Job Description

A hospice care aide helps individuals unable of helping themselves. These aides travel to the patient s home and help them perform activities required for daily life. Hospice care aides are often associated with the elderly or disabled who are unable to take care of themselves, so they perform all required functions specific to the patient.

Most of the time, a hospice aide is the first position an individual may take at the beginning of their career. Alternatively, an individual may seek such a career if they are interested in the nursing field, but do not have the required education or training. Most functions performed are rather fundamental in nature, and do not require much training.

Hospice aides usually work within a hospice environment or travel to the patient s home. They help the individual eat, get up out of bed, or help them with the bathroom or a bedpan. The number of duties involved depends on the nature and the status of the patient, since they are expected to help the patient function as normally as possible. If working within a hospice environment, they work with individual patients to make their final days as comfortable as possible. If working in a home environment, they spend time sitting with or caring for an elderly or disabled patient who requires extra care.

Many times, families of patients in either a home or hospice environment can t commit to 24 hour care, so they hire a hospice aide to help during the times they are absent. Hospice aides travel to the home and work in shifts providing round the clock supervision and care. They aren t usually a registered nurse or hold a license and cannot usually administer medication, so the responsibilities are typically fundamental.

Training/Educational Requirements

There is no educational or training requirement for this position. A hospice aide learns from on-the-job training with a registered nurse or a nurse s aide. They learn the proper way to do things such as help a patient out of bed or assist with a bedpan by observing other nurses and by practicing. Oftentimes the family of the patient has a particular way they want things done, so the best training comes from working directly on the job.

The National Association for Home Care and Hospice ( NAHC ) offers certification for personal and home care aides throughout the country. This certification is not mandatory although individuals may opt to gain it to demonstrate they have met established industry standards. It can be a simple certification and lend way to further development within this role. Those individuals wishing to gain their certification must complete a 75-hour course, observe and document work in up to 17 different skills signed off by a registered nurse. In addition, they must successfully pass a final exam. Although this isn t a requirement, it can certainly lend way to more opportunities for advancement.

There are some high school courses that prepare for this role. At the early stages, an individual can do a co-op job to better prepare themselves for becoming a hospice aide. There are some standalone classes an individual can take in patient care which prepares them for the real thing. Individuals interested in a career as a hospice aide receive basic training from the company they work for before they are sent to a patient s home.

When working in a hospice environment, the facility will provide some training not only in how to care for a patient but also in how to make them comfortable as they prepare for death. There are certain personality traits such as patience and compassion that are required for such a role since most patients are elderly, disabled, or dying.

How to Get Hired

Most individuals wishing to get a job as a hospice aide look for employment with a home health care services company. This is usually the best way to get hired because the patients and their families go to such a company to hire this type of individual. It helps to take training classes or work through a high school co-op to prepare for this role. In turn, this also helps to get hired much faster.

For the hospice aide wishing to get hired into a hospice environment, it helps to apply to the facility directly. Usually these facilities are looking for background and experience since patients who come in here require a certain type of personality. It is also helpful to have experience working with a dying patient.

Although there is no specific educational requirement for this position, experience helps to get hired. It is especially true for families who are hiring someone to come to their home and care for their loved one.

Job Prospects, Employment Outlook, and Career Development

There is expected to be a better than average increase in the hiring of hospice aides. This role is expected to grow as families continue to look for individuals to care for their loved ones in a home or hospice environment. Since this is an entry-level position, the requirements for hiring are much lower. Therefore, it appeals to those who may not have a college education. There is a high potential for growth since people oftentimes don t want to work within an entry-level role.

As more hospice centers become a popular environment, the need for hospice aides will increase. So, too, will the need to keep aging or disabled family members in their home rather than sending them to a nursing home. This means there will be an increased need for individuals in this role, and within the home health care services business.

