    How Much Can I Borrow?

    1) How Much You Earn

    The amount you can borrow will vary between lenders but the rule of thumb is three and a half times your annual earnings.

    You may get up to four times your earnings, particularly if you have a good mortgage broker.

    For a couple buying together typical variations would include:

    Couple 1. two and a half times both annual incomes.

    Couple 2. three to three and a half times the greater income plus one year of the second income.

    Here s a secret

    Assuming you have a regular income and clean credit history you re likely to get a loan fairly easily.

    Despite the impression you may be given that you ve got to jump through the hoops, there is strong competition between lenders to get your business.

    Some lenders now use more sophisticated credit rating methods, where they examine your income and your outgoings.

    The idea is that every borrower has unique circumstances. Someone with teenage children and high outgoings can t afford to borrow as much as a singleton earning the same salary.

    2) Depending on How Much The Property is Worth.

    Most lenders will loan up to 75% of the property s value. (This is known as the loan to value ratio).

    Some lenders might lend more a mortgage broker would know who but you would probably have to pay over the odds eg a higher interest rate.

    Depending on the area you want to buy in, the lender may refuse a loan. for example if they feel the property isn t expensive enough for the area.

    More often, it s the opposite case where a property is seen as too expensive.

    Some mortgage lenders will put a limit on the amount they ll allow on certain types of property. For example, thatch-roofed, timber framed . and houseboats? Well that s a whole different deal.

    3) Depending On How Much The Mortgage Lender Thinks You Can Afford

    You may be able to get a mortgage which stretches your budget to the limit but leaves you in trouble when you have to pay the other costs involved in buying your home and its future running costs

    Some lenders will want to estimate this by checking your average outgoings eg your household bills, any debts etc. Some will get you to fill in a detailed questionnaire either by hand or on the phone or online etc.

    Note that how much you can borrow is not necessarily what you can afford

    If you re a first time buyer it will always help if you can show you ve been paying regular rent for a similar amount to what your intended mortgage payments will be.

    All you need to know about home-schooling

    From overcrowded classrooms to ruthless bullies, some children fare better when their education takes place at home. We tell you more about home-schooling and some SuperMoms who have chosen it.

    Don’t take the decision to home-school lightly

    Mari-Lynn Kent, who started home-schooling her daughter Emma (11) this year, wrestled with the decision to home-school for about a year. “It’s a big decision and if you make the wrong choice, your child suffers.”

    A number of factors motivated Mari-Lynn to home-school her daughter. Emma has a very busy extramural schedule, but trouble with bullies at school and the sexual conversations between 11-year-olds on the playground were also motivating factors. “I felt the care wasn’t there, especially when it came to the bullying,” says Mari-Lynn. “The child sitting next to Emma was hitting her. I had to speak to the teacher three times before she was moved.”

    Discipline is key

    Home-schooling allows Emma to focus on her passion – dance. “We try to work ahead, keeping Fridays open for her extramural activities such as ballet, piano, drama and choir,” says Mari-Lynn.

    Mari-Lynn, a mother of two, has a very structured approach to home-schooling. A distance education company supplies her with a curriculum and tells her what textbooks she needs. “I wanted everything under one umbrella. The company also uses teachers to set up exams and the curriculum,” she explains. “They advise you on how much work you should do each week so you don’t fall behind.”

    What about exams?

    The company requires students to write exams in June and December. “She can write them at home, but there needs to be a moderator. I’m not allowed to moderate, but can ask someone else, who’ll have to sign an affidavit, swearing that Emma wrote in an exam environment.”

    If Emma then ever goes back to a traditional school, she will have the certificates to prove that her education is up to standard and she won’t have to write an entry level exam. Mari-Lynn plans on home-schooling her daughter until high school. “Emma wants to go to a traditional high school and if she still feels that way later, then I’ll send her to one.”

    A more informal approach

    Other parents prefer an informal approach to home-schooling, says Bouwe van der Eems from the Association for Homeschooling. “Some parents prefer to be guided by the child’s interests, focusing on the subjects their children are interested in. You don’t have to follow a one size fits all curriculum like a slave. Parents can pick and choose from a variety of curriculums.”

    How do home-schooled children matriculate?

    From Grade 10 onwards, you need to follow the national curriculum if you want your child to finish school with a South African matric qualification. A home-schooled pupil must submit projects and write exams just like those in traditional schools. Parents can do this through home-school curriculum providers.

    Many people prefer the international matric qualifications, like the British Cambridge or US GED diploma, over a South African matric. “These qualifications are recognised by universities. Unlike a South African matric qualification, you don’t need to complete and hand in projects. You just prepare and write the exams,” van der Eems says.

    Benefits of home-schooling

    • A child can work at her own pace, says Anel Annandale, a Cape Town child psychologist. “Often children who are home-schooled learn to take greater responsibility for their own learning and development.”
    • It gives you and your child greater flexibility when it comes to time, says Annandale. “It allows a child with a specific gift for a generally time-consuming interest, such as a gifted gymnast or horse rider, to schedule schoolwork around her practice and training schedule,” she adds.
    • “Peer pressure, bullying and high levels of competitiveness are less often a factor for home-schooled children,” Annandale says.
    • Home-schooling can lead to closer family relationships, says Raquel Ferreira, a Johannesburg educational psychologist. “Extended periods of time together strengthen family relationships, not only between the child and his parents, but also with his siblings. As they get to know each other, the family members form bonds that last a lifetime,” she says.
    • One-on-one tutoring means that there is more time available to actually learn and teach, says Annandale. “Teachers in traditional schooling systems often waste a lot of time on discipline issues which arise purely because of the large number of students in a class,” she says.
    • “Home-schooling is often better suited to children with learning difficulties because parents can focus more in-depth on specific subject areas that their children struggle with,” Annandale says. “Many of the behaviour issues that are often associated with learning difficulties, such as the hyperactivity and impulsivity that children with ADHD (Attention deficit hyperactivity disorder) often exhibit are tolerated more easily in a home-schooling environment than it may be in a traditional classroom.”
    • If a child can’t cope in traditional or remedial school, then home-schooling might be your best option, says Megan Robinson, an educational psychologist from Johannesburg. “Home-schooling can work well for the unique child who’s tried all other options and hasn’t coped. Similarly, a child who’s suffering from an illness or physical difficulty may have no other option. A child with a severe emotional or social disorder will also cope well in a home-school environment.”

    Drawbacks of home-schooling:

    • “Home-schooling often offers difficulty for university entrance and coping skills,” Robinson says. “Often children who go through home-schooling battle to adjust to the demands placed on them in the tertiary environment or in the working world.
    • Children might miss out on learning opportunities, she adds. “Normal schooling teaches the child about more than just the general curriculum and provides the developing child with vital skills and life lessons. For example, it teaches social interaction, discipline and structure, moral development, leadership skills and other valuable life skills.”
    • Children who are home-schooled often miss out on healthy competition and extracurricular memories, says Ferreira. “Parents need to involve their children in club sports so as to still allow this area of development.”
    • It might be impossible for parents to juggle work with home-schooling. “Often a parent has to stop working so that they have time to home-school, but this then has the associated drawbacks of loss of income for the family,” Annandale says.
    • Annandale warns that home-schooled children sometimes have fewer opportunities to socialise with other children their own age and may become withdrawn and reserved in social settings. “Children who are very sociable or really enjoy or excel at group activities and group sports generally fare better in a traditional schooling system.”
    • Some subjects are best taught by skilled teachers, says Ferreira. “Some home-schooling parents have a hard time with subjects such as algebra or physics. Parents may need to look into the use of private tutors for this reason.”

