Best places to stay in Bath, United Kingdom #motels #in #fort #lauderdale

    #bath hotels

    #

One of the jewels of England, Bath’s Roman and Georgian history are everywhere, whether you want to simply enjoy being surrounded by some of the best architecture in the world or to more closely examine the city s fascinating past. Our pick of the best places to stay in Bath are mostly centrally located and all make an ideal base for exploring this beautiful city. For those who’d prefer an apartment, we recommend the selection from our online partner, Booking.com .

Sites such as the ravishing Pulteney Bridge which, like Florence s Ponte Vecchio, was designed for practicality as well as beauty with built in shops. It s the perfect place to visit if one of you prefers sipping cappuccinos to history.

The Roman Baths from which the city takes its name were built by the Romans around the only hot spring in Britain and are still filled with natural warm water. As well as audio guides in eight languages the admission fee includes guides specially for children in both English and French. Genius.

The Fashion Museum s 150 figures chart fashion from the late 16th century to the present day. There are exhibits of menswear, womenswear, day and evening wear and alternative fashion. You can even try on a crinoline at the Corsets and Crinolines display.

Bath is hilly which you should bear in mind if walking is a problem. It can be tricky for motorists too so if you are driving, it is advisable to make sure your hotel has parking.

This entrancing city has a vast choice of places to stay. There are dozens of smaller hotels in 18th century houses, handsome from the outside, though sometimes rather cramped within. And the historical vibe (Beau Brummel, Jane Austen, etc.) tempts some hoteliers to adopt a deadly repro/chintzy style. Beware.

But don’t forget that it’s very busy – year round! If you want to ensure a room in one of the best places to stay in Bath, you’ll need to plan ahead and book well in advance.

    Smaller Hotels

The following are small hotels which avoid camping up the history and are strongly recommended as some of the best places to stay in Bath.

    • Centrally yet quietly located, The Kennard boasts ornate decor and Jane Austen inspired garden. The helpful owners attention to detail and delicious breakfasts prepared with ingredients sourced from local farms make it deservedly popular.
    • Harrington’s Hotel is an excellent value option a stone’s throw from all the major attractions. It’s great for families too with a couple of self catering apartments across the road. Best of all, it’s got its own parking (albeit a couple of minutes walk away)!
    • The Queensberry is a chic privately owned townhouse hotel occupying several houses in a Georgian terrace. Smart and modern it is handy for the Fashion Museum being directly opposite the Assembly Rooms.
    • The Halcyon is a great choice if you want somewhere a bit funky and buzzy in the city centre. There is a cocktail bar and if you want to eat breakfast downstairs and not in bed you ll pay extra. As if.

Super smart places to stay in Bath

In the upper echelons of price and grandeur, competition is fierce and the Big Three; the Bath Priory, Bath Spa and Royal Crescent, have each splashed out heavily on spa facilities. All would also claim to be high up there amongst the best places to stay in Bath.

    • Highly recommended, is the Bath Priory. though it is just on the edge of the city and not in the centre. Decorated in traditional English country house style, it has an award-winning Michelin-starred restaurant, indoor and outdoor pools, glorious four acre gardens which also supply the kitchen with vegetables and extremely comfortable and luxurious rooms and bathrooms. Bath is quite small so the centre of the city is easily accessible from the hotel.
    • The well-known Royal Crescent hotel is hard to beat if you are planning a treat. Situated right in the middle of one of the most jaw-droppingly impressive pieces of architecture in Britain, there is little chance of waking up here and wondering where you are. Effortlessly combining traditional and modern interiors it is a classy delight both inside and out. And the fabulously grown-up spa is a child-free zone.
    • Of the three, the Macdonald Bath Spa Hotel can’t be ignored, but is hard to really love. Standing in seven acres of formal grounds, it is competent, imposing and slightly dull, though its spa facilities are excellent.

    For a city centre apartment

    • If you don’t think that any of our picks of hotels or B Bs fits the bill, how about an aparmtent? You can find a good selection with our online partner, Booking.com .

Sue Ryder Care – Wheatfields Hospice

I can only review one aspect of Wheatfields Hospice as I have only experienced the brick-a-brack sale they have twice a week to raise money, and not the care home element of it.