Working Environment

Depending on the type of facility a hospice aide wishes to work within, the environment may be slightly different. If focused within home health care, the individual will often travel to patient s homes. The assignment may last for a short time or may become a regular job. Individuals in this role care for more than one patient in their home or sometimes travel between a variety of different patient s homes. It all depends on the patients, the workload and the hours required. However, if focused within the home health care niche, hospice aides can expect the working environment to be an actual home. If focused within the hospice niche, it is expected to work within such a facility or institution. These facilities strive to create a cozy environment for their patients so they can be comfortable before they die. This work environment is more like an actual medical facility, but feels cozier.

Salary and Benefits

As of 2006, the average earnings for a hospice aide was about $9.34 an hour. The range averaged anywhere from $7.99 all the way up to $13 per hour depending on the facility. In some instances, a home health care company pays slightly higher depending on the environment and patient s needs. The more experience an individual has, the more they can expect to earn per hour.

Since these are hourly positions, most of the time hospice aides work without any true benefits. Any travel expenses incurred are the individuals responsibility and are often not reimbursed. Instead, a flat hourly rate is given to hospice aides. For those working within a hospice environment, they can expect some benefits if they are hired by the facility directly.

The University of Texas at Arlington s College of Nursing and School of Social Work will use nearly $900,000 during the next three years to fund research to improve connections between nurses and social workers before they meet in the health care workplace.

The U.S. Department of Health and Human Services Health Resources and Services Administration recently awarded an $862,532 Advanced Nursing Education Grant to Judy LeFlore, a professor and interim associate dean for research of the UT Arlington College of Nursing . The funding reflects the growing popularity of interprofessional education and the important role social workers and nurses play in treating patients holistically, she said.

There is a realization from the federal government all the way down to academia that education of health care professionals in silos is not very effective or efficient. We need to shift the task of learning to work together as a team to the education end, rather than the service end, LeFlore said.

LeFlore will work with School of Social Work professors Gail Adorno and Joan Blakey as well as fellow nursing professors Mindi Anderson, Patricia Thomas and Sharolyn Dihigo and clinical instructors Lindy Moake and Sara Moore.

Beginning in Fall 2013 and again in Fall 2014, about 40 graduate students, from the School of Social Work and the pediatric and neonatal nurse practitioner program, will be selected for the grant program. They ll work together in small groups for the next year to come up with care plans for selected case studies. Professors will teach the students about each profession s roles and responsibilities, distinct professional values, as well as identifying barriers to teamwork between social workers and nurses in the workplace.

There aren t many models for how to teach nurses and social workers to work collaboratively. Yet, great working relationships between nurses and social workers translates into patients receiving better, more comprehensive care, Adorno said. This initiative with nursing will allow us to develop a unique model of interprofessional education enhanced by technology in a specific practice setting neonatal and pediatric health care.

In addition to in-person collaboration, the student teams will use the virtual world of Second Life to meet and work together on projects. They ll also work through simulations with computerized patients in UT Arlington s Smart Hospital . a state-of-the-art clinical lab with 32 high-tech simulation stations. The program also will have a service-learning component, which will include volunteer work at SafeHaven of Tarrant County, a domestic violence shelter with locations in Arlington and Fort Worth.

LeFlore and the research team hope to use what they learn to design a course on team dynamics for social work and nursing graduate students to take together in the future. They believe the model they establish could be applied to other areas of care, such as the growing field of geriatrics.

How does seller financing work in a home sale?

I’ve been looking for an affordable house, which is no easy task given my moderate income and slightly messed up credit record. I noticed an ad for a house that says “Seller financing available!” What does this mean, and can it help me?

Answer

You may want to look into this further. In rare cases, a seller will agree to loan a buyer part or all of the money to buy the property. The seller’s motivation may be to create an incentive to buyers who can’t borrow enough from a bank or commercial lender to buy the house. Or the seller’s reasons may be tax related, since financing your purchase would allow the seller to spread out the income from the sale over a number of years.

Seller financing can be carried out in one of two ways.

The first is for the seller to “take back” a mortgage on the house. You, the buyer, sign both a promissory note (promising to repay the loan) and either a mortgage or a deed of trust (allowing the seller to foreclose if you fail to pay). In return, the seller signs a deed transferring title to you. Because you hold the title, you can sell the house or refinance. But you must keep making the agreed-upon payments to the seller.