    Beware the challenges

    • Home-schooling is hard work, says van der Eems. “You need to prepare and be willing to make sacrifices, especially when it comes to your time. The home-school never closes. It’s open 24 hours a day, seven days a week.”
    • Home-schooling isn’t the norm, he adds, and not everyone might agree with your choice to home-school your child and you might often need to defend your choice.
    • Schools have more teaching resources than home-schooling parents. “Parents may not have access to the same resources that schools do. For instance, practically demonstrating a chemistry experiment may require a lot more effort and money than it would in a traditional school with a fully-equipped science lab,” Annandale says.

    Are you ready for the responsibility?

    Home-schooling is a huge responsibility warns van der Eems. “When you take your child’s education into your own hands, you take on a huge responsibility,” he says. “It can weigh on your conscience and you can question if you’re doing the right thing for your child’s education and their future. However, there are many support groups and internet discussion forums where parents can support one another and ask for advice.”

    I was a chronic trouble maker in the past and got a hold of Joe at GUAB.     The Best! My 20 year old daughter slapped her boyfriend during an argument and went to jail for the first time in her life. She was terrified and I was terrified for her. She got a 1,000 dollar bond and would have had to sit in jail waiting for her court date if it had not been for the staff at Give U A Break.     HOSPICE FUNDING:
    WHAT YOU NEED TO KNOW

    While the hospice’s source of funds may be the last thing on your mind, hospices are businesses. The business end of hospice can affect the care your loved one receives. Hospices receive funds from government programs or private insurance, from donations made by the public or other corporations, and from grants donated by charitable foundations. Hospices are reimbursed by Medicare, Medicaid, or private insurance for care provided to the terminally ill.

    Except for continuous around-the-clock home nursing care, which is paid for on an hourly basis, all other services are reimbursed on a per-diem basis. Hospices are not reimbursed on a fee for service basis: rather, hospices are paid, on a basis of how many days the patient is enrolled in the program and received services: the per-diem basis. In return for payment, Medicare, Medicaid and private insurance companies expect the hospices to provide all services which the patient and family need which are related to the terminal illness. The Centers for Medicare Services (CMS) regulations mandate that the hospice comply with the regulations.

    And the per-diem basis of payment assumes that the hospice agency will actually provide those services; this payment system places much trust in the hospice management. However, fewer services provided to a patient results in more money retained by the hospice. Obviously, the system can reward fraudulent hospices with increased income while honest, dedicated hospices provide full services as required.

    Donations to Hospices

    While nonprofit hospices can solicit as well as receive charitable donations, for-profit hospices can only receive donations. After the death of their loved one, some families suggest making memorial donations to the hospice they used. This is a very significant source of funds to the hospice and helps to cover expenses incurred in running the hospice.

    Nonprofit charitable hospices are supposed to provide hospice services to those persons who do not have coverage and cannot afford to pay for care. So when you donate to a nonprofit hospice, there is a greater likelihood that charitable hospice services will be provided.

    When you donate to a hospice, you cannot be sure that donations made to the local hospice will actually be used for the purpose you designate. Even if you designate a purpose, there are legal tricks of the trade, or accounting, which can make it possible for a hospice to shift the donated money or other money to other purposes than what you wished. Although there are laws stating that restricted donations must be used for the purpose designated by the donor, certain recent court cases have challenged the strictness with which these laws are enforced. Practically speaking, the government does not always look into what a hospice may do with any donations.

    While donating directly to a nonprofit hospice allows you to claim a charitable federal tax deduction, you can only be sure that your money is used for a particular purpose if you personally buy something and donate it to the hospice. Some hospices have several branch locations covering a large area. If you donate to one hospice, the management may take the money and transfer it to a completely different location. or it may use the money to help pay for unreasonably high salaries for executives.

    For-profit hospices have no obligation to provide services to anyone who does not have coverage from Medicare, Medicaid or private insurance. They can turn people away and tell people to go elsewhere. If you do choose to donate to a for-profit hospice, just remember that the for-profit corporation may take that money and pay its Chief Executive Officer many hundreds of thousands of dollars each year in salary and benefits!

    No Need to Ever Pay Privately for Hospice Services

    Whether or not your loved one has Medicare, Medicaid or private insurance, you should never have to pay out of your own pocket for hospice covered services. If a hospice asks you to pay for private duty nursing out of your own pocket when there are uncontrolled symptoms, when you already have Medicare, Medicaid or private insurance, it is very likely committing health care fraud illegally! It is important that you report this to your regional U.S. Office of Inspector General and U.S. Attorney’s office. Double-billing is a very real and despicable scam committed by rogue hospices taking advantage of the dying. Don’t let this happen to you!

    Charitable Provision of Hospice Services

    If you do not have any coverage by Medicare, Medicaid or private insurance, a nonprofit hospice can provide services to you FREE OF CHARGE as part of its charitable mission. The nonprofit status of the hospice often requires it to provide charitable services. Find a larger nonprofit hospice if you have no coverage; the nonprofit hospices are dedicated to the mission of serving those in need.

    Is Palliative or Hospice Nursing For You?

    “It must be so depressing,” is often the first comment. It’s followed by, “Oh, you must be special because it has to be really hard.” The first time I heard those comments, I was actually taken aback. But these are often the first things you hear when you tell someone that you work as a palliative or hospice nurse.

    I don’t work in palliative care anymore. But I had the privilege of doing so, part-time, for almost five years. It wasn’t depressing and it wasn’t “really hard.” In fact, it was the most rewarding nursing job I’ve ever done.

    In nursing school, we’re taught how to help people as they recover from their illnesses, injuries, or surgeries. Death is discussed, most often in terms of us trying to avoid it. However, there are some lessons on how to deal with dying patients and their family members.

    More schools are introducing a palliative care and hospice component to their nursing programs . But it is only recently that palliative and hospice care is coming into its own as a specialty that nurses want to work in.

    Often, the question asked is “why?” Why would someone want to work in palliative or hospice care? To me, it’s very simple. It’s a completely different type of nursing career than what we’re used to in the hospitals. We know that these people are dying and that they will die, sooner than later. Our goal isn’t to prolong their life, but to help them live their life as comfortably as is possible in their remaining time.

    It used to be that people just waited to die. But with modern medicine and know-how, we can help make those final days comfortable enough for many for them to enjoy them. They can visit with family members and friends. They can tie up loose ends. They can laugh and cry. They can take the time to say good-bye to those they love.

    When I was a nursing student —and later working on the floors—I came across many situations where dying patients weren’t told they were dying. In fact, often the patients would ask outright; only to be told, “No, you’re not dying” by their family members. They had decided it was best for the patient not to know.

    Even back when I was 20 and not all that experienced, I knew that was wrong. It was wrong to rob someone of the opportunity to end their life in a way that they could finish what they could finish.