Formalities aside, I can now express my utter joy in sharing this charity shop style gem with all the Yelp readers. It is AMAZING! It is open at very odd times so some planning is required but is basically made up of a few small warehouse type buildings and a few large porter cabins and is a bargain hunters DREAM. One section houses second (or more) hand furniture like tables and sofas, and old bike parts. I picked up a large wooden mirror from here for only a few quid a while ago. Then there is a large section dedicated to clothing of all kinds. In fits of creativity I have raided this place for old clothes to cut up and turn into new clothes. Next to this there is book section with most books priced in true charity shop style at only 50p each, maybe a pound for a hardback. And there is even more – a room dedicated to junk of all kinds – plated, jars, old glasses, broken umbrellas, vases, trays, ornamental bits of glass. the list goes on. Basically any of the useless items you re likely to find in your granny s house amongst which are lying some glorious treasures. And finally, if you feel totally worn out by all this browsing there is a tea room with tea, coffee and home made cake – also a perfect place to hobnob with the local OAPs. So, finally, this is a great place to get down to if you ve moved into a new house and are looking for some cheap furniture or anything else second hand.

Deutsche Bank cuts ties with 3,400 clients in trading business

    By Arno Schuetze | FRANKFURT

FRANKFURT Deutsche Bank’s Global Markets division will cut ties with about 3,400 clients in its debt and equities sales activities, the bank said on Friday. Deutsche Bank will immediately cease debt sales services to some financial institutions and hedge funds as well as equity sales activities, the execution of equities trading orders and equity structuring activities for some clients, a spokesman said, citing an internal memo.

Germany’s biggest bank is looking to shed clients that cost more to service than they bring in in returns as part of efforts to turn around its business and boost its capital.

Deutsche Bank Chief Executive John Cryan is stepping up the restructuring process as the bank finalises talks with U.S. justice authorities over a multi-billion dollar fine related to U.S. mortgages.

In October 2015, Cryan had said at a strategy presentation that Deutsche Bank would reduce the number of clients in its Global Markets and Corporate & Investment Banking divisions by about 50 percent.

“We expect to off-board about half of the current list of clients as the economic returns in these relationships are inadequate to us,” he said at the time. He also said that 80 percent of the investment bank’s income came from 30 percent of clients.

Deutsche Bank is still among the top 5 trading houses in debt and top 10 in equities globally, according to research firm Coalition. But the bank has lost market share as it retreats from a period of expansion, in which it had focused mainly on revenue growth and less on profitability.

    Being Pet-Friendly

When people think pet-friendly–extremely pet-friendly–many think Europe. However, in our many years of experience, we’ve seen the US become more and more more pet-friendly, to the point where you can feel comfortable finding not only pet-friendly hotels and other lodging for your dog or cat, but also a wide array of pet-friendly services and venues–in small towns as well as large cosmopolitan areas.

In the old days you could only find pet-friendly Bed and Breakfasts and Inns, while larger hotel chains were wary of accepting pets. That’s all changed. Now, they’ve learned that being pet-friendly is not only the right attitude–it’s also good business. That’s why you can now drive into practically any town and find a hotel, bed & breakfast, inn, cabin or cottage that is dog friendly or cat friendly, or both.

Of course, being “pet-friendly” means different things to different lodgings. Some allow all pets and all sizes. Some have size restrictions and fees or deposits. Others might put you a smoking room. At petswelcome, we made a conscious decision to list ALL lodgings that accept pets, no matter the policy. And while $150 non-refundable “deposit” is egregious, as you move down the line, the distinctions become less offensive and harder to judge. The bottom line is that we figured it was best to simply list the pet policy and let you decide. One person’s outrage is another person’s pet paradise.

You can use the search form above or browse through the states below to find a complete list of all pet-friendly cities in that particular state. Our lodgings also include Bed & Breakfasts, Cabins and Cottages, Inns and Motels.

Best Interest Rate on Senior Citizens Bank Fixed Deposits July 2017

Highest Interest Rate on Bank Fixed Deposits for Senior Citizens July 2017

The post lists down the rate of interest on Bank fixed deposits for Senior Citizens as of July 4, 2017.

You might want to bookmark this page as the FD interest rates would be updated every month. Would help you in better decision making.

    Fixed Deposit Highlights:

    • Most banks offer FDs for tenure of 7 Days to 10 Years.
    • The Ratnakar Bank IDBI Bank do offer fixed deposits up to 20 years too .
    • For very short Term Deposits the interest rate is similar to that of Savings Account and so you should not worry about FD. Also Interest up to Rs 10,000 in Saving Account is Tax free .
    • Most banks compound interest quarterly
    • Banks offer Loan/Overdraft against the amount available in Fixed Deposit. The interest is generally 0.5% to 1% more than that offered to FD.
    • TDS (Tax deduction at source) at the rate of 10% is deducted, if the interest income is more than Rs 10,000 in financial year per bank. You can fill Form15G/H
    • You can fill Form15G/H if you want to avoid TDS .
    • There might be penalty for pre-mature withdrawal of Fixed Deposits

    Fixed Deposit Interest Rates:

The highest interest rate is offered by The Deutsche Bank at 8.25% for 1 to 2 years Fixed Deposit.