The second and less popular possibility is for the seller to keep title to the property for as long as it takes you to pay off the loan. The contract you and the seller sign is known by various names, including “contract for deed,” “contract of sale,” “land sale contract,” or “installment sales contract.”

It works like this: The contract states that the seller will keep title to the property until you pay off the loan. (You normally pay the loan off in a series of regular payments, similar to a standard mortgage.) After you pay off the entire loan, the seller signs a deed transferring title to you. Because the seller keeps the title over the life of the loan, you cannot sell or refinance the property until all payments are made and the title is transferred — an obvious reason for the unpopularity of these contracts.

Fall 2012: Pedagogy in Urban Classrooms

This course examines the relationships between political, economic, cultural, and educational contexts and what occurs in urban schools and classrooms. The course defines pedagogy broadly, as consequences of sets of relationships among factors both external and internal to schools. Sur le thème de la fabrication additive (3D)

Jeudi 29 juin a eu lieu la cinquième Soirée des partenaires de l’école, organisée par le service des Relations industrielles. L’événement a rassemblé environ quatre-vingt personnes, industriels, représentants du secteur économique régional, universitaires et étudiants.

Une table-ronde, animée par Fabien SOULIÉ, enseignant-chercheur, a abordé le thème de « La fabrication additive dans les applications industrielles, citoyennes, de la recherche et de la formation ».

Stéphane ABED, président de la société Poly shape, Yann LEFEBVRE, président du FabLab « LabSud », André CHRYSOCHOOS, directeur du centre PRO3D et Christian JORGENSEN, professeur au CHU de Montpellier et directeur de l’unité INSERM “Cellules souches, plasticité cellulaire, médecine régénératrice et immunothérapies”, sont intervenus pour partager leurs expériences et échanger sur les applications de ces technologies.

Les diverses interventions des invités ont permis de montrer le spectre très large des champs d’applications de ces nouvelles techniques de mise en œuvre de la matière, allant de l’industrie de pointe à la recherche médicale avancée, sans oublier les applications standardsaccessibles à tous ” précise Fabien Soulié.

Les présentations ont aussi su faire ressortir toutes les étapes de la démarche de conception, de dimensionnement et d’optimisation, préalables à la phase d’impressionproprement dite ” ajoute André Chrysochoos, concluant que “la fabrication additive est apparue aux participants comme une solution complémentaire aux techniques classiques de fabrication, mais possédant ses propres limites “.

Which Hospice companies are Good to work for?

Maybe it’s already been mentioned but it might be good to look at nonprofit hospice companies. They tend to be less focused on money and have more services for the patient/family.

That’s not always the case, though. Where I live, the not for profit hospice is more concerned with their public image than pt. care. I work for a for profit hospice and our administration is very concerned with pt. care. heartbeat

I don’t know that I totally agree with the profit vs non-profit and that is why I did not mention it in my last post. For-profit companies will tell you that the only difference between them and NFP companies is that the NFP companies do not have to pay taxes into the community. We have 7 competing hospices in the area that I am no more impressed by the NFP agencies than the FP’s. There is an older, national NFP here in the area and I interviewed with them at one time–wages were 80% of the market average here in the area and the benefits were horrible(the excuse was they were a NFP and couldn’t afford to pay anymore)–this company is also known for working their nursing staff into the ground (they are leaving in droves right now) and not accepting the expensive patients . They also have an inpatient unit that several in the community feel is misused to substitute for continuous care and our company has taken several of their patients that have wanted to stay at home for end of life care as they were informed that their patients are typically transferred to their inpatient center to die). As I have said, there are good and bad FP ‘s and NFP’s. If I would suggest anything to look at, it would be wheter a company is locally owned or nationally owned. Nationally owned companies are paying an entire staff at the top of the chain and the money has to come form somewhere (sometimes shows up in higher nursing caseloads or poor patient suppiles). Not that all of them are bad–some locally owned companies are started by people with intentions other than patient care. The important thing is to ask the right questions and take your time selecting the right company. I would continue to insist that your best resources are speaking with current and previous employees of the company you are considering working for. I say this because once you are trained and in a position, it becomes harder to leave if you find out the company misrepresented itself as most of this that are in this profession struggle with knowing that when we leave, there will be even less people to care for the patients.

