more sense because depending on the term, you could score a higher interest rate.

For example, a 30-day term deposit with ING Direct will see you pocket interest at 3.25 percent p.a. while a two-year term deposit will deliver a more profitable 6.00 percent p.a. interest. So $10,000 will earn $26.71 in interest in 30 days, while after two years your nest egg will be up by $1,200.

As with all financial decisions, shop around for the term deposit that suits your needs. And ensure you won t need the money before your term expires otherwise you will have to wear the cost in hefty fees. If you are starting from scratch, a high interest savings account is a better option because most do not require a minimum opening balance.

The ING Direct Savings Maximiser account will dish out 4.75 percent p.a to new customers who signs up before November 30. This special introductory rate will last until 31 January 2010. Other generous high interest savings accounts include UBank s USaver account, which offers 5.11 percent p.a interest, and Westpac s Reward Saver with a 4.70 percent p.a rate.

There are hundreds of savings accounts to choose from, so researching and comparing interest rates and terms and conditions is essential.

New York Short-Term Disability Benefits (DBL)

If you are injured or you become ill while not on the job, you may be eligible for New York State short-term disability benefits under its Disability Benefits Law (DBL). While these benefits are limited both in amount and number of payments, they can be very helpful in providing cash to those who are temporarily unable to work.

Who Is Eligible for New York Short-Term Disability Benefits?

In order to be eligible for short-term disability benefits, you must have become injured or ill while not at work but must be employed, or recently employed, at the time of illness or injury. (Those who are injured on the job are covered under a different set of rules.) Additionally, pregnancy is covered under short-term disability.

The employees who are covered by disability include:

  • An individual who is working or has recently worked (and is collecting unemployment) at least four consecutive weeks at a job that is considered to be owned by a covered employer.
  • Individuals who change from one covered employer to another covered employer. As long as your employment was continuous, coverage for short-term disability starts on your first day of work.
  • Domestic workers who work 40 hours or more for one employer. An example of this would be a nanny or personal assistant.
  • Individuals who are not employed by a covered employer but elect for voluntary coverage.

What is a covered employer?

The definition of covered employer generally includes all employers who have one or more employees. Some exclusions to this definition include government workers, religious leaders, those providing work in return for charity care, and high school students who work part-time or only during vacations.

What Agency Handles Short-Term Disability Benefits in New York?

The New York State Workers Compensation Board, under the New York State Department of Labor, oversees short-term disability benefits. New York State law requires that every covered employer provides insurance to cover short-term disability benefits for their employees. Under the Workers Compensation Board, the New York State Insurance Fund (NYSIF) provides insurance to a vast majority of employers in New York State.

The New York State Workers Compensation Board can be contacted regarding short-term disability insurance for employees who are disabled due to non-work-related illnesses or injuries at (518) 462-8881 or (800) 353-3092. NYSIF may be contacted directly if the insurance fund is your employer s insurer.

Another option that is available to employers is the ability to apply to the Board for an exemption that allows them to self-insure, meaning they would pay short-term disability benefits themselves instead of paying for insurance to do so. If your company self-insures, contact them directly to coordinate disability payments.

How Do I File a Claim for Short-Term Disability?

It is very important to note that if you become disabled and qualify for short-term disability insurance, you must file your claim within 30 days of becoming disabled.

If you are working — or are unemployed and have been collecting unemployment benefits for less than four weeks — at the time your disability began, you will file a DB-450 form. These forms can be obtained through your employer. Completed DB-450 forms should be sent to: NYSIF Disability Benefits Claims; 15 Computer Drive West; Albany, NY 12205.

If NYSIF is not your employer s insurance provider, contact the Worker s Compensation Board.

If you are unemployed and have been collecting unemployment benefits for longer than four weeks at the time you become disabled, you will file a DB-300 form. These forms can be obtained on the NYSIF website, from your local Workers Compensation Board, or by calling 800-353-3092. Completed DB-300 forms should be sent to: NYS Workers Compensation Board Disability Benefits; 100 Broadway; Menands/Albany, NY 12241.

Both forms require you to fill out a section about your disability, as well as a section to be filled out by your treating physician regarding your disability. Medical evidence regarding your disability may be required for the entire period you are requesting benefits.

For those who are employed or unemployed for less than four weeks, there is an additional section that must be filled out by your employer.

How Much Can I Expect to Receive in Benefits?

Disability benefits will pay 50% of your average wages (calculated over the prior eight weeks) up to a maximum of $170 per week.

Benefits will begin on your eight consecutive day out of work; the first seven days is an unpaid waiting period. You can receive benefits for a maximum of 26 weeks in a 52-week period. You will receive payment every two weeks.