    I would often say, “How lonely must it be to be dying,” and know that the people around you are lying to you and won’t talk about it. I say that because in my heart, I truly believe that the vast majority of dying people know that they are dying, even if they are told that it isn’t so. I can’t imagine what it must be like to be denied that honesty right up to the end.

    Palliative care and hospice nursing allow for that honesty. It allows us, as nurses, to help the patients do what it is they need to do. We provide to them the most intimate of care possible. What is more intimate than being there to listen to them, to talk to them, to care for them, in their final days?

    The reward from palliative care comes out throughout the time we have with our patients. When they can move from one stage to another, when they can ask for help and get it, when they can talk openly about what they want and what they can do.

    In the end, we know we have our job when we know that the passing has been as pain-free as is possible, when the families have had a chance to say good-bye, and when we can turn around and give the same care to the next patient who needs us.

    I have been a hospice nurse 7 years and can say this is the most rewarding and humbling experience of my life. We are allowed into a patients and their families lives in the most personal time of their lives. We care for the families, not just the patient. Itis continuous education for the families by the nurse walking them through their grief as well as the grief experienced by the patient. I have taken part in making finsl wishes come true.We have prepared a luau for one patient, a wedding for another, a baptism for another. Oh and the patient who wanted to see the sun rise once more. We bundled her in blankets, placed her in a gerichair and rolled up and down a snow filled driveway for the perfect view of the sunrise. Her expression was priceless. But then she requested watching the sun set. No problem, our routine was mastered by that point, and oh what a view it was that wjnter evening. So, you see, hospice nursing is so much more about symptom mgmt with medication. Its caring for the spiritual, emotionsl, and social sides of the patient and the family all while respecting final wishes and treating others ss you want to be treated ehe your time comes.

    I have had the honor of working in a faith based, not-for-profit Hospice for 10+ years and feel truly blessed to be able to do the work we do there. This is the reason I went to nursing school! Hospice and palliative care is CARE. care to listen, touch, clean, teach, pray, share, and do all you can within your knowledge and skill to help this patient, family and friends walk this journey with peace, dignity and comfort. And yes, it is very rewarding- you get more than you give often time. You learn something with each patient. That helps you take even better care of the next one.

    Hospice nursing means doing all the things you wanted to do when you were in nursing school. Comfort, care, touch, teach, laugh and cry with patients and their families. It is not depressing to me, but it is not for a nurse who gets seriously attached to every pt and can’t accept death as part of life. I have met the most amazing patients who have chosen to take control of the life they have left. It is a privilege to walk the journey with these pts and their families. It is the most rewarding nursing I have ever done.

    My choice out of nursing school was oncology. I worked there for 8 yrs. Loved it and loved my patients. Nursing staff worked together and we had our patients from diagnosis to death. We were fortunate enough to have support from our doctors and administration. We also had a psychologist who came to our floor weekly. This was for patients but more for staff. On any given day we could check obits and usually there was someone we knew. Some of our docs were not good at explaining DNR status. I explained hundreds of times to patients and families. Most people do not understand what goes on during a code. I explained. Or to encourage families to adhere to the requests made by their family member. And to think of the patient and not make selfish decisions. Palliative care should be in every hospital. Our floor was very upbeat and we did everything to make sure it was never a depressing place.
    We are all going to die at some point. My job was to ease the patient and family into the inevitable. Keep the patient comfortable. Know that sometimes you are medicating the family. If family thought the patient was hurting because they groaned or grimaced, they asked for meds to be given. As a nurse, it’s not up to u to judge whatever decision is made. Talk to the family and encourage them to keep talking to the patient. Let them know u are there and that u will be ok. Give them the peace they need to slip away

    Hospice Marketing Are You Tapping The Target Who Can Dramatically Grow Your Census?

    When it comes to hospice marketing. Most hospices focus almost exclusively on marketing to medical providers. Makes sense right? After all they have the power to refer. But doing so ignores a powerful demographic. One that has the need, the ability, the drive and the opportunity to refer to your hospice. She is the in home gatekeeper and she should be a key focus for your hospice.

    We know the struggles with consumers thinking all hospices are the same; we know the struggles of ‘the list’ given to patients by case managers. But I can tell you from 20 years of ROI studies of our hospice gatekeeper direct strategies consumers have power and they will motivate their physician to refer and they will demand the hospice partner of their choice. To the point of dramatic census growth, averaging 60 percent with our partners.

    We have several case studie s but here is one that is a great example of the power of targeting the in home gatekeeper. We partnered with a hospice that had only three provider liaisons working out of three locations at the time. They decided to expand their service area throughout the entire state. Expansion came without the power of acquisition, meaning no relationships, no existing patients. And no additional provider liaisons. Since relationships with physicians and other medical providers take a while to build, we took our story directly to the in home gatekeeper. We created an intense gatekeeper direct strategy involving social media, web, traditional marketing, advertising and recreating conversations we knew would resonate deeply with her. We used specific strategies to lower her psychological purchase price of hospice care and to build a connect between her and our hospice partner. Their census grew 300% taking them from three locations to 35 and positioning them as the largest hospice provider in their state.

    We continue to see significantly more individuals benefiting from hospice when a sound gatekeeper direct focus works in tandem with building supportive and educational relationships with providers. So for most hospices what seems to be missing in their efforts to connect to this powerful referral source?

    Though many hospices’ messaging begins to speak to the gatekeeper (Web, Facebook, brochures, advertising), they often are focused more on listing services, explaining what she doesn’t know about hospice, debunking hospice myths, or talking about their hospice and sharing testimonials. Rather than connecting to this powerful decision maker in real and profound ways that resonate with her. This takes understanding this gatekeeper better than they understand themselves. We’ve spent two decades gathering intensive data on what will drive her action to choose a certain hospice. If your hospice wants to care for more individuals and be able to provide the full benefit of hospice sooner, then look to the in home gatekeeper; she can be your hospice’s best friend.

    Consider re-inventing your conversations with her. Truly examine how you are communicating with her. Then build strategies and messages based on intensive research that tell your hospice’s story like it has never been told before, in a way she will find hard to ignore. Then give these messages the power to be heard.

    How to Clean a Microfiber Couch

    Microfiber is a stain-resistant material with a smooth, velvety texture. Thus, microfiber furniture is trendy, aesthetic, and yet easy to maintain. They are super absorbent and easy to clean because of the microscopic fibers.

    However, if you do not clean your microfiber couch regularly then it can become dreadfully dirty. Most microfiber couches, feature a mix of pure microfiber and polyester-based material.

    They cannot be cleaned with water alone as it is likely to leave behind water stains or marks. Nevertheless, you can use water safely if your microfiber furniture’s tag is marked with a ‘W’ which refers to water.

    How to Clean a Microfiber Sofa

    Step for Cleaning

    1. Remove the cushions from the couch.
    2. Vacuum the couch or sofa thoroughly to get rid of dust and debris.
    3. Clean the couch with a slightly damp microfiber cloth to get rid of surface dirt. Prefer to use a white rather than a colored cleaning cloth as while scrubbing it can leave behind its own color on the couch.
    4. Remove tough stains by spraying rubbing alcohol (isopropyl alcohol) or clear alcohol (gin, vodka) on the affected area and scrubbing it with a soft, absorbent, cotton cloth, scrub sponge, or a soft scrub brush.