I work for a nonprofit, part of a larger religious nonprofit. The downside is that being under senior services-which is traditionally low paid, I think we are way underpaid. Hospice should be advanced home health and pay for the years of general nursing experience you come with.

But the plus is that the focus is on compassionate care, for the most part. We would not turn down a patient with financial needs, we have resources to help those in dire need of basic comforts. Our nurses are not pushed to meet financial goals. Now, they are overworked because we are too short staffed, too short staffed because we are not paid well enough to attract the experience we need, but there are measures being taken to address those things. At least I feel that there is interest in improving our org and fundamentally many good people. We are currently changing leadership and I am hoping that they hire an outsider with strong leadership skills to both corral some of the internal management issues and better advocate for us.

Yes Yes Yes. People, there is a nursing shortage! Good help is hard to find. Believe me when I tell you that life is too short to be working in a position or job that you don’t enjoy. If you are true of heart and want to consider hospice nursing a profession, take the time to investigate the company and the position–don’t be afraid to ask questions–you have the right to get up every day to go into a postion that is rewarding (not that some days won’t be difficult) and just as much right to be able to balance home and work and to go to bed at the end of the day with a clear conscience.

Is it possible for a new RN to get into hospice without hospital training? Would oncology by a suitable specialty to start with? I think I would enjoy home care eventually. Any suggestions would be greatly appreciated!

How does hospice care work

In order to receive hospice care, two physicians must confirm your eligibility. This typically includes your attending physician or specialist and the Optum Hospice Care physician.

After it is confirmed that hospice care is the right care for you, one of our interdisciplinary team members will review what you and your loved ones can expect from hospice care. During this time, you will gain an in-depth understanding of the clinical care and psychosocial support available through hospice care. Once the paperwork and review is finalized, your hospice care begins.

Optum Hospice Care begins its visits almost immediately. Members of the interdisciplinary care team, such as the hospice physician and nurse case manager, nursing assistants, chaplain, social worker and trained volunteers will be in touch to make sure you get the care you need. Optum also manages ordering and setting up of any necessary medical equipment, such as a nursing bed, or medications so you and your loved ones can focus on other things, such as spending time together.

The cost of hospice care, including medical equipment and prescriptions related to the terminal illness, is typically covered by Medicare. Medicaid and most private insurance plans.

Depending on the patient’s insurance benefit, the admit process will vary. Only Medicare requires hospice care to be received in the last six months of life, recognizing the patient might outlive that time period and can receive hospice care for longer, given he/she is clinically appropriate.

Because African Americans make up a substantial percentage of the urban population, and are also over represented among urban residents facing unrelenting social and economic problems, the BSW, MSW and PhD programs have a major focus on preparing its graduates to address, systematic, ally and strategically, issues of poverty and socioeconomic disadvantage, interpersonal and community violence, substance abuse and mental health problems, social injustice and discrimination.

The MSW department’s mission complements that of the university and is consistent with the purposes of social work education by preparing autonomous social work professionals to become leaders in the urban community. The department’s emphasis on alleviating poverty and human suffering, oppression, and injustice through multi-level and specialized fields of practice further addresses the hallmarks of the social work profession.

Ask people what they want in a job, and meaningfulness looms large. For decades, Americans have ranked purpose as their top priority — above promotions, income, job security, and hours. Work is a search “for daily meaning as well as daily bread,” wrote Studs Terkel after interviewing hundreds of people in a striking array of jobs. Yet all too often, we feel that our work doesn’t matter. “Most of us have jobs that are too small for our spirit. Jobs are not big enough for people.”

What makes a job meaningless? After more than 40 years of research. we know that people struggle to find meaning when they lack autonomy, variety, challenge, performance feedback, and the chance to work on a whole product or service from start to finish. As important as these factors are, though, there’s another that matters more.

Consider the following jobs. They all meet some of the criteria above, yet about 90 percent of people fail to find them highly meaningful.