For comparing the best interest rates on fixed deposits over different duration of investment, we have it divided into following 5 slabs:

    • FD for Less than 1 Year
    • FD for 1 to 2 years
    • FD for 2 to 5 Years
    • FD for 5 to 10 years
    • FD for More than 10 years

We show the highest interest rates on fixed deposits for the above duration buckets. We have also compared the best interest rates on offer by that being offered by State Bank of India (SBI), ICICI Bank and Post Offices.

Interest Rate (Senior Citizens) for FD Less than 1 Year:

The highest interest rate is offered by The Ratnakar Bank (241 days to 364 days) at 7.60% .

    Shankar Iyer says:

I suppose there has to be an extensive study before stats are pur out esp. with reference to Ratnakar Bank. If you could check Varacha Bank, Surat Peoples Bank, Sutex Bank, Sarvoday Coop Bank ( all coop banks in Surrat) whose financials are available and NPAs are available for view. These give better returns than Ratnakar. Also, see Gujarat Rajya Karmachari Coop Bank. Investments upto Rs. 1 lac is insured. Also, the investor needs to be educated on the maneouveing of applicant names while opening accounts with coop banks, so that only the benefit of sr citizen is obtained, but the deposits are also insured.

I agree with your views and you can use various combinations of names of account holders to get benefit of deposit insurance. Historically co-operative banks have been riskier and so they offer higher interest rate. Also claiming insurance in case of default is not easy and it may take time. So it s individuals call weather to take risk for slightly higher interest rates.

    Gramercy Park Hotel

    Loading map, please wait.

    The hotel at a glance

For our New York vacation, we we were looking for a hotel in midtown Manhattan, a location perfect for shopping and visiting NYC. We finally selected the Gramercy Park Hotel.

It was very different from other hotels in New York. We were enchanted by the hotel decor designed by Ian Schrager in an artfully revisited bohemian style. His artistic sensuality literally carried us away. We admired the skillful blend of furniture styles and the clever choice of works of art enhanced by a Renaissance color palette inspired by the famous Raphael. We unreservedly liked this concept of modern luxury, the beautiful rugs, the heavy bright pink velvet curtains framing the mahogany windows. We were totally mesmerized by the beauty of the place and amazed by the designer’s talent and creativity. But the best was yet to come.

In the morning we took an ‘à la carte’ breakfast in the Gramercy terrace, a wonderful environment with its lush vegetation. The service was fast and friendly. In this landscaped indoor garden, we immediately forgot that we were in the heart of New York City, especially when the sun gratified us with its warm presence.

We also loved to spend time at the Rose Bar and Jade Bar with their most original atmosphere. Located on the 16th floor of the hotel, it is a private lounge-type area where we could sit leisurely surrounded by masterworks of renowned 20-century artists and where we thoroughly enjoyed the cozy atmosphere as well as the the well-chosen music and chatted with other guests before returning to the tranquility of our room.

Our stay at the Gramercy Park was really fabulous and even better than we had hoped for. I absolutely recommend this original and luxurious hotel, the perfect choice for anyone who hates getting bored.

Our Hidden History of Corporations in the United States #united #states #corp

    #

Our Hidden History of Corporations in the United States

When American colonists declared independence from England in 1776, they also freed themselves from control by English corporations that extracted their wealth and dominated trade. After fighting a revolution to end this exploitation, our country s founders retained a healthy fear of corporate power and wisely limited corporations exclusively to a business role. Corporations were forbidden from attempting to influence elections, public policy, and other realms of civic society.

Initially, the privilege of incorporation was granted selectively to enable activities that benefited the public, such as construction of roads or canals. Enabling shareholders to profit was seen as a means to that end. The states also imposed conditions (some of which remain on the books, though unused) like these*:

    • Corporate charters (licenses to exist) were granted for a limited time and could be revoked promptly for violating laws.
    • Corporations could engage only in activities necessary to fulfill their chartered purpose.
    • Corporations could not own stock in other corporations nor own any property that was not essential to fulfilling their chartered purpose.
    • Corporations were often terminated if they exceeded their authority or caused public harm.
    • Owners and managers were responsible for criminal acts committed on the job.
    • Corporations could not make any political or charitable contributions nor spend money to influence law-making.