      Unlike water or any water-based solution, alcohol evaporates quickly and hence, it is less likely to saturate the fabric. If the tag on your sofa or couch reads S-W then you can use water-based cleaning solutions like a gentle soap, carpet cleaner, upholstery shampoo, etc.
    5. Let the fabric dry. To speed up the drying process, you may use a blow dryer on the wet spots.
    6. Finally, scrub the surface of the fabric with a soft, clean scrub brush in circular motion. It helps fluff up the microfiber and soften the stiff fibers.

    How Remove Stains and Other Problems

    • Apart from rubbing alcohol and clear alcohol, distilled water and baby wipes are considered useful for clearing stains, even ink stains and water rings from a microsuede couch. You can try a dry cleaning solvent, too.
    • For removing nail polish or other tough stains, consider using an aerosol hairspray or simply a nail polish remover on the stained spot.
    • To remove pet hair from the couch, use a sticky lint roller. Besides, you may pick up the pet hair with rubber gloves.
    • When dealing with vomit stains, use a mixture of white vinegar and water, both mixed in equal parts. Spray the solution on the stain and scrub it with a soft cloth or scrub sponge. As for the vomit smell, you may sprinkle baking soda on the affected area and leave it overnight. In the morning, vacuum it away. You can use this trick to get rid of urine stains and smells, too.
    • In case there is chewing gum stuck on the fabric, rub an ice cube on it. Ice helps harden the chewing gum so that it can be peeled off easily.
    • Using a steam cleaner with fabric attachment (set to delicate mode) is also considered good for cleaning microfiber sofa, cushion, loveseat, and so on.

    Additional Tips

    • Before cleaning the entire sofa or couch, make sure you test your cleaning solution on a small portion of the sofa. Cleaning microfiber furniture with soaps and detergents is usually discouraged as it tends to ruin the material’s ability to repel stains and damage its texture.
    • It is recommended to avoid saturating the fabric of your couch with water or any cleaning solvent as it may leave behind stains.
    • Though microfiber furniture is stain-resistant, it is not completely immune to stains. So, deal with fresh spills and stains immediately. Spills in particular, should be blotted with a paper towel as soon as possible so that the fluid does not seep in the padding present underneath.
    • You should clean the couch at least one a month. Plus, vacuum clean the dust and dirt once or twice a week, especially if you have pets.

    Venezuela protests: What you need to know

    (CNN) Venezuela has been riven by violent protests in recent weeks as opposition leaders face off with President Nicolas Maduro and his supporters.

    Anti-government protesters want Maduro to step down, accusing him eroding democracy. Maduro, meanwhile, has ordered the Venezuelan armed forces onto the streets to maintain order.

    The political turmoil comes against the backdrop of a worsening economic crisis. Despite having the largest proven oil reserves in the world, Venezuela is fast running out of cash. and its people have struggled for years with food and medical shortages, coupled with skyrocketing prices.

    As the protests continue, here’s what you need to know:

    Why are they protesting?

Interior and justice minister Nestor Reverol told reporters that the “lamentable” incident was under investigation. He said that moments before Yaminne was run over, demonstrators hurled a Molotov cocktail at the armored vehicle, opened the side door and “brutally assaulted” the driver.

    Venezuela is in crisis, and while there is no simple solution to the country’s woes, the opposition argues it can fix the failing economy.

    When there is food on the shelves, prices are so high that few Venezuelans can afford it. Many have taken to eating out of the trash.

    Medicine remains in short supply, too. Venezuelans hunt for penicillin and other remedies at pharmacies everywhere, often without success. Public hospitals have fallen apart, causing people, including infants, to die because of the scarcity of basic medical care.

    Venezuela recently asked the United Nations for help to relieve serious shortages of medicines.

    Where has all the money gone?

    The country only has $10.5 billion in foreign reserves left. according to recent Central Bank of Venezuela data. For the rest of the year, Venezuela owes roughly $7.2 billion in outstanding debt payments. In 2011, Venezuela had roughly $30 billion in reserves. In 2015, it had $20 billion.

    Massive government overspending, a crashing currency, mismanagement of the infrastructure and corruption are all factors that have sparked high inflation in Venezuela. Inflation is expected to rise 1,660% this year and 2,880% in 2018, according to the International Monetary Fund.

    Another key problem is the relatively low price of oil, which stands at half of what it was in 2014. Venezuela has more oil reserves than any other nation, and oil shipments make up more than 90% of its total exports.

    The low price is making it nearly impossible for the country to pay its debts and import food, medicine and other essentials. Hyperinflation has wiped out salaries and the value of the currency, the bolivar, sending prices for all kinds of goods skyrocketing.

    The outlook isn’t great either. Unemployment is set to surpass 25% this year, according to the IMF, and the economy is expected to remain in recession this year and the next after shrinking a massive 18% last year.

    What are Venezuela’s neighbors doing about the turmoil?

    President Donald Trump has not said much publicly about the crisis in Venezuela. In the early days of his presidency, he tweeted a photo of himself with Lilian Tintori. the wife of opposition figure Leopoldo Lopez. In the tweet, Trump called for Venezuela to release its political prisoners. US Secretary of State Rex Tillerson told reporters the United States was “concerned that the government of Maduro is violating its own constitution and is not allowing the opposition to have their voices heard, nor allowing them to organize in a way that expresses the views of the Venezuelan people.”

    The Organization of American States recently tried to declare Venezuela in violation of its democratic charter but was denied the necessary votes by Caribbean and Central American nations that have depended for years on cheap Venezuelan oil. OAS Secretary General Luis Almagro, a Uruguayan, routinely calls Venezuela a dictatorship.

    In March, Venezuelan Foreign Minister Delcy Rodríguez called Almagro a liar and a criminal mercenary at the OAS.

    The region seems split in its support for Maduro. Argentina, Brazil, Chile, Colombia, Costa Rica, Guatemala, Honduras, Mexico, Paraguay, Peru and Uruguay signed a joint statement Monday asking for the Venezuelan government “to guarantee the right to peaceful demonstration,” and avoid violence against protesters. They have also urged the government to call for elections.

    Even the Vatican has gotten involved in the Venezuela crisis. Pope Francis sent an envoy to Caracas to mediate talks between the opposition and the government in 2016. While those talks failed, Francis said he is willing to meet with the opposing parties to help resolve the conflict.

    What Does Dark Green Poop Mean?

    Why are we here?

    Are we alone in the universe?

    Why is my poop green?

    The first two questions are a bit beyond the purview of this site, but the last one can be answered quite readily.

    Dark green poop may be surprising or alarming upon your first encounter, but it actually has a very simple biological explanation and, as far as the brilliant rainbow of feces is concerned, is not normally a cause of significant concern. There are also certain steps you can try taking when your poop is green in order to prevent a reoccurrence. But we’re getting ahead of ourselves. Let’s start with the basic question and move on from there.

    What Causes Green Stool?

    Understanding why you have dark green feces first requires knowing why it’s normally brown in the first place. Poo is a mix of undigested food, bile, bacteria, and dead blood cells. The brown coloration happens during the stool’s journey through the digestive tract where intestinal bacteria break down and feast on the leftover bile and other cell detritus it contains. The underlying process is surprisingly complicated but the main takeaway here is that poo is normally greenish until exposure to intestinal bacteria, where turns it brown.