Enriching the Meaningfulness of a Job

Becoming a neurosurgeon isn’t for everyone. The good news is that there are steps we can take to make jobs more meaningful — for ourselves and others.

In many cases, our jobs do have an impact, but we’re too distant from the end users of our products and services. Think of automotive safety engineers who never meet the drivers of their cars or medical scientists who don’t see a patient. By connecting directly with these end users, we can see our past and potential impact. When university fundraisers met a single student whose scholarship was funded by their work, they increased 142 percent in weekly phone minutes and over 400 percent in weekly revenue. When radiologists saw a patient’s photo included in an x-ray file, they wrote 29 percent longer reports and made 46 percent more accurate diagnoses.

This is why leaders at John Deere invite employees who build tractors to meet the farmers who buy their tractors, leaders at Facebook invite software developers to hear from users who have found long-lost friends and family members thanks to the site, and leaders at Wells Fargo film videos of customers describing how low-interest loans have rescued them from debt. When we see the direct consequences of our jobs for others, we find greater meaning. “The greatest untapped source of motivation,” Susan Dominus explains. “is a sense of service to others.”

Of course, some jobs are simply not designed to have a major impact on others. In these situations, people often make the mistake of treating their job descriptions as fixed, overlooking the fact that they can take initiative to alter their own roles. Wrzesniewski, Jane Dutton, and Justin Berg call this job crafting — adding, emphasizing, revising, delegating, or minimizing tasks and interactions in pursuit of greater meaning. For example, hospital cleaners who lack patient contact stepped up to provide emotional support to patients and their families, and technology associates began volunteering for mentoring, teaching, and training roles.

When people craft their jobs, they become happier and more effective. In an experiment at Google. colleagues and I invited salespeople and administrators to spend 90 minutes doing the Job Crafting Exercise — they mapped out ways to make their tasks and interactions more meaningful and contribute more to others. Six weeks later, their managers and coworkers rated them as happier and more effective. When they developed new skills to support more significant changes, the happiness and performance gains lasted for at least six months.

Like all things in life, meaning can be pushed too far. As the psychologist Brian Little observes. if we turn our trivial pursuits into magnificent obsessions, we gain meaning at the price of manageability. When the weight of the world is on our shoulders, we place ourselves at risk for burnout.

Yet most people are facing the opposite problem in their jobs, of too little meaning rather than too much. Against this backdrop, the chance to help others can be what makes our work worthwhile. “Suffering ceases to be suffering once it finds a meaning,” wrote Viktor Frankl in Man’s Search For Meaning . “Being human always points, and is directed, to something or someone, other than oneself — be it a meaning to fulfill or another human being to encounter. The more one forgets himself — by giving himself to a cause to serve or another person to love — the more human he is.”