For 100 years after the American Revolution, legislators maintained tight controll of the corporate chartering process. Because of widespread public opposition, early legislators granted very few corporate charters, and only after debate. Citizens governed corporations by detailing operating conditions not just in charters but also in state constitutions and state laws. Incorporated businesses were prohibited from taking any action that legislators did not specifically allow.

States also limited corporate charters to a set number of years. Unless a legislature renewed an expiring charter, the corporation was dissolved and its assets were divided among shareholders. Citizen authority clauses limited capitalization, debts, land holdings, and sometimes, even profits. They required a company s accounting books to be turned over to a legislature upon request. The power of large shareholders was limited by scaled voting, so that large and small investors had equal voting rights. Interlocking directorates were outlawed. Shareholders had the right to remove directors at will.

In Europe, charters protected directors and stockholders from liability for debts and harms caused by their corporations. American legislators explicitly rejected this corporate shield. The penalty for abuse or misuse of the charter was not a plea bargain and a fine, but dissolution of the corporation.

In 1819 the U.S. Supreme Court tried to strip states of this sovereign right by overruling a lower court s decision that allowed New Hampshire to revoke a charter granted to Dartmouth College by King George III. The Court claimed that since the charter contained no revocation clause, it could not be withdrawn. The Supreme Court s attack on state sovereignty outraged citizens. Laws were written or re-written and new state constitutional amendments passed to circumvent the (Dartmouth College v Woodward ) ruling. Over several decades starting in 1844, nineteen states amended their constitutions to make corporate charters subject to alteration or revocation by their legislatures. As late as 1855 it seemed that the Supreme Court had gotten the people s message when in Dodge v. Woolsey it reaffirmed state s powers over artificial bodies.

But the men running corporations pressed on. Contests over charter were battles to control labor, resources, community rights, and political sovereignty. More and more frequently, corporations were abusing their charters to become conglomerates and trusts. They converted the nation s resources and treasures into private fortunes, creating factory systems and company towns. Political power began flowing to absentee owners, rather than community-rooted enterprises.

The industrial age forced a nation of farmers to become wage earners, and they became fearful of unemployment a new fear that corporations quickly learned to exploit. Company towns arose. and blacklists of labor organizers and workers who spoke up for their rights became common. When workers began to organize, industrialists and bankers hired private armies to keep them in line. They bought newspapers to paint businessmen as heroes and shape public opinion. Corporations bought state legislators, then announced legislators were corrupt and said that they used too much of the public s resources to scrutinize every charter application and corporate operation.

Government spending during the Civil War brought these corporations fantastic wealth. Corporate executives paid borers to infest Congress and state capitals, bribing elected and appointed officials alike. They pried loose an avalanche of government financial largesse. During this time, legislators were persuaded to give corporations limited liability, decreased citizen authority over them, and extended durations of charters.

Attempts were made to keep strong charter laws in place, but with the courts applying legal doctrines that made protection of corporations and corporate property the center of constitutional law, citizen sovereignty was undermined. As corporations grew stronger, government and the courts became easier prey. They freely reinterpreted the U.S. Constitution and transformed common law doctrines.

One of the most severe blows to citizen authority arose out of the 1886 Supreme Court case of Santa Clara County v. Southern Pacific Railroad . Though the court did not make a ruling on the question of corporate personhood , thanks to misleading notes of a clerk, the decision subsequently was used as precedent to hold that a corporation was a natural person. This story was detailed in The Theft of Human Rights , a chapter in Thom Hartmann s recommended book Unequal Protection .

From that point on, the 14th Amendment, enacted to protect rights of freed slaves, was used routinely to grant corporations constitutional personhood. Justices have since struck down hundreds of local, state and federal laws enacted to protect people from corporate harm based on this illegitimate premise. Armed with these rights, corporations increased control over resources, jobs, commerce, politicians, even judges and the law.

A United States Congressional committee concluded in 1941, The principal instrument of the concentration of economic power and wealth has been the corporate charter with unlimited power .

Many U.S.-based corporations are now transnational, but the corrupted charter remains the legal basis for their existence. At Reclaim Democracy. we believe citizens can reassert the convictions of our nation s founders who struggled successfully to free us from corporate rule in the past. These changes must occur at the most fundamental level the U.S. Constitution.

Two killed in San Jose car crash

    Sunday, December 25, 2016

An early Christmas morning head on collision in South San Jose resulted in two fatalities, including that of a 14-year-old boy, according to police.

Officers responded at 12:28 a.m. to a report of a major collision at Capitol Expressway and Snell Avenue, police said.