    It’s also important to distinguish between dark green stool (1 ) in adults and any dark green poop from a baby. During the first few days of life, an infant will pass dark green stool known as meconium. This is normal and is the result of waste that was accumulated during gestation. Meconium usually persists for several days until the baby begins breast or formula feeding, at which point it will start to turn yellow over time. Dark green poop in a breastfed baby or one being bottle-fed is therefore not normally a cause for alarm.

    Now that the preliminaries are out of the way, let’s look at what dark green poop might mean.

    Excess Nutrition

    Sometimes, the color of your poo has less to do with your own body and more with what you’re eating. Consuming large amounts of green, leafy vegetables like kale or broccoli can sometimes result in your poo appearing dark green. Iron-rich foods such as beans or meats will produce a similar effect and may leave you with a dark green, almost black poop. This is more easily seen in the dark green poop a two month old might have if they are fed iron-fortified milk. In all of these cases, the underlying mechanism is the same. There is a biological limit to how much of any nutrient your body can make use, of and anything leftover can get excreted, dyeing the resulting poo. Iron supplements or supplements that contain chlorophyll or fructose can also cause green stools.

    Food Coloration

    Food dyes are generally not absorbed by the body and will get excreted along with other waste. If you have recently eaten a large number of foods that use green coloring (such as promotional St. Patrick’s Day treats), you may find yourself producing green stool over the following days. How susceptible someone is to food coloring will vary from one individual to the next. It’s fully possible, for instance, for you and a friend to eat the same dyed foods but for only one of you to have a green poo as a result.

    Medication

    Antibiotics, particularly the powerful ones that get prescribed for major infections, are capable of reducing the levels of bacteria in your intestinal tract. As mentioned earlier, these are the same bacteria responsible for why poo normally appears brown and their loss means your feces can’t be processed as thoroughly during its trip through the body. Due to this, green stool is a known side effect of some antibiotics.

    Gastrointestinal Distress

    Anything that causes food to move too quickly through the intestines can produce green stool since this cuts down on the amount of time it can be processed. This means that any situation capable of causing severe diarrhea, such as food poisoning, salmonella, a parasite, or irritable bowel syndrome, would also be able to produce dark green bowel movements. In other words, anything capable of aggravating your bowels will also be capable of producing dark green diarrhea.

    Cleansing

    A colon cleanse is a type of home treatment some individuals use to try and flush “toxins” from the body. Cleanses come in numerous varieties but typically employ some combination of supplements, enemas, and/or laxatives. Since colon cleanses essentially force-evacuate your bowels, it’s possible for them to produce green stool by virtue of triggering a bowel movement prematurely, before feces has been properly processed.

    Pregnancy

    It’s also possible to have dark green poop while pregnant. During pregnancy, a woman’s body produces more blood, and if she doesn’t get enough iron, she could become anemic. If that happens, she might then take iron supplements, a side effect of which is black or green poop. Iron supplements can cause other discomfort, such as constipation, so it may be in the best interest of pregnant women to increase their iron intake through diet, by consuming liver, red meat, and leafy greens.

    Treating Dark Green Poop

    Anyone can be affected by dark green poop; toddlers and adults alike. Treatment for dark green poo begins with identifying what the actual cause was. Fortunately, this is not a difficult task. Meconium is easy to rule out simply by asking whether the person was born recently.

    Similarly, you should be able to remember whether or not you recently engaged in a colon cleanse (enemas are kind of hard to forget). If you suspect antibiotics or a nutritional supplement are involved, it’s best to consult with your doctor on how this can be determined. It’s strongly inadvisable to discontinue an antibiotic without your doctor’s approval since you were likely prescribed that medicine for a good reason.

    Lastly, conditions such as food poisoning or irritable bowel syndrome come with other associated symptoms that can help identify them. In these instances, the green stool is likely accompanied by diarrhea, stomach cramps, nausea, vomiting, or other signs of digestive disagreement.

    Once your cause is identified, treatment becomes relatively simple:

    • Meconium goes away on its own, so time is the only remedy needed.
    • Talk to your doctor about switching to a new, less severe antibiotic to give your intestinal bacteria time to recover.
    • Over-enthusiastic eating of green foods or iron-rich items can be curtailed with simple diet adjustments, as can any indulgence in overly-dyed treats.
    • Food poisoning requires bed rest, maintaining fluid intake, and easing in to bland foods once you’re able to keep them down. Most food poisoning cases resolve within a few days with or without treatment.
    • Avoid giving yourself a colon cleanse.

    When to See Your Doctor

    A one-off incidence of green stool is rarely a cause for concern and can be safely ignored. If you find that you are having recurrent or repeat episodes of green bowel movements, however, a doctor’s appointment may be in order.

    Often, an exam for green stool will involve some questions about any recent dietary or lifestyle changes that may be affecting digestion and the stool itself may get examined if nothing can be ruled out.

    One important thing to keep in mind is that although most causes of green stool are benign, this does not stop green stool from occurring alongside other more problematic situations.

    If your stool seems to have mucus or blood in it (bright red or tarry, almost black) or is very watery, medical attention may be advised. This also applies if you experience rectal pain, intermittent bouts of constipation, fever, or loss of appetite. These symptoms should always be paid attention to and investigated regardless of whether you have dark green poop or not.

You can use these details about a hotel’s star rating, amenities, and location to reveal the hotel’s name, with the help of independent websites like Bid Goggles. Hotwire Revealed. Bidding for Travel. and Better Bidding .

    My fave tool is Bid Goggles (bidgoggles.com). a pet project by some travelers and computer programmers. After you’ve targeted a promising deal on Hotwire. copy the mystery hotel’s amenities, star rating, and neighborhood into the Bid Goggles tool (open in a separate browser tab or window). The site matches the Hotwire description with the description of similar hotels that other travelers have visited.

    For example, I recently helped a family member book a room in Pittsburgh for a wedding. I fired up Hotwire and quickly locked in on a “four-star, downtown Pittsburgh area hotel” with a $131 a nightly rate. Hotwire listed this hotel’s amenities as including a fitness center, a business center, high-speed Internet access, and spa services. I went to Bid Goggles and punched in these details about the property. The site predicted that the hotel would be either the Renaissance Hotel or the Omni William Penn Hotel. I went ahead with the booking and lo and behold it was the Omni William Penn.

    Hotwire Revealed is another site that works similarly and successfully.

    Both sites are far, far from perfect, though. Nobody has a magic decoder ring that can reveal every one of Hotwire’s listings perfectly every time. But the sites provide close-enough guesses. In the above example, Bid Goggles had predicted the Omni William Penn and the Renaissance Hotel. I looked at the websites of those hotels and was pleased with how they looked and their general location that I felt confident that even if I didn’t get either of those hotels I would get a hotel that was fairly similar. The process boosted my confidence that I was doing right by my relatives when planning their trip.

    The information about successful Hotwire bookings is more comprehensive and more frequently updated at the Bidding For Travel forums and, to a lesser extent, at Better Bidding. But there’s a trade-off: Those forums require more patience to use than the other sites.