Termination Guidelines

  • Clarify expectations and obligations from the outset. The Office Policies are the best way to articulate and discuss such expectations and obligations. (Zur Institute’s Clinical Forms .)
  • Be familiar with the relevant guidelines and standards in your professions’ Codes of Ethics with regards to termination and abandonment issues. Summary of the different Codes of Ethics on Termination Psychotherapy and Counseling
  • Some terminations are short and swift, while others may be long and protracted. Then termination can have different meanings and be just an end of a phase in intermittent-long-term therapy. The form and type of termination depends on the client, setting of therapy, therapeutic orientation, quality and type of therapist-client relationship and the therapist.
  • If necessary, review with patients their insurance coverage, limits to managed care contracts, and how utilization review may impact on termination. Set up arrangements for addressing patient treatment needs if continued authorization is denied.
  • Make adequate arrangements for coverage during any periods of planned or unplanned absences.
  • Provide patients with referrals to other treatment sources, if needed, and work to assist them in their transition to other health care providers.
  • Be cautious in regard to termination of patients who are in crisis. Try to avoid terminating clients who are in acute or temporary crisis due to payment issues.
  • The average number of visits among U.S. patients receiving psychotherapy in 2007 was 8 sessions, according to a study published in the American Journal of Psychiatry.
  • 41 percent of patients in the U.S. quit psychotherapy “prematurely,” according to a 2010 study in the journal Psychotherapy Theory, Research, Practice, Training.
  • Many clients unilaterally decide to drop out of therapy. They may do that with a phone message or by simply not showing up to their next scheduled appointment. You must remember that it is the client’s prerogative and choice whether to continue in therapy or not. Except in extreme situations, such as when the client poses a danger to self or others, you need to respect their choice. Do not tacitly condone patients dropping out of treatment when your clinical judgment indicates continued care is needed. When clinically and otherwise appropriate, notify the patient of your assessment and recommendations. There is no ethical, clinical or legal mandate to send a registered letter to client. Different clients and situations may require different actions and, at times, lack of action.
  • The question of whether therapists need to send a letter to clients who unilaterally dropped out (i.e. pre-mature termination) was recently addressed by Davis & Younggren in a 2009 PPRP article, where they clearly stated “In ordinary circumstances, however, letters are typically unnecessary and potentially counterproductive to the natural dissolution of the relationship (Davis, 2008). For instance, the client might feel embarrassed or scolded for his or her oblique termination and be less inclined to return. The client might perceive the psychotherapist’s actions as controlling and unnecessarily intrusive. It might seem that the psychotherapist is trying to break up with the client or get rid of him or her with such a formal action. Routine letters of closure not only present an unrealistic administrative burden on the provider, they add to the risk of negative client reactions.” (p. 575)
  • It is not unusual for therapy to break down rather than go through a smooth, clear or distinct termination process with patients with personality disorders (i.e. BPD) or those who were diagnosed with severe mental illness, such as schizophrenia or Bi-Polar disorders. Termination with these clients can be very abrupt or very long, painful, confusing and tumultuous. Obviously, each case should be handled according to the specific context of therapy. The context of therapy includes client, setting, therapy and therapist factors.
  • You can terminate treatment with clients (or another person with whom the client has a relationship) who threaten or stalk you, your family member or your employees via phone, email, online, in-person or other means. You can also terminate treatment with clients who intrude into your private life via the Internet or in “real” life. You are allowed to protect your privacy and secure your own, your family members’ and employee’s sense of privacy and safety. (Document, document and document.)
  • You must terminate therapy when it becomes reasonably clear that the patient no longer needs the service, is not likely to benefit, or is being harmed by continued service. For those patients who have ongoing treatment needs, when appropriate, one should offer to provide them with assistance with referrals to other appropriately trained and accessible professionals (except when we are being threatened, assaulted, stalked, or other relevant situations).
  • Document discussions of termination issues, agreements reached, decisions made and their rationale, and, when relevant, document the recommendations and follow-ups. Purchase a Termination Summary form
  • Termination of treatment is not always a permanent ending of the professional relationship. Termination is often not relevant during, or an end of phase in, intermittent-long-term psychotherapy. These forms of therapy may continue throughout the life span of individuals and families.
  • Termination is a phase of each patient’s treatment. If possible and appropriate, plan for it, prepare for it, process it. Additionally, each clinician should consider termination in light of their theoretical orientation and treatment approach, each patient’s/client’s diagnosis and treatment needs, and any relevant diversity factors that might impact the process.

    How do IRS tax relief programs actually work?

    So how does this work exactly? You run up a huge tax debt with the IRS, wave a magic wand and then voila — you can settle your IRS debt for pennies on the dollar? Let s get real. there s no way that IRS tax relief actually works like that.

    Sure, the late-night TV commercials and tax resolution firms make it seem easy: Just hire us for $2,500 and we can settle your tax bill for pennies on the dollar! It all seems so easy and/or unbelievable. So what gives?

    The truth about getting IRS tax relief on your back taxes lies somewhere in between. Settling tax debts with the IRS isn t easy, but it isn t a scam. In this article we want to tell the story of how IRS tax debt relief programs work in real life, and one of our favorite clients, Ethel.

    We don t need easy, we just need possible

    We represented Ethel, a retired woman who was suffering from thyroid cancer. Her prognosis was good, but treatment had to be ongoing. Ethel could no longer drive to the doctor, and because there was no public transportation where she lived she had to get a taxi to go to her treatment once a week.