For pregnancy, women are covered for six weeks after a normal pregnancy and eight weeks after a Caesarian section (those these lengths may be extended if there are complications). WOmen filing for post-childbirth benefits receive the same payment as those filing for other disabilities.

Medical costs are not covered by disability insurance.

How to Find Hotels for a Long Term Stay

Make a list of amenities you will require during your extended stay. Not all long-term hotels have the same services. Keep this list with you whenever you inquire about a hotel room.

Can you please put wikiHow on the whitelist for your ad blocker? wikiHow relies on ad money to give you our free how-to guides. Learn how .

Ask your employer if you have a corporate discount with any hotel chains. Many corporations sign up for a loyalty program. Input your discount code along with your search terms on the hotel chain’s national website to see the costs of a long-term stay.

Learn the names of hotel chains that specialize in extended stays. These include the Staybridge Suites, Extended Stay America, Hawthorne Suites, Homewood Suites, Residence Inn, TownePlace and Candlewood Suites. Keep these names in mind as you search online or in the phone book.

Use the phone book. The number of extended stay options differs greatly depending upon your location. If you do not have a local phone book, go to yellowpages.com to access phone book directories.

Do a yellowpages.com and general search for extended stay hotel and the name of the city you will be staying in. This will give you the first options, and is likely to recommend hotels that specialize in extended stays.

Call the long-term stay hotels directly to inquire about the cost and amenities. The websites are unlikely to give you any discounts. Call the hotel and ask for long-term stay discounts.

  • Smaller cities and towns are unlikely to have long-term stay hotels listed on this site. It is best used if you are going to a large metropolitan area.

Go to hotels.com or kayak.com to search for hotels in the right location. If you want to stay in a specific area, then you should establish the hotels that are located near your conference center or office.

  • Call these hotels individually and ask for extended stay discounts. Many hotels in areas that are growing offer extended stay rates, even if they do not advertise them in the phone book or online. You are guaranteeing them they will book a room, so do not be afraid to ask for a deeper discount.

Search for a hotel-quality stay at a residence. Search on Airbnb.com to find someone who is renting out their house for an extended period of time. These properties usually have similar amenities as long-term stay hotels, such as laundry, Internet and a kitchenette.

Write down the costs and amenities for each hotel. Create a spreadsheet. Comparison shopping will help you to get the best possible price.

  • Remember to calculate costs like a rental car or parking. Although you might be offered a reduced rate, these costs can easily make an all-inclusive hotel more attractive.

Book your stay with a credit card. Inquire about the cancellation policy in case you need to cut your trip short. Inquire how easily you can extend your stay.

  • Booking with a credit card allows you to reserve without a deposit at many locations. You will also be able to request a refund that will be credited to your account, if it is allowed with the hotel’s cancellation policy. It is recommended that you book early to secure discounts.

[Less]

Homemaker Services make it possible for people to live in their own homes or return to their homes by helping them complete household tasks that they can’t manage alone. Homemaker services aides may clean houses, cook meals or run errands.

Home Health Aides help those who live in their own homes instead of residential care facilities. Home health aides may offer care to people who need more extensive personal care than family or friends are able to or have the time or resources to provide.

Assisted Living Facility 2 Assisted Living Facility

These facilities are living arrangements that provide personal care and health services for people who may need assistance with Activities of Daily Living (ADLs). The level of care provided is not as extensive as that which may be provided in a nursing home. Assisted living is not an alternative to a nursing home, but an intermediate level of long term care.

[Less]

Staybridge Suites Phoenix-Glendale

Holiday Inn Express & Suites Scottsdale – Old Town

Holiday Inn Express & Suites Phoenix Airport

Everything worked out great. We are at the end of our season, but I will definitely use you in the future for my travels needs for 2007. The rates you gave were very competitive better than ones that I get online or over the phone.

V.V. Blacksburg, VA

Your system is very friendly to the customer and understandable from the very first page. Thank you again for your program and system.

D.P. Rockford, IL

My name is Joey Tovar and I own a windmill construction company. I used Extended Stayer twice now for my employees- once in California and once in Texas. I can honestly say that I have saved hundreds of dollars by using Extended Stayer. We used to pay whatever expedia and hotels.com charged but with extended stayer hotels are knocking down their price by 20-30% off those rates because we stay months at a time.

J.T. Colorado Springs, CO

Hoteliers want your business so much that they’ll reduce their rates up to 70% in some cases. They do this because the extended stay traveler uses the least amount of resources (think breakfast, utilities and hotel staff time) all while being guaranteed revenue for them, even when the hotel is slow.