    Are You (Still) Painting the Picture? Ensuring High Quality Documentation in Hospice

    It’s been almost twenty years since the Health Care Finance Administration (now the Centers for Medicare and Medicaid Services ) sent a memo to hospices announcing the first widespread, in-depth review of hospice medical records and patient eligibility. Focused Medical Review, as it was called in 1994, was just the beginning of the government’s search for fraud and waste in hospice.

    Prior to this action, hospices that had largely avoided widespread regulatory scrutiny were suddenly being held accountable to a much higher documentation standard than ever before. During this audit, hospices struggled to demonstrate through their documentation that patients who had already died, often within days or weeks of admission, met eligibility requirements for hospice. In the days before Webinars or online education, hospices were challenged with the means to disseminate education to their staff to teach them to “paint the picture” of medical eligibility.

    Today, hospices are once again facing intense scrutiny of their documentation as Medicare Administrative Contractors (MACs) are sending notices of Probe Edits and Additional Document Requests (ADRs). The burden on staff and financial resources to respond to these audits is taking a toll on hospices under review. Once again, many hospices are finding that they lack the documentation required to paint a picture of eligibility.

    The criteria used to determine eligibility for the Hospice Medicare Benefit is specific to each MAC, and changes periodically based on new prognostication tools and research. Hospices should use the disease-specific criteria outlined in each MAC’s Local Coverage Determinations (LCDs) to determine if someone is or continues to be eligible for hospice under Medicare.

    Documentation of disease-specific criteria, along with supporting information such as description of the patient’s illness trajectory, pertinent co-morbid and secondary conditions, severity of symptoms, declining functional, cognitive and nutritional status, and overall burden of illness are critical to depicting why this patient will probably die within six-months or less.

    All hospice staff should be fully aware of and utilize their LCD guidelines and disease-specific criteria to document eligibility for each admission and every recertification.

    Ask yourself the following questions to determine if your hospice team has the education and tools that they need to paint the picture of eligibility:

    Are the LCDs easily accessible to all admission and IDT staff – either on their laptop, smartphone or in a handy printed resource?

    Does your team refer to LCD criteria when discussing recertification during IDT meetings?

    Does each patient’s medical record include documentation regarding his or her prognosis, both upon admission and in each recertification note?

    Do your physician narratives contain key elements based upon LCD guidelines that lead the reader to the conclusion that the patient is terminally ill?

    Do you conduct monthly chart audits to ensure that best practices for documenting medical eligibility are being followed?

    Are ALL staff trained and regularly retrained on how to document to the LCDs, including documentation of co-morbid conditions to support medical eligibility?

    If you answered “No” to any of the questions below, your hospice may be at risk for financial penalties in the event of an audit. Comprehensive documentation that paints the picture of initial and ongoing eligibility is paramount to the ongoing survival of your hospice. Training, along with ongoing chart reviews, is the best insurance you can have against future MAC audits, probes, and ADRs.

    While the hospice’s source of funds may be the last thing on your mind, hospices are businesses. The business end of hospice can affect the care your loved one receives. Hospices receive funds from government programs or private insurance, from donations made by the public or other corporations, and from grants donated by charitable foundations. Hospices are reimbursed by Medicare, Medicaid, or private insurance for care provided to the terminally ill.

    Except for continuous around-the-clock home nursing care, which is paid for on an hourly basis, all other services are reimbursed on a per-diem basis. Hospices are not reimbursed on a fee for service basis: rather, hospices are paid, on a basis of how many days the patient is enrolled in the program and received services: the per-diem basis. In return for payment, Medicare, Medicaid and private insurance companies expect the hospices to provide all services which the patient and family need which are related to the terminal illness. The Centers for Medicare Services (CMS) regulations mandate that the hospice comply with the regulations.

    And the per-diem basis of payment assumes that the hospice agency will actually provide those services; this payment system places much trust in the hospice management. However, fewer services provided to a patient results in more money retained by the hospice. Obviously, the system can reward fraudulent hospices with increased income while honest, dedicated hospices provide full services as required.

    Donations to Hospices

    While nonprofit hospices can solicit as well as receive charitable donations, for-profit hospices can only receive donations. After the death of their loved one, some families suggest making memorial donations to the hospice they used. This is a very significant source of funds to the hospice and helps to cover expenses incurred in running the hospice.

    Nonprofit charitable hospices are supposed to provide hospice services to those persons who do not have coverage and cannot afford to pay for care. So when you donate to a nonprofit hospice, there is a greater likelihood that charitable hospice services will be provided.

    When you donate to a hospice, you cannot be sure that donations made to the local hospice will actually be used for the purpose you designate. Even if you designate a purpose, there are legal tricks of the trade, or accounting, which can make it possible for a hospice to shift the donated money or other money to other purposes than what you wished. Although there are laws stating that restricted donations must be used for the purpose designated by the donor, certain recent court cases have challenged the strictness with which these laws are enforced. Practically speaking, the government does not always look into what a hospice may do with any donations.

    While donating directly to a nonprofit hospice allows you to claim a charitable federal tax deduction, you can only be sure that your money is used for a particular purpose if you personally buy something and donate it to the hospice. Some hospices have several branch locations covering a large area. If you donate to one hospice, the management may take the money and transfer it to a completely different location. or it may use the money to help pay for unreasonably high salaries for executives.

    For-profit hospices have no obligation to provide services to anyone who does not have coverage from Medicare, Medicaid or private insurance. They can turn people away and tell people to go elsewhere. If you do choose to donate to a for-profit hospice, just remember that the for-profit corporation may take that money and pay its Chief Executive Officer many hundreds of thousands of dollars each year in salary and benefits!

    No Need to Ever Pay Privately for Hospice Services

    Whether or not your loved one has Medicare, Medicaid or private insurance, you should never have to pay out of your own pocket for hospice covered services. If a hospice asks you to pay for private duty nursing out of your own pocket when there are uncontrolled symptoms, when you already have Medicare, Medicaid or private insurance, it is very likely committing health care fraud illegally! It is important that you report this to your regional U.S. Office of Inspector General and U.S. Attorney’s office. Double-billing is a very real and despicable scam committed by rogue hospices taking advantage of the dying. Don’t let this happen to you!

    Charitable Provision of Hospice Services

    If you do not have any coverage by Medicare, Medicaid or private insurance, a nonprofit hospice can provide services to you FREE OF CHARGE as part of its charitable mission. The nonprofit status of the hospice often requires it to provide charitable services.     When cancer is advanced it means that it can’t be cured and is likely to cause death within a limited period of time. The amount of time is difficult to predict but it could be weeks to several months. Doctors might also say that the illness is terminal. This distressing news can affect you and the people close to you in different ways. This page is about how you may feel and what can help you cope. There is information about

    Your first reactions

    There is no right or wrong way to react when you are told your cancer is too advanced to cure. Everyone responds in their own way. For most of us of course, this is very shocking news. Even if you thought it may happen, hearing it from your doctor can still be very upsetting.

    Some people become silent. They can’t believe what they are hearing and don’t know what to say or do. Some start to cry and feel as though they won’t be able to stop. Others may become very angry and scared. Some people feel numb and as though they have no emotions. These are all very common reactions.