    As we looked at Ethel s financial situation and $49,000 tax bill, we determined that she qualified for three different types of tax relief. Option one: Because she had old debts, her tax returns were filed, and she had other credit card debt, she could file Chapter 7 bankruptcy and wipe out all of the tax debts. Ethel didn t want to file bankruptcy; she liked her credit cards and didn t want to have to rely on anyone to sign for loans in the future. Even though bankruptcy would result in a net payment to the IRS of $0, this option wasn t all that palatable to her.

    We then went over option two: The IRS has a limited amount of time to collect on an assessed tax. Typically, with some exceptions, it is ten years. Ethel s debt was 5 year old from the date of assessment. Which meant in five years, the debts would be written off. Based on Ethel s financials we believe that she could be placed in hardship status with the IRS. This means the IRS wouldn t try to collect anything from her, and she wouldn t have to pay the IRS anything (except tax refunds the IRS would intercept). Getting into hardship status was more or less a sure thing. However, Ethel didn t want this tax bill hanging over her for 5 more years.

    She wanted to know if there was a quicker way to get rid of it. And there is. Using the Offer in Compromise Doubt as to Collectability tax settlement program, we calculated that Ethel could submit $2,000 to settle a tax bill of $49,000. We submitted Ethel s Offer in Compromise package and hoped for a quick resolution.

    Unbelievably, the IRS disallowed Ethel s taxi expenses — they said it was not a reasonable or necessary expense . I explained that Ethel would die without her cancer treatment, so I disagreed. let s say somewhat passionately.

    But this is the way government works. What makes sense to you and me, doesn t make sense to a bureaucrat. Luckily, they weren t the last word.

    I filed an appeal of the Offer in Compromise rejection, and expected an easy win. But still, the IRS appeals officer was giving me a hard time about Ethel s taxi fare. WTF.

    I used some of our negotiations techniques (sorry I can t share them publicly — they re too valuable for our clients) and there was a pause by the Appeals Officer. Finally he said you bring up a good point. Your $2,000 offer is more than the IRS could expect to collect, so I am going to recommend its acceptance.

    The lesson of Ethel

    Even easy IRS tax relief may not be easy. Sometimes the most persuasive negotiation techniques in the world must be used to get the IRS to act in a way that is humane. Everything ended well for Ethel! She finished up her chemotherapy, and is cancer free. And now she is back to driving!

    The Forensic Practice graduate certificate program, formerly known as the Forensic Social Work program, is offered on a part-time basis creating flexibility for all students. A majority of the courses are offered online, as well as in the evenings, allowing students to co-ordinate their schedule accordingly. Subsequently, our Independent Research Project course is offered every semester where students select an agency of choice to volunteer and gain experience. This program will also be of interest to seasoned professionals working in a range of health and human services agencies who wish to refine their skills in the area of forensic practice. Many recent graduates of university and college programs seeking to receive specialized knowledge in forensic practice may also find this program to be extremely beneficial for professional development.

    Forensic Practice consists of health and human service workers who occupy and come into contact with professions found in corrections and parole, child welfare and protection, and mental health and well-being. It encompasses criminal and civil matters, interventions with violent offenders, issues of child maltreatment, assessing parenting capacity and substance abuse.     This paper introduces a critique of the experimentalist strand of the evidence-based practice movement in social work. It does so by drawing on the available body of knowledge that is found within the field of judgment and decision making. The nexus between experimentalist evidence-based practice and rational choice approaches to social work decision making is demonstrated. Subsequently, it argues that the properties of social work decision tasks are more likely to facilitate naturalistic than analytical decision making strategies. Recognition-primed decision making is introduced as a possible alternative to practice guidelines and decision aids. Finally, the consequences of embracing the principles of recognition-primed decision making are considered regarding social work practice, education and research.