After a preliminary investigation, officers determined a white 2012 Chevrolet Malibu being driven by a 25-year-old woman was traveling west on Capitol Expressway. For an unknown reason, the Chevy crossed into the opposite lanes of traffic, colliding head on into a silver 2004 Lexus, according to police.

The Lexus, which was being driven by a man, was also carrying the man’s wife and their 14-year-old son.

All four victims were taken to local hospitals, police said.

The driver of the Lexus and his wife both suffered injuries not considered life-threatening, according to police.

The 14-year-old boy and the 25-year-old driver were both pronounced dead a short while later. Their identities have not been released.

Investigators believe the deceased driver of the Chevy may have been driving while under the influence of alcohol.

The collision marks the city’s 47th fatal collision this year, police said.

What is the GASTRIC SLEEVE? Information on cost, benefits of vertical sleeve?

    #

    GastricSleeve (Vertical Sleeve Gastrectomy)

The gastric sleeve is the lastest weight loss procedure that shows promising results. By restricting food absorbtion and reducing hunger the gastric sleeve might be your best option. Please consult with our surgeon or staff to find out if this is the best procedure for you. Advantages of gastric sleeve include lower cost, less dumping, fewer complications and does not require adjustments after the initial surgery.

The vertical sleeve gastrectomy is a restrictive form of weight loss surgery in which approximately 85% of the stomach is removed leaving a cylindrical or sleeve shaped stomach with a capacity ranging from about 60 to 150 cc, depending upon the surgeon performing the procedure. Unlike many other forms of bariatric surgery, the outlet valve and the nerves to the stomach remain intact and, while the stomach is drastically reduced in size, its function is preserved. Again, unlike other forms of surgery such as the Roux en Y gastric bypass, the sleeve gastrectomy is not reversible.

Because the new stomach continues to function normally there are far fewer restrictions on the foods which patients can consume after surgery, albeit that the quantity of food eaten will be considerably reduced. This is seen by many patients as being one of the great advantages of the sleeve gastrectomy, as is the fact that the removal of the majority of the stomach also results in the virtual elimination of hormones produced within the stomach which stimulate hunger.

Perhaps the greatest advantage of the gastric sleeve lies in the fact that it does not involve any bypass of the intestinal tract and patients do not therefore suffer the complications of intestinal bypass such as intestinal obstruction, anemia, osteoporosis, vitamin deficiency and protein deficiency. It also makes it a suitable form of surgery for patients who are already suffering from anemia, Crohn’s disease and a variety of other conditions that would place them at high risk for surgery involving intestinal bypass.

Facts about the gastric sleeve or sleeve gastrectomy:

Alternative names: vertical sleeve gastrectomy, sleeve gastrectomy, greater curvature gastrectomy, parietal gastrectomy, gastric reduction and vertical gastroplasty.

Surgery for high BMI patients. For patients with a particularly high body mass index (typically 50+) many forms of weight loss surgery are either difficult to perform or present increased risk. As a result, a vertical sleeve gastrectomy (or increasingly a laparoscopic sleeve gastrectomy) is sometimes performed as the first of a two-part weight loss solution to provide an initial drop in weight which then makes other bariatric follow up possible at a reduced level of risk.

Surgery for low BMI patients. For obese patients with a relatively low body mass index the vertical sleeve gastrectomy can also prove a good choice, especially where existing conditions (such as anemia or Crohn’s disease) prevent them from having other forms of bariatric surgery. In addition, patients may choose this form of surgery if they are concerned about the long-term affects of bypass surgery or object to having a ‘foreign’ body implanted into their body, as is the case with lap band surgery.

    Laparoscopic Sleeve Gastrectomy Surgery

During sleeve gastrectomy, the surgeon will remove the larger, rounded part of the stomach.