    You might find that many questions come into your mind, such as

    • Why me?
    • Do I deserve this?
    • Why can’t you find a treatment to help me?
    • There must be something that will stop this cancer – can’t you just try anything?

    It is natural to feel desperate, upset, angry, or that you don’t believe the news. Be sure to give yourself the time and space to take in what is happening. You might want to be on your own. Or you may need to spend time with your partner, family or friends to help you deal with the news. Of course they may also be very upset and feel that they don’t know what to say. Even if all you can do at first is get upset together, that can be a huge help.

    If you don’t feel like talking straight away, you don’t have to. Just let the people around you know that. As hard as it can be, try not to push your emotions aside completely. If you can manage it, it is better to express how you feel and allow your emotions to come to the surface – even if that is uncomfortable and hard to cope with.

    Feelings you may have

    Over the first few days, you may go through a range of very strong emotions. The emotions may change very quickly and sometimes you might feel numb or as though everything is happening to someone else. Some people say they feel very calm and detached when they are first told they don’t have long to live.

    At times, you will probably feel shock, anger, and sadness. These emotions can feel overwhelming at times. This news will mean that you can’t plan your future in the way you had hoped. Dying may mean leaving behind a partner, children, and other important people in your life. You may wonder how they will cope and won’t want to see them upset.

    These thoughts may be too painful to cope with at times. You might be unable to stop crying and worrying. All this is perfectly normal and understandable.

    You may find it difficult to look around and see life going on as normal for most people. It can feel very strange to watch people going about their daily lives as usual – shopping, driving and working. Coping with this roller coaster of feelings can be very exhausting. You may feel as if you are stuck under a huge black cloud and that there is no point in doing anything.

    Most people will have some or all of these emotions. This usually changes gradually. Many people say that the intensity and distress lessens in time. This doesn’t mean that you stop worrying or feeling upset. But the feelings get more bearable. You will most likely be able to think about your situation a little more calmly and plan what you want to do.

    Looking at your options

    It can be helpful to find out what support is available. Some people want to make plans about what they would like to do before they die. You may like to ask your doctor about

    • How to control cancer symptoms
    • What is likely to happen
    • Who to talk to about your concerns

    Having some idea about what to expect can help you feel less anxious. You might also want to think about where you want to be at the end of your life.

    Sharing your feelings

    Sharing your fears and sadness with people you love and trust may be a great relief for you.

    Many people say that talking about their feelings helps them to cope. It also helps your friends and family to understand more about your situation. In turn, this will help them to help and support you. Other people find sharing their thoughts and emotions too difficult, and would rather keep things to themselves. It is important to do whatever feels best for you.

    Don’t let other people pressure you into talking if you don’t feel ready. This is a very personal, emotional time. You can choose how you handle things. If you would like to talk, make sure you choose people you can talk to easily, who will understand how you feel and be able to support you.

    If, after some time, you still feel overwhelmed and that you can’t cope, try speaking to someone outside your immediate family and friends.

    Our advanced cancer organisations section lists counselling organisations that offer this kind of support. They also have information about coping with grief, terminal illness, and dying with cancer.

    If you are by yourself

    If you don’t have people nearby to help with practical things, you can ask for help. One of your health care team may be able to arrange volunteers to help out at home, or come to visit you in hospital. Ask your specialist cancer nurse or doctor about this.

    If you need help with personal care, such as washing or dressing, social services may be able to help. They can arrange a care package for you.

    You might be wary of letting people you don’t know into your life. But most people find that they do need support at some stage from other people or organisations. And the people that you are put in touch with will be understanding and aware of your feelings and need for personal space. They will want to support you in the best possible way they can.

    You might find help and support at your religious organisation or through other organisations you are involved with.

    Staying hopeful

    It is important that people nearing the end of their life, and those close to them, are aware of what is likely to happen. But only if they want to know this. Everyone needs to have some sense of hope for their future. When you are dying, this hope may be that you can visit a place that you love. Or you may hope that you can enjoy being with your family and friends for a time. Some people believe that there is life after death and find that this gives them hope.

    It may help you to share your hopes with other people, so they can help you fulfil them. Many people hope for comfort, dignity, friendship and love to surround them in their final days.

    Getting a Cheap Hotel: Does It Matter What Booking Site You Use?

    There are countless hotel booking sites out there, but how do you decide which one to use? They all deliver a wide range of search results when you re looking for a hotel, and it s hard to tell if there are any differences. I decided to do some testing and find out which sites offer the best deals.

    As much as I love hostels and other forms of cheap accommodation, there is something nice about the luxuriousness of hotels. They are clean and quiet, feature comfy beds, strong showers, and lots of soap I can take for later.

    But luxury comes at a price. Hotels certainly aren t cheap and I hate spending a lot of money for a room I am only going to be in for a few hours. It s why I mostly avoid hotels—I don t think they are a good use of money (and there are far better accommodation options out there ). If I m not in a hostel, you can usually find me at a cheap guesthouse or at an Airbnb.

    But I ve been traveling a lot for work lately, and with the seemingly endless hotel booking sites out there, I decided this was a good time to do some testing. Does it matter which site you use to book a hotel?

    I also picked five cities to research: London, Los Angeles, Paris, NYC, and Seattle. I picked bookings close to the current date of research as well as far in advance, on both weekdays and weekends. (I did my research end of March, which is why the dates are different than what you d expect from a post published in early May.)

    I searched six booking websites: Expedia. Hotels.com. Booking.com. Hotwire. Priceline. and a new one called TravelPony in two, three, and four star categories. Below are the data tables with the lowest price shown (Priceline rates are based on their search listings, not the bidding section of the site).     Are You (Still) Painting the Picture? Ensuring High Quality Documentation in Hospice

    It’s been almost twenty years since the Health Care Finance Administration (now the Centers for Medicare and Medicaid Services ) sent a memo to hospices announcing the first widespread, in-depth review of hospice medical records and patient eligibility. Focused Medical Review, as it was called in 1994, was just the beginning of the government’s search for fraud and waste in hospice.

    Prior to this action, hospices that had largely avoided widespread regulatory scrutiny were suddenly being held accountable to a much higher documentation standard than ever before. During this audit, hospices struggled to demonstrate through their documentation that patients who had already died, often within days or weeks of admission, met eligibility requirements for hospice. In the days before Webinars or online education, hospices were challenged with the means to disseminate education to their staff to teach them to “paint the picture” of medical eligibility.

    Today, hospices are once again facing intense scrutiny of their documentation as Medicare Administrative Contractors (MACs) are sending notices of Probe Edits and Additional Document Requests (ADRs). The burden on staff and financial resources to respond to these audits is taking a toll on hospices under review. Once again, many hospices are finding that they lack the documentation required to paint a picture of eligibility.

    The criteria used to determine eligibility for the Hospice Medicare Benefit is specific to each MAC, and changes periodically based on new prognostication tools and research. Hospices should use the disease-specific criteria outlined in each MAC’s Local Coverage Determinations (LCDs) to determine if someone is or continues to be eligible for hospice under Medicare.

    Documentation of disease-specific criteria, along with supporting information such as description of the patient’s illness trajectory, pertinent co-morbid and secondary conditions, severity of symptoms, declining functional, cognitive and nutritional status, and overall burden of illness are critical to depicting why this patient will probably die within six-months or less.