    I recently set up our new accountant with the BM vpn and NW clients so that
    she might be able to access our quickbooks 2000 database via a mapped drive
    while connected to our BM3.7 server. She can connect fine, and althought QB
    (a known resource hog) is fairly slow for most common operations over the
    vpn (both client and server have SDSL connections over 500kbps), it is not
    entirely unworkable. SOME particular operations, however, take an
    IMPOSSIBLY long time (especially find operations in the records DB),
    literally many, many orders of magnitude slower than everything else.

    Is this really just due to the number of drive accesses involved in these
    operations, or is there some other buggy issue at work here?

    Does anyone know how to troubleshoot/work around this problem effectively?
    We are really trying to avoid having to shutdown/lock the QB system long
    enough for our accountant to copy the DB to a local machine, do her work,
    and then write it back out to our system. We really need a way for her to
    be able to work remotely AND concurrently with the rest of us.

    You will probably need to take some traces (Ethereal on the PC’s
    involved) to see what kind of traffic is involved. Might be as simple
    as needing to reduce MTU size on one or both ends of that connection.

    Perhaps simpler would be to use pcANYWHERE, VNC or terminal server/RDP
    to remote control an internal PC over the VPN to do the work remotely.

    My experience with Quickbooks over a lan is that the newer versions can be a
    little slow as it is. I would venture to say that there is just way too
    much data to push through that small DSL pipe.

    I apologize for my own sluggish response. I actually found a post on
    another site that echoed Craig’s suggestion of running quickbooks on a local
    machine and having the remote client connect to it using pcAnywhere or the
    like so that all the heavy drive accesses remain local. I’m going to give
    that a try.

    Lackawanna College’s Surgical Technology Program is designed to be completed over a two-year period with day, evening, and Saturday courses as well as on- and off-campus clinical hours. Each program also includes summer session courses and clinical lab hours.

    Acceptance into Lackawanna College’s Surgical Technology Program is based on a selective admissions process with a maximum of 15 students selected annually.

    Degree information

    Through a balance of classroom instruction and clinical experience, the student will acquire applied skills and knowledge in the field of Surgical Technology.

    Surgical technologists work in the operating room under the direct supervision of a surgeon and a registered nurse. Surgical technologists help facilitate the draping, and prepping of the surgical patient. They gown, and glove other team members. They get all the instruments, supplies, equipment necessary for specific surgical procedures. As a member of the sterile team, surgical technologists will hand all necessary instruments and supplies to the surgeon and other team members.

    Surgical Technology Program classes are given days, nights and on weekends (based on the availability of the instructors). Clinical externships, however, must take place during the daytime.

    • To facilitate the hands-on, supervised experience in the operating room performing the duties of a surgical technologist.

    Prospective students must meet the college’s minimum requirements. Applicants must submit proof of high school or GED completion with official high school transcript or GED equivalent. Lackawanna College will determine the applicant’s current academic ability based on a combination of following:

      • Recommended SAT scores: 470 Verbal and 470 Math.
      • Recommended ACT scores: 20 or higher in each relevant section.
      • Most scores are valid up to 3 years from test date.
    • College Transfer Credits

      • Applicants without valid SAT/ACT scores and who have not previously earned college credits will be required to take the ACCUPLACER exam for determination of current academic ability. Many home health aides will represent themselves as independent contractors, ostensibly relieving the hiring individual of these tax obligations. However, it is the responsibility of the hiring individual to be sure that the aide truly is an independent contractor and is therefore paying their own taxes. In many instances, the aide will not meet the legal criteria as an independent contractor. If the aide has not met his or her tax obligations, this responsibility falls to the employer. This can be a serious obligation because it may involve interest on back taxes, civil fines and the possibility of criminal penalties. Potential private employers should seek the advice of a labor lawyer to assure appropriate hiring practices with respect to federal tax laws.

    Case Example: Mr. L hired a home health aide for his mother on the basis of a recommendation from a neighbor who also employed this individual. Mrs. L needed help because of a minor stroke that left her unsteady on her feet. Initially, Mrs. L only needed four hours of help per day to assure that she had a bath, that two meals were prepared and that the weekly shopping was completed. Occasionally, the aide also accompanied Mrs. L to some of her many medical appointments.