    • The remaining stomach looks like a sleeve (or hose or tube) and holds about 15 percent as much food as the original stomach.
    • The surgeon will remove the larger, rounded part of the stomach from the body. (This is the only gastric surgery in which part of the stomach is taken out of the body.)
    • Unlike gastric bypass, which changes stomach openings, sleeve gastrectomy leaves the openings intact.
    • It may be a safer and more effective option than gastric bypass for patients with very high BMI, those with medical problems like anemia, Crohns disease, osteoporosis, extensive prior surgeries and other complex medical conditions.
    • Sleeve gastrectomy may be safer than gastric bypass for patients who have a number of health risks.
    • It lowers the risk of ulcers compared to gastric bypass.
    • The surgery cuts away the part of the stomach that produces grehlin, a stomach hormone that stimulates hunger.
    • Though the stomach is smaller, the openings are left intact, so digestion can go on as normal.
    • The body is free of foreign objects like the LAP-BAND or Realize Band.
    • May be converted to gastric bypass or duodenal switch if necessary for additional weight loss
    • Expected excess weight loss for stand-alone procedure is 60 to 70% at two years
    • Sleeve gastrectomy is more common in Europe, but most American health insurance carriers still consider it an investigational procedure and do not cover the cost. But that is rapidly changing so please consult our office to see if your insurance will cover the gastric sleeve.
    • Anytime you have anesthesia or surgery, there is a risk of blood clots, other complications or death.
    • Do not smoke. Smoking would put you at high risk for infection, blood clots, slow healing and other life-threatening complications.
    • Complications can occur with the stapling, such as leaks or bleeding.
    • You may need malabsorptive surgery intestinal bypass or duodenal switch in addition to your sleeve gastrectomy in order to lose all the weight you need and want to lose.
    • The smaller portion of the stomach may stretch.
    • Foods that you eat now may cause discomfort, nausea or vomiting after your surgery.
    • Gastric surgery puts you at higher than normal risk of developing gallstones and gallbladder disease.
    • You will not lose weight or maintain your weight loss unless you eat a healthy diet and exercise regularly. This is the reason we stress long-term follow-up with our center and your doctor.

    The hotel at a glance

Star studded, intimate, well located and relaxed, the Covent Garden Hotel is the perfect headquarters for those looking to raid London’s theatre district.

When stars start to flock to a boutique hotel, you know that the place has got discretion down pat and if you’re looking for the same getaway experience, the Covent Garden Hotel is one of the best around. Staff are friendly and helpful without being obtrusive, and even though things can get very glamorous over here; the laid back and home away from home vibe is conducive to friendly conversations and relaxation.

The hotel’s guest areas, the Tiffany Library and the Drawing room, give out a classic vibe with the wood paneling and fireplace, but the quirky furniture reminds you that you’re in a Firmdale hotel and that fun and games are what this place is about. If you have any doubts, you can just head down to the hotel’s screening room for a private movie session, or sip a drink in the Brasserie max’s youthful atmosphere.

As usual, room designs are upbeat and the chintz motifs really give this hotel its signature flair which we’re sure many guests have tried to emulate back home. The cream of the crop here is the Covent Garden Loft suite. This two level room is the perfect symbol for this hotel’s home away from home tag with its perky yet not over the top design. If you had commissioned Kit Kemp to decorate your home, this is probably what it would end up looking like and anyone who has lounged in the suite’s sitting room might be tempted to do just that.

An In-Depth Look At The Fraud Charges Against Vitas Hospice Services –

United States. An In-Depth Look At The Fraud Charges Against Vitas Hospice Services

Earlier this month, the United States Department of Justice (пїЅDOJпїЅ) filed a suit against Vitas Hospice Services, L.L.C. and its subsidiary entities (пїЅVitasпїЅ) alleging that Vitas submitted false claims for hospice services which were excessive, unnecessary, or not provided, and also alleging that Vitas admitted patients to hospice who were not terminally ill.

    Hospice Care

Hospice care is provided to terminally ill patients who have elected to forego curative treatment in exchange for palliative, comfort care in their own homes.пїЅ Hospice care is intended to make available the medical services necessary for a patient to be able to remain in his or her home without the need to run to the emergency room every time uncontrollable symptoms arise. Most hospice agencies derive a significant portion of their revenue from Medicare Part A for services provided to program beneficiaries, including Vitas, whose revenue is historically 90% derived from Medicare.

In order to be eligible for hospice care, a patient must be terminally ill with a prognosis of six months or less if the patientпїЅs illness runs its normal course. Hospice care is provided at four levels of care: routine home care, continuous home care (also called crisis care), inpatient respite care, and general inpatient care, with each level of care being reimbursed at vastly different rates. Specifically, in fiscal year 2013, MedicareпїЅs daily reimbursement for continuous care was $742 more per patient than the daily reimbursement rate for routine home care. Hospice care is not intended to be an around-the-clock service but rather intermittent care provided by a range of clinical and non-clinical personnel who make visits to a patientпїЅs home several times a week.

    Continuous or пїЅCrisisпїЅ Care

The need for crisis care arises when a patientпїЅs symptoms become uncontrollable and require nursing care on a continuous basis for a brief period of time, which could range from eight hours (the minimum amount of continuous care necessary to bill for the higher rate) to several days. The first part of the DOJпїЅs complaint focuses on allegedly inappropriate and excessive billing for continuous care services. The DOJ alleges that Vitas set quotas for the percentage of crisis care which it expected its agencies to meet.пїЅ In one instance, for example, VitasпїЅs Vice President of Operations allegedly emailed a marketing employee and general manager demanding to know why the agencyпїЅs crisis care margins had dropped in a particular month.