    All hospice staff should be fully aware of and utilize their LCD guidelines and disease-specific criteria to document eligibility for each admission and every recertification.

    Ask yourself the following questions to determine if your hospice team has the education and tools that they need to paint the picture of eligibility:

    Are the LCDs easily accessible to all admission and IDT staff – either on their laptop, smartphone or in a handy printed resource?

    Does your team refer to LCD criteria when discussing recertification during IDT meetings?

    Does each patient’s medical record include documentation regarding his or her prognosis, both upon admission and in each recertification note?

    Do your physician narratives contain key elements based upon LCD guidelines that lead the reader to the conclusion that the patient is terminally ill?

    Do you conduct monthly chart audits to ensure that best practices for documenting medical eligibility are being followed?

    Are ALL staff trained and regularly retrained on how to document to the LCDs, including documentation of co-morbid conditions to support medical eligibility?

    If you answered “No” to any of the questions below, your hospice may be at risk for financial penalties in the event of an audit. Comprehensive documentation that paints the picture of initial and ongoing eligibility is paramount to the ongoing survival of your hospice. Training, along with ongoing chart reviews, is the best insurance you can have against future MAC audits, probes, and ADRs.

    Do You Have ‘The Little Blue Book’ in Your House?

    If you are wondering what I’m talking about, it is for you that I am writing this blog! Here’s the bottom line: We were all born one moment of one day, and each of us is going to die one moment of another day — we just don’t get to know when that will be. For some of us, that is a major source of anxiety. Furthermore, we live in a society that has kept us in the dark about what to expect when we, or someone we love, dies. This absence of knowledge not only makes us ill-prepared to face death, but it feeds our fear of death, which in turn diminishes our enjoyment of life.

    It is perfectly normal to have some anxiety or fear about death, and in fact, most death-related anxiety is actually about the process of dying rather than about being dead. That’s where the little blue book comes in — this little 14-page blue booklet gently, kindly, and in a matter-of-fact manner explains the dying experience in such a way that it brings its readers great comfort. So why not reach for that comfort sooner rather than later? Why live with death anxiety on autopilot in the background of your mind? Why wait and find yourself called to the bedside of a dying loved one unprepared for what you will encounter and not knowing what to do?

    Most people working in the field of dying, death, and grieving know about “the little blue book,” as it has come to be known. Many hospice and palliative care organizations around the country give this book to their dying patients and their loved ones to ease them through the dying experience. It is entitled Gone From My Sight: The Dying Experience and is written by Barbara Karnes, RN, who was one of the pioneers of the hospice movement in this country in the early 1970s.     This blog is not meant to be an advertisement for Barbara and her work, but rather I am shouting from the rooftops to spread the word that each and every one of us has the opportunity to prevent the needless suffering that our fundamental ignorance about the dying process brings. Not knowing what is normal and what the signs of the end of life are, we often cling desperately to life, relying on physicians to come up with a pill or procedure that will prolong our lives. Sometimes this is reasonable, but often the dying and their loved ones simply lack the understanding of the dying process that would enable them to consider the relative wisdom of further medical interventions or the timing of opening the door to palliative care.

    Those of us who work in the field of dying, death, and grieving have satchels of stories about the ways people suffer unnecessarily through their own death or at the bedside of a loved one. Not knowing what to expect, what is normal, and how to support and comfort a loved one who is dying makes us ill at ease which detracts from a tranquil environment for the dying.

    As someone who writes often about dying, death, and grieving, I am quite passionate about the need for a basic education of the general public about this topic. That’s why I would go so far as to say that if I were queen of the world, I would make Barbara’s books required reading for everyone and as important as a fire extinguisher to have in your home!

    The vast majority of us will die of old age and/or prolonged illness. Having Barbara’s books on hand when you or a loved one begins the end of life’s journey empowers you all to do your very best to provide a loving, comforting, and supportive sendoff. Rather than worrying about what you should or shouldn’t do, what’s normal and what isn’t, and how to be the greatest comfort to your loved one, empower yourself with some basic education and serve as an example to others. Sitting at your dying mother’s bedside, when you know that what you are observing is normal, you can encourage her and let her know she is doing a great job of dying. In this case, a little knowledge is a very powerful, wise, and comforting thing.

    Find out if you’re an inpatient or an outpatient—it affects what you pay

    Your hospital status—whether you’re an inpatient or an outpatient—affects how much you pay for hospital services (like X-rays. drugs. and lab tests ) and may also affect whether Medicare will cover care you get in a skilled nursing facility (SNF) following your hospital stay.

    • You’re an inpatient starting when you’re formally admitted to the hospital with a doctor’s order. The day before you’re discharged is your last inpatient day.
    • You’re an outpatient if you’re getting emergency department services. observation services, outpatient surgery. lab tests, or X-rays, or any other hospital services, and the doctor hasn’t written an order to admit you to a hospital as an inpatient. In these cases, you’re an outpatient even if you spend the night in the hospital.

    Observation services are hospital outpatient services you get while your doctor decides whether to admit you as an inpatient or discharge you. You can get observation services in the emergency department or another area of the hospital.

    The decision for inpatient hospital admission is a complex medical decision based on your doctor’s judgment and your need for medically necessary hospital care. An inpatient admission is generally appropriate when you’re expected to need 2 or more midnights of medically necessary hospital care, but your doctor must order such admission and the hospital must formally admit you in order for you to become an inpatient.

    Here are some common hospital situations and a description of how Medicare will pay. Remember, you pay your deductible. coinsurance. and copayment .

    Inpatient or outpatient

    You’re in the Emergency Department (ED) (also known as the Emergency Room or “ER”) and then you’re formally admitted to the hospital with a doctor’s order.

    Outpatient until you’re formally admitted as an inpatient based on your doctor’s order. Inpatient following such admission.

    Your inpatient hospital stay

    Your doctor services

    You visit the ED and are sent to the intensive care unit (ICU) for close monitoring. Your doctor expects you to be sent home the next morning unless your condition worsens. Your condition resolves and you’re sent home the next day.

    You come to the ED with chest pain, and the hospital keeps you for 2 nights. One night is spent in observation and the doctor writes an order for inpatient admission on the second day.

    Outpatient until you’re formally admitted as an inpatient based on your doctor’s order. Inpatient following such admission.

    Your inpatient hospital stay

    Doctor services and hospital outpatient services (for example, ED visit, observation services, lab tests, or EKGs)

    You go to a hospital for outpatient surgery, but they keep you overnight for high blood pressure. Your doctor doesn’t write an order to admit you as an inpatient. You go home the next day.

    Doctor services and hospital outpatient services (for example, surgery, lab tests, or intravenous medicines)

    Your doctor writes an order for you to be admitted as an inpatient, and the hospital later tells you it’s changing your hospital status to outpatient. Your doctor must agree, and the hospital must tell you in writing—while you’re still a hospital patient before you’re discharged—that your hospital status changed.

    Remember, even if you stay overnight in a regular hospital bed, you might be an outpatient. Ask the doctor or hospital.

    The copayment for a single outpatient hospital service can’t be more than the inpatient hospital deductible. However, your total copayment for all outpatient services may be more than the inpatient hospital deductible.