The DOJпїЅs complaint alleges that Vitas conditioned its patients and their families to expect crisis care by misleading them into believing that the Medicare hospice benefit routinely covers around-the-clock care.пїЅ Because of this alleged marketing ploy, the DOJ surmises that patients sometimes chose Vitas over other providers. The complaint also alleges that Vitas disseminated written materials to its staff which incorrectly trained the staff on how and when to initiate crisis care. According to the complaint, several staff members gave investigators explanations of crisis care which were inconsistent with Medicare requirements.

According to the complaint, many patients failed to meet the eligibility requirements for crisis care. One Vitas nurse allegedly told the DOJ that, on more than one occasion, she would arrive at a patientпїЅs home to provide crisis care, only to learn that the patient was at church, the beauty parlor, or playing bingo. According to the complaint, an internal audit conducted by Vitas itself showed that only 50% of the continuous care records reviewed contained clinical documentation consistent with the Medicare requirements for such care. According to the DOJ, when compared to other hospice providers, Vitas bills Medicare for twice as many crisis care days as all other hospice providers combined. The complaint also alleges that VitasпїЅs crisis care billings were almost six times what would be expected if its crisis care statistics were in line with the national average.

The complaint also alleges specific instances of inappropriate use of continuous care, including treating a patientпїЅs back pain with a heating pad, performing daily dressing changes, and continuing to provide continuous care for patients whose acute symptoms had been treated and stabilized. According to the complaint, in one unfortunate incident, a Vitas nurse allegedly failed to recognize that a patient was suffering from opioid neurotoxicity and continued to administer higher levels of morphine to the patient, which only increased her pain and led to seizures. The complaint goes on to allege that Vitas then billed Medicare for 16 days of crisis care for the patient, whose critical condition was created by Vitas itself. On another occasion, Vitas allegedly offered crisis care to another patient simply because the patient was considering aggressive curative therapy instead of continuing hospice care.

    Inappropriate Admissions

The second part of the DOJпїЅs complaint focuses on inappropriately admitting patients to hospice when such patients did not meet the Medicare criteria for hospice eligibility. The complaint alleges that VitasпїЅs corporate culture promoted increasing its agenciesпїЅ censuses without regard to whether services were actually necessary. Vitas allegedly paid bonuses to its non-clinical staff based on the number of patients enrolled in the program, and Vitas took adverse employment actions against marketers who failed to meet monthly admission goals. According to the complaint, one former manager told investigators that his bonuses were based on the number of patient admissions and the length of time that he could get a patient to stay on hospice services.

The DOJ further alleges that VitasпїЅs medical staff reported that they felt pressured by Vitas to admit or readmit patients who were inappropriate for hospice services. The complaint details an alleged incident in which one former Vitas admissions nurse said that if he did not admit a patient whom he found to be ineligible, he would be pressured to reconsider his decision until he finally determined that the patient was eligible. Another Vitas nurse purportedly stated that, at weekly meetings, discharging more than four patients per meeting was frowned upon by business managers, and medical staff were told to stop discharging patients even if patients were not eligible for hospice care. The same nurse also allegedly reported to investigators that Vitas instructed her to falsely write in the medical record that a patient experienced symptoms which he did not actually experience and further instructed her not to write in the medical records that a patientпїЅs health was improving.

According to the complaint, several Vitas physicians reported being under pressure from management to increase the number of patients admitted to hospice care and were oftenпїЅ overruled when they did not believe that patients were eligible for hospice or when they determined that patients should be discharged because they were not dying. The complaint includes a detailed description of several instances when, according to the DOJ, the medical record clearly did not support the need for hospice care.

    Damages Sought by DOJ

Based on all of its allegations, the DOJ charges Vitas with knowingly presenting or causing to be presented false or fraudulent claims to Medicare in violation of the False Claims Act and seeks unspecified damages, which would be trebled as required by law. Finally, the DOJ also includes as causes of action payment by mistake and unjust enrichment.

    Lessons for Providers

The allegations against Vitas are of course just that пїЅ merely allegations. But if proven to be true, some of VitasпїЅs practicesпїЅ are obviously flawed: donпїЅt instruct staff to falsify medical records; donпїЅt provide a higher level of service than is necessary; donпїЅt base discharges on non-clinical reasons. Some of VitasпїЅs alleged business practices, however, are not obviously problematic. From a business standpoint, what more accurate way is there to measure a marketerпїЅs performance than by looking at the number of admissions which she generates? What would be completely benign business practices in any other profession could subject someone to great civil and criminal liability in the healthcare arena.

Hospice providers are encouraged to use the DOJпїЅs complaint as a learning tool for understanding what types of issues and business practices are likely to attract the attention of investigators (and whistleblowers). Understanding the issues and potentially problematic business practices up front should enable providers to design compliance policies and procedures that will help them avoid some of the pitfalls that have befallen Vitas.

    Bethany Arms Motel Apartments

The biggest draw back was the not so private balcony. The night auditor told me it was private. You step out the door and you can pretty much see the people on the adjacent balcony. If a family rented a floor, fine. But I really wanted to leave the door and sleep with the sound of the ocean. Can t do that because over this railing between balconies, someone could get in my room. The couch by the door folds into a bed. I pulled it out and found some strange debris. I could just leave the door open and hope no one is tempted to come in. Smh.

It s late. Spontaneous drive through Delaware has shown me, I m afraid, what there is to see here. Rehoboth was a mad house. Traffic was horrendous. Reminder: it s after Labor Day. Dewey Beach was packed. Kept on driving and found this place. While I may have found a cot on the boardwalk in Rehoboth for 3 or 400 hundred dollars, I like a quieter place.

Found it here. Hotel offers a lot. The room is big. Has a full size fridge and stove, but I can t forget that clear unlabeled bottle of blue goo by the sink. Common sense tells me it s dish detergent. If there isn t soap in the bathroom, why is there soap by the kitchen sink?

I m really not a nit picker. I ve stayed all over the east coast in everything from large upscale chains to small family owned hotels, even old ones like this.

Not impressed for $230 a night.

Could climb right over and be in their private balcony too

    Stop following Kial S.

Bethany Arms is a quaint, very old, motel. It doesn t have a bunch of special amenities or anything but it serves its purpose just fine.

It s got a great view of the beach if you re on the second floor of building D and it s right next to the boardwalk so you can walk right across to the beach.

They get three stars because it meets expectations and the people were very nice.

This is an old motel that looks straight from the 70 s, but it is clean and is in a great location. You re right on beach and only half a block (like a 10 second walk) to all the restaurants and shops in Bethany beach. Since it s an old motel, their pricing is lower than anywhere around. I stayed there May 31-June 2 and the place was maybe 20% full, yet every day there was a whole crew of about a dozen people going through every room to clean. If you want the ritz, this isn t it. But if you want a room in a great location with the basic amenities (mini fridge, parking, AC, clean bathroom and beds) then this is your place.

If you want to stay in Bethany Beach, then the location is 4 or 5 stars.

There are 5 separate buildings in the complex. This is the first time we ve stayed here, so we ve only experienced one room in one building. Our premium suite was very dated and a bit decrepit. Like it might have been all that 50 years ago, but now the fixtures are rusted, tile and paint faded, furniture sagging. and it smelled like 50 years worth of accumulated must when we walked in. We did have a kitchenette, but it was sparse (fine for storing beer and sandwich fixings – but no microwave for your oatmeal, and you wouldn t want to cook an actual meal in it). We weren t expecting much. and it just about met our expectations.

On the plus side, the staff was helpful, and extended our checkout time with no issue (since the room was not booked until a day later). Also, the other buildings may be nicer or more current, I just don t know. At the end of the day, we enjoyed ourselves, and the place wasn t awful – just on the low side of average. We might stay again sometime.

This place is stuck in the 1970 s yet the prices are 2011. It seemed clean and all but it had an old, moldy smell in rooms. With the prices that this place is charging I would expect them to update their rooms. Staff is extremely careless over the phone and in person. Only when I was checking out a nice lady said thank you, other guy was mentally in some other place. Maybe because there are no other competition around the area this place thinks they can get away. Well Hilton might be just around the corner building a small hotel with better prices, I would start worrying.

    Was this review ?

As I was searching for a hotel for my two night stay in Bethany, the Bethany Arms Motel was one of the lower priced options. It s a great location, right on the boardwalk. There are several different buildings which are each a different price depending on the view, the size, and if they offer a kitchenette. I went for the cheapest option and the room was simple but had everything we needed (fridge, microwave, etc). They also had an outdoor shower so you didn t have to fight the crowds for the ones on the boardwalk. The front desk was friendly and helpful.

Best places to stay in Bath, United Kingdom #cheap #hotel #sites
